Respiratory Volume Monitoring to Assess the Effect of Airway Maneuvers on Ventilation during Upper Endoscopy

Introduction: Propofol use during endoscopic procedures has become increasingly popular and assessing and maintaining airway patency is a significant challenge. Anesthesiologists often use airway maneuvers to maintain airway patency and ventilation during procedural sedation. A novel, non-invasive, Respiratory Volume Monitor (RVM) that provided continuous, real-time measurements of minute ventilation (MV), tidal volume (TV) and respiratory rate (RR) was used to monitor respiratory performance before, during, and after endoscopic procedures, quantify MV changes before and after airway maneuvers, and to quantify propofol-induced respiratory depression. Methods: RVM traces were obtained from 25 patients undergoing sedation for endoscopic procedures. Airway maneuvers were performed in 19/25 patients. All 25 patients received propofol as the primary sedative. Results: Forty-five airway maneuvers were performed. During these maneuvers, all respiratory parameters increased relative to pre-maneuver levels. On average, MV increased by 24% ± 5% (mean ± SEM), TV 14% ± 5% and RR: 17% ± 6%. The cohort average MVBASELINE was 9.5 ± 0.7 L/min (TV = 670 ± 60 ml, RR = 15 ± 0.7). Following propofol MV decreased transiently, reaching nadir five minutes after the last dose of propofol at 82% ± 10% of baseline (MV = 7.5 ± 1.0 L/min). The reduction in MV was driven by reduction in TV, not RR. Conclusions: Data demonstrated that RVM was able to track changes in ventilation and was able to quantify respiratory changes following airway maneuvers. All patients had a significant reduction in ventilatory volumes after propofol. Five minutes after the last dose of propofol, MV and TV were significantly reduced while RR was not, suggesting that monitoring respiratory rate alone was not a sufficient indicator of respiratory status.

Schwann cells(SCs)are essential in the development of the peripheral nervous system(PNS),in PNS inj ury,and the aging PNS.SCs comprise greater than 90%of the nucleated cells in peripheral nerves(campana,2007).Myelinating SCs insulate large-diameter axons,forming a 1:1relationship and promoting saltatory conduction of axonal action potentials.Non-myelinating SCs ensheathe clusters of smaller diameter axons in Remak bundles.Both types of SCs provide essential metabolic and trophic support to axons,which is essential for maintaining axon integrity in healthy peripheral nerves.

Patient-reported outcomes in subjects with neuroendocrine tumors of the pancreas

AIM: To assess the patient-reported outcomes (PROs) of pancreatic neuroendocrine tumor (PNET) patients. METHODS: Fifty-one consecutive patients (21 male, 30 female, 61.0 ± 10.3 years) with proven PNETs were studied. An SF-12 questionnaire capable of exploring the physical (PCS) and mental (MCS) aspects of daily life was used. Four questionnaires were also used [12 items General Health Questionnaire (GHQ-12) for non-psychotic psychiatric disorders, State Trait AnxietyInventory (STAI) Y-1 and Y-2 for anxiety and BDI-for depressive symptoms] to explore the psychological aspects of the disease. Forty-four sex-and age-matched Italian normative subjects were included and evaluated using the SF-12, STAI Y-1 and Y-2 questionnaires.RESULTS: Seven patients refused to participate to the study; they were clinically similar to the 44 participants who agreed to complete the questionnaires. PNET patients had a PCS score (44.7 ± 11.0) were not signifi cantly different from the norms (46.1 ± 9.9, P = 0.610), whereas the MCS score was signifi cantly lower in patients (42.4 ± 13.0) as compared to the norms (48.2 ± 9.8, P = 0.036). GHQ-12 identified 11 patients (25.0%) as having non-psychotic psychiatric disorders.The STAI scores were similar in the patients and inthe normative population. Finally, BDI-identifiedeight patients (18.2%) with moderate depression and 9 (20.5%) with mild depression whereas 27 patients (61.4%) had no depression.CONCLUSION: The PNET patients had a good physical but an impaired mental component of their quality of life; in addition, mild or moderate depressive symptoms are present in about 40% of PNET patients.

Virtual anthropology:a preliminary test of macroscopic observation versus 3D surface scans and computed tomography(CT)scans

Virtual anthropology(VA)is based on applying anthropological methods currently used to analyse bones to 3D models of human remains.While great advances have been made in this endeavour in the past decade,several interrogations concerning how reliable these models are and what their proper use should be remain unanswered.In this research,a fundamental assumption of VA has been investigated:if the way we perceive and apply an anthropological method is truly similar when looking at bones macroscopically and through various 3D media.In order to answer,10 skulls of known age and sex were scanned using a computed tomography(CT)scanner and a 3D surface scanner.Two observers separately applied a defined staging method to eight suture sites on these skulls,first looking at the bone macroscopically,then at the 3D surface scan,and finally on the CT scan.Two rounds of observation were carried out by each observer.Intra-and inter-observer error were evaluated,and two sample t-tests used to evaluate if the different types of medium used yielded significantly different observations.The results show a high degree of inter-observer error,and that data obtained from 3D surface scans differ from macroscopic observation(confidence level 95%,P≤0.05).CT scans,in these settings,yielded results comparable to those obtained through macroscopic observations.These results offer many possibilities for future research,including indications on the kind of anthropological methods and anatomical landmarks that might be reliably transferable to the virtual environment.All current methods used in traditional anthropology should be tested,and if they prove unreliable,new techniques to analyse bones from virtual models should be developed.

A one-dimensional polynomial chaos method in CFD–Based uncertainty quantification for ship hydrodynamic performance

A one-dimensional non-intrusive Polynomial Chaos （PC） method is applied in Uncertainty Quantification （UQ） studies for CFD-based ship performances simulations. The uncertainty properties of Expected Value （EV） and Standard Deviation （SD） are evaluated by solving the PC coefficients from a linear system of algebraic equations. The one-dimensional PC with the Legendre polynomials is applied to：（1） stochastic input domain and （2） Cumulative Distribution Function （CDF） image domain, allowing for more flexibility. The PC method is validated with the Monte-Carlo benchmark results in several high-fidelity, CFD-based, ship UQ problems, evaluating the geometrical, operational and environmental uncertainties for the Delft Catamaran 372. Convergence is studied versus PC order P for both EV and SD, showing that high order PC is not necessary for present applications. Comparison is carried out for PC with/without the least square minimization when solving the PC coefficients. The least square minimization, using larger number of CFD samples, is recommended for current test cases. The study shows the potentials of PC method in Robust Design Optimization （RDO） and Reliability-Based Design Optimization （RBDO） of ship hydrodynamic performances.

Preliminary experience of helical tomotherapy for locally advanced pancreatic cancer

Radiotherapy for locally advanced pancreatic cancer is technically difficult and frequently associated with high-grade digestive toxicity.Helical tomotherapy(HT)is a new irradiation modality that combines megavoltage computed tomography imaging for patient positioning with intensity-modulated fan-beam radiotherapy.Its recent availability opens new fields of exploration for pancreatic radiotherapy as a result of its ability to tailor very well-defined dose distributions around the target volumes.Here,we report the use of HT in two patients with locally advanced pancreatic cancer. Doses to the bowel,kidneys and liver were reduced significantly,which allowed for excellent treatment tolerance without any high-grade adverse effects in either patient.

Modern post-mortem investigations use an increasing number of digital imaging methods,which can be collected under the term“post-mortem imaging”.Most methods of forensic imaging are from the radiology field and are therefore techniques that show the interior of the body with technologies such as X-ray or magnetic resonance imaging.To digitally image the surface of the body,other techniques are regularly applied,e.***.three-dimensional(3D)surface scanning(3DSS)or photogrammetry.Today’s most frequently used techniques include post-mortem computed tomography(PMCT),post-mortem magnetic resonance imaging(PMMR),post-mortem computed tomographic angiography(PMCTA)and 3DSS or photogrammetry.Each of these methods has specific advantages and limitations.Therefore,the indications for using each method are different.While PMCT gives a rapid overview of the interior of the body and depicts the skeletal system and radiopaque foreign bodies,PMMR allows investigation of soft tissues and parenchymal organs.PMCTA is the method of choice for viewing the vascular system and detecting sources of bleeding.However,none of those radiological methods allow a detailed digital view of the body’s surface,which makes 3DSS the best choice for such a purpose.If 3D surface scanners are not available,photogrammetry is an alternative.This review article gives an overview of different imaging techniques and explains their applications,advantages and limitations.We hope it will improve understanding of the methods.

CT-scan vs.3D surface scanning of a skull:first considerations regarding reproducibility issues

Three-dimensional surface scanning(3DSS)and multi-detector computed tomography(MDCT)are two techniques that are used in legal medicine for digitalizing objects,a body or body parts such as bones.While these techniques are more and more commonly employed,surprisingly little information is known about the quality rendering of digitalized three-dimensional(3D)models provided by each of them.This paper presents findings related to the measurement precision of 3D models obtained through observation of a study case,where a fractured skull reconstructed by an anthropologist was digitalized using both post-mortem imaging methods.Computed tomography(CT)scans were performed using an 8-row MDCT unit with two different slice thicknesses.The variability of 3D CT models superimposition allowed to assess the reproducibility and robustness of this digitalization technique.Furthermore,two 3D surface scans were done using a professional high resolution 3D digitizer.The comparison of 3D CT-scans with 3D surface scans by superimposition demonstrated several regions with significant differences in topology(average difference between+1.45 and-1.22 mm).When comparing the reproducibility between these two digitalizing techniques,it appeared that MDCT 3D models led in general to greater variability for measurement precision between scanned surfaces.Also,the reproducibility was better achieved with the 3D surface digitizer,showing 3D models with fewer and less pronounced differences(from+0.32 to-0.31 mm).These experiments suggest that MDCT provides less reproducible body models than 3D surface scanning.But further studies must be undertaken in order to corroborate this first impression,and possibly explain the reason for these findings.

Planning Movement of a Robotic Arm for Assembly of Products

This paper presents an algorithmic proposal of a movements" planner for a robotic manipulator. Its application is the planning of trajectories from wooden cuttings in a band saw. This algorithm is based on heuristics. A simulator was developed in order to allow the user to define the table"s shape and figure that you want to trim. The simulator was developed in Java from Sun Microsystems, and next it will be applied in ? Cyberbotics Webots.
