safety and success of precut biliary sphincterotomy: Is it linked to experience or expertise?

AIM： To determine the rates of success and complications of precut biliary sphincterotomy （PBs） based on prior experience and to compare the complication rates between PBs and standard endoscopic sphincterotomy （Es）. METHODs： A retrospective evaluation of prospectively collected non-randomized data at an academic tertiary referral center, The study included all patients in an eight-year period who underwent PBs and Es by a single endoscopist who had no formal training in PBs. The main outcorne measures of the study were success and complications of PBs with a comparison to complications of Es.REsULTs： A total of 2939 endoscopic retrograde chola ngiopancreatographies （ERCPs） were performed during the study period, including 818 （28%） Es and 150 （5%） PBs procedures. selective biliary cannulation via PBs was successful at the first attempt in 75% of the patients. Cannulation was achieved in an additional 13% of the patients at a subsequent attempt （total 87%）. Complication rate from PBs was 45% higher than Es, but did not differ significantly [7% （10/50） vs 5% （38/818）, P = 0.29]. None of the complications from PBs was severe. A significant trend towards increasing success existed with regard to the endoscopist＇s first attempt at precut （P = 0.0393, Cochran-Armitage exact test for trend, Z = -1.7588）. CONCLUsION： Despite the lack of specific training in this technique, PBs was performed with a high success rate and a complication rate similar to or less than reports from other experienced centers. These results suggest that endoscopic experience and perhaps innate endoscopic skill may play an important role in the outcome of this procedure.

“If somebody Needed Help, I Went Over”: social Capital and Therapeutic Communities of Older Adult Farmers in British Columbia Floods

Older adults in disaster contexts are often thought of as a passive, vulnerable population that lacks agency and capacities to cope in the aftermath. However, it can be argued that older adults may have underrecognized strengths that can be utilized pre-, peri-, and post-disaster. One of these strengths is older adults' unique social capital that stems from long-standing connections with other members of their respective communities. Using data from in-depth, semistructured interviews with farmers in British Columbia 3–11 months after the 2021 floods, this research explored the experiences of older adult farmers' recovery. The farmers discussed how they leveraged their social capital to aid in their recovery efforts from the flood event. By using their bonding social capital, older adult farmers transformed their existing, deep-rooted connections into post-disaster assistance. This, in turn, generated the idea of the therapeutic community, helping community members cope in the aftermath. This research indicated the need to further examine how older adults in disaster settings can be viewed as assets with community knowledge and skills as opposed to solely as a vulnerable population.

Pilfering of western juniper seed caches by scatter-hoarding rodents varies by microsite and canopy type

Molecular basis of ataxia telangiectasia and related diseases

Ataxia telangiectasia (AT) is a rare human disease characterized by extreme cellular sensitivity to radiation and a predisposition to cancer, with a hallmark of onset in early childhood. several human diseases also share similar symptoms with AT albeit with different degrees of severity and different associated disorders. While all AT patients contain mutations in the AT-mutated gene (ATM), most other AT-like disorders are defective in genes encoding an MRN protein complex consisting of Mre11, Rad50 and Nbs1. Both ATM and MRN function as cellular sensors to DNA double-strand breaks, which lead to the recruitment and phosphorylation of an array of substrate proteins involved in DNA repair, apoptosis and cell-cycle cbeckpoints, as well as gene regulation, translation initiation and telomere maintenance. ATM is a member of the family of phosphatidylinositol 3-kinase-like protein kinases (PIKK), and the discovery of many ATM substrates provides the underlying mechanisms of heterologous symptoms among AT patients. This review article focuses on recent findings related to the initial recognition of double-strand breaks by ATM and MRN, as well as a DNA-dependent protein kinase complex consisting of the heterodimer Ku70/Ku80 and its catalytic subunit DNA-PKcs, another member of PIKK. This possible interaction suggests that a much greater complex is involved in sensing, transducing and co-ordinating cellular events in response to genome instability.

Endocrine correlates of mate choice and promiscuity in females of a socially monogamous avian mating system with alternative male reproductive phenotypes

While our understanding of male reproductive strategies is informed by extensive investigations into endocrine mechanisms, the proximate mechanisms by which females compete for mates and adjust reproduction to social environment remains enigmatic. We set out to uncover endocrine correlates of mate choice, social environment, and reproductive investment in female red-backed fairy-wrens Malurus melanocephalus. In this socially monogamous, yet highly sexually promiscuous species, females experience discrete variation in the phenotype of their mates, which vary in both plumage signals and level of paternal care, and in the composition of their breeding groups, which consist of either the pair alone or with an additional cooperative auxiliary; fe- male investment varies according to these social parameters. We found that androgen, estrogen, and glucorticoid levels varied with reproductive stage, with highest androgen and estrogen concentrations during nest construction and highest corticosterone concentrations during the pre-breeding stage. These stage-dependent patterns did not vary with male phenotype or auxiliary presence, though androgen levels during pre-breeding mate selection were lower in females obtaining red/black mates than those obtaining brown mates. We found no evidence that androgen, estrogen, or corticosterone levels during the fertile period were re- lated to extra-pair young （EPY） frequency. This study demonstrates clear changes in steroid levels with reproductive stage, though it found little support for variation with social environment. We suggest hormonal responsiveness to social factors may be physiologically constrained in ways that are bypassed through exogenous hormone manipulations.

What do coaches want to know about sports-related concussion?A needs assessment study

Purpose: This study aims to identify the concussion-related training and education needs of Gaelic Athletic Association(GAA) coaches in Ireland,as well as the preferred method of concussion education ***: We used a self-report questionnaire to collect data from a convenience sample of 108 GAA coaches in Ireland. Data were captured on(1) informational needs and desires,(2) preferred methods of delivery, and(3) concussion practices and procedures. Questionnaires were completed electronically from June 3 rd–september 29 th, ***: Coaches indicated that they were most interested in receiving information about the(1) signs and symptoms of concussion,(2) assessment of concussion, and(3) return-to-play guidelines. Over two-thirds of participants indicated that in-person training would be the most effective mode of delivery of concussion education for this population. Additionally, only 10% coaches reported that before the start of the season they talked to their athletes about concussion management and safety, and this was more common among coaches who reported being formally educated about ***: Our findings reveal a disconnect between the concussion education needs and the education that is currently provided to GAA coaches, in terms of content and delivery modality. Our results suggest a need for a multifaceted approach to concussion education, tailored to the needs and learning preferences of the target population.

Repeatability of decision-making behaviour in male threespine stickleback Gasterosteus aculeatus： Effects of dummy vs. live stimuli

Individuals select from a number of behaviours when responding to various situations and the decisions they make may affect their fitness. The costs and benefits of these responses vary among individuals causing them to differ even in identical situations. One example of this type of situation is when territorial males encounter both a male and female simultaneously, gene- rating a trade-off that likely leads to individual differences due to differing costs of various actions among males. This situation commonly occurs in threespine stickleback, Gasterosteus aculeatus. However, for selection to act effectively, individuals must behave in a consistent manner and measuring repeatability can aid in understanding how selection may shape such trade-offs. Males of this species exhibit consistent individual differences in their response to dummy males and females but it is unknown ff patterns are similar when feedback from the stimuli is present. To assess this, male threespine stickleback were tested with dummy and live male and female conspecifics, presented separately and simultaneously. While the same trends were found re- gardless of stimulus type, males were more aggressive towards the live conspecifics than to the dummies. Repeatability values were similar within a treatment regardless of whether live or dummy conspecifics were used, suggesting that individuals show the same level of consistency. This study adds to our understanding of consistent individual differences by demonstrating that feed- back may not affect responses to conflicting stimuli and that male threespine stickleback respond in a consistent manner to both dummy and live stimuli
