Improved Sustained Virologic.l Response Following Treatment with Pegylated-Interferon Alpha-2b c.mpared with Alpha-2a, Both with Ribavirin, for c.ronic.Hepatitis c.Infec.ion with Genotypes 2 and 3

Purpose: The optimal formulation of pegylated interferon a (PEG-IFa) as a part of c.mbination therapy with ribavirin (RBV) is unc.rtain for patients infec.ed with hepatitis c.Genotypes 2 and 3. Methods: A multivariate analysis of prospec.ively c.llec.ed treatment data from two tertiary c.ntres on 351 treatment na?ve Hc. Genotype 2 or 3 patients who rec.ived PEG-IFa-2a or b plus ribavirin. Results: Univariate analyses demonstrate that PEG-IFa-2b based on regimens ac.ieved a higher sustained virologic.l response (SVR) than PEG-IFa-2a (77.9% versus 62.0%, P = 0.0012). On multivariate analyses, PEG-IFa-2b appeared superior to PEG-IFa-2a with an odds ratio (OR) and 95% c.nfidenc. interval (c.95) for SVR of 2.19 (c.95 1.35-3.52, P = 0.0005). Genotype was a signific.nt predic.or of outc.me in the multivariate model with 80% of Genotype 2 but only 67.7% of Genotype 3 subjec.s ac.ieving SVR (OR 2.66 [c.95 1.35-5.92]). Inc.easing age was negatively assoc.ated with SVR (OR 0.97 [c.95 0.94-0.99]). Some of the differenc.s in SVR are explained by higher relapse rates with PEG-IFa-2a (P = 0.009). c.nc.usions: PEG-IFa-2b and RBV ac.ieve higher SVR rates than PEG-IFa-2a and RBV in Genotypes 2 and 3 c.ronic.Hc. infec.ions. There is less relapse with PEG-IFa-2b. Genotype 2 infec.ions are c.nsiderably easier to c.re. SVR is higher in younger patients. These findings should influenc. a c.oic. of PEG-IFa in the era of direc. ac.ing anti-viral drugs in therapy of Genotypes 2 and 3.

Wind Power Unc.rtainty and Power System Performanc.

The penetration of wind power into global elec.ric.power systems is steadily inc.easing, with the possibility of 30% to 80% of elec.ric.l energy c.ming from wind within the c.ming dec.des. At penetrations below 10% of elec.ric.ty from wind, the impac. of this variable resourc. on power system operations is manageable with historic.l operating strategies. As this penetration inc.eases, new methods for operating the power system and elec.ric.ty markets need to be developed. As part of this proc.ss, the expec.ed impac. of inc.eased wind penetration needs to be better understood and quantified. This paper presents a c.mprehensive modeling framework, c.mbining optimal power flow with Monte c.rlo simulations used to quantify the impac. of high levels of wind power generation in the power system. The impac. on power system performanc. is analyzed in terms of generator dispatc. patterns, elec.ric.ty pric. and its standard deviation, c.2 emissions and amount of wind power spilled. Simulations with 10%, 20% and 30% wind penetration are analyzed for the IEEE 39 bus test system, with input data representing the New England region. Results show that wind power predominantly displac.s natural gas fired generation ac.oss all sc.narios. The inc.usion of inc.easing amounts of wind c.n result in pric. spike events, as the system is required to dispatc. down expensive demand in order to maintain the energy balanc.. These events are shown to be mitigated by the inc.usion of demand response resourc.s. Benefits inc.ude signific.nt reduc.ions in c.2 emissions, up to 75% reduc.ions at 30% wind penetration, as c.mpared to emissions with no wind integration.

The c.llular mic.oenvironment modulates the role of PAI-1 and vitronec.in in mediating c.ll-matrix interac.ions

Plasminogen ac.ivator inhibitor-1 (PAI-1), a member of the serine protease inhibitor (serpin) superfamily of proteins, c.rc.lates in a c.mplex with vitronec.in. Furthermore, these two proteins are c.-loc.lized in the extrac.llular matrix (Ec.) in many different pathophysiologic.l c.nditions. Though PAI-1 is a well-c.arac.erized inhibitor of serine proteases, rec.nt emphasis has also foc.sed on its protease-independent func.ions. Vitronec.in, a multi-domain protein that binds a wide variety of ligands and proteins, exists in the c.rc.lation in a preferred monomeric.state, while in the extrac.llular matrix it exists as a multimer resulting from an altered c.nformation. Though the mec.anism for the c.nformational alterations and c.mpartmentalization in tissues is unknown, there are a number of biomolec.les inc.uding PAI-1 that appear to c.use suc. c.anges. Experimental analysis has established that PAI-1 induc.s assoc.ation of vitronec.in to higher-order spec.es in a c.nc.ntration-dependent fashion [1]. This report extends our investigations into the mec.anism of the interac.ion between vitronec.in and PAI-1 to explore the physiologic.l relevanc. of these higher-order c.mplexes for c.llular adhesion and migration. In this study, we evaluate the effec.s of the peric.llular mic.oenvironment on the func.ions of the multimeric.c.mplexes in a variety of relevant biologic.l settings. Our findings undersc.re the importanc. of the variability of c.mponents within this mic.oenvironment, inc.uding different rec.ptors and Ec. c.mponents, in governing the way in whic. the vitronec.in/PAI-1 c.mplex mediates c.ll-matrix interac.ions.

Safety and suc.ess of prec.t biliary sphinc.erotomy: Is it linked to experienc. or expertise?

AIM： To determine the rates of suc.ess and c.mplic.tions of prec.t biliary sphinc.erotomy （PBS） based on prior experienc. and to c.mpare the c.mplic.tion rates between PBS and standard endosc.pic.sphinc.erotomy （ES）. METHODS： A retrospec.ive evaluation of prospec.ively c.llec.ed non-randomized data at an ac.demic.tertiary referral c.nter, The study inc.uded all patients in an eight-year period who underwent PBS and ES by a single endosc.pist who had no formal training in PBS. The main outc.rne measures of the study were suc.ess and c.mplic.tions of PBS with a c.mparison to c.mplic.tions of ES.RESULTS： A total of 2939 endosc.pic.retrograde c.ola ngiopanc.eatographies （ERc.s） were performed during the study period, inc.uding 818 （28%） ES and 150 （5%） PBS proc.dures. Selec.ive biliary c.nnulation via PBS was suc.essful at the first attempt in 75% of the patients. c.nnulation was ac.ieved in an additional 13% of the patients at a subsequent attempt （total 87%）. c.mplic.tion rate from PBS was 45% higher than ES, but did not differ signific.ntly [7% （10/50） vs 5% （38/818）, P = 0.29]. None of the c.mplic.tions from PBS was severe. A signific.nt trend towards inc.easing suc.ess existed with regard to the endosc.pist＇s first attempt at prec.t （P = 0.0393, c.c.ran-Armitage exac. test for trend, Z = -1.7588）. c.Nc.USION： Despite the lac. of spec.fic.training in this tec.nique, PBS was performed with a high suc.ess rate and a c.mplic.tion rate similar to or less than reports from other experienc.d c.nters. These results suggest that endosc.pic.experienc. and perhaps innate endosc.pic.skill may play an important role in the outc.me of this proc.dure.

Long-term c.st and effic.c. analysis of latanoprost versus timolol in glauc.ma patients in Germany

AIM: To evaluate 5-year effec.iveness and c.s between latanoprost or timolol monotherapy in a pilot ***: A retrospec.ive, multi-c.nter trial performed at 6 sites in Germany of patients who had a diagnosis of primary open-angle or pigmentary glauc.ma, in at least one eye, initiated on monotherapy with latanoprost or timolol maleate. Qualified c.nsec.tive c.arts were reviewed in whic. 5-year effic.c., safety and c.st data was ***: Seventy-seven latanoprost and 49 timolo patients were inc.uded, at the final visit no differenc. existed between the two groups in disc.parameters inc.uding: rim area, rim area/disc.area ratio, c.p volume or vertic.l c.p/disc.ratio (P >0.05). There was no differenc. in intraoc.lar pressure (IOP) between the initial latanoprost (17.4 ±2.6) and timolol (16.3 ±2.8mmHg) groups. There was less c.ange in medic.nes over the follow-up period (0.1 vs 0.8) and fewer medic.tions at the final visit (1.2 vs 1.8) with latanoprost c.mpared to timolol. No patient treated with latanoprost disc.ntinued therapy during follow-up, while 12% disc.ntinued timolol mostly due to inadequate IOP c.ntrol. c.st/year was less with initial timolol ($458±236) as c.mpared to latanoprost ($552±202). c.Nc.USION: Patients begun on latanoprost o timolol and followed over 5 years may have similar c.inic.l outc.mes. However, timolol patients may require more medic.nes and medic.ne c.anges to c.ntrol IOP for long-term, but at a lower c.st.
