Grading system modification and management of blunt aortic injury

Background The traditional approach to blunt aortic injury （BAI） has been emergent intervention. This study aimed to utilize a modified imaging grading system that may allow us to categorize these injuries as needing emergent, urgent, or non-operative management. Methods From January 2003 to december 2011, 28 patients with BAI were managed at our institution. Imaging and medical records were reviewed retrospectively. BAI was classified into 4 grades based on imaging studies. Grade la： intimal tear, Grade Ib： intramural hematoma; Grade Ⅱ： intimal injury with periaotic hematoma; Grade Ilia： aortic transection with pseudoaneurysm, Grade Illb： multiple aortic injuries; and Grade IV： free rupture. Progression and clinical outcomes of ABI were analyzed. Results Of the 28 patients, 22 were males and 6 were females with mean age of 38 （range, 7-69） years, Twenty-five （89.3%） had descending thoracic aortic injury, two （7.1%） had abdominal aortic injury and one （3.6%） presented with multiple aortic injuries. Three patients （10.7%） with Grade I, 1 （3.6%） Grade Ⅱ, 22 （78.6%） Grade Ⅲ, and 2 （7.1%） Grade IV injuries. Twenty-five patients underwent thoracic endovascular aortic repair and 3 were managed medically. Median time between injury and surgical intervention was （2±1） days. One （3.6%） patient developed paraplegia after thoracic endovascular aortic repair （TEVAR）. One Type 2 endoleak spontaneously sealed within 1 month, and another patient died from ruptured Type 1 endoleak 3 years later. Median follow-up time was 16 （range, 1-96） months. Perioperative 30-day mortality rate was 3.6%. Conclusions This study based on our modified BAI grading system indicated that Grade Ⅰ BAI can be managed conservatively. Grade Ⅱ injury requires close observation and repeated computerized tomography angiogram （CTA） within 48-72 hours. Impact of Insurance Status on the Prognosis of Acute Lower Limb Ischemia

The objective of this study was to investigate if the insurance status of patients impacted the treatment options and prognosis in acute limb ischemia (ALI). A retrospective chart review was performed at a single university tertiary care center using ICd-9 codes for the diagnosis and procedure for ALI from January 2000 to January 2011. A total of 96 patients were diagnosed with ALI, comprising of 66 males and 30 females with a mean age of 56 years (range was 19 - 80 years). Time to presentation and prognosis (rate and level of amputation) were analyzed using insurance status as the independent variable. Patients covered under commercial insurance were compared to patients with Medicare and Medicaid and to patients without any insurance coverage. Statistical analysis was performed using the proportion z test to evaluate differences among the groups investigated. A “p” value of ≤0.05 was considered significant. In this study, ALI occurred more commonly in African Americans (p = 0.0029) and in patients without insurance coverage regardless of race (p = 0.0034). Chronic obstructive pulmonary disease (COPd), hypertension (HTN), and acute renal failure (ARF) were significantly higher in the uninsured group, compared to the insured group (p = 0.0005, 0.0055, and 0.0034, respectively). The time to hospital admission was significantly longer in uninsured patients compared to the insured group (p = 0.0449). The rates of major amputation above the ankle were 46% in patients with commercial insurance, 62% in the government insurance (Medicare and Medicaid) group, and 51% in the uninsured group. There was no significant difference in major versus minor amputation in patients with commercial insurances. The Mesoproterozoic(1.11 Ga)Umkondo large igneous province(LIP)in southern Africa and Antarctica was emplaced ind is dominated by low-Ti tholeiitic doleritic-gabbroic *** is of particular interest because it is the least studied LIP in southern Africa with both sublithospheric and lithospheric mantle sources proposed and it coincides with the early assembly of Rodinia,so it has importance in understanding the nature of magmatism and tectonics in and around the Kalahari craton during the *** this study,we compiled a large database of existing(~750)and new(~100)major and trace element data for the Umkondo province,as well as 42 new Sr-Nd isotopic measurements,to provide constraints on its magma sources and geochemical *** element compositional variations in the low-Ti tholeiites are explained by low-pressure(1 kbar)three-phase fractional crystallisation(olivine,clinopyroxene and plagioclase)of a parent magma with~10 wt.%MgO in oxidising conditions(QFM+1).Inverse models show that the low-Ti tholeiitic magmas were derived as residual melts after the crystallization of 12%-33%olivine from primary komatiitic-basaltic magmas(up to~20 wt.%MgO)in equilibrium with mantle olivine(Fo90).Low Sm/Yb and TiO2/Yb-Nb/Yb indicate that the primary magmas were derived by 2%-20%shallow(40-50 km)partial melting of spinel *** Sm/Yb is restricted to dyke swarms and may imply limited magma production from deeper(up to~70 km)garnet lherzolite-like *** low-Ti tholeiites of the Umkondo province are enriched in large ion lithophile elements(Rb-Sr-Cs-K)and depleted in high-field strength elements(Zr-Hf-Nb-Ta),indicating the involvement of crustal material and/or the subcontinental lithospheric *** is supported by covariations in Th/Nb,Nb/Yb,Nb/La and Ce/Sm with generally negativeΔ***-Nd isotopes lend support to the notion that the Umkondo magmas were derived from depleted and/or enriched sublithospheric mantle sources and subs

Unequal distribution of Innovation Efforts for Neglected Tropical diseases: The Role of Funding Evaluation Criteria

Background: International research and innovation efforts for neglected tropical diseases have increased in recent decades due to disparities in overall health research funding in relation to global burden of disease. However, within the field of neglected tropical diseases some seem far more neglected than others. In this research the aim is to investigate the distribution of resources and efforts, as well as the mechanisms that underpin funding allocation for neglected tropical diseases. Methodology: A systematic literature review was conducted to establish a comprehensive overview of known indicators for innovation efforts related to a wide range of neglected tropical diseases. Articles were selected based on a subjective evaluation of their relevance, the presence of original data, and the breadth of their scope. This was followed by thirteen in-depth open-ended interviews with representatives of private, public and philanthropic funding organizations, concerning evaluation criteria for funding research proposals. Results: The findings reveal a large difference in the extent to which the individual diseases are neglected with notable differences between absolute and relative efforts. Criteria used in the evaluation of research proposals relate to potential impact, the probability of success and strategic fit. Private organizations prioritize strategic fit and economic impact;philanthropic organizations prioritize short-term societal impact;and public generally prioritize the probability of success by accounting for follow-up funding and involvement of industry. Funding decisions of different types of organizations are highly interrelated. Conclusions: This study shows that the evaluation of funding proposals introduces and retains unequal funding distribution, reinforcing the relative neglect of diseases. Common psychiatric disorders(CPds)and depression contribute significantly to the global epidemic of type 2 diabetes(T2d).We postulated a possible pathophysiological mechanism that through Bridge-Symptoms present in depression and CPds,promotes the establishment of emotional eating,activation of the reward system,onset of overweight and obesity and,ultimately the increased risk of developing *** plausibility of the proposed pathophysiological mechanism is supported by the mechanism of action of drugs such as naltrexonebupropion currently approved for the treatment of both obesity/overweight with T2d and as separate active pharmaceutical ingredients in drug addiction,but also from initial evidence that is emerging regarding glucagon-like peptide 1 receptor agonists that appear to be effective in the treatment of drug *** hope that our hypothesis may be useful in interpreting the higher prevalence of CPds and depression in patients with T2d compared with the general population and may help refine the integrated psychiatric-diabetic therapy approach to improve the treatment and or remission of T2d.

Objective To compare the efficacy of using“distal acupoints only”vs.“local acupoints mainly combined with distal acupoints”in cases of acute low back pain(ALBP).Methods A total of 102 eligible patients aged 18–65 years with ALBP lasting less than 6 weeks will be randomized in a 1:1 ratio to the distal acupoints only group(dPOG)and the local acupoints mainly combined with the distal acupoints group(LPMG).during a 4-week treatment period,patients in the dPOG will receive acupuncture at distal acupoints only(Houxi[SI 3]and Yaotongxue[EX-UE 7])twice a week,while those in the LPMG group will receive acupuncture at local acupoints(mainly Shenshu[BL 23]and dachangshu[BL 25])combined with distal points(Weizhong[BL 40]).The patients in both groups will be evaluated at every session of treatment,and the follow-up will be performed for 3 *** primary outcome measure will be the change in ALBP intensity,assessed using visual analogue scale scores before and after *** secondary outcome measure will be the evaluation of functional disability using the Oswestry disability *** This study compares the dPOG and LPMG to explore the feasibility of the dPOG in the treatment of ALBP.

Structural elucidation (automatic determination of the structure of amolecule from its spectra) is frequently hampered by combinatorial explosion when trying to assemblethe identified substructures. We devised a new method which can avoid this pitfall by a systematicexamination of allowed ^(13)C chemical shifts ranges for all substructures chemically possible andcombined with a progressive pruning thanks to neighbouring relationships appearing from 2d NMR. Thismethod is explained by a detailed example.
