Assessing Thai Hospitals’Evacuation Preparedness Using the Flexible Surge Capacity Concept and Its Collaborative Tool

According to the concept of“flexible surge capacity,”hospitals may need to be evacuated on two occasions:(1)when they are exposed to danger,such as in war;and(2)when they are contaminated,such as during the Covid-19 *** the former,the entire hospital must be evacuated,while in the latter,the hospital becomes a pandemic center necessitating the transfer of its non-contaminated staff,patients,and routine activities to other *** occasions involve several degrees of evacuation—partial or total—yet all require deliberate surge planning and collaboration with diverse *** study aimed to investigate the extent of hospital evacuation preparedness in Thailand,using the main elements of the flexible surge capacity concept.A mixed method cross-sectional study was conducted using a hospital evacuation questionnaire from a previously published multinational hospital evacuation *** tool contained questions regarding evacuation preparedness encompassing surge capacity and collaborative elements and an open-ended inquiry to grasp potential *** 143 secondary care,tertiary care,and university hospitals received the questionnaire;43 hospitals provided *** findings indicate glitches in evacuation protocols,particularly triage systems,the inadequacies of surge planning and multiagency collaboration,and knowledge limitations in community *** conclusion,the applications of the essential components of flexible surge capacity allow the assessment of hospital preparedness and facilitate the evaluation of guidelines and instructions through scenario-based training exercises.

Protein arginine methyltransferase-6 regulates heterogeneous nuclear ribonucleoprotein-F expression and is a potential target for the treatment of neuropathic pain

Protein arginine methyltransferase-6 participates in a range of biological functions,particularly RnA processing,transcription,chromatin remodeling,and endosomal ***,it remains unclear whether protein arginine methyl transferase-6 modifies neuropathic pain and,if so,what the mechanisms of this *** this study,protein arginine methyltransferase-6 expression levels and its effect on neuropathic pain were investigated in the spared nerve injury model,chronic constriction injury model and bone cancer pain model,using immunohistochemistry,western blotting,immunoprecipitation,and label-free proteomic *** results showed that protein arginine methyltransferase-6 mostly co-localized withβ-tubulinⅢin the dorsal root ganglion,and that its expression decreased following spared nerve injury,chronic constriction injury and bone cancer *** addition,PRMT6 knockout(Prmt6~(-/-))mice exhibited pain ***,the development of spared nerve injury-induced hypersensitivity to mechanical pain was attenuated by blocking the decrease in protein arginine methyltransferase-6 ***,when protein arginine methyltransferase-6 expression was downregulated in the dorsal root ganglion in mice without spared nerve injury,increased levels of phosphorylated extracellular signal-regulated kinases were observed in the ipsilateral dorsal horn,and the response to mechanical stimuli was ***,protein arginine methyltransferase-6 appeared to contribute to spared nerve injury-induced neuropathic pain by regulating the expression of heterogeneous nuclear ***,protein arginine methyltransfe rase-6-mediated modulation of hete rogeneous nuclear ribonucleoprotein-F expression required amino atids 319 to 388,but not classical H3R2 *** findings indicated that protein arginine methyltransferase-6 is a potential therapeutic target fo r the treatment of peripheral neuro pathic pain.

BACKGROUnD Cervical cancer is a formidable global health issue,particularly affecting women in lower-middle-income countries with little or no access to preventative vaccines,screening programs,and treatment *** case report presents a unique case of a large cervical cancer achieving complete response(CR)with concurrent chemoradiotherapy(CCRT),highlighting the effectiveness of this treatment approach even in advanced stages and underscoring the importance of adaptive radiotherapy(RT)in optimizing patient *** SUMMARY We present the case of a 53-year-old woman who presented with four years of abnormal vaginal bleeding and was found to have p16-positive,moderately differentiated cervical squamous cell *** tumor measured 14 cm×12 cm×8 cm,the largest size reported in the literature to achieve CR with *** this monumental feat,the patient remained disease-free and is currently on follow-up for 2 years;however,she continued to suffer from substantial morbidity caused by a vesicovaginal fistula and hydronephrosis,underscoring the continuing impact of cervical cancer on quality of *** In this case report,we highlight the effectiveness of CCRT in achieving CR,even in cases of bulky cervical cancer,with adaptive RT offering a customized strategy to improve patient *** also emphasize the necessity for multidisciplinary team discussions and highlight the need for strategies to mitigate treatment-related toxicities and long-term complications.

Energy consumption simulations of rural residential buildings considering differences in energy use behavior among family members

The“average occupant”methodology is widely used in energy consumption simulations of residential buildings;however,it fails to consider the differences in energy use behavior among family *** on a field survey on the Central Shaanxi Plain,to identify the energy use behavior patterns of typical families,a stochastic energy use behavior model considering differences in energy use behavior among family members was proposed,to improve the accuracy of energy consumption simulations of residential *** results indicated that the surveyed rural families could be classified into the following four types depending on specific energy use behavior patterns:families of one elderly couple,families of one middle-aged couple,families of one elderly couple and one child,and families of one couple and one ***,on typical summer days,the results of daily building energy consumption simulation obtained by the“average occupant”methodology were 25.39%and 28%lower than the simulation results obtained by the model proposed in this study for families of one elderly couple and families of one middle-aged couple,and 13.05%and 23.05%higher for families of one elderly couple and one child,and families of one couple and one *** typical winter days,for the four types of families,the results of daily building energy consumption simulation obtained by the“average occupant”methodology were 21.69%,10.84%,1.21%,and 8.39%lower than the simulation results obtained by the model proposed in this study,respectively.

Let Y be a Gromov-Hausdorff limit of complete Riemannian nmanifolds with Ricci curvature bounded from below. A point in Y is called k-regular, if its tangent is unique and is isometric to a k-dimensional Euclidean space. By Cheeger-Colding and Colding-naber, there is k 〉 0 such that the set of all k-regular point ：Rk has a full renormalized measure. An open problem is if Rl = 0 for all l 〈 k？ The main result in this paper asserts that if R1 ≠ 0, then Y is a one-dimensional topological manifold. Our result improves Honda＇s result that under the assumption that 1 ≤ dimH（Y）〈 2.

Improved encoding structure and decoding algorithms for spinal codes

To improve the error correction performance, an innovative encoding structure with tail-biting for spinal codes is designed. Furthermore, an adaptive forward stack decoding(A-FSD) algorithm with lower complexity for spinal codes is proposed. In the A-FSD algorithm, a flexible threshold parameter is set by a variable channel state to narrow the scale of nodes accessed. On this basis, a new decoding method of AFSD with early termination(AFSD-ET) is further proposed. The AFSD-ET decoder not only has the ability of dynamically modifying the number of stored nodes, but also adopts the early termination criterion to curtail complexity. The complexity and related parameters are verified through a series of simulations. The simulation results show that the proposed spinal codes with tail-biting and the AFSD-ET decoding algorithms can reduce the complexity and improve the decoding rate without sacrificing correct decoding performance.

near-perfect replication on amorphous alloys through active force modulation based on machine learning/neural network parameter prediction

As a microforming technique, micro/nano-structural replication possesses advantages of high precision and efficiency. With the remarkable superplasticity in the supercooled liquid region, amorphous alloys or metallic glasses(MGs) are regarded as ideal materials for miniature fabrication. However, due to the intrinsic metastable nature of supercooled liquids, the design of imprinting processes for MGs poses a challenge. In the past, process parameters have largely relied on trial-and-error *** this work, a low-frequency active force modulation system is employed to apply a stable, precise, and real-time feedback stress field for imprinting of MG samples. Low-frequency vibrations can facilitate the filling of microstructures on the template surface by reducing the effective viscosity of the supercooled liquid. With the dataset composed of orthogonal experiments, a machine learning strategy based on back-propagation(BP) neural networks was utilized to construct a 3D parameter space for temperature, stress, and time, and to predict the corresponding filling ratio. Furthermore, the optimal combination of imprinting process parameters was identified, and its filling ratio was experimentally validated to reach as high as 0.94. The near-perfect replication of microstructures confirms the superiority of the active force modulation system and the data-driven strategy of machine learning-assisted parameter design. At the same time, this one-step microforming process provides a new approach to addressing the accuracy-cost trade-off dilemma in precision manufacturing.

Some new Trends of Deep Learning Research

Deep learning has been attracting increasing attention in the recent decade throughout science and engineering due to its wide range of successful *** real problems, however, most implementation stages for applying deep learning still require inevitable manual interventions, which naturally conducts difficulty in its availability to general users with less expertise and also deviates from the intelligence of humans. It is thus a challenging while critical issue to enhance the level of automation across all elements of the entire deep learning framework, like input amelioration, model designing and learning, and output adjustment. This paper tries to list several representative issues of this research topic, and briefly describe their recent research progress and some related works proposed along this research line. Some specific challenging problems have also been presented.

Many social events spread fast through the Internet and arouse wide community discussions. Those on-line public opinions emerge into diverse topics along the time. Moreover, the strength of the topics is fluctuating. How to catch both primary topics and trend of topics over the shifting on-line discussions are not only of theoretical importance for scientific research, but also of practical importance for societal management especially in current China. To try the cutting-edge text analytic technologies to deal with unstructured on-line public opinions and provide support for social problem-solving in the big data era is worth an endeavour. This paper applies dynamic topic model （DTM） to explore the changing topics of new posts collected from Tianya Zatan Board of Tianya Club, the most influential Chinese BBS in China's Mainland. By analysis of the hot and cold terms trends, we catch the topics shift of main on-line concerns with illustrations of topics of school bus and environment in December of 2011. An algorithm is proposed to compute the strength fluctuation of each topic. With visualized analysis of the respective main topics in several months of 2012, some patterns of the topics fluctuation on the board are summarized.
