BACKGROUND Needle-knife precut papillotomy(NKP)is typically performed ***,it remains unclear whether pancreatic stent(PS)placement can improve the outcomes of *** To explore whether PS placement improves the success rate of NKP in patients with difficult biliary *** This single-center retrospective study included 190 patients who underwent NKP between January 2017 and December 2021 after failed conventional biliary *** cases with incidental pancreatic duct cannulation during conventional biliary cannulation,the decision for pre-NKP PS placement was made at the endoscopist's *** primary outcome was the difference in the NKP success rate between patients with and without PS placement;the secondary outcome was the adverse event *** Among the 190 participants,82 received pre-NKP PS(PS-NKP group)whereas 108 did not[freehand or freehand NKP(FH-NKP)group].Post-NKP selective biliary cannulation was successful in 167(87.9%)patients,and the PS-NKP had a significantly higher success rate than the FH-NKP group(93.9%vs 83.3%,P=0.027).The overall adverse event rates were 7.3%and 11.1%in the PS-NKP and FH-NKP groups,respectively(P=0.493).A periampullary diverticulum(PAD)and significant intraoperative bleeding during NKP were independently associated with NKP failure;however,a pre-NKP PS was the only predictor of NKP *** the 44 participants with PADs,the PS-NKP group had a non-significantly higher NKP success rate than the FH-NKP group(87.5%and 65%,respectively;P=0.076).CONCLUSION PS significantly improved the success rate of NKP in patients with difficult biliary cannulation.

Traditional air quality data have a spatial resolution of 1 km or above, making it challenging to resolve detailed air pollution exposure in complex urban areas. Combining urban morphology, dynamic traffic emission, regional and local meteorology, physicoche.ical transformations in air quality models using big data fusion technology, an ultra-fine resolution modeling system was developed to provide air quality data down to street level. Based on one-year ultra-fine resolution data, this study investigated the effects of pollution heterogeneity on the individual and population exposure to particulate matter(PM_(2.5)and PM_(10)),nitrogen dioxide(NO_(2)), and ozone(O_(3)) in Hong Kong, one of the most densely populated and urbanized cities. Sharp fine-scale variabilities in air pollution were revealed within individual city blocks. Using traditional 1 km average to represent individual exposure resulted in a positively skewed deviation of up to 200% for high-end exposure individuals. Citizens were disproportionally affected by air pollution, with annual pollutant concentrations varied by factors of 2 to 5 among 452 District Council Constituency Areas(DCCAs) in Hong Kong, indicating great environmental inequities among the population. Unfavorable city planning resulted in a positive spatial coincidence between pollution and population, which increased public exposure to air pollutants by as large as 46% among districts in Hong Kong. Our results highlight the importance of ultra-fine pollutant data in quantifying the heterogeneity in pollution exposure in the dense urban area and the critical role of smart urban planning in reducing exposure inequities.

This paper presents a corrosion assessment of copper spent nuclear fuel disposal canisters in crystalline rock,using hydrogeological modeling.A simplified approach is considered,to avoid complex and time-consuming computer *** simplified case is presented as a base case,with changes in the hydrogeological parameters presented as variant *** results show that in Taiwan’s base case,decreasing the hydraulic conductivity of the rock or decreasing the hydraulic conductivity of dikes results in a shorter transport path for sulfide and an increase in corrosion ***,the estimated canister failure time is still over one million years in the variant cases.

Drying paddy with low-pressure superheated steam(LPSS)can effectively increase theγ-aminobutyric acid content in *** study aimed to investigate the characteristics and mathematical models(MMs)of thin-layer drying of paddy with *** experimentally obtained data wereﬁtted by nonlinear regression with 5 MMs commonly used for thin-layer drying to calculate the goodness of fit of the ***,the thin-layer drying of paddy with LPSS was modeled with two machine learning methods as a Bayesian regularization back propagation(BRBP)neural network and a support vector machine(SVM).The results showed that paddy drying with LPSS is a reduced-rate drying *** drying temperature and operating pressure have a significant impact on the drying *** the same pressure,increasing the drying temperature can accelerate the drying *** the same temperature,increasing the operating pressure can accelerate the drying *** comparison of the model evaluation indexes showed that 5 common empirical MMs(Hederson and Pabis,Page,Midilli,Logarithmic,and Lewis)for thin-layer drying can achieve excellent fitting effects for a single experimental ***,the regression fitting of the indexes by calculating the coefficient(s)of each model showed that the empirical MMs produce poor fitting *** BRBP neural network-based model was slightly better than the SVM-based model,and both were significantly better than the empirical MM(the Henderson and Pabis model),as evidenced by a comparison of the training root mean square error(RMSE),testing RMSE,training mean absolute error(MAE),testing MAE,training R2,and testing R2 of the Henderson and Pabis model,the BRBP neural network model,and the SVM-based *** results indicate that the MMs established by the two machine learning methods can better predict the moisture content changes in the paddy samples dried by LPSS.

From 1990,over 17 years field experiment was carried out in paddy field cultivated from infertile upland to evaluate the response of rice productivity,soil organic carbon（SOC）,and total N to long-term NPK fertilization or NPK combined with organic *** field trials included NPK（N,P,K fertilizer）,NPKRS（NPK combined with rice straw）,NPK2RS（NPK combined with double amount of rice straw）,NPKPM（NPK combined with pig manure） and NPKGM（NPK combined with green manure） and the cropping system was rice-rice（Oryza sativa L.） *** rice yield,straw biomass,and harvesting index increased steadily with cultivation time in all *** annual rice yield from 1991 to 2006 was ranged from 7 795 to 8 572 kg ha-1 among *** yields in treatments with organic amendments were usually higher than that in treatment with *** of SOC and total N also increased gradually in the cultivation years and reache. the level of 7.82 to 9.45 and 0.85 to 1.03 g kg-1,respectively,in *** fertilities in treatments with che.ical fertilization combined with organic amendments were relatively appropriate than those in treatment with *** was obvious discrepancy between cumulative characters of rice yield and soil organic fertility in newly formed paddy *** with relatively high rate of crop productivity improvement,cumulative rates of SOC and total N were much lower in our *** and total N contents were still less than half of those in local highly productive paddy soils after 17 years cultivation in subtropical *** work helps to better understand the development of infertile paddy soils and to estimate the potential of yield improvement in this region.

BACKGROUND Accumulating clinical evidence has shown that diabetes mellitus(DM)is a serious risk factor for cardiovascular disorders and an important factor for adverse cardiovascular *** To explore the value of the combined determination of the neutrophil-lymphocyte ratio(NLR)and red blood cell distribution width(RDW)in the early diagnosis and prognosis evaluation of DM complicated with heart failure(HF).METHODS We retrospectively analyzed clinical data on 65 patients with type 2 DM(T2DM)complicated with HF(research group,Res)and 60 concurrent patients with uncomplicated T2DM(control group,Con)diagnosed at Zhejiang Provincial People’s Hospital between January 2019 and December *** NLR and RDW values were determined and comparatively analyzed,and their levels in T2DM+HF patients with different cardiac function grades were *** receiver operating characteristic(ROC)curves were plotted to determine the NLR and RDW values(alone and in combination)for the early diagnosis of *** correlation between NLR and RDW with the presence or absence of cardiac events was also *** Higher NLR and RDW levels were identified in the Res vs the Con groups(P<0.05).The NLR and RDW increased gradually and synchronously with the deterioration of cardiac function in the Res group,with marked differences in their levels among patients with grade II,III,and IV HF(P<0.05).ROC curve analysis revealed that NLR combined with RDW detection had an area under the curve of 0.915,a sensitivity of 76.9%,and a specificity of 100%for the early diagnosis of ***,HF patients with cardiac events showed higher NLR and RDW values compared with HF patients without cardiac *** NLR and RDW were useful laboratory indicators for the early diagnosis of DM complicated with HF,and their joint detection was beneficial for improving diagnostic ***,NLR and RDW values were directly proportional to patient outcomes.
