Discrepancies between clinical and autopsy findings in patients who had an acute stroke

Objectives According to international observations,the incidence of clinical autopsies is declining worldwide,plummeting below 5%in the USA and many European *** is an unfavourable trend as,in 7%-12%of cases,recent clinicopathological studies found discrepancies that might have changed the therapy or the outcome if known *** previous large-scale observations have examined varied patient populations,we aimed to focus on the differences between the clinical and pathological diagnostic findings in only patients who had a *** and methods We assessed the postmortem non-neuropathological and neuropathological findings of 534 consecutive patients who had a stroke who passed *** neoplasms,pneumonias,thromboembolisms and haemorrhagic transformations revealed only by autopsy were considered severe abnormalities;in addition,benign abnormalities important from an educational or scientific point of view were also *** In 26 of the 534 cases(4.9%),the presence of systemic neoplasms had already been confirmed in the clinical stage;however,8(1.5%)malignant tumours were only detected during ***,80(15%)thromboembolic events,73(13.6%)pneumonias and 66(18%)haemorrhagic transformations were only diagnosed at *** hospital stay(from admission to death)resulted in fewer discrepancies between clinical and autopsy diagnosis of thromboembolic events and pneumonias(pke is reliably diagnosed with imaging techniques,postmortem autopsies are also important in patients who had a stroke as autopsies may reveal clinically silent diseases(eg,tumour),and contribute to knowing the actual incidence of stroke-related thromboembolic and pneumonia complications.

Dietary supplementation of some antioxidants against hypoxia

The present study aims to clarify the protective effect of supplementation with some antioxidants,such as idebenone(200 mg/kg,ip),melatonin(10 mg/kg,ip) and arginine(200 mg/kg,ip) and their combination,on liver function(***,albumin,alanine aminotransferase,aspartate aminotransferase and alkaline phosphatase),energetic parameters(adenosine triphosphate,adenosine diphosphate,adenosine monophosphate,inorganic phosphate,total adenylate,adenylate energy charge and potential phosphate).The effect on glycolytic and glycogenolytic enzymes(glucose,glycogen,glycogen phosphorylase,pyruvate kinase and phosphofructokinase against hypoxia) was also *** drugs were administered 24 and 1 h prior sodium nitrite *** biochemical parameters were estimated 1 h after sodium nitrite *** of sodium nitrite(75 mg/kg,sc) produced a significant disturbance in all biochemical parameters of liver function,energetic parameters and glycolytic and glycogenolytic *** damage was confirmed by histopathological examination of the liver as compared to *** marked changes in hepatic cells induced by sodium nitrite were completely abolished by pretreatment with the drug combination,suggesting potential protection against sodium nitrite-induced *** could be concluded that a combination of both idebenone and melatonin or idebenone and arginine provides potential protection against sodium nitrite-induced hypoxia by improving biochemical parameters and preserving liver histology.

Previous studies have demonstrated that advanced Alzheimer＇s disease（AD）patients have deficiency of eye ***,there have been no reports on eye movement in the early stages of *** aim of this study was to evaluate pursuit ocular movements（POM）provided by a vision-based non-intrusive eye tracker in patients with early *** values were significantly lower in AD patients than in normal controls（P 〈 0.01）.In AD patients,POM values were not closely correlated with the Mini-Mental State Examination scores（P = 0.3）.There was no significant difference in POM values among patients treated with or without anticholinesterase *** used a vision-based method,for non-intrusive eye tracking,which can be proposed as a possible tool for supporting the diagnosis of early AD.

Antioxidant potential,tannin and polyphenol contents of seed and pericarp of three Coffea species

Objective:To investigate the antioxidant activity,total phenolic and total tannin content of the pericarp and the seed of Coffea benghalensis(***) and Coffea liberica compared to Coffea arabica(***).Methods:The antioxidant potential,total tannin and polyphenol contents of the immature and mature seed and pericarp of *** and Coffea liberica were quantified and compared to *** chemiluminescence(ECL).2,2-diphenyl-1-picrylhydruzyl(DPPH).oxygen radical absorbance capacity,Folin-Ciocaltcau method and total tannin content assays were ***:Trolox equivalent(TE/g plant material) values obtained by ECL and DPPH methods showed loose correlation(r^2=0.587)while those measured by oxygen radical absorbance capacity assay were higher without correlation in each plant.A closer correlation was detected between the ECL method and the percentage antioxidant activity of the DPPH technique(r^2=0.610 7) in each species,however the immature pericarp of *** showed much higher DPPH scavenging potential than was seen in the ECL *** immature pericarp of *** expressed the highest tannin and polyphenol content,and a high polyphenol level was also detected in the immature seed of *** immature pericarp of Bengal and Liberian coffees showed the largest amount of phenolic ***:The obtained data highlight the potential role of *** as a new source of natural antioxidants and polyphenols compared to ***.

Parkinson＇s disease has a negative impact on health-related quality of life in Parkinson＇s disease patients. Depression, cognitive impairment, coping strategies, dyskinesia, gait disorders and complications of dopaminergic drugs are the variables that most affect health-related quality of life. The ecological model of human development focuses attention on both individual and social environmental factors as targets for health interventions. From this perspective, the aim of this cross-sectional survey was to evaluate the influence of gender, family size and perceived autonomy on health-related quality of life in Parkinson＇s disease patients in nOrtheastern Sicily, Italy. Ninety Parkinson＇s disease patients, attending the Movement Disorders Clinic at IRCCS Centro Neurolesi ＂Bonino-Pulejo＂（Messina）, were consecutively enrolled. The Unified Parkinson Disease Rating Scale motor subscale （UPDRS-Ⅲ） scores, the Parkinson Disease Questionnaire-39 Item scores （as a disease-specific measure of health-related quality of life）, scores on the Short Form （36） Health Survey Questionnaire （as a generic measure）, and answers to a brief checklist were recorded. A total of 85 Parkinson＇s disease patients （49% males and 51% females; mean age 70.8 ± 8.6 years mean UPDRS-Ⅲ 24.15 ± 6.55; mean disease duration 5.52 ± 4.65 years） completed the booklet of questionnaires. In the multivariate regression analysis, we included clinical and social variables as independent predictors of health-related quality of life. Our results suggest a potential compounding effect of ecological intrapersonal and interpersonal levels on health-related quality of life outcomes. Gender, self-evaluated autonomy and family size significantly impacted health-related quality of life. If quality of life is used as an indicator of treatment outcomes, an ecological perspective of the case history will be important to disclose relevant prognostic information and trigger personalized health care interventions.

Bridging the Gap between Evidence and Practice: A Systematic Review—When Is the Best Time to Clamp the Infant’s Umbilical Cord in Term Low-Risk Women?

Historically, in midwifery and obstetric care, the umbilical cord was not usually clamped until all pulsation of the cord had ceased. This is now referred to as delayed cord clamping or DCC (not clamping umbilical cord before two minutes of life). During the last one hundred years with changes in the ways women give birth, especially the shift toward hospital based birth in the Western world, it became commonplace for the cord to be clamped within 20 - 30 seconds of birth (immediate cord clamping or ICC). Authors have postulated various reasons for this shift in timing of umbilical cord clamping to include a possible decreased risk of postpartum hemorrhage, the need for resuscitation of the newborn and sampling of cord blood for stem cells and cord blood analysis. Recent evidence suggests that newborns (particularly in low-resourced settings) would benefit greatly from a policy of DCC with decreased risks of childhood anemia, subsequent sequelae and the need for supplementation. This systematic review examined recent literature from the last eight years using commonly used academic search engines with associated keywords. The results were carefully collated into a table of findings and outcomes.

Interaction of the major inflammatory bowel disease susceptibility alleles in Crohn’s disease patients

AIM:To investigate the interaction of interleukin-23 receptor(IL23R)(rs1004819 and rs2201841),autophagy-related 16-like 1(ATG16L1)(rs2241880), caspase recruitment domain-containing protein 15 (CARD15)genes,and IBD5 locus in Crohn's disease(CD) patients. METHODS:A total of 315 unrelated subjects with CD and 314 healthy controls were *** and specific genotype combinations of a total of eight variants were *** variants of IBD5locus(IGR2198a_1 rs11739135 and IGR2096a_1 rs12521868),CARD15(R702W rs2066845 and L1007fs rs2066847),ATG16L1(rs2241880)and IL23R (rs1004819,rs2201841)genes were genotyped by PCR-RFLP,the G908R(rs2066844)in CARD15 was determined by direct sequencing. RESULTS:The association of ATG16L1 T300A with CD was confirmed[P=0.004,odds ratio(OR)=1.69, 95%CI:1.19-2.41],and both IL23R variants were found to represent significant risk for the disease(P= 0.008,OR=2.05,95%CI:1.20-3.50 for rs1004819 AA;Pk *** almost all cases,the combined risk of susceptibility pairs was higher in patients carrying two different risk-associated gene variants together than individuals with just one *** highest OR was found for IL23R rs2201841 homozygous genotype with combination of positive CARD15 status(P<0.001,OR=9.15,95% CI:2.05-40.74). CONCLUSION:The present study suggests a cumulative effect of individual IBD susceptibility loci.

20世纪初,德国考察队从克孜尔石窟揭取了大量壁画,其中的大部分现藏于德国柏林亚洲艺术博物馆。然而,考察队对这些壁画所出洞窟的记录存在一定错漏。近年,新疆龟兹研究院与德国柏林亚洲艺术博物馆合作,对该馆所藏克孜尔石窟壁画进行了深入调查,并通过实地考察与逐一核对,确认了大部分德藏克孜尔壁画所属的洞窟及原来所在的位置,为相关研究提供了便利。
