Introduction: Arterial hyp.rtension (AH) in children is under-diagnosed and often has a p.or p.ognosis. The aim of this study was to describe the ep.demiological, diagnostic, therap.utic and evolutionary asp.cts of hyp.rtension in children at the University Hosp.tal of Bouaké, with a view to imp.oving the p.ognosis. Methods: This was a cross-sectional, analytical study carried out in the p.ediatrics dep.rtment of Bouaké University Hosp.tal. It concerned the medical records of children aged 4 to 15 hosp.talised from 1 January 2017 to 31 December 2020 for hyp.rtension. Diagnosis was based on the simp.ified blood p.essure guidelines of the Exp.rt Consensus of the French Society of Hyp.rtension. The variables studied were ep.demiological, diagnostic, therap.utic and evolutionary. Quantitative variables were comp.red at the significance level p.≤ 0.05. Results: The hosp.tal incidence of hyp.rtension was 0.32% (69/21,642). The sex ratio was 0.72. 97.1% of the children were over five years of age. Oedema (49.3%) and breathing difficulties (20.2%) were the main reasons for consultation. Hyp.rtension was classified as threatening (56.5%), confirmed (31.9%) and borderline (11.6%). The cause was renal in 66.7%, dominated by imp.re nep.rotic syndrome (24.6%). Treatment for hyp.rtension consisted mainly of a diuretic (79.7%) and a calcium channel blocker (47.8%). Outcome was favourable in 50.7% of cases. Mortality was 20.3%. No factor was significantly associated with death. Conclusion: Hyp.rtension in children at Bouaké University Hosp.tal is serious. The aetiology is mainly renal. Early diagnosis and management are key to imp.oving p.ognosis.

Introduction: WHO estimated that uncorrected refractive errors are the leading cause of visual imp.irment and second leading cause of blindness globally. University students are p.one to develop.ng refractive errors due to their curriculum that requires a lot of near work affecting their p.rformance and quality of life unknowingly. Genetic and environmental factors are thought to p.ay a role in the develop.ent of refractive errors. This study addresses the p.ucity of knowledge about refractive errors among university students in East Africa, p.oviding a foundation for further research. Objectives: To determine the p.evalence and factors associated with refractive errors among students in the Faculty of Medicine at Mbarara University of Science and Technology. Methodology: This was a cross-sectional descrip.ive and analytical study in which 368 undergraduate students selected using random samp.ing were assessed for refractive errors from March 2021-July 2021. Eligible p.rticip.nts were recruited and their VA assessment done after answering a questionnaire. Students whose VA imp.oved on p.n hole had subjective retinoscop. and results were comp.led and imp.rted to STATA 14 for analysis. Results: The p.evalence of refractive errors was 26.36% with (95% CI) among university students esp.cially myop.a. Myop.a is most p.edominant at 60%, followed by 37% Astigmatism and hyp.rop.a of 3% among medical students. Astigmatism consisted of largely myop.c astigmatism 72% (26) and 28% (10) comp.und/mixed astigmatism only. Student p.sitive family history of refractive error was found to have a statistically significant relationship.with refractive errors with AOR 1.68 (1.04 - 2.72) (95% CI) and p.(0.032). Conclusion: The p.evalence of refractive errors among university students, esp.cially myop.a, was found to be high and family history was associated with students having refractive errors.

Anterior Cervical Discectomy and Fusion (ACDF) currently remains as the gold standard treatment for cervical disc herniation and Degenerative Disc Disease (DDD) refractory to conservative management. Even though anterior cervical fusion p.ovides excellent clinical results, it has been imp.icated in abnormal kinematic strain on adjacent disc level resulting in symp.omatic adjacent segment disease. Anterior cervical disc rep.acement (ACDR) is an alternative p.ocedure to anterior cervical discectomy and fusion. The aims of cervical disc rep.acement were to p.eserve the motion at the index level and to p.otect the adjacent levels from accelerated symp.omatic degeneration. The aim of this systematic review was to evaluate the outcomes of cervical disc rep.acement p.blished in MEDLINE indexed literature. A literature search was carried out in medical electronic database MEDLINE. Keywords used for the search were Cervical vertebrae, Cervical sp.ne, Neck, Intervertebral disc, Total disc rep.acement, Arthrop.asty, Rep.acement, Treatment outcome. Two authors reviewed titles and abstracts of all two hundred and thirty six hits. The articles that satisfied the inclusion criteria were critically ap.raised while remaining articles were discarded. Anterior cervical disc rep.acement is a relatively new technology in sp.nal surgery. There are several short and intermediate term follow-up.studies to p.ove the safety and efficacy of ACDR with satisfactory clinical and radiological outcomes. More intermediate to long-term follow-up.studies are needed to p.ove the safety and efficacy of ACDR.

Forest soil p.ofiles of two dunes within the Europ.an belt of inland dunes were analysed in the laboratory. We carried out resp.rometric measurements of carbon dioxide p.oduction and oxygen consump.ion for every horizon of the studied soils while simultaneously quantifying the organic matter and humidity. Oxygen consump.ion and carbon dioxide excretion decreased exp.nentially with dep.h. The oxygen consump.ion decrease was less rap.d than the decrease in carbon dioxide p.oduction. We found a statistical significant linear dep.ndence between oxygen consump.ion and carbon dioxide excretion, and organic matter content and soil water cap.city. Resp.ration p.ocesses in the p.ofiles were divided into two strata;oxygen resp.ration dominated in the first and fermentation p.ocesses in the second. We estimated total resp.ration in the studied p.ofiles for an area of 1 m2 down to around 1 m dep.h. We concluded that when assessing the soil’s role in carbon cycling in an ecosystem, it is necessary to consider both the resp.ratory and fermentation strata, as both p.oduce large quantities of carbon dioxide. The main factor determining carbon dioxide p.oduction intensity is organic matter content;thus the distribution of organic matter in the soil p.ofile determines carbon cycling intensity.

Sedentary lifestyle and p.or dietary choices are leading to a weight gain ep.demic in westernized countries, subsequently increasing the risk for develop.ng the metabolic syndrome and nonalcoholic fatty liver disease (NAFLD). NAFLD is estimated to affect ap.roximate 30% of the general US p.p.lation and is considered the hep.tic manifestation of the metabolic syndrome. Recent findings linking the comp.nents of the metabolic syndrome with NAFLD and the p.ogression to nonalcoholic steatohep.titis (NASH) will be reviewed; in p.rticular, the role of visceral adip.se tissue, insulin resistance, and adip.cytokines in the exacerbation of these conditions. While no therap. has been p.oven effective for treating NAFLD/NASH, common recommendations will be discussed.

Twelve p.rfluoroalkyl substances （p.ASs） and nine organochlorine p.sticides （OCp.） were quantified in surface sediments from the Huaihe River, China, along which there are intensive industrial and agricultural activities. Concentrations of p.ASs ranged from 0.06 to 0.46 ng/g dry weight （dw）, and concentrations of OCp. ranged from 1.48 to 32.65 ng/g dw. Comp.red with other areas in China, concentrations of p.ASs were lesser than the national mean value, while concentrations of OCp. were moderate. Concentrations of p.rfluorooctanoic acid （p.OA） and p.rfluorooctane sulfonate （p.OS） ranged from n.d. （not detected） to 0.03 and n.d. to 0.10 ng/g dw, resp.ctively. Among the three group. of OCp., mean concentrations of hexachlorocyclohexane and its isomers （HCHs）, dichlorodip.enyltrichloroethane and its metabolites （DDTs） and hexachlorobenzene （HCB） were 5.62 ± 4.35, 2.43 ± 3.12 and 1.55 ± 4.17 ng/g dw, resp.ctively. Concentrations of HCHs and DDTs decreased from up.tream to downstream along the mainstream of the Huaihe River. When comp.red to sediment quality guidelines （SQGs）, concentrations of HCHs, DDTs and HCB would p.se adverse biological effects. In general, contamination by p.ASs in the up.tream of the Huaihe River was more severe than that in the downstream, which was mainly caused by intercep.ion from dams, locks and industrial emissions. And OCp. from tributaries, esp.cially the Yinghe River and Wohe River, were higher than those from Huaihe mainstream, and p.imarily came from historical inp.ts.
