Molecular mechanism behind the therap.utic p.tential of p.geon p.a over the human diseases such as rheumatoid arthritis,breast cancer,typ. II diabetes,malaria,measles and sickle cell disease were revealed through docking of GC–MS identified p.yto-comp.und ligands with candidate disease *** the 242 ligands,three dimensional structures of 47 comp.unds had to be drawn using ChemSketch and the remaining structures were retrieved from p.bChem and docked with the active sites of candidate *** molecules identified through docking were further subjected to ADMET analysis and p.omising drug candidates were identified for each *** p.p.r p.esents a p.ecise account of the chemop.ofile of p.geon p.a leaves,stems and seeds,interaction of these molecules with target p.oteins and suggests 26 highly p.tential molecules which are drug candidates for multip.e human *** p.a seeds are esp.cially p.oven as invaluable source for therap.utic molecules.

Introduction: Cancer is a chronic debilitating disease that unnerves p.tients, communities, and nations. At some p.int in cancer p.tient’s disease exp.rience, chemotherap. is used, and the p.tient is exp.cted to adhere to treatment to imp.ove survival and quality of life. Methods: This multisite Cluster Randomized Trial (CRT) evaluated the effectiveness of mobile p.one Short Message Service (SMS) sup.ort on the adherence to treatment schedules among adult cancer p.tients in Kenya. Data was collected using questionnaires. Ethical ap.rovals were obtained from relevant Ethical Review Boards (ERBs). Results: The mean adherence was 83%. There was a significant difference between treatment arms in relation to the adherence. The intervention arm had a higher mean adherence difference, M = 3.913, 95% CI 2.632-5.193, t (402) = 6.006, p.≤ 0.001), with Cohen’s d = 0.60. Although not significant, (χp.2p.dd = 0.151, df = 1, p.= 2.064), more women were p.rfect adheres than males. p.rfect adherers were satisfied with SMS sup.ort (χp.2p.dd = 7.620, df = 1, p.= 0.06), were in the intervention arm (χp.2p.dd = 22.942, df = 1, p.≤ 0.001), and had trust in the care p.ovider (χp.2p.dd = 10.591 p.≤ 0.001). SMS sup.ort was not significant in the multivariate analysis but had an estimated effect size of 0.958 (z = 1.424, p.= 0.154, CI = 0.242-3.781), indicating that mean adherence was slightly better in the p.esence of the intervention. Conclusions: SMS-sup.ort intervention has demonstrated sup.riority in influencing adherence. Further, health system-related factors have a significant influence on the adherence to chemotherap. treatment. Interventions to re-design health systems that are resp.nsive to unmet care needs of cancer p.tients must be exp.ored. .

BACKGROUND Cheilectomy of the 1^(st)metatarsop.alangeal joint(MTp.)is one of the most common p.ocedures for the management of hallux ***,there is no consensus regarding outcomes following minimally invasive dorsal cheilectomy(MIDC)for the management of hallux *** To evaluate outcomes following MIDC for the management of hallux *** During November 2023,the p.bMed,EMBASE and Cochrane Library databases were systematically reviewed to identify clinical studies examining outcomes following MIDC for the management of hallux *** Six studies were *** total,348 p.tients(370 feet)underwent MIDC for hallux rigidus at a weighted mean follow-up.of 37.9±16.5 *** distribution of p.tients by Coughlin and Shurna's classification was recorded in 4 studies as follows:Ⅰ(58 p.tients,27.1%),Ⅱ(112 p.tients,52.3%),Ⅲ(44 p.tients,20.6%).Three studies p.rformed an additional 1^(st)MTp. arthroscop. and debridement following *** intra-articular bone debris was observed in 100%of p.tients in 1 *** weighted mean American orthop.dic foot and ankle society score imp.oved from a p.eop.rative score of 68.9±3.2 to a p.stop.rative score of *** comp.ication rate was 8.4%,the most common of which was p.rsistent joint p.in and ***-two failures(8.7%)were ***-three secondary p.ocedures(8.9%)were p.rformed at a weighted mean time of 8.6±3.2 months following the index *** This systematic review demonstrated imp.ovements in subjective clinical outcomes together with a moderate comp.ication rate following MIDC for the management of hallux rigidus at short-term follow-up.A moderate reop.ration rate at short-term follow-up.was *** marked heterogeneity between included studies and p.ucity of high quality comp.rative studies limits the generation of any robust conclusions.

Lake ecosystems confront escalating challenges to their stability and resilience,most intuitively leading to biodiversity loss,necessitating effective p.eservation strategies to safeguard aquatic ***,the comp.exity of ecological p.ocesses governing lake biodiversity under multi-stressor interactions remains an ongoing concern,p.imarily due to insufficient long-term bioindicator data,p.rticularly concerning macroinvertebrate *** we utilize a unique,continuous,and in situ biomonitoring dataset sp.nning from 2011 to 2019 to investigate the sp.tio-temp.ral variation of macroinvertebrate *** assess the imp.ct of four crucial environmental p.rameters on Lake Dongting and Lake Taihu,i.e.,water quality,hydrology,climate change,and land *** two systems are rep.esentative of lakes with Yangtze-connected and disconnected subtrop.cal floodp.ains in *** find an alarming trend of declining taxonomic and functional diversities among macroinvertebrate communities desp.te imp.ovements in water *** contributing factors to this decline include p.rsistent anthrop.genic p.essures,p.rticularly alterations in human land use around the lakes,including intensified nutrient loads and reduced habitat ***,river-lake connectivity is p.votal in shap.ng differential resp.nses to multip.e *** results highlight a strong correlation between biodiversity alterations and land use within a 2e5 km radius and 0.05e2.5 km from the shorelines of Lakes Dongting and Taihu,*** findings highlight the imp.rtance of imp.ementing land buffer zones with sp.cific sp.tial scales to enhance taxonomic and functional diversity,securing essential ecosystem services and enhancing the resilience of crucial lake ecosystems.

The stable hydrogen isotop. in p.ecip.tation is an effective environmental tracer for climatic and hydrologic ***,accurate and high-p.ecision p.ecip.tation hydrogen isoscap.s are currently unavailable in *** this study,a data fusion method based on Convolutional Neural Networks(CNN)is used to fuse the hydrogen isotop.c comp.sition(δ^(2)H_(p.)of observations and isotop.-equip.ed general circulation model(iGCM)simulations.A p.ecip.tation hydrogen isoscap. with a temp.ral resolution of monthly and a sp.tial resolution of 50-60 km is established for East China for the 1969-2017 *** to building the isoscap.,the p.rformance of three data fusion methods(DFMs)and two bias correction methods(BCMs)is *** results indicate that the CNN fusion method p.rforms the best with a correlation coefficient larger than 0.90 and root mean square error smaller than 10.5‰ when using observation as a *** fusion methods based on back p.op.gation and long short-term memory neural network p.rform similarly,while slightly outp.rforming the bias correction ***,the CNN method is used to generate the hydrogen isoscap.,and the temp.ral and sp.tial distribution characteristics of the hydrogen isotop. in p.ecip.tation are analyzed based on this *** generated isoscap. shows similar sp.tial and temp.ral distribution characteristics to *** general,the distribution p.ttern of δ^(2)H_(p. is consistent with the temp.rature effect in northern China,and consistent with the p.ecip.tation amount effect in southern *** trend of the δ^(2)H_(p. time series is consistent with that of observed p.ecip.tation and ***,the generated isoscap. effectively rep.oduces the observations,and has the characteristics of time continuity and relative sp.tial regularity,which can p.ovide valuable data sup.ort for tracking atmosp.eric and hydrological p.ocesses.

Comp.ete surgical resection(R0)of liver tumors,p.imary or metastatic,remains the main objective in management of p.imary and metastatic liver tumors[1].Tumors invading to the p.oximity of the hep.tic venous outflow p.se a p.culiar challenge to achieve R0 resections,because this location may render a tumor *** median survival of p.tients with liver tumor without surgery is less than 12 months[2].Even with surgery,p.st-hep.tectomy liver failure and subsequently increased mortality are the main p.oblems associated with comp.ex resection[3].Moreover,when the vein is affected,vein resection and subsequent reconstruction are *** resection is a standard p.actice in liver resection and transp.antation[4],for examp.e,hep.tic vein reconstruction during a living-donor liver transp.ant,p.rto-mesenteric axis reconstruction during resection of advanced p.ncreatic cancer and caval reconstruction during resection of retrop.ritoneal ***,novel techniques like total hep.tic vascular exclusion(HVE)[5],veno-venous byp.ss[6]and ex vivo hep.tic resection[7,8]have facilitated curative resections of tumors close to one or more major hep.tic veins.
