Successful rescue of acute liver failure and hemophagocytic lymphohistiocytosis following varicella infection: A case report and review of literature

Herein we report a case of acute liver failure(ALF) and hemophagocytic lymphohistiocytosis(HLH) induced by varicella infection, successfully rescued by a combination therapy of acyclovir, supportive care, and immunosuppression with dexamethasone and etoposide. A previously healthy 16-year-old boy presented with generalized rash, fever, severe abdominal pain, and abnormal liver function within 4 d. Chickenpox was suspected, and acyclovir and intravenous immunoglobulin were started on admission. However, the patient's condition deteriorated overnight with soaring transaminases, severe coagulopathy and encephalopathy. On the fourth day of admission, pancytopenia emerged, accompanied by hypofibrinogenemia and hyperferritinemia. The patient was diagnosed with ALF. He also met the diagnostic criteria of HLH according to the HLH-2004 guideline. Polymerase chain reaction(PCR) amplifications of varicella-zoster virus(VZV) were positive, confirming that VZV was a causative trigger for ALF and HLH. In view of the devastating immune activation in HLH, immunosuppression therapy with dexamethasone and etoposide was administered, in addition to high dose acyclovir. The patient's symptoms improved dramatically and he finally made a full recovery. To our knowledge, this is only the second report of a successful rescue of ALF associated with HLH, without resorting to liver transplantation. The first case was reported in a neonate infected by herpes simplex virus-1. However, survival data in older children and adults are lacking, most of whom died or underwent liver transplantation. Our report emphasizes the clinical vigilance for the possible presence of HLH, and the necessity of extensive investigation for underlying etiologies in patients presenting with indeterminate ALF. Early initiation of specific therapy targeting the underlying etiology, and watchful immunosuppression such as dexamethasone and etoposide, together with supportive therapy, are of crucial importance in this life

Some qualitative behaviors of stationary solutions for the 2-D Lengyel-Epstein system are considered from the local bifurcation viewpoint in this paper. First, local bifurcation branches of hexagonal stationary solutions are constructed in the special case when the habitat domain is a rectangle. Next, the type of the bifurcation diagram near the bifurcation points is discussed.

High-dependency units play a key role in the treatment of a Chinese military patient who developed liver failure while abroad

High-dependency units(HDUs)provide high-level care to patients who suffer from single organ failure,with the exception of respiratory failure requiring mechanical ventilation;HDUs serve as an intermediary between general wards and Intensive Care *** to military and civilian needs,our hospital has established a unique HDU for patients with liver disease in China.A Chinese military officer in the United nations Peacekeeping Forces in South Sudan was transferred to our HDU for liver failure treatment in *** patient’s disease status,nutrition,sleep habits,and psychological behaviour were monitored on different *** patient was provided with vascular monitoring,telemetry,pulse oximetry,drug treatment,nutritional support,sleep intervention,psychological intervention,and humanistic care by a multidisciplinary treatment *** treatment,the patient recovered and avoided liver *** on the experience with this HDU,this new model may create an efficient treatment process for military and civilian patients with severe liver disease at home or abroad.

Chewing substances with or without tobacco and risk of cardiovascular disease in Asia:a meta-analysis

Objective:To assess whether people who ever use any form of chewing substance in Asia are at increased risk of cardiovascular disease(CVD).Methods:PubMed and ISI Web of Science were searched for relevant studies,with no limitation on language or study *** were included if they provided quantitative estimate of the association between ever use of chewing substance and the occurrence of *** authors independently implemented inclusion criteria,abstracted study characteristics,and performed *** relative risks were estimated on the basis of a random effect *** used Q statistic and Egger's test to examine heterogeneity across studies and potential publication bias,***:Eight eligible studies were *** relative risk of CVD for ever using chewing substances with or without tobacco was 1.26(95% confidence interval(CI) 1.12-1.40),which was unchanged when restricted to cohort studies [1.25(1.08-1.42)] or cohort studies in Taiwan [1.31(1.12-1.51)].The summary relative risk for ischemic heart disease was 1.27(1.02-1.52),and was lowered to 1.26(0.85-1.67) after exclusion of a cross-sectional *** overall relative risk for cerebrovascular disease was 1.32(1.08-1.56).On the basis of the Taiwan data,the summary relative risk of CVD for betel(Areca catechu) chewing was 1.30(1.17-1.44).Data on dose-response were limited to betel chewing in Taiwan,suggesting a relationship between risk of CVD and cumulative *** large cohorts in Taiwan reported a greater risk of CVD with betel chewing than with ***:An association was detected between betel chewing with or without tobacco and the risk of *** chewing may impose a greater CVD risk than *** effort is needed in developing betel chewing cessation *** relationship between betel chewing and subgroups of CVD requires further investigation.

Exceptional points enhance sensing in silicon micromechanical resonators

Exceptional points(EPs)have recently emerged as a new method for engineering the response of open physical systems,that is,systems that interact with the *** systems at the EPs exhibit a strong response to a small ***,we show a method by which the sensitivity of silicon resonant sensors can be enhanced when operated at *** our experiments,we use a pair of mechanically coupled silicon micromechanical resonators constituting a parity-time(PT)-symmetric *** perturbations introduced on the mechanically coupled spring cause the frequency to split from the EPs into the PT-symmetric regime without broadening the two spectrum linewidths,and this frequency splitting scales with the square root of the perturbation *** overall signal-tonoise ratio is still greatly enhanced,although the measured noise spectral density of the EP sensing scheme has a slight increase comparable to the traditional *** results pave the way for resonant sensors with ultrahigh sensitivity.

Application of GNSS-PPP on Dynamic Deformation Monitoring of Offshore Platforms

The real-time dynamic deformation monitoring of offshore platforms under environmental excitation is crucial to their safe *** Global navigation Satellite System-Precise Point Positioning(GNSS-PPP)has been considered for this purpose,its monitoring accuracy is relatively ***,the influence of background noise on the dynamic monitoring accuracy of GNSS-PPP remains ***,it is imperative to further validate the feasibility of GNSS-PPP for deformation monitoring of offshore *** address these concerns,vibration table tests with different amplitudes and frequencies are *** results demonstrate that GNSS-PPP can effectively monitor horizontal vibration displacement as low as±30 mm,which is consistent with ***,the spectral characteristic of background noise in GNSS-PPP is similar to that of GNSS-RTK(Real Time Kinematic).Building on this observation,an improved Complete Ensemble Empirical Mode Decomposition with Adaptive Noise(CEEMDAN)has been proposed to de-noise the data and enhance the dynamic monitoring accuracy of *** monitoring application research is also undertaken,successfully extracting and analyzing the dynamic deformation of an offshore platform structure under environmental excitation using GNSS-PPP monitoring in conjunction with improved CEEMDAN *** comparing the de-noised dynamic deformation trajectories of the offshore platform during different periods,it is observed that the platform exhibits reversible alternating vibration responses under environmental excitation,with more pronounced displacement deformation in the direction of load *** research results confirm the feasibility and potential of GNSS-PPP for dynamic deformation monitoring of offshore platforms.

Incubation temperature induced developmental plasticity of cold responsive physiological phenotypes in Japanese Quails

Embryonic development is a critical period for phenotype *** variation during embryonic development can induce changes in postnatal phenotypes of *** thyroxine secretion and aerobic metabolic activity of small birds are important phenotypes closely related to their winter *** the context of climate change,it is necessary to determine whether temperature variation during incubation in birds leads to developmental plasticity of these cold responsive *** incubated Japanese Quail(Coturnix japonica)eggs at 36.8℃,37.8℃,and 38.8℃,and raised the chicks to 35-day old at 22℃with same raising conditions,then all the quails were exposed to gradually temperature dropping environment(from 15℃to 0℃).After cold treatment,serum T3 level,resting metabolic rate,skeletal muscle and liver metabolomes of the birds were *** serum T3 levels were significantly lower in the 38.8℃group and significantly higher in the 36.8℃group compared to the 37.8℃*** metabolic rate in the 38.8℃group was significantly lower compared to the 37.8℃*** with the 37.8℃group,metabolites involved in the tricarboxylic acid cycle in the liver were significantly lower in the 38.8℃group,and metabolites related to lipid oxidation metabolism and fatty acid biosynthesis were significantly lower in the skeletal muscles in the 38.8℃group but significantly higher in the 36.8℃*** results indicate that incubation temperature variation can lead to developmental plasticity in cold responsive physiological *** incubation temperature may impair the capacity of birds coping with cold challenge.
