Brain protective effect of dexmedetomidine vs propofol for sedation during prolonged mechanical ventilation in non-brain injured patients

BACKGROUND Dexmedetomidine and propofol are two sedatives used for long-term *** remains unclear whether dexmedetomidine provides superior cerebral protection for patients undergoing long-term mechanical *** To compare the neuroprotective effects of dexmedetomidine and propofol for sedation during prolonged mechanical ventilation in patients without brain *** Patients who underwent mechanical ventilation for>72 h were randomly assigned to receive sedation with dexmedetomidine or *** Richmond Agitation and Sedation Scale(RASS)was used to evaluate sedation effects,with a target range of-3 to *** primary outcomes were serum levels of S100-βand neuron-specific enolase(NSE)every 24 *** secondary outcomes were remifentanil dosage,the proportion of patients requiring rescue sedation,and the time and frequency of RASS scores within the target *** A total of 52 and 63 patients were allocated to the dexmedetomidine group and propofol group,*** data were comparable between *** significant differences were identified between groups within the median duration of study drug infusion[52.0(IQR:36.0-73.5)h vs 53.0(IQR:37.0-72.0)h,P=0.958],the median dose of remifentanil[4.5(IQR:4.0-5.0)μg/kg/h vs 4.6(IQR:4.0-5.0)μg/kg/h,P=0.395],the median percentage of time in the target RASS range without rescue sedation[85.6%(IQR:65.8%-96.6%)vs 86.7%(IQR:72.3%-95.3),P=0.592],and the median frequency within the target RASS range without rescue sedation[72.2%(60.8%-91.7%)vs 73.3%(60.0%-100.0%),P=0.880].The proportion of patients in the dexmedetomidine group who required rescue sedation was higher than in the propofol group with statistical significance(69.2%vs 50.8%,P=0.045).Serum S100-βand NSE levels in the propofol group were higher than in the dexmedetomidine group with statistical significance during the first six and five days of mechanical ventilation,respectively(all P<0.05).CONCLUSION Dexmedetomidine demonstrated

Immunotherapy and associated immune regulation strategies gained huge attraction in order to be utilized for treatment and prevention of respiratory *** specifically nanomedicines can be used to regulate host immunity in lungs in the case of respiratory diseases including coronavirus disease 2019(COVID-19)***-19 causes pulmonary embolisms,thus new therapeutic options are required to target thrombosis,as conventional treatment options are either not effective due to the complexity of the immunethrombosis *** this review,we discuss regulation of immune response in respiratory diseases especially *** further discuss thrombosis and provide an overview of some antithrombotic nanoparticles,which can be used to develop nanomedicine against thrombo-inflammation induced by COVID-19 and other respiratory infectious *** also elaborate the importance of immunomodulatory nanomedicines that can block pro-inflammatory signalling pathways,and thus can be recommended to treat respiratory infectious diseases.

Pathogenic changes of community-acquired pneumonia in a children’s hospital in Beijing,China before and after COVID-19 onset:a retrospective study

Background This study aimed to analyze the pathogenic characteristics of community-acquired pneumonia(CAP)in a children’s hospital before and after the coronavirus disease 2019(COVID-19)pandemic and to provide testimony for preventing CAP in the *** A retrospective analysis was *** information was collected from the electronic medical record system of the hospital.A total of 2739 children were included from February 1,2019,to January 31,*** Among these 2739 patients were 1507(55.02%)males and 1232(44.98%)females;the median age was 3.84 *** were 2364 cases during the pre-COVID-19 period and 375 cases during the post-COVID-19 *** number of hospitalized children after the pandemic was 84.14%*** median age after the onset was 1.5 years younger than that before the onset(4.08 years old)(Z=−7.885,Pbacteria(50.42%)>viruses(29.57%)>fungi(3.43%).During the post-COVID-19 period,the pathogen proportions were bacteria(56.53%)>viruses(53.60%)>MP(23.47%)>fungi(3.73%).Conclusions There was a significant decrease in the number of children with CAP hospitalized after the pandemic,especially among school-age children,and the pathogen proportions of CAP with MP and IV were significantly *** inferred that CAP was effectively prevented in school-age children because of the strong mitigation measures.

Exceptional points enhance sensing in silicon micromechanical resonators

Exceptional points(EPs)have recently emerged as a new method for engineering the response of open physical systems,that is,systems that interact with the *** systems at the EPs exhibit a strong response to a small ***,we show a method by which the sensitivity of silicon resonant sensors can be enhanced when operated at *** our experiments,we use a pair of mechanically coupled silicon micromechanical resonators constituting a parity-time(PT)-symmetric *** perturbations introduced on the mechanically coupled spring cause the frequency to split from the EPs into the PT-symmetric regime without broadening the two spectrum linewidths,and this frequency splitting scales with the square root of the perturbation *** overall signal-tonoise ratio is still greatly enhanced,although the measured noise spectral density of the EP sensing scheme has a slight increase comparable to the traditional *** results pave the way for resonant sensors with ultrahigh sensitivity.

We investigate the resource allocation problem of a cell-free massive multiple-input multiple-output system under the condition of colluding eavesdropping by multiple passive *** address the problem of limited pilot resources,a scheme is proposed to allocate the pilot with the minimum pollution to users based on access point selection and optimize the pilot transmission power to improve the accuracy of channel *** at the secure transmission problem under a colluding eavesdropping environment by multiple passive eavesdroppers,based on the local partial zero-forcing precoding scheme,a transmission power optimization scheme is formulated to maximize the system’s minimum security spectral *** results show that the proposed scheme can effectively reduce channel estimation error and improve system security.

Astronomical forcing and sedimentary noise modeling of lake-level changes in the Middle Eocene Chezhen Sag,Bohai Bay Basin,eastern China

The accurate determination of geological age is a key to understanding the history and process of paleolake evolution and oil and gas exploration in continental lake ***,improving the accuracy of geological age has always been a difficult scientific problem.A 609-m-thick,continuous lacustrine mudstone and sandstone succession in Chezhen Sag(eastern China)provides an ideal middle Eocene sedimentary record for establishing a high-resolution stratigraphic chronology *** on spectrum analysis and sliding window spectrum analysis of the natural gamma(GR)logging data of well Che 271(C271)in Chezhen Sag,the periods of 405 kyr and 40.1 kyr were filtered by a Gaussian bandpass filter,and a“floating”astrochronological time scale(ATS)was *** total number of 405 kyr eccentricity cycles were 13.6 and 40.1 kyr obliquity cycles were 138 which recorded from the upper member 4(Es4U)to the member 3(Es3)of the Eocene Shahejie Formation,and the depositional duration was 5.53 *** Coefficient(COCO)analysis and evolutionary Correlation Coefficient(eCoCo)analysis found that the optimal sedimentary rate of different *** noise simulation revealed the history of paleolake water changes in the Middle Eocene in the Chezhen Sag,according to which four sequences are *** study shows that the lake level change of Chezhen Sag in the middle Eocene shows prominent 1.2 Myr cycles and an antiphase well-coupled relationship with obliquity ***,we propose a model to explain the relationship between the orbital cycle and lake level change in the continental lake *** the obliquity of the earth increases,the middle and high latitudes of the earth will be closer to the sun,the direct sunlight will be higher,and the meridional sunshine will increase,thus accelerating the evaporation process of lake basin *** the seasonal changes are obvious(maximum period of 1.2 Myr ultra-long obliquity),this effect is more signific

Response of Bacterial Community and Enzyme Activity of Greenhouse Tomato under Different Irrigation Systems

The micro-sprinkler irrigation mulched(MSM)has been suggested as a novel water-saving approach in con-trolled environment ***,the effects of microbial community structure and enzyme activity in the rhizosphere soil on crop growth under MSM remain *** study conducted a randomized experimen-tal design using greenhouse tomatoes to investigate changes in bacterial community structure and enzyme activity in rhizosphere soil under different irrigation frequencies(F)and amounts(I)of ***ﬁndings revealed that with the increase of F or I,The total count of soil bacteria in tomatoesﬁrst rose and then fell in terms of Opera-tional Taxonomic Units(OTUs)classiﬁ*** to other F,the most abundance of nitrogen and phos-phorus metabolism genes and enzyme activities were observed with a 5-day ***,the diversity of soil bacterial community structure initially rose before eventually declining with the increase of the *** 1.00 Epan(cumulative evaporation of a 20 cm standard pan)under MSM helped boost the abundance of nitrogen and phosphorus metabolism functional genes in soil bacteria,ensuring higher enzyme activities related to nitro-gen,carbon,and phosphorus metabolism in the rhizosphere soil of ***’yield initially rose before eventually declining with the increase in F or I,whereas I had a more signiﬁcant effect on yield.A 1.00%increase in I yielded a minimum of 39.24%increase in tomato *** study showed a positive correlation between soil bacterial community,soil enzyme activity,and greenhouse tomato yield under *** the results comprehensively,the combined irrigation mode of F of 5 d and I of 1.00 Epan was recommended for greenhouse tomatoes under *** conclusion provides theoretical support for water-saving practices and yield improve-ment in facility agriculture,especially tomato cultivation.

Application of GNSS-PPP on Dynamic Deformation Monitoring of Offshore Platforms

The real-time dynamic deformation monitoring of offshore platforms under environmental excitation is crucial to their safe *** Global navigation Satellite System-Precise Point Positioning(GNSS-PPP)has been considered for this purpose,its monitoring accuracy is relatively ***,the influence of background noise on the dynamic monitoring accuracy of GNSS-PPP remains ***,it is imperative to further validate the feasibility of GNSS-PPP for deformation monitoring of offshore *** address these concerns,vibration table tests with different amplitudes and frequencies are *** results demonstrate that GNSS-PPP can effectively monitor horizontal vibration displacement as low as±30 mm,which is consistent with ***,the spectral characteristic of background noise in GNSS-PPP is similar to that of GNSS-RTK(Real Time Kinematic).Building on this observation,an improved Complete Ensemble Empirical Mode Decomposition with Adaptive Noise(CEEMDAN)has been proposed to de-noise the data and enhance the dynamic monitoring accuracy of *** monitoring application research is also undertaken,successfully extracting and analyzing the dynamic deformation of an offshore platform structure under environmental excitation using GNSS-PPP monitoring in conjunction with improved CEEMDAN *** comparing the de-noised dynamic deformation trajectories of the offshore platform during different periods,it is observed that the platform exhibits reversible alternating vibration responses under environmental excitation,with more pronounced displacement deformation in the direction of load *** research results confirm the feasibility and potential of GNSS-PPP for dynamic deformation monitoring of offshore platforms.
