Lack of Efficacy of Ulinastatin Therapy During Cardiopulmonary Bypass Surgery

Background： It was believed that inflammatory response induced by cardiopulmonary bypass （CPB） was blamed for complications after cardiac surgery. To improve the outcome, many pharmacological interventions have been applied to attenuate inflammatory response during CPB. The objective of this study was to investigate the effect of ulinastatin （urinary trypsin inhibitor [UTI]） on outcome after CPB surgery. Methods： Totally, 208 patients undergoing elective valves replacement between November 2013 and September 2014 were divided into Group U （n = 70） and Group C （n = 138） based on they received UTI or not. Categorical variables were compared between groups using Fisher＇s exact test, and continuous variables using unpaired Student＇s t-test or Mann-Whitney U-test. One-way analysis of variance and Dunnett＇s or Tukey＇s tests were used to compare values at different time points within the same group, The risk of outcomes was estimated and adjusted by multivariable logistic regression, propensity scoring, and mixed-effect models for all measured variables. Results： Both the serious complications in total, including death, acute lung injury, acute respiratory distress syndrome and acute kidney injury, and the other complications, including hemodialysis, infection, re-incubation, and tracheotomy were similar between the two groups （P 〉 0.05）. After adjusted by multivariable logistic regression and the propensity score, UTI still cannot be found any benefit to improve any outcomes after cardiac surgery. Also, no statistical differences with regard to duration of postoperative mechanical ventilation, the length of Intensive Care Unit and hospital stays （P 〉 0.05）. Conclusion： UTI did not improve postoperative outcomes in our patients after cardiopulmonary bypass surgery.

Is there a role of TNFR1 in acute lung injury cases associated with extracorporeal circulation？

The signaling pathway for tumor necrosis factor-a （TNF-a） and its receptors is up-regulated during ex- tracorporeal circulation （ECC）, and recruits blood neutrophil into the lung tissue, which results in acute lung injury （ALl） In this study, we evaluated the role of tumor necrosis factor receptor I （TNFR1） in ECC-induced ALl by blocking TNF-a binding to TNFR1 with CAY10500. Anesthetized Sprague-Dawley （SD） rats were pretreated intravenously with phos- phate buffered saline （PBS） or vehicle （0.3 ml ethanol IV） or CAY10500, and then underwent ECC for 2 h. The oxy- genation index （OI） and pulmonary inflammation were assessed after ECC. OI was significantly decreased, while TNF-a and neutrophil in bronchoalveolar lavage fluid （BALF） and plasma TNF-a increased after ECC. Pretreatment of CAY10500 decreased plasma TNF-a level, but did not decrease TNF-a levels and neutrophil counts in BALF or improve OI. Lung histopathology showed significant alveolar congestion, infiltration of the leukocytes in the airspace, and increased thickness of the alveolar wall in all ECC-treated groups. CAY10500 pretreatment slightly reduced leukocyte infiltration in lungs, but did not change the wet/dry ratio in the lung tissue. Blocking TNF-a binding to TNFR1 by CAY10500 intravenously slightly mitigates pulmonary inflammation, but cannot improve the pulmonary function, indicating the limited role of TNFR1 pathway in circulating inflammatory cell in ECC-induced ALl.

Pretreatment elevated fibrinogen level predicts worse oncologic outcomes in upper tract urothelial carcinoma

This study aimed to further validate the prognostic role of fibrinogen in upper tract urothelial carcinoma(UTUC)in a large Chinese cohort.A total of 703 patients who underwent radical nephroureterectomy were retrospectively *** levels of≥4.025 g l?1 were defined as *** regression analysis was performed to determine the association between fibrinogen and adverse pathological ***–Meier analysis and Cox regression models were used to assess the associations of fibrinogen with cancer-specific survival(CSS),disease recurrence-free survival(RFS),and overall survival(OS).Harrell c-index and decision curve analysis were used to assess the clinical utility of multivariate *** median follow-up duration was 42(range:1–168)*** regression analysis revealed that elevated fibrinogen was associated with higher tumor stage and grade,lymph node involvement,lymphovascular invasion,sessile carcinoma,concomitant variant histology,and positive surgical margins(all Pnalysis demonstrated that elevated fibrinogen was independently associated with decreased CSS(hazard ratio[HR]:2.33;Pnalysis showed an added benefit for CSS prediction when fibrinogen was added to the *** fibrinogen may be a strong independent predictor of worse oncologic outcomes in UTUC;therefore,it may be valuable to apply this marker to the current risk stratification in UTUC.

Coniopteris simplex is a common component in many Jurassic ***,due to morphological variations in sterile fronds and incomplete preservation of reproductive organs,its classification has been controversial for a long ***,we present new material collected from the Middle Jurassic Yan’an Formation in the Ordos Basin,Inner *** new material reveals important morphological characters,including hemi-dimorphic fronds,two forms of sterile pinnae,short-stalked sori and sporangia on fertile fronds,annulus of sporangium composed of about 30 cells,in situ rounded triangular spores with protruded suture,and the lips of trilete laesura encircled by one or two rows of small *** on these observations,we emended the diagnosis of this common *** spatio-temporal distribution of Coniopteris simplex shows that the iconic species was restricted to the paleolatitudes ranging from 19.5°N to 48.9°N in the Northern Hemisphere during the Early—Late Jurassic,mainly distributed in the tropical to paratropical zones,and the paleolongitude ranging from 9.6°E to 129.3°E,which may be limited by the opening of the Viking Corridor and the splitting of the Central Atlantic Ocean.

Time-of-flight resolved stimulated Raman scattering microscopy using counter-propagating ultraslow Bessel light bullets generation

We present a novel time-of-flight resolved Bessel light bullet-enabled stimulated Raman scattering(B2-SRS)microscopy for deeper tissue 3D chemical imaging with high resolution without a need for mechanical *** accomplish the tasks,we conceive a unique method to enable optical sectioning by generating the counterpropagating pump and Stokes Bessel light bullets in the sample,in which the group velocities of the Bessel light bullets are made ultraslow(e.g.,vg≈0.1c)and tunable by introducing programmable angular dispersions with a spatial light *** theoretically analyze the working principle of the collinear multicolor Bessel light bullet generations and velocity controls with the relative time-of-flight resolved detection for SRS 3D deep tissue *** have also built the B2-SRS imaging system and present the first demonstration of B2-SRS microscopy with Bessel light bullets for 3D chemical imaging in a variety of samples(e.g.,polymer bead phantoms,biological samples such as spring onion tissue and porcine brain)with high *** B2-SRS technique provides a>2-fold improvement in imaging depth in porcine brain tissue compared to conventional SRS *** method of optical sectioning in tissue using counter-propagating ultraslow Bessel light bullets developed in B2-SRS is generic and easy to perform and can be readily extended to other nonlinear optical imaging modalities to advance 3D microscopic imaging in biological and biomedical systems and beyond.

Integrating single-cell Rna sequencing with spatial transcriptomics reveals immune landscape for interstitial cystitis

Interstitial cystitis(IC)is a severely debilitating and chronic disorder with unclear etiology and pathophysiology,which makes the diagnosis difficult and treatment *** investigate the role of immunity in IC bladders,we sequenced 135,091 CD45^(+) immune cells from 15 female patients with IC and 9 controls with stress urinary incontinence using single-cell Rna sequencing(scRna-seq).22 immune subpopulations were identified in the constructed *** them,M2-like macrophages,inflammatory CD14^(+) macrophages,and conventional dendritic cells had the most communications with other immune ***,a significant increase of central memory CD4^(+)T cells,regulatory T cells,GZMK^(+)CD8^(+)T cells,activated B cells,un-switched memory B cells,and neutrophils,and a significant decrease of CD8+effector T cells,Th17 cells,follicular helper T cells,switched memory B cells,transitional B cells,and macrophages were noted in IC *** enrichment analysis identified a virus-related response during the dynamic change of cell proportion,furthermore,the human polyomavirus-2 was detected with a positive rate of 95%in urine of patients with *** integrating the results of scRna-seq with spatial transcriptomics,we found nearly all immune subpopulations were enriched in the urothelial region or located close to fibroblasts in IC bladders,but they were discovered around urothelium and smooth muscle cells in control *** findings depict the immune landscape for IC and might provide valuable insights into the pathophysiology of IC.

Electroreduction of nitrate has been gaining wide attention in recent years owing to it's beneficial for converting nitrate into benign N_(2) from the perspective of electrocatalytic denitrification or into value-added ammonia from the perspective of electrocatalytic NH_(3) *** reason of the undesired formation of ammonia is dominant during electroreduction of nitrate-containing wastewater,chloride has been widely used to improve N_(2) ***,selective electroreduction of nitrate to N2 gas in chloride-containing system poses several *** this review,we focus on the key strategies for efficiently enhancing N_(2) selectivity of electroreduction of nitrate in chloride-free system,including optimal selection of elements,combining an active metal catalyst with another metal,manipulating the crystalline morphology and facet orientation,constructing core–shell structure catalysts,*** summarizing the strategies,four possible reaction pathways of electro-reduction of nitrate to N_(2) are ***,this review attempts to provide practical strategies for enhancing N2 selectivity without the aid of electrochlorination and highlight directions for future research for designing appropriate electrocatalyst for final electrocatalytic denitrifi-cation.

Phase-modulated multi-foci microscopy for rapid 3D imaging

3D imaging technology is pivotal in monitoring the functional dynamics and morphological alterations in living cells and ***,conventional volumetric imaging associated with mechanical z-scanning encounters challenges in limited 3D imaging speed with inertial ***,we present a unique phase-modulated multifoci microscopy (PM^(3)) technique to achieve snapshot 3D imaging with the advantages of extended imaging depths and adjustable imaging intervals between each focus in a rapid *** accomplish the tasks,we utilize a spatial light modulator (SLM) to encode the phases of the scattered or fluorescence light emanating from a volumetric sample and then project the multiple-depth images of the sample onto a single charge-coupled device camera for rapid 3D *** demonstrate that the PM^(3)technique provides~55-fold improvement in imaging depth in polystyrene beads phantom compared to the depth of field of the objective lens ***^(3)also enables the real-time monitoring of Brownian motion of fluorescent beads in water at a 15 Hz volume *** precisely manipulating the phase of scattered light on the SLM,PM^(3)can pinpoint the specific depth information in living zebrafish and rapidly observe the 3D dynamic processes of blood flow in the zebrafish *** work shows that the PM^(3)technique developed is robust and versatile for fast 3D dynamic imaging in biological and biomedical systems.
