Objective To explore the prescribing patterns of outpatients receiving antibiotics for upper respiratory tract infections(URTIs)in rural Western china and to identify the correlation in terms of doctors and patients *** Totally 7 678 prescriptions for URTIs were collected from 680 primary health village clinics of 40 counties across 10 provinces of Western *** outcome variables were used in the analysis:the occurrence of prescribing at least one antibiotic drug for an URTI and the occurrence of prescribing two or more antibiotics for an *** logistic regression models were used to examine the socioeconomic and demographic determinants of the above two outcome *** The percentage of prescribing at least one antibiotic for URTIs was 48.6% while the percentage of prescribing antibiotic combination(two or more antibiotics)was 4.6%.The two measurements of antibiotic utilization differed remarkably among the 10 *** diagnosed with tonsillitis and faucitis had higher odds(OR=8.86 for tonsillitis and OR=4.64 for faucitis)of antibiotic prescription than patients with other diagnosis of *** with tonsillitis and faucitis also had higher odds(OR=3.82 for tonsillitis and OR=2.71 for faucitis)of multiple antibiotic prescription than those with other diagnosis of *** number of drugs per prescription and injection in prescriptions were also significant predictors of antibiotic and multiple antibiotic utilization for *** It is concluded that the percentage of antibiotic prescription for URTIs is higher in rural Western china than in most of other countries with available data and that prescriptions of antibiotics for URTIs are associated with residence regions of patients,URTI diagnosis and background information on drug prescription.

盆栽条件下,以平邑甜茶幼苗为试验材料,设置连作土(对照)、灭菌连作土、连作土添加1.5 g·kg^(-1)腐植酸、连作土添加腐植酸生物降解膜、连作土添加1.5 g·kg^(-1)腐植酸和腐植酸生物降解膜5个处理,通过测定处理后平邑甜茶生物量和植株根系生长状况、土壤微生物数量、土壤酶活性、土壤养分含量等指标,研究腐植酸及其生物降解膜对连作土壤环境及平邑甜茶幼苗生长的影响。8月取样结果表明,与对照相比,腐植酸和腐植酸生物降解膜复合处理使得平邑甜茶幼苗株高、地径、鲜质量、干质量分别提高了72.56%、58.50%、196.20%、250.81%;根表面积、根体积和根系超氧化物歧化酶、过氧化物酶、过氧化氢酶活性较连作土相比分别提高了74.93%、143.75%和63.40%、100.85%、72.57%;腐植酸及其生物降解膜还可以改善土壤环境,施用后土壤中细菌数量明显增加,真菌数量减少,细菌/真菌值增大,细菌、放线菌数量分别比对照提高了115.79%、135.14%,真菌数量降低了87.25%,细菌/真菌值增大了16倍,且土壤有机质及营养元素含量也有所提升,9月变化趋势与8月一致。综上所述,腐植酸及其生物降解膜改善了土壤环境,促进了平邑甜茶幼苗的生长,可有效减轻平邑甜茶的连作障碍。

Objective In china, 24 cases of human infection with highly pathogenic avian influenza(HPAI) H5 N6 virus have been confirmed since the first confirmed case in 2014. Therefore, we developed and assessed two H5 N6 candidate vaccine viruses(CVVs).Methods In accordance with the World Health Organization(WHO) recommendations, we constructed two reassortant viruses using reverse genetics(RG) technology to match the two different epidemic H5 N6 viruses. We performed complete genome sequencing to determine the genetic stability. We assessed the growth ability of the studied viruses in MDCK cells and conducted a hemagglutination inhibition assay to analyze their antigenicity. Pathogenicity attenuation was also evaluated in vitro and in *** The results showed that no mutations occurred in hemagglutinin or neuraminidase, and both CVVs retained their original antigenicity. The replication capacity of the two CVVs reached a level similar to that of A/Puerto Rico/8/34 in MDCK cells. The two CVVs showed low pathogenicity in vitro and in vivo, which are in line with the WHO requirements for *** We obtained two genetically stable CVVs of HPAI H5N6 with high growth characteristics,which may aid in our preparedness for a potential H5N6 pandemic.

目的:建立LC-MS/MS法测定人血浆中甲麦角新碱浓度,对肌注0.2 mg马来酸甲麦角新碱注射液后药动学进行研究。方法:以文拉法辛为内标,血浆以NaOH碱化后经乙酸乙酯液液萃取浓缩后进行LC-MS/MS分析。采用高效液相色谱分离系统,色谱柱为CAPCELL CORE C_(18)色谱柱(2.1 mm×100 mm,2.7μm),流动相为甲醇(0.05%乙酸)-水(5 mmol·L^(-1)乙酸铵,0.05%乙酸)(65∶35);采用质谱检测法,多反应监测(MRM)方式监测m/z 340.2→m/z 208.1(甲麦角新碱)和m/z 278.3→m/z 215.1(内标文拉法辛)。结果:建立的LC-MS/MS法在0.025~10 ng·mL^(-1)范围内甲麦角新碱色谱响应与浓度相关性良好,定量限为0.025 ng·mL^(-1),日内、间精密度RSD均小于7%,准确度在96.7%~112.4%,方法稳定性较好。16名受试者单次肌注0.2 mg马来酸甲麦角新碱注射液后AUC_(0-8)为(12.165±3.345)ng·mL^(-1)·h,AUC_(0-∞)为(12.900±3.800)ng·mL^(-1)·h,C_(max)为(4.707±2.541)ng·mL^(-1),t_(max)为(0.577±0.251)h,t_(1/2)为(1.831±0.457)h,MRT为(2.412±0.246)h,CL为(16.925±5.281)L·h^(-1),V_d为(43.183±14.041)L。结论:该法灵敏准确,简便易行,适用于低剂量甲麦角新碱血药浓度的测定及其药代动力学研究。研究结果显示马来酸甲麦角新碱注射液肌注后吸收较快,0.5 h左右达峰;部分受试者用药后发生恶心、呕吐及头痛等不良事件,但程度均为轻微。

Establishment of an optimized CTC detection model consisting of EpCAM,MUC1 and WT1 in epithelial ovarian cancer and its correlation with clinical characteristics

Objective:Emerging studies have demonstrated the promising clinical value of circulating tumor cells(CTCs)for diagnosis,disease assessment,treatment monitoring and prognosis in epithelial ovarian ***,the clinical application of CTC remains restricted due to diverse detection techniques with variable sensitivity and specificity and a lack of common ***:We enrolled 160 patients with epithelial ovarian cancer as the experimental group,and 90 patients including 50 patients with benign ovarian tumor and 40 healthy females as the control *** enriched CTCs with immunomagnetic beads targeting two epithelial cell surface antigens(EpCAM and MUC1),and used multiple reverse transcription-polymerase chain reaction(RT-PCR)detecting three markers(EpCAM,MUC1 and WT1)for *** then we used a binary logistic regression analysis and focused on EpCAM,MUC1 and WT1 to establish an optimized CTC detection ***:The sensitivity and specificity of the optimized model is 79.4%and 92.2%,*** specificity of the CTC detection model is significantly higher than CA125(92.2%vs.82.2%,P=0.044),and the detection rate of CTCs was higher than the positive rate of CA125(74.5%vs.58.2%,P=0.069)in early-stage patients(stage I and II).The detection rate of CTCs was significantly higher in patients with ascitic volume≥500 mL,suboptimal cytoreductive surgery and elevated serum CA125 level after 2 courses of chemotherapy(P26.3%vs.11.9%;26.4%vs.13.4%,P43).Conclusions:Our study presents an optimized detection model for CTCs,which consists of the expression levels of three markers(EpCAM,MUC1 and WT1).In comparison with CA125,our model has high specificity and demonstrates better diagnostic values,especi

Pedestrian Injuries and the Relevant Burden in Shanghai, china: Implications for control

Objective To study the associations of pedestrian injuries with age, income and educational level in Shanghai and to analyze the relative disease burden. Methods Information on pedestrian-related cases and deaths were collected from 494 hospitals and mortality registry systems from 1992 to 2010, and a multistage cluster sampling survey conducted in 2006. Logistic regression model was used in the analyses. Results The age group of 5-9 had the highest mortality and morbidity among children. Mortality increased obviously among those aged 60 or above. Individuals with an educational level under the primary school and with the lower family average income were more likely to suffer pedestrian-related injuries. Multivariate Logistic analysis demonstrated that lower income and lower educational level increased the risk of pedestrian injuries with the odds ratio of 1.40 （95% Cl： 1.15-1.71） and 1.70 （95% Cl： 1.20-2-40）, respectively. About 13.54% of the share of GDP for the healthcare, social security and welfare industries in Shanghai was occupied by the burden of pedestrian-related injuries in 2006. Conclusion Pedestrian-related injury has inverse association with victims＇ income and educational level. Children of 5-9 years old and adults over 60 with lower educational level and lower monthly income are the target persons to be intervened.
