Hypobaric hypoxia (HH) exposure can cause serious brain injury as well as life-threatening cerebral edema in severe cases. Previous studies on the mechanisms of HH-induced brain injury have been conducted primarily using non-primate animal models that are genetically distant to humans, thus hindering the development of disease treatment. Here, we report that cynomolgus monkeys (Macaca fascicularis) exposed to acute HH developed human-like HH syndrome involving severe brain injury and abnormal behavior. Transcriptome profiling of white blood cells and brain tissue from monkeys exposed to increasing altitude revealed the central role of the HIF-1 and other novel signaling pathways, such as the vitamin D receptor (VDR) signaling pathway, in co-regulating HH-induced inflammation processes. We also observed profound transcriptomic alterations in brains after exposure to acute HH, including the activation of angiogenesis and impairment of aerobic respiration and protein folding processes, which likely underlie the pathological effects of HH-induced brain injury. Administration of progesterone (PROG) and steroid neuroprotectant 5α-androst-3β,5,6β-triol (TRIOL) significantly attenuated brain injuries and rescued the transcriptomic changes induced by acute HH. Functional investigation of the affected genes suggested that these two neuroprotectants protect the brain by targeting different pathways, with PROG enhancing erythropoiesis and TRIOL suppressing glutamate-induced excitotoxicity. Thus, this study advances our understanding of the pathology induced by acute HH and provides potential compounds for the development of neuroprotectant drugs for therapeutic treatment.

BACKGROUND Improved adenoma detection at colonoscopy has decreased the risk of developing colorectal ***,whether image-enhanced endoscopy(IEE)further improves the adenoma detection rate(ADR)is *** To compare IEE with white-light imaging(Wli)endoscopy for the detection and identification of colorectal *** This was a multicenter,randomized,controlled *** were enrolled between September 2019 to April 2021 from 4 hospital in *** were randomly assigned to an IEE group with Wli on entry and IEE on withdrawal(n=2113)or a Wli group with Wli on both entry and withdrawal(n=2098).The primary outcome was the *** secondary endpoints were the polyp detection rate(PDR),adenomas per colonoscopy,adenomas per positive colonoscopy,and factors related to adenoma *** A total of 4211 patients(966 adenomas)were included in the analysis(mean age,56.7 years,47.1%male).There were 2113 patients(508 adenomas)in the IEE group and 2098 patients(458 adenomas)in the Wli *** ADR in two group were not significantly different[24.0%vs 21.8%,1.10,95%confidence interval(CI):0.99-1.23,P=0.09].The PDR was higher with IEE group(41.7%)than with Wli group(36.1%,1.16,95%CI:1.07-1.25,P=0.01).Differences in mean withdrawal time(7.90±3.42 min vs 7.85±3.47 min,P=0.30)and adenomas per colonoscopy(0.33±0.68 vs 0.28±0.62,P=0.06)were not *** analysis found that with narrowband imaging(NBI),between-group differences in the ADR,were not significant(23.7%vs 21.8%,1.09,95%CI:0.97-1.22,P=0.15),but were greater with linked color imaging(30.9%vs 21.8%,1.42,95%CI:1.04-1.93,P=0.04).the second-generation NBI(2G-NBI)had an advantage of ADR than both Wli and the first-generation NBI(27.0%vs 21.8%,P=0.01;27.0%vs 21.2.0%,P=0.01).CONCLUSION This prospective study confirmed that,among chinese,IEE didn’t increase the ADR compared with Wli,but 2G-NBI increase the ADR.

原发性免疫性血小板减少症(ITP)中少见但至关重要的危重出血事件,给患者的预后、生活质量和治疗决策带来严重影响。尽管有一些研究探讨了ITP中与危重出血相关的风险因素,但目前尚缺乏大样本数据、大规模多中心研究结果以及针对ITP患者致命出血事件的预测模型。本研究首次采用国际血栓与止血学会新提出的ITP致命出血标准,利用大样本数据开发了首个基于机器学习的在线应用,用于预测ITP患者的致命出血.研究中,我们使用中国各地大型多中心数据进行开发,并对全国39家医疗中心进行为期一年的外部测试,得到了较好的训练、验证和测试数据集预测能力该基于新算法的便捷网络工具能够快速识别ITP患者的出血风险,辅助临床决策,有望未来降低不良事件的发生。

Background Malnutrition is common in patients with cancer,and can negatively impact their quality of life(QoL)and even ***,there is currently no large data available on the prevalence of malnutrition in chinese cancer *** study evaluated the prevalence of malnutrition and the QoL of chinese patients with locoregional,recurrent or metastatic *** We conducted a nationwide observational,multi-center,hospital-based cross-sectional study within the chinese Society of Nutritional Oncology(CSNO)*** of the patients were diagnosed with one of the following 18 different types of malignant tumors:lung cancer,gastric cancer,liver cancer,colorectal cancer,breast cancer,esophageal cancer,cervical cancer,endometrial cancer,nasopharyngeal carcinoma,malignant lymphoma,leukemia,pancreatic cancer,ovarian cancer,prostate cancer,bladder cancer,brain cancer,biliary tract malignant tumors or gastrointestinal stromal *** patients were enrolled from 72 hospitals located in different regions of *** patients’nutritional status was evaluated based on the body mass index(BMI),loss of bodyweight,laboratory measurements and patient generated-subjective global assessment(PG-SGA)*** cancer patients’physical status and QoL were assessed by the Karnofsky Performance Status(KPS)questionnaire and the European Organization for Research and Treatment of Cancer(EORTC)QLQ-C30 questionnaire,*** From December 2013 to April 2016,23,994 patients hospitalized for cancer treatment(such as surgery,chemotherapy or radiotherapy)were enrolled in the *** patients included 12,494(52.9%)males and 11,124(47.1%)*** mean age was 55.8±13.7 *** proportions of patients in cancer stagesⅠ,Ⅱ,Ⅲ,Ⅳand uncertain were 11.5%,20.3%,27.5%,30.2%and 10.5%,*** the 23,994 inpatients,the proportions of patients who were underweight(BMI18.5 kg/m2),normal(18.5 kg/m2

Background Malnutrition is common in patients with cancer,and this adversely affects the survival and quality of life of *** Society for Nutritional Oncology issued a multi-center,large-scale,long-term follow-up prospective study,the Investigation on Nutrition Status and Clinical Outcome of Patients with Common Cancers in china(INSCOC study)since *** is an extension to the previous 2013-2020 study *** study still sought to:①address the prognostic impact of nutritional factors and quality of life on cancer patient survival;②describe the overall and cancer-specific incidence and/or distribution of malnutrition and different measurements of patient quality of *** and study design This is an observational,multi-centered,hospital-based prospective cohort *** collection will be performed at baseline(within 48 hours after patient admission),during the hospital stay and 30 days after hospital ***-up will be conducted for 1-20 years after *** primary outcome will be the all-cause mortality/overall survival,and secondary outcomes will be the length of hospital stay and costs of *** factors will include demographic characteristics,tumor characteristics,information about chronic diseases,hematological measurements(e.g.,red blood cell count,total lymphocyte counts,hemoglobin,albumin,prealbumin,creatinine,C-reactive protein,IL-6),anthropometric measurements(e.g.,height,weight,arm circumference,arm muscle circumference,triceps skinfold thickness,and waist circumference),body composition parameters,PG-SGA scores,quality of life(as indicated by the QLQ-C30 questionnaire),muscle mass(as indicated by the calf circumference),muscle strength(as indicated by the handgrip strength),muscle function(as indicated by the six-meter walking speed test)and physical status assessments(as indicated by the Karnofsky Performance Status scores).This clinical study protocol was approved by local Ethics Committees of

营养不良对恶性肿瘤患者的不利影响贯穿其整个病程.肿瘤患者的营养状况如何未见大样本报道,而且已有报道差异很大,本研究旨在调查中国常见恶性肿瘤患者营养不良状况及营养治疗现状.本次观察性多中心研究采用两阶段随机抽样方法抽取全国22个主要省市80家三甲医院共47488例16种常见恶性肿瘤住院患者,以患者主观整体评估(Patient-Generated Subjective Global Assessment,PG-SGA)评估患者营养状况,以定量评估区分营养不良严重程度:0~1分无营养不良,2~3分、4~8分、≥9分分别定义为可疑/轻度、中度、重度营养不良.方差分析比较不同特征肿瘤患者PG-SGA评分.肿瘤患者中、重度营养不良的发病率为58.2%(中度32.1%、重度26.1%),22.2%为可疑/轻度营养不良,只有19.6%的患者无营养不良.营养不良的发病存在瘤种、年龄、性别、肿瘤分期、治疗情况及地区差异:胰腺癌患者PG-SGA评分最高(9.58±5.74),乳腺癌患者PG-SGA评分最低(3.51±3.49);2±5.10);女性肺癌(5.87vs.5.60,P=0.006)和胃癌(8.36 vs.7.81,P1)PG-SGA评分高于男性;TNM分期较高的患者其PG-SGA评分也较高,其中胰腺癌Ⅲ、Ⅳ期患者PG-SGA评分最高,乳腺癌Ⅰ~Ⅲ期患者PG-SGA评分均低于4分(3.05±2.83);接受手术治疗的患者PG-SGA评分最高(6.22±4.74),而目前还没有接受任何治疗的患者评分最低(5.61±4.68);华中区域PG-SGA评分最低(4.82±4.16),华东区域PG-SGA评分最高(7.31±5.53).此外,在某些肿瘤类型中,不同医疗保险类型、受教育水平、职业、居住地、民族的患者PG-SGA评分也有显著差异,农民、小学及以下低教育水平患者营养状况最差.68.78%的肿瘤患者没有获得任何营养治疗,重度营养不良(PG-SGA≥9)肿瘤患者的无营养治疗比例仍然高达55.03%.以上结果说明,中国常见恶性肿瘤患者营养不良发生率高而营养治疗率较低,肿瘤患者的营养状况与其年龄、性别、肿瘤类型、TNM分期、治疗情况、行政区域、医疗保险类型、受教育水平、职业、民族等因素有关.我们呼吁重视肿瘤患者的营养状况及其营养治疗,重点关注上消化道、老年、农民、低教育水平及进展期肿瘤患者等营养不良高风险人群,整体提高肿瘤患者的临床营养治疗率.
