Dear Editor,Inflammatory bowel diseases(IBD)such as ulcerative colitis(UC)and Crohn's disease are affecting increasing numbers of patients worldwide.1 These chronic diseases significantly impact patients'quality of life as they are incurable and disabling.1 Patients with IBD have had an increasing rate of hospitalizations,half of all UC patients are hospitalized at least once during their lifetime,and readmissions after a UC-related hospitalization are common.

The sediment particles play a huge role in shaping the bed load transport.In this research,240 water-tunnel experiments are carried out to investigate the incipient velocity of the observation particles in two particle arrangements.To accurately predict the incipient velocity of the observation particles,the equation is conceived by the rolling instability mechanism.The incipient velocity equations and experimental data are used to analyze the trend of dispersive pressure and the effect of arrangement position on velocity.We find that it is appropriate to choose the coefficient of drag as 0.261 and the coefficient of lift as 0.198 for the incipient velocity equation of spherical particles on the hemispherical bed surface.Furthermore,the dispersive pressure is closely related to the flow state,particle size,and particle arrangement,which leads to the incipient velocity of the observation particle being at a minimum when the interference particle angle is 45°.Finally,the particle spacing and the projected area changed with the arrangements,directly affecting the incipient velocity of the observed particle.The analysis of four aspects for the coefficients,dispersive pressure,different particle spacing,and projected area will facilitate the prediction of particle incipient velocity,especially on hemispherical beds.

Fluorescence Anisotropy(FA)is an effective biochemical detection method based on molecular rotations.Graphene oxide(GO)has been extensively used as an FA amplifier.However,the enhancement of FA by GO alone is limited and the strong scattering of GO will easily make the measurement of FA inaccurate.In order to address these problems,an octopus-like DNA nanostructure(ODN)was designed and coupled with GO to enhance the FA together in this work.By mimicking the multi-clawed structure of the octopus,the ODN can be adsorbed on GO tightly,which not only could improve the sensitivity because of the double FA enhancement abilities of GO and ODN,but also could improve the specificity due to the decrease of the nonspecific interaction in complex samples.Furthermore,ODN could maintain a certain distance between the fluorophore and GO to reduce the fluorescence quenching efficiency of GO,which could improve the accuracy.This method has been applied for the detection of hepatitis B virus DNA(HBV-DNA)in a range of 1-50 nmol/L and the limit of detection(LOD)was 330 pmol/L.In addition,the proposed method has been successfully utilized to detect HBV-DNA in human serum,indicating that this method has a great practical application prospect.

OBJECTIVES To establish a scoring system combining the ACEF score and the quantitative blood flow ratio(QFR) to improve the long-term risk prediction of patients undergoing percutaneous coronary intervention(PCI).METHODS In this population-based cohort study, a total of 46 features, including patient clinical and coronary lesion characteristics, were assessed for analysis through machine learning models. The ACEF-QFR scoring system was developed using 1263consecutive cases of CAD patients after PCI in PANDA Ⅲ trial database. The newly developed score was then validated on the other remaining 542 patients in the cohort.RESULTS In both the Random Forest Model and the Deep Surv Model, age, renal function(creatinine), cardiac function(LVEF)and post-PCI coronary physiological index(QFR) were identified and confirmed to be significant predictive factors for 2-year adverse cardiac events. The ACEF-QFR score was constructed based on the developmental dataset and computed as age(years)/EF(%) + 1(if creatinine ≥ 2.0 mg/d L) + 1(if post-PCI QFR ≤ 0.92). The performance of the ACEF-QFR scoring system was preliminarily evaluated in the developmental dataset, and then further explored in the validation dataset. The ACEF-QFR score showed superior discrimination(C-statistic = 0.651;95% CI: 0.611-0.691, P libration(Hosmer–Lemeshow χ^(2)= 7.070;P = 0.529) for predicting 2-year patient-oriented composite endpoint(POCE). The good prognostic value of the ACEF-QFR score was further validated by multivariable Cox regression and Kaplan–Meier analysis(adjusted HR = 1.89;95% CI: 1.18–3.04;log-rank P linical and coronary lesion-based functional variables(ACEF-QFR)was developed, and its ability for prognostic prediction in patients with PCI was further validated to be significantly better than the post-PCI physiological

In this study,the effects of soluble readily biodegradable COD(sCOD)and particulate slowly biodegradable COD(pCOD)on anammox process were investigated.The results of the longterm experiment indicated that a low sCOD/N ratio of 0.5 could accelerate the anammox and denitrification activity,to reach as high as 84.9%±2.8%TN removal efficiency.Partial denitrification-anammox(PDN/anammox)and denitrification were proposed as the major pathways for nitrogen removal,accounting for 91.3% and 8.7% of the TN removal,respectively.Anammox bacteria could remain active with high abundance of anammox genes to maintain its dominance.Candidatus Kuenenia and Thauera were the predominant genera in the presence of organic matter.Compared with sCOD,batch experiments showed that the introduction of pCOD had a negative effect on nitrogen removal.The contribution of denitrification to nitrogen removal decreased from approximately 14%to 3% with increasing percentage of pCOD.In addition,the analysis result of the process data using an optimized ASM1 model indicated that high percentage of pCOD resulted in serious N_(2)O emission(the peak value up to 0.25mg N/L),whichwas likely due to limited mass diffusion and insufficient available carbon sources for denitrification.However,a high sCOD/N ratio was beneficial for alleviating N_(2)O accumulation.

Background: Erosive esophagitis (EE) is a gastroesophageal reflux disease characterized by mucosal breaks in the esophagus. Proton pump inhibitors are widely used as maintenance therapy for EE, but many patients still relapse. In this trial, we evaluated the noninferiority of vonoprazanvs. lansoprazole as maintenance therapy in patients with healed EE.Methods: We performed a double-blind, double-dummy, multicenter, phase 3 clinical trial among non-Japanese Asian adults with endoscopically confirmed healed EE from April 2015 to February 2019. Patients from China, South Korea, and Malaysia were randomized to vonoprazan 10 mg or 20 mg once daily or lansoprazole 15 mg once daily for 24 weeks. The primary endpoint was endoscopically confirmed EE recurrence rate over 24 weeks with a noninferiority margin of 10% using a two-sided 95% confidence interval (CI). Treatment-emergent adverse events (TEAEs) were recorded.Results: Among 703 patients, EE recurrence was observed in 24/181 (13.3%) and 21/171 (12.3%) patients receiving vonoprazan 10 mg or 20 mg, respectively, and 47/184 (25.5%) patients receiving lansoprazole (differences: -12.3% [95% CI, -20.3% to-4.3%] and -13.3% [95% CI, -21.3% to -5.3%], respectively), meeting the primary endpoint of noninferiority to lansoprazole in preventing EE recurrence at 24 weeks. Evidence of superiority (upper bound of 95% CI <0%) was also observed. At 12 weeks, endoscopically confirmed EE recurrence was observed in 5/18, 2/20, and 7/20 of patients receiving vonoprazan 10 mg, vonoprazan 20 mg, and lansoprazole, respectively. TEAEs were experienced by 66.8% (157/235), 69.0% (156/226), and 65.3% (158/242) of patients receiving vonoprazan 10 mg, vonoprazan 20 mg, and lansoprazole, respectively. The most common TEAE was upper respiratory tract infection in 12.8% (30/235) and 12.8% (29/226) patients in vonoprazan 10 mg and 20 mg groups, respectively and 8.7% (21/242) patients in lansoprazole group.Conclusion: Vonoprazan maintenance therapy was well-tolerated and noninferior to lansoprazole f

BACKGROUND The prognostic value of coronary collateral circulation(CC)in patients undergoing chronic total occlusion(CTO)percutaneous coronary intervention(PCI)is underdetermined.The purpose of the study was to assess the prognostic value of current two CC grading systems and their association with long-term outcomes in patients with CTO underwent PCI.METHODS We consecutively enrolled patients with single-vessel CTO underwent PCI between January 2010 and December2013.All patients were categorized into well-developed or poor-developed collaterals group according to angiographic Werner's CC(grade 2 vs.grade 0–1)or Rentrop(grade 3 vs.grade 0–2)grading system.The primary endpoint was 5-year cardiac death.RESULTS Of 2452 enrolled patients,the overall technical success rate was 74.1%.Well-developed collaterals were present in686 patients(28.0%)defined by Werner's CC grade 2,and in 1145 patients(46.7%)by Rentrop grade 3.According to Werner's CC grading system,patients with well-developed collaterals had a lower rate of 5-year cardiac death compared with those with poordeveloped collaterals(1.6%vs.3.3%,P=0.02),those with suboptimal recanalization was associated with higher rate of 5-year cardiac death compared with optimal recanalization(4.7%vs.0.8%,P=0.01)and failure patients(4.7%vs.1.6%,P=0.12).However,the similar effect was not shown in Rentrop grading system.CONCLUSIONS In patients with the single-vessel CTO underwent PCI,well-developed collaterals by Werner's CC definition were associated with lower rate of 5-year cardiac death.Werner's CC grading system had a greater prognostic value than Rentrop grading system in patients with CTO underwent PCI.

Hepatic ischemia/reperfusion injury(HIRI) is a serious complication that occurs following shock and/or liver surgery. Gut microbiota and their metabolites are key upstream modulators of development of liver injury. Herein, we investigated the potential contribution of gut microbes to HIRI.Ischemia/reperfusion surgery was performed to establish a murine model of HIRI. 16 S r RNA gene sequencing and metabolomics were used for microbial analysis. Transcriptomics and proteomics analysis were employed to study the host cell responses. Our results establish HIRI was significantly increased when surgery occurred in the evening(ZT12, 20:00) when compared with the morning(ZT0, 08:00);however, antibiotic pretreatment reduced this diurnal variation. The abundance of a microbial metabolite3,4-dihydroxyphenylpropionic acid was significantly higher in ZT0 when compared with ZT12 in the gut and this compound significantly protected mice against HIRI. Furthermore, 3,4-dihydroxyphenylpropionic acid suppressed the macrophage pro-inflammatory response in vivo and in vitro. This metabolite inhibits histone deacetylase activity by reducing its phosphorylation. Histone deacetylase inhibition suppressed macrophage pro-inflammatory activation and diminished the diurnal variation of HIRI. Our findings revealed a novel protective microbial metabolite against HIRI in mice. The potential underlying mechanism was at least in part, via 3,4-dihydroxyphenylpropionic acid-dependent immune regulation and histone deacetylase(HDAC) inhibition in macrophages.
