Background: Oxidative stress and inflammation are related to the pathophysiology of diabetes mellitus (DM), being involved in the development of micro-and macrovascular complications. Physical activity is beneficial for DM patients, but little is known about the relationship between redox and inflammation biomarkers and the level of physical activity in these patients. Based on this, this research aims to evaluate the effects of physical activity level on redox stress parameters and inflammatory markers in T2DM patients. Methods: Eighty-four patients with T2DM were divided according to their physical activity level: group A (n = 48), sedentary;group B (n = 11) active (3 times a week, 150 min) and group c (n = 25), highly active (5 times a week, 150 - 300 mins, at least). Anthropometric and biochemical parameters, superoxide dismutase (SOD) and glutathione peroxidase (GPx) activities, as well as GSH, sRAGE and IcAM-1 levels were assessed. Results: Glycated haemoglobin, total cholesterol, and LDL-cholesterol levels were lower in the highly active group in comparison to other groups. Plasma SOD activity was higher in group c compared to Group A, while IcAM-1 levels were significantly higher in group B when compared to other groups. conclusion: Our results suggest that the practice of physical activity is beneficial to T2DM patients, especially at high volume and frequency.

Profile of People Living with HIV/AIDS in a Large Municipality of Sao Paulo State, Brazil （2012-2013）

HIV/AIDS has brought to light the challenge of incorporating the many influences between living conditions, social characteristics and health services performance to an adequate care for PLWHA （people living with AIDS）. Vulnerability of these populations is under the responsibility of specialized care units whose assistance does not always occur according to their real needs and demands. Therefore, this study aimed to analyze demographic, social and clinical profiles of PLWHA, as well as their follow-up in SS （Specialized Health Services） in Ribeir^o Preto, Brazil. It is a descriptive study conducted by the application of structured questionnaires to 253 patients with HIV/AIDS in follow-up during the years of 2012-2013. Variables were analyzed by descriptive statistics procedures. The findings pointed out gender parity, aging population, low education and economic predominance of class c. Regarding clinical characteristics, there was a predominance of asymptomatic individuals, with no clinical manifestations of AIDS or major comorbidities. The main mode of transmission was through sexual contact. The results led to the need of adequating the assistance provided to the specificities inherent to PLWHA. The care provision should cross an interdisciplinary perspective, targeting recognition of problems and ensuring comprehensive health care adequate to users＇ needs and demands.

Parathyroid ultrasonography and bone metabolic profile of patients on dialysis with hyperparathyroidism

AIM To evaluate the parathyroid ultrasonography and define parameters that can predict poor response to treatment in patients with secondary hyperparathyroidism due to renal *** This cohort study evaluated 85 patients with chronic kidney disease stage V with parathyroid hormone levels above 800 pg/mL. All patients underwent ultrasonography of the parathyroids and the following parameters were analyzed: Demographic characteristics(etiology of chronic kidney disease, gender, age, dialysis vintage, vascular access, use of vitamin D), laboratory(calcium, phosphorus, parathyroid hormone, alkaline phosphatase, bone alkaline phosphatase), and the occurrence of bone changes, cardiovascular events and death. The χ~2 test were used to compare proportions or the Fisher exact test for small sample frequencies. Student t-test was used to detect differences between the two groups regarding continuous *** Fifty-three patients(66.4%) had parathyroid nodules with higher levels of parathyroid hormone, calcium and phosphorus. Sixteen patients underwent parathyroidectomy and had higher levels of phosphorus and calcium × phosphorus product(P = 0.03 and P = 0.006, respectively). They also had lower mortality(32% vs 68%, P = 0.01) and lower incidence of cardiovascular or cerebrovascular events(27% vs 73%, P = 0.02). calcium × phosphorus product above 55 mg^2/dL^2 [RR 1.48(1.06, 2.08), P = 0.03], presence of vascular calcification [1.33(1.01, 1.76), P = 0.015] and previous occurrence of vascular events [RR 2.25(1.27, 3.98), P ctors for mortality in this population. Evaluation of the Eta Simulations Nested in Three Global climate Models

To provide long-term simulations of climate change at higher resolution, Regional climate Models (RcMs) are nested in global climate models (GcMs). The objective of this work is to evaluate the Eta RcM simulations driven by three global models, the HadGEM2-ES, BESM, and MIROc5, for the present period, 1961-1990. The RcM domain covers South America, central America, and caribbean. These simulations will be used for assessment of climate change projections in the region. Maximum temperatures are generally underestimated in the domain, in particular by MIROc5 driven simulations, in summer and winter seasons. Larger spread among the simulations was found in the minimum temperatures, which showed mixed signs of errors. The spatial correlations of temperature simulations against the cRU observations show better agreement for the MIROc5 driven simulations. The nested simulations underestimate precipitation in large areas over the continent in austral summer, whereas in winter overestimate occurs in southern Amazonia, and underestimate in southern Brazil and eastern coast of Northeast Brazil. The annual cycle of the near-surface temperature is underestimated in all model simulations, in all regions in Brazil, and in most of the year. The temperature and precipitation frequency distributions reveal that the RcM and GcM simulations contain more extreme values than the cRU observations. Evaluations of the climatic extreme indicators show that in general hot days, warm nights, and heat waves are increasing in the period, in agreement with observations. The Eta simulations driven by HadGEM2-ES show wet trends in the period, whereas the Eta driven by BESM and by MIROc5 show trends for drier conditions.

Background: cystinosis is a multisystemic autosomal recessive deficiency of the lysosomal membrane transporter protein (cystinosin) caused by mutations in cTNS gene. Objective: This study summarizes the Portuguese experience in the diagnosis and management of patients with this rare disease over the past few years and reports recurrent mutations in the cTNS gene. Methods: Unrelated patients from different pediatric and adult hospitals all over Portugal with non-nephrotic proteinuria, hypercalciuria, hypokalemia impaired proximal reabsorption of amino acids, glycosuria and hypophosphatemia, suggestive of a Fanconi syndrome and ocular problems, were studied. Intra-leukocyte cystine levels were determined and molecular analysis was performed, to determine the presence or absence of the 57-kb deletion in cTNS, followed by direct sequencing of the coding exons of cTNS. Results: From 1998 to 2017, twenty-one cystinotic patients were biochemically diagnosed. From the remaining seventeen (four deceased), eleven were studied for cTNS gene. Five out of eleven patients were homozygous for the 57-kb deletion (10/22;45.5%), and other five were compound heterozygous for this variant (15/22;68.2%). The other mutations found were p.Q128X (c.721 c>T;2/22), p.S139F (c.755 c>T;4/22) and c.18-21delGAcT (p.T7FfsX7;1/22). All of these seventeen cystinotic patients are in treatment. Approximately 84% are adults, 16% are young children, and 54.5% are kidney transplant recipient. conclusions: The authors would like to emphasize the importance of first screening for the 57-kb deletion since it is very common in our population. This genetic study is the first in our country and it could be the basis for future genetic counseling in Portuguese population.
