Re-bleeding events in patients with obscure gastrointestinal bleeding after negative capsule endoscopy

AIM: To investigate long-term re-bleeding events after a negative capsule endoscopy in patients with obscure gastrointestinal bleeding(OGIB) and the risk factors associated with the ***: Patients referred to Hospital Egas Moniz(Lisboa, Portugal) between January 2006 and October 2012 with OGIB and a negative capsule endoscopy were retrospectively analyzed. The following study variables were included: demographic data, comorbidities, bleeding-related drug use, hemoglobin level, indication for capsule endoscopy, post procedure details, work-up and follow-up. Re-bleeding rates and associated factors were assessed using a Cox proportional hazard analysis. The Kaplan-Meier method was used to estimate the cumulative incidence of re-bleeding at 1, 3 and 5 years, and the differences between factors were ***: The study population consisted of 640 patients referred for OGIB investigation. Wireless capsule endoscopy was deemed negative in 113 patients(17.7%). A total of 64.6% of the population was female, and the median age was 69 years. The median follow-up was forty-eight months(interquartile range 24-60). Re-bleeding occurred in 27.4% of the cases. The median time to re-bleeding was fifteen months(interquartile range 2-33). In 22.6%(n = 7) of the population, small-bowel angiodysplasia was identified as the culprit lesion. A univariate analysis showed that age > 65 years old, chronic kidney disease, aortic stenosis, anticoagulant use and overt OGIB were risk factors for re-bleeding; however, on a multivariate analysis, there were no risk factors for re-bleeding. The cumulative risk of re-bleeding at 1, 3 and 5 years of follow-up was 12.9%, 25.6% and 31.5%, *** who presented with overt OGIB tended to rebleed sooner(median time for re-bleeding: 8.5 mo vs 22 mo).CONCLUSION: Patients with OGIB despite a negative capsule endoscopy have a significant re-bleeding risk; therefore, these patients require an extended follow-up strategy.

Provisioning of Inter-Domain QoS-Aware Services

Cooperation among service providers, network providers, and access providers in the Internet allows the creation of new services to offer to customers that are in other domains, thus increasing revenue. However, the Internet heterogeneous environment, where each provider has its own policies, infrastructure and business goals, hinders the deployment of more advanced communication services. This paper presents a Quality of Service （QoS） for Inter-Domain Services （QIDS） model that allows inter-domain QoS-aware services to be defined, configured, and adapted in a dynamic and on-demand fashion, among service providers. This is accomplished by： 1） the use of a common communication channel （business layer） where service providers publish and search for services, and interact with each other to contract and manage these services; 2） the templates to specify the business and technical characteristics of the services; 3） the automatic composition of services using service dements （smaller services） a^cording to performance and service-specific QoS parameters; and 4） the creation and enforcement of configuration rules for the underlying infrastructure. A prototype was implemented to validate QIDS and performance tests were conducted on an inter-domain Border Gateway Protocol （BGP）/Multiprotocol Label Switching （MPLS） Virtual Private Network （VPN） scenario.

Matching dependencies （MDs） are used to declaratively specify the identification （or matching） of cer- tain attribute values in pairs of database tuples when some similarity conditions on other values are satisfied. Their en- forcement can be seen as a natural generalization of entity resolution. In what we call the pure case of MD enforce- ment, an arbitrary value from the underlying data domain can be used for the value in common that is used for a match- ing. However, the overall number of changes of attribute val- ues is expected to be kept to a minimum. We investigate this case in terms of semantics and the properties of data clean- ing through the enforcement of MDs. We characterize the in- tended clean instances, and also the clean answers to queries, as those that are invariant under the cleaning process. The complexity of computing clean instances and clean query an- swering is investigated. Tractable and intractable cases de- pending on the MDs are identified and characterized.

Diagnosis and management of choledocholithiasis in the golden age of imaging, endoscopy and laparoscopy

Biliary lithiasis is an endemic condition in both Western and Eastern countries, in some studies affecting 20% of the general population. In up to 20% of cases, gallbladder stones are associated with common bile duct stones (CBDS), which are asymptomatic in up to one half of cases. Despite the wide variety of examinations and techniques available nowadays, two main open issues remain without a clear answer: how to cost-effectively diagnose CBDS and, when they are finally found, how to deal with them. CBDS diagnosis and management has radically changed over the last 30 years, following the dramatic diffusion of imaging, including endoscopic ultrasound (EUS) and magnetic resonance cholangiography (MRC), endoscopy and laparoscopy. Since accuracy, invasiveness, potential therapeutic use and cost-effectiveness of imaging techniques used to identify CBDS increase together in a parallel way, the concept of “risk of carrying CBDS” has become pivotal to identifying the most appropriate management of a specific patient in order to avoid the risk of “under-studying” by poor diagnostic work up or “over-studying” by excessively invasive examinations. The risk of carrying CBDS is deduced by symptoms, liver/pancreas serology and ultrasound. “Low risk” patients do not require further examination before laparoscopic cholecystectomy. Two main “philosophical approaches” face each other for patients with an “intermediate to high risk” of carrying CBDS: on one hand, the “laparoscopy-first” approach, which mainly relies on intraoperative cholangiography for diagnosis and laparoscopic common bile duct exploration for treatment, and, on the other hand, the “endoscopy-first” attitude, variously referring to MRC, EUS and/or endoscopic retrograde cholangiography for diagnosis and endoscopic sphincterotomy for management. Rheumatoid arthritis （RA） is a chronic inflammatory disease characterized by synovial joint inflammation and cartilage and bone tissue destruction. Although there exist some treatment strategies for RA, they are not completely safe and effective. Therefore, it is important to develop and test new drugs for RA that specifically target inflamed/swollen joints and simultaneously attenuate other possible damages to healthy tissues. Nanotechnology can be a good alternative to consider when envisioning predse medication for treating RA. Through the use of nanopartides, it is possible to increase bioavailability and bioactivity of therapeutics and enable selective targeting to damaged joints. Herein, recent studies using nanoparticles for the treatment of RA, namely with liposomes, polymeric nanoparticles, dendrimers, and metallic nanoparticles, have been reviewed. These therapeutic strategies have shown great promise in improving the treatment over that by traditional drugs. The results of these studies confirm that feasibility of the use of nanopartides is mainly due to their biocompatibility, low toxicity, controlled release, and selective drug delivery to inflamed tissues in animal RA models. Therefore, it is possible to claim that nanotechnology will, in the near future, play a crucial role in advanced treatments and patient-specific therapies for human diseases such as RA.

DNA damage levels in systemic lupus erythematosus patients with low disease activity:An evaluation by comet assay

Oxidative stress has been implicated in the inflammatory process of Systemic Lupus Erythematosus (SLE), particularly by the formation of anti-DNA autoantibodies, which can lead to DNA damage. The aim of this study was to investigate, through comet assay, whether the level of DNA damage in SLE patients is different from that of healthy subjects. Twenty-five adult SLE patients with SLEDAI up to ten, and 25 healthy subjects were paired according to age, gender and Body Mass Index (BMI). Other anthropometric variables were also assessed. Comet assay was assessed as the marker of oxidative stress described as DNA Damage (DD) percentage. Waist Circumference (WC), Hip Circumference (HC) and BMI were also performed. Exclusion criteria for patients and controls comprised smoking and other chronic disorders. Level of damage index was remarkably higher in SLE patients than in controls, and no significant differences between the groups were found for age, BMI, WC and HC. No stratification concerning gender was performed, since there were just two males per group. No correlation was observed between BMI and DD (%). DD increased in SLE, which reflects the oxidant/antioxidant imbalance in these patients. These findings support an association between oxidative stress and SLE. This stronger correlation observed in patients with low disease activity may be useful in elucidating the mechanisms of disease pathogenesis.

Long term results of no- alcohol laser epithelial keratomileusis and photorefractive keratectomy for myopia

AIM: To evaluate the long term clinical results of mechanical no-alcohol-assisted laser epithelial keratomileusis(LASEK) versus standard photorefractive keratectomy(PRK) for low-moderate ***: Twenty-five eyes treated with LASEK and twenty-five eyes treated with PRK were evaluated with a mean follow-up duration of 60 mo. Mechanical separation of the epithelium was performed with blunt spatula and without application of alcohol. Laser ablation was performed with the MEL-70 excimer laser. All patients were examined daily until epithelial closure; at 1, 3, 6,and 12 mo, and every year subsequently. Main outcome measures were uncorrected distance visual acuity(UDVA), corrected distance visual acuity(CDVA),manifest refraction, haze, efficacy and safety ***: Twenty-one eyes and 22 eyes completed follow-up of 60 mo in LASEK and PRK group respectively. Manifest refraction at 60 mo follow-up was-0.01 and 0.26 in LASEK and PRK group respectively. In the LASEK group mean UDVA and mean CDVA after60 mo were 20/22 and 20/20 respectively(P >0.01). In the PRK group mean UDVA and mean CDVA at 60 mo follow-up were 20/20 and 20/20 after 60mo(P >0.01). The efficacy indexes were 0.87 and 0.95, and the safety indexes were1.25 and 1.4 respectively for LASEK group and PRK ***: Both standard PRK and no-alcohol LASEK offer safe and effective correction of low-moderate myopia in the long term without any statistically significant difference between the two groups.
