Adaptation of a Brazilian Version of the North/Northeast Region for the Brief Pain Inventory

Purpose: The adaptation of BPI-B into North/Northeast of Brazil. The purpose of this study was the translation and adaptation of the BPI to Portuguese language, as spoken in Brazil, aiming at its posterior usage to measure both intensity and interference of pain in cancer patients’ life. Methods: The BPI-B was developed from the original BPI, using back-translation and committee review. The Back Translation was compared to the original BPI, as a result, the North/Northeast Brazilian version proved to have the same goals, and is similar to other current versions, observing its psychometrics properties. The inventory presented a final sample of 475 patients, whose average age was 54.37 years old (DP = 14.56), most female (58.9%). One hundred ninety-six patients in elementary school took part. It had its objective to group multiple indicators that responded to validation, precision and parsimony criteria. The patients answered the BPI at the very moment they were diagnosed as cancer cases. All of them were above 18 years old and they were also undergoing treatment at Cancer Ambulatory in a Hospital in Ceará, Brazil. The retest was carried, after about a month of the first application. In order to verify the reliability of inventory adaptation, the exploratory factorial analysis was used as the oblique rotation axis. Results: Exploratory factor analysis confirmed two underlying dimensions, pain severity, and pain interference, with Cronbach’s α 0.833 and 0.733, respectively. Conclusion: A proposition of a north/northeast Brazilian version of BPI turned out to be adequate, gathering evidences of adaptation and internal consistency similar to already validated versions.

Structural Characteristics of Mulato Grass I under Different Cutting Heights

This work aimed to evaluate the effects of different individual heights of cut on structural characteristics of Brachiaria hibrida cv. Mulato I in four periods of the year. The study was carried out at the Fazenda Experimental Vale do Acaraú, in Sobral, Ceará, Brazil, from January to December, 2008. The experimental design was a split plot, with the plots representing four pasture conditions through manual cuts of 10 cm, 20 cm, 30 cm and 40 cm heights, which were done every 30 days. Each treatment had three replications. The subplots were represented by four periods of evaluation: January-February-March, April-May-June, July-August-September and October-November-December. Fertilization was performed with 350 kg of N/ha, divided into monthly applications. Irrigation was conducted in alternate days during the dry season. The variables studied were: mean height before cutting (cm);dry matter content (%);total dry matter production (t/ha);percentage of leaves;percentage of stems;percentage of dead material;leaf blade/stem ratios;number of total leaves per tiller;number of live leaves per tiller;average size of leaves;and tiller density. Cutting at 10 cm caused a decrease in total dry matter production, underestimating the potential production. Cutting heights between 20 and 30 cm contributed positively to increase the total number of leaves, number of green leaves, average size of the leafs and total dry matter production. Under the present experimental conditions, cutting around 40 cm was not recommended because of a decreasing in the leaf/stem ratio and a increasing in stem fraction and dry matter content, compromising the structure of the canopy.

Holistic Evaluation of the Morbidity Due to Diabetes Mellitus Type 2 and Its Main Risk Factors in the State of San Luis Potosi, Mexico

Objective: To evaluate the morbidity due to diabetes mellitus type 2 within the State of San Luis Potosí, México, through a strong methodology, through which the multivariate relations were identified of the main social and environmental determiners in the disease, thus managing to quantify their respective levels of responsibility. Material and Methods: This evaluation began as a hypothesis of a multicasual theoretical model on diabetes mellitus and its main determining factors, which was analyzed through the application of multivariate exploratory statistical methodologies and confirmed as it is the case of the principal components analysis and the structural equation models. Results: Three components were extracted that explain the 96% of the total variance of the indicators;the main risk factors which were identified in the first component were, the use of the car, age, homes with TV use, urban life and feminine population;the indicators from the second and third component have little influence in the impact of the disease. Conclusions: the study shows the usefulness of the model for the analysis and prioritization of the environmental and social determiners of the disease, information that could sustain the design of public guidelines for the prevention and control of the analyzed disease.

AIM: To evaluate the demographic characteristics and clinical phenotypes of inflammatory bowel disease(IBD) in a geographic area in Northeastern ***: This retrospective study was conducted at the Hospital of the Federal University of Piauí in Northeastern Brazil. Demographic characteristics and clinical phenotypes of IBD were analyzed in relation to the time of diagnostic confirmation, which was defined as the date of disease onset. Data were collected between January 2011 and December 2012 and included all census patients 18 years of age or older during that period for whom there was diagnostic confirmation of Crohn's disease(CD), ulcerative colitis(UC), or unclassified colitis according to the Montreal criteria. We also analyzed the period of time between the onset of clinical manifestations and the diagnosis of IBD(delay in the diagnosis). Statistical analyses included means and standard deviations for numeric variables and the Pearson χ2 adherence test for nominal variables. The annual index occurrence and overall prevalence of IBD at our institution were also calculated, with P values < 0.05 indicating statistical significance. This study was approved by the Institutional Ethics and Research ***: A total of 252 patients with IBD were included, including 152(60.3%) UC patients and 100(39.7%) CD patients. The clinical and demographic characteristics of all patients with IBD showed a female to male ratio of 1.3:1.0 and a mean age of35.2(SD = 14.5) years. In addition, the majority of patients were miscegenated(171, 67.9%), had received higher education(157, 62.4%), lived in urban areas(217, 86.1%), and were under the age of 40 years(97, 62.5%). For patients with CD, according to the Montreal classification, the predominant features present from the onset of disease were an age between 17 and 40 years(A2); colonic disease location(L2); and nonstricturing, nonfistulizing disease behavior(B1). Comparative Study between Endometrial Resection and Electrocoagulation in Patients with Abnormal Uterine Bleeding

Objective: To compare clinical outcomes between two first-generation endometrial ablation techniques. Design: Prospective comparative coorte. Setting: Tertiary public hospital, university teaching center. Seventy-three patients with abnormal uterine bleeding unresponsive to clinical treatment submitted to endometrial ablation from October 2011 to September 2013. Methods and Main Outcome Measures: Patients were assigned to either monopolar U-shaped electrode resection with rollerball electrocoagulation (group A, n = 36) or rollerball electrocoagulation alone (group B, n = 37). Mean follow-up length was 359 (280 - 751) and 370 days (305 - 766) in groups A and B, respectively. Bleeding pattern, associated symptoms, failure/success rates were assessed 30, 90, 180 and 360 days post-procedure. Findings: Patient characteristics were similar in both groups (P ≥ 0.05). Surgery duration (mean of 48.5 [±12.0] vs. 31.9 [±5.6] min, P Conclusions: Endometrial ablation using rollerball electrocoagulation alone may be considered safer than resection with rollerball electrocoagulation, which requires shorter surgical time and less distention medium, and is associated with lower postoperative vaginal discharge incidence. Success rate did not statistically differ between groups, but study parameters in absolute values and percents were superior in group B.

Differential Growth of Genotypes of Physic Nut Conditioned by Nitrogen Fertilization

The adequate supply of nitrogen is essential for the plant metabolism. This nutrient has an irreplaceable role on the vegetative and reproductive growth of physic nut;therefore the correct management of the fertilization is very important, particularly in tropical regions, which present considerable losses of nitrogen by leaching and volatilization processes. This study was made with the objective of evaluating the growth of genotypes of physic nut conditioned by nitrogen fertilization. The experiment was conducted in controlled environment, following a factorial scheme 12 × 4, with 12 Brazilian genotypes of Jatropha curcas L. and 4 levels of nitrogen fertilization (0%, 50%, 100% and 150% of the recommendation), in completely randomized design, with four replications. The growth of the genotypes was evaluated at 100 days of cultivation. Positive response to the increase in the nitrogen supply was observed in most genotypes, with gain in plant height, stem diameter, number of leaves, leaf area and root volume. The levels of nitrogen fertilization promoted differential growth between genotypes, being possible to identify genotypes with superior growth for each level.

Comparative Study on Cerebral Processing among Elderly People with Either Intellectual or Mechanical Work Activities, by Means of Electroencephalographic Techniques

Background: The cognitive capacity is one of the determinants of quality of life in old age. In EEG, gamma rhythm represented electrical correlates of execution of cognitive activities. Objective: To compare the cortical areas activated during executive activities through the gamma rhythm (25 - 45 Hz) and the performance in the Frontal Assessment Battery (FAB) among older people who exercised intellectual and mechanical work during their lifetime. Methods: We compared the electroencephalographic record of the gamma rhythm potential (25 - 45 Hz) in the cortical quadrants during the execution of the FAB among 13 elderly people who exercised intellectual activity and 12 elderly people who performed mechanical work. Results: In the FAB, the intellectual group obtained a better performance with a mean score of 16.2/18, while the mechanical group obtained an average score of 14.9/18. Only a significant difference between groups was observed in the similarity test, with intellectual group leadership. There was no significant difference between groups in the overall analysis of gamma rhythm potential in the brain quadrants during the tests. Conclusion: Elderly people who performed intellectual activity performed better than the elderly who exercised mechanical work in the execution of the Frontal Evaluation Battery (FAB). There was a greater quantification of gamma rhythm in the group of elderly individuals who exercised intellectual activity, compared to those who exercised mechanical activity.

Decomposition of the interaction of common black bean group genotypes with the environment

The purpose of this paper was to assess which environmental factor(s) (location, sowing season or years) in the common black bean genotypes interaction was more expressive and to verify the existence of genotypes with broad and/or specific adaptations to sowing season. Data of grain yield of thirteen genotypes were tested in twelve trials of value of cultivation and use during 2003 and 2004 in the south-central region of Brazil. Analyses of variance were performed with the decomposition of genotypes × environments interaction, which allowed a partial isolation of factors, and subsequently, eight trials were used which allowed a complete isolation of factors. The most important interactions were genotypes × years (R2 = 2.5%) and genotypes × locations (R2 = 1.9%), while genotypes × sowing seasons was the least important (R2 = 1.4%). Thus, it is more important to evaluate the genotypes in different locations and years than at different seasons. Most genotypes showed wide adaptation, but some lines showed strong specific adaptation.

Novel pharmacological therapy in type 2 diabetes mellitus with established cardiovascular disease: Current evidence

Cardiovascular diseases(CVDs) remain the leading cause of death in the world and in most developed countries. Patients with type 2 diabetes mellitus(T2DM)suffer from both microvascular and macrovascular diseases and therefore have higher rates of morbidity and mortality compared to those without T2DM. If current trends continue, the Center for Disease Control and Prevention estimates that 1 in 3 Americans will have T2DM by year 2050. As a consequence of the controversy surrounding rosiglitazone and the increasing prevalence of diabetes and CVDs, in 2008 the Food and Drug Administration(FDA) established new expectations for the evaluation of new antidiabetic agents, advising for pre and,in some cases, post-marketing data on major cardiovascular events. As a direct consequence, there has been a paradigm shift in new antidiabetic agents that has given birth to the recently published American Diabetes Association/European Association for the Study of Diabetes consensus statement recommending sodium-glucose cotransporter-2 inhibitors(SGLT2i) and glucagon like peptide-1 receptor agonists(GLP-1RA) in patients with T2DM and established CVD. As a result of over a decade of randomized placebo controlled cardiovascular outcome trials, the aforementioned drugs have received FDA approval for risk reduction of cardiovascular(CV) events in patients with T2DM and established CV ***2i have been shown to have a stronger benefit in patients with congestiveheart failure and diabetic kidney disease when compared to their GLP-1RA counterparts. These benefits are not withstanding additional considerations such as cost and the multiple FDA Black Box warnings. This topic is currently an emerging research area and this mini-review paper examines the role of these two novel classes of drugs in patients with T2DM with both confirmed, and at risk for, CVD.

Genetic Variability among Genotypes of Physic Nut Regarding Seed Biometry

With probable center of origin in Brazil, the species Jatropha curcas L., known as physic nut, is a tropical oilseed with potential for cultivation aiming at the production of biodiesel. This study was conducted with the objective of investigating the genetic variability regarding the morphology of seeds of physic nut, using biometric analyses to identify variables that have potential for the study of the diversity of the species and the classification of genotypes. Seeds of 22 genotypes of physic nut, from the germplasm bank of Embrapa Agroenergia (Brazil) were evaluated regarding characteristics of size and mass. The genotypes of Jatropha curcas L. selected by the Brazilian breeding program presented high diversity for characteristics of the seeds, allowing the exploration of this genetic variability to classify genotypes in different groups. In addition, variables related to the biomass of the seeds present high relative contributions to the diversity observed in the genotypes.
