Volumetric intravascular ultrasound comparisons of drug-eluting stent thrombosis and in-stent restenosis

Objectives We compared intravascular ultrasound (IVUS) findings of drug-eluting stent (dES)-treated lesions that developed stent thrombosis versus in-stent restenosis (ISR) to identify underlying mechanical differences. Methods IVUS findings in 15 post-dES thrombosis patients were compared with 45 matched ISR patients who had no evidence of stent thrombosis. Results Minimum stent area [MSA, (3.7±0.8) mm2 vs (4.9±1.8) mm2, P=0.01], minimum stent diameter [(1.9±0.3) mm vs (2.3±0.4) mm, P=0.005], mean stent area [(5.2±0.8) mm2 vs (7.2±2.1) mm2, Pd both focal [MSA/reference lumen area, (54.7±15.9)% vs (75.0±20.1)%, P=0.001] and diffuse stent expansion [mean stent area/reference lumen area, (76.6±23.0)% vs (110.3±23.3)%, Pbosis group (vs the ISR group). An MSA d in the stent thrombosis group (vs the ISR group). The MSA site occurred more frequently in the proximal stent segment within the stent thrombosis group compared to the ISR group (60% vs 24.4%, P=0.01). There were no differences in edge dissection, stent fracture, or stent-vessel wall malapposition between the two groups. Independent predictors of stent thrombosis were diffuse stent expansion (OR=1.5, P=0.03) and proximal location of the MSA site (OR=12.7, P=0.04). Conclusion dES-treated lesions that develop thrombosis or restenosis are often underexpanded. Underexpansion appears to be more severe in dES-thrombosis lesions. Lesions with diffuse underexpansion and a proximal (vs distal) underexpanded MSA site are more predisposed to thrombus formation than ISR.

Objective Angiographic assessment of a left main coronary artery (LMCA) stenosis is often difficult and unreliable. Intravascular ultrasound (IVUS) assessment of absolute lumen dimensions has been shown to correlate with fractional flow reserve (FFR) and to predict clinical outcome in patients with a LMCA stenosis. Methods during 21 months period (October, 2004 to July, 2006), 153 patients (Ostial lesions, n=47; Non-ostial lesion, n=106) underwent IVUS evaluation specifically to assess the severity of an angiographically inconclusive LMCA narrowing. IVUS analysis included plaque morphology; external elastic membrane (EEM), lumen, plaque cross-sectional areas (CSA), plaque burden (plaque CSA/ EEM) and remodeling index (lesion EEM CSA/reference EEM CSA). Results Overall, minimum lumen area (MLA) and diameter (MLd) and plaque burden measured 8.2 mm2, 2.6 mm, and 59.3 %, respectively. An MLAbeen shown to correlate with a FFR d 44.5% in non-ostial lesions. In particular, ostial LMCA lesions had a larger MLA and a smaller plaque burden than non-ostial lesions (Table). Conclusions Patients referred for LMCA evaluation commonly have insignificant narrowing. Negative remodeling was prominent at the LMCA ostium. These patients deserve IVUS assessment before revascularization.

Objective We used intravascular ultrasound (IVUS) to assess incidence, predictors, morphology, and angiographic findings of edge dissections and intramural hematomas after drug-eluting stent (dES) implantation. Methods We studied 887 patients with 1 045 non-in-stent restenosis lesions in 977 native arteries undergoing dES implantation with IVUS imaging, and compared the dissected stent end to the non-dissected stent end. Results Eighty-two dissections were detected; 51.2% (42/82) involved the proximal and 48.8% (40/82) the distal stent edge. When compared to the non-dissected stent end, residual plaque area [(8.0±4.3) mm2 vs (5.2±3.0) mm2, Pburden [(52±12)% vs (36±15)%, Pd stent edge symmetry (1.17±0.11 vs 1.14±0.08, P=0.02) were larger; plaque burden≥50% was more frequent (62% vs 17%, Pd calcium deposits (52.5% vs 35.6%, P=0.03) more common; and the lumen/stent area (0.86±0.16 vs 1.02±0.18, Pdissected end. Independent predictors of stent edge dissection were residual plaque eccentricity (OR=1.3, Pd residual plaque burden≥50% (OR=7.3, Pd in 34.1% (28/82) of *** predictors of intramural hematomas were plaque eccentricity (OR=1.4, P=0.005), plaque burden≥50% (OR=7.1, P=0.02), and mean lumen diameter to stent diameter ratio (OR=0.37, P=0.04).Concluslon IVUS identified edge dissections after 9.4% of dES implantations. Residual plaque eccentricity and significant plaque burden predicted coronary stent edge dissections. dissections in less diseased reference segments with an arc of normal vessel wall (greater plaque eccentricity) more often evolved into an intramural hematoma.

目的了解"伤寒疫苗国际合作项目"监测区人口统计学特征的变化,探讨调查的质量控制方法及作用。方法用统一的调查表对监测区内的住户逐户开展了3次人口统计学调查,将数据录入以VisualFoxpro为软件平台的数据库并进行统计分析;采用不同的抽样方法对调查进行质量控制,分析质控结果。结果3次人口统计学调查所调查总户数分别为35001户(共115539人)、41908户(134780人)及48160户(136129人)。5～60岁人口比例约占总人口数的86%;家庭规模以1户4人以下居多,占调查总户数的80%。2001～2005年,城市迁出人口和迁入人口分别占城市总人口数的18.9%和15.38%,农村地区迁出人口和迁入人口分别占农村总人口数的21.13%和8.80%。质量控制结果为:用不同的方法进行复查,在第2次调查中的复查符合率分别为88.5%和90.0%;第3次调查中复查符合率分别为92.8%和87.7%。在第2、3次人口统计学调查中所复查的问卷准确性评分分别为4.35和4.22(总分为5分)。结论质量控制在调查中起着至关重要的作用,保证了调查结果的可靠性。

With the purpose to determine the frequency and type of cardiac lesions in naturally exposed dogs to Trypanosoma cruzi, ninety one stray dogs, capture by the Canine and Feline Control Center （dog pound） from the municipality of Merida, were studied. before euthanasia, blood samples were taken to detect 72 cruzi antigens by indirect immunofluorescence antibody test and Western blot and to detect the genome of parasite by Polymerase Chain Reaction. Immediately after euthanasia, hearts were macroscopically evaluated and a sample of the middle right atrial wall of each dog was taken for histopathological analyses. dNA was also obtained from paraffin blocks of seropositives animals with microscopic lesions to detect 72 cruzi genome. Of ninety one dogs, thirteen were seropositive. All seropositive dogs showed an association （P 〈 0.05） with lymphocytoplasmatic myocarditis. The presence of the 72 cruzi genome was also detected by PCR in cardiac septum tissue of seropositive dogs and in all the cases with microscopic lesions indicating the high pathogenicity of the local circulating strain. No association with macroscopic lesions was observed in seropositive dogs. Also, the presence of dirofilaria immitis （d. immitis） was found in 6% of dogs evaluated. This study demonstrates a high tropism to cardiac tissue and virulence of the strains of 72 cruzi circulating in the studied dog population.

AIM：To assess the proportion of refractive errors in the Mexican population that visited primary care optometry clinics in fourteen states of Mexico. METHOdS：Refractive data from 676 856 patients aged 6 to 90 y were collected from optometry clinics in fourteen states of Mexico between 2014 and 2015. The refractive errors were classified by the spherical equivalent（SE）, as follows：sphere＋？ cylinder. Myopia（SE〉-0.50 d）, hyperopia（SE〉＋0.50 d）, emmetropia（-0.50≤SE≤＋0.50）, and astigmatism alone（cylinder≥-0.25 d）. A negative cylinder was selected as a ***：The proportion（95% confidence interval） among all of the subjects was hyperopia 21.0%（20.9-21.0）, emmetropia 40.7%（40.5-40.8）, myopia 24.8%（24.7-24.9） and astigmatism alone 13.5%（13.4-13.5）. Myopia was the most common refractive error and frequency seemed to increase among the young population（10 to 29 years old）, however, hyperopia increased among the aging population（40 to 79 years old）, and astigmatism alone showed a decreasing trend with age（6 to 90y; from 19.7% to 10.8%）. There was a relationship between age and all refractive errors（approximately 60%, aged 50 and older）. The proportion of any clinically important refractive error was higher in males（61.2%） than in females（58.3%; P〈0.0001）. From fourteen states that collected information, the proportion of refractive error showed variability in different geographical areas of ***：Myopia is the most common refractive error in the population studied. This study provides the first data on refractive error in Mexico. Further programs and studies must be developed to address the refractive errors needs of the Mexican population.

A series of 1,3-substituted 8-styrylxanthines (11a-d) was synthesized, under chemo- and regioselective conditions, in a good overall yield. The compounds showed affinity towards both A1 and A2A-adenosine receptors by radioligand binding by means of in vitro assays. The (E)-3-ethyl-1-propyl-8-styrylxanthine (11a) showed the greatest affinity towards the A2A receptor, whereas (E)-3-pentyl-1-propyl-8-styrylxanthine (11d) showed the greatest affinity for the A1 receptor. When the 8-styrylxanthines 11a (A15Et) and 11c (A15bu) were administrated in rats, which were previously injured with 6-hydroxydopamine at the substantia nigra pars compacta (SNc), the turning behavior decreased 50%. based on these results we propose to A15Et as a potential compound to treat some symptoms of Parkinson’s disease.

We tested the prediction that at coarse spatial scales, variables associated with climate, energy, and productivity hy- potheses should be better predictor（s） of bat species richness than those associated with environmental heterogeneity. distribution ranges of 64 bat species were estimated with niche-based models informed by 3629 verified museum specimens. The influence of environmental correlates on bat richness was assessed using ordinary least squares regression （OLS）, simultaneous autoregressive models （SAR）, conditional autoregressive models （CAR）, spatial eigenvector-based filtering models （SEVM）, and Classification and Regression Trees （CART）. To test the assumption of stationarity, Geographically Weighted Regression （GWR） was used. bat species richness was highest in the eastern parts of southern Africa, particularly in central Zimbabwe and along the western border of Mozambique. We found support for the predictions of both the habitat heterogeneity and climate/productivity/energy hypothe- ses, and as we expected, support varied among bat families and model selection. Richness patterns and predictors of Miniopteridae and Pteropodidae clearly differed from those of other bat families. Altitude range was the only independent variable that was sig- nificant in all models and it was most often the best predictor of bat richness. Standard coefficients of SAR and CAR models were similar to those of OLS models, while those of SEVM models differed. Although GWR indicated that the assumption of stationa- rity was violated, the CART analysis corroborated the findings of the curve-fitting models. Our results identify where additional data on current species ranges, and future conservation action and ecological work are needed.
