Statistical data driven approach of COVID-19 in Ecuador: R_(0) and R_(t) estimation via new method

The growth of COVID-19 pandemic throughout more than 213 countries around the world have put a lot of pressures on governments and health services to try to stop the rapid expansion of the *** 2009,H1n1 Influenza pandemic,statistical and mathematical methods were used to track how the virus spreads around *** of these models that were developed at the beginning of the XXI century are based on the classical susceptible-infected-recovered(SIR)model developed almost a hundred years *** evolution of this model allows us to forecast and compute basic and effective reproduction numbers(R_(t) and R_(0)),measures that quantify the epidemic potential of a pathogen and estimates different *** this study,we present a traditional estimation technique for R_(0) with statistical distributions by best fitting and a Bayesian approach based on continuous feed of prior distributions to obtain posterior distributions and computing real time R_(t).We use data from COVID-19 officially reported cases in Ecuador since the first confirmed case on February *** of the lack of data,in the case of R_(0) we compare two methods for the estimation of these parameters below exponential growth and maximum likelihood *** do not make any assumption about the evolution of cases due to limited information and we use previous methods to compare scenarios about R_(0) and in the case of R_(t) we used Bayesian inference to model uncertainty in contagious proposing a new modification to the well-known model of Bettencourt and Ribeiro based on a time window of m days to improve *** R_(0) with exponential growth criteria was 3.45 and with the maximum likelihood estimation method was *** results show that Guayas,Pichincha and Manabíwere the provinces with the highest number of cases due to *** reasons explain the increased transmissibility in these localities:massive events,population density,cities dispersion patterns,and

We report the magnetic properties of Tb/Si multilayers obtained by rf-sputtering at the Tb layer thickness LTb = 3 nm. Analysis of the magnetization processes indicates more complex behaviour than canonical spinglass transition. It is more probable that these multilayers contain both Tb superparamagnetic particles and Tb-Si spin-glass alloys.

BACKGroUnD During the second wave of the coronavirus disease 2019(COVID-19)pandemic,a subset of critically ill patients developed delayed respiratory deterioration in the absence of new infection,fluid overload or extra-pulmonary organ *** To describe the clinical and laboratory characteristics,outcomes,and management of these patients,and to contrast this entity with other post COVID-19 immune dysregulation related inflammatory *** This was a retrospective observational study of adult patients admitted to the medical intensive care unit of a 2200-bed university affiliated teaching hospital,between May and August 2021,who fulfilled clearly defined inclusion and exclusion *** was assessed by a change in PaO_(2)/FiO_(2) ratio and levels of inflammatory markers before and after immunomodulation,duration of mechanical ventilation after starting treatment,and survival to *** Five patients developed delayed respiratory deterioration in the absence of new infection,fluid overload or extrapulmonary organ dysfunction at a median interquartile range(IQR)duration of 32(23-35)d after the onset of *** patients had elevated inflammatory markers,required mechanical ventilation for 13(IQR 10-23)d,and responded to glucocorticoids and/or intravenous *** patient died(20%).COnCLUSIOn This delayed respiratory worsening with elevated inflammatory markers and clinical response to immunomodulation appears to contrast the well described Multisystem Inflammatory Syndrome–Adults by the paucity of extrapulmonary organ *** diagnosis can be considered in patients presenting with delayed respiratory worsening,that is not attributable to cardiac dysfunction,fluid overload or ongoing infections,and associated with an increase in systemic inflammatory markers like C-reactive protein,inteleukin-6 and ferritin.A good response to immunomodulation can be *** delayed inflammatory pulmonary syndrome may re

The structure and the magnetic properties of rf-sputtered nauoscale Tb in [Tb/Ti]n and [Tb/Si] multilayers have been studied experimentally. It is found that the structure of Tb layers changes, transforming from a fine-crystalline state of Tb into an amorphous state as the magnetic layer thickness decreases. The observed displacement of a magnetic ordering temperature to the lower temperature range and the shift of magnetic hysteresis obtained in the ZFC-FC terms may be caused by the size effect and the amorphization. Both the first and the second suppress the exchange interaction and decrease the crystalline magnetic anisotropy. The type of material of nonmagnetic spacers between the Tb layers plays a certain role in these changes.

Single stage management of Gustilo type Ⅲ A/B tibia fractures： Fixed with nail ＆ covered with fasciocutaneous flap

Purpose： To evaluate the role of immediate and definitive management of Gustilo type Ⅲ A/B tibia fractures with intramedullary nailing and fasciocutaneous flap. Methods： From August 2010 to July 2012, 22 patients with Gustilo Grade Ⅲ A/B tibia fractures were managed with a single stage treatment of ipsilateral fasciocutaneous flap ＆ reamed intramedullary nailing and were included in the study. The severity of the injury was calculated with Ganga Hospital injury severity score. Results： The mean age of patients was 41 years and the follow-up time ranged from six months to one year. Among the 22 patients, 73% were type Ⅲ B fractures with upper leg involved in 55% of them. The time interval from injury to completion of surgery was 8 14 h. The incidence of bone infection requiring secondary procedure was 9%; the major and minor soft tissue complication rate was 9% and 14% respectively. The limb salvage rate was 100%. Conclusion： Multidisciplinary management of severe lower limb trauma is important and provides good outcomes, lntramedullary nailing and immediate flap fixation can achieve early bone union and good soft tissue coverage, leading to good outcomes in patient with Grade Ⅲ A ＆ B tibia fractures.

Activity Induced by a naphthalene-Prazosin Derivative on Ischemia/Reperfusion Injury in Rats

In this study, a new naphthalene-prazosin derivative (compound 5) was synthetized with the objective of evaluating its activity on ischemia/reperfusion injury. The Langendorff technique was used to evaluate the effect of the compound 5 on ischemia/reperfusion injury. Additionally, the mechanism of action involved in the activity exerted by the compound 5 on perfusion pressure and coronary resistance was evaluated by measuring left ventricular pressure in absence or presence of following compounds;prazosin, metoprolol, indomethacin and nifedipine. The results showed that the compound 5 reduced infarct size compared with the control conditions. Other results showed that the compound 5 significantly increases (p = 0.05) the perfusion pressure and coronary resistance in isolated rat heart. In addition, other data indicate that the compound 5 increases left ventricular pressure in a dose-dependent manner (0.001 to 100 nM);however, this phenomenon was significantly inhibited by nifedipine at a dose of 1 nM (p = 0.05) and this effect was independent of cAMP levels. In conclusion, these data suggest that the naphthalene-prazosin derivative exerts a cardio protective effect via the calcium channels activation and consequently induces changes in the left ventricular pressure levels. This phenomenon results in a decrease of myocardial necrosis after ischemia and reperfusion.
