Dynamic magnetic resonance defecography in 10 asymptomatic volunteers

AIm: Evaluation of the wide range of normal findings in asymptomatic women undergoing dynamic magnetic resonance (mR) defecography. mETHODS: mR defecography of 10 healthy female volunteers (median age: 31 years) without previous pregnancies or history of surgery were evaluated. The rectum was filled with 180 mL gadolinium ultrasound gel mixture. mR defecography was performed in the supine position. The pelvic floor was visualized with a dynamic T2-weighted sagittal plane where all relevant pelvic floor organs were acquired during defecation. The volunteers were instructed to relax and then to perform straining maneuvers to empty the rectum. The pubococcygeal line (PCGL) was used as the line of reference. The movement of pelvic floor organs was measured as the vertical distance to this reference line. Data were recorded in the resting position as well as during the defecation process with maximal straining. Examinations were performed and evaluated by two experienced abdominal radiologists without knowledge of patient history. RESULTS: Average position of the anorectal junction was located at -5.3 mm at rest and -29.9 mm during straining. The anorectal angle widened significantly from 93° at rest to 109° during defecation. A rectocele was diagnosed in eight out of 10 volunteers showing an average diameter of 25.9 mm. The bladder base was located at a position of +23 mm at rest and descended to -8.1 mm during defecation in relation to the PCGL. The bladder base moved below the PCGL in six out of 10 volunteers, which was formally defined as a cystocele. The uterocervical junction was located at an average level of +43.1 mm at rest and at +7.9 mm during straining. The uterocervical junction of three volunteers fell below the PCGL; described formally as uterocervical prolapse. CONCLUSION: Based on the range of standard values in asymptomatic volunteers, mR defecography values for pathological changes have to be re-evaluated.

Comparison of high-resolution ultrasound and mR-enterography in patients with inflammatory bowel disease

AIm:To compare the results of high-resolution ultrasound(HR-US) and magnetic resonance enterography(mRE) examinations in patients with inflammatory bowel disease(IBD).mETHODS:The reports of 250 consecutive cases with known IBD,who had an mRE and HR-US examination,were retrospectively *** a patient-based approach we evaluated morphological disease features such as affected bowel wall,stenosis,abscess and *** comparison between the two modalities was based on the hypothesis,that any pathological change described in any imaging modality was a true finding,as no further standard of reference was available for complete ***:Two hundred and fifty examinations representing 207 different patients were *** modalities assessed similar bowel wall changes in 65% of the examinations,with more US findings in 11% and more mRE findings in 15%.When the reports were analyzed with regard to "bowel wall inflammation",US reported more findings in 2%,while mRE reported more findings in 53%.Stenoses were assessed to be identical in 8%,while US found more in 3% and mRE in 29%(P more findings in 2%(n = 4) while mRE detected more in 6%(n = 16).US detected more fistulas in 1%(n = 2),while mRE detected more in 13%(n = 32)(P most common reason for no detected pathology by US was a difficult to assess anatomical region(lesser pelvis,n = 72).CONCLUSION:US can miss clinically relevant pathological changes in patients with IBD mostly due to difficulty in assessing certain anatomical regions.

Current Injection Provokes Rapid Expansion of the Guard Cell Cytosolic Volume and Triggers Ca^2＋ Signals

High-resolution microscopy opens the door for detailed single-cell studies with fluorescent reporter dyes and proteins. We used a confocal spinning disc microscope to monitor fluorescent dyes and the fluorescent protein Venus in tobacco and Arabidopsis guard cells. multi-barreled microelectrodes were used to inject dyes and apply voltage pulses, which provoke transient rises in the cytosolic Ca^2＋ level. Voltage pulses also caused changes in the distribution of Lucifer Yellow and Venus, which pointed to a reversible increase of guard cell cytosolic volume. The dynamic cytosolic volume changes turned out to be provoked by current injection of ions. A reduction of the clamp current, by blocking K^＋ uptake channels with Cs^＋, strongly suppressed the cytosolic volume changes. Cs^＋ not only inhibited the expansion of the cytosol, but also inhibited hyperpolarization-induced elevations of the cytosolic Ca^2＋ concentration. A complete loss of voltage-induced Ca^2＋ signals occurred when Ca^2＋-permeable plasma membrane channels were simultaneously blocked with La^3＋. This shows that two mechanisms cause hyperpolarization-induced elevation of the cytosolic Ca^2＋-concentration：（i） activation of voltage-dependent Ca^2＋-permeable channels, （ii） osmotically induced expansion of the cytosol, which leads to a release of Ca^2＋ from intracellular stores.

Effects of supplementation of green tea extract on the milk performance of peripartal dairy cows and the expression of stress response genes in the liver

Background: We hypothesised that supplementation of green tea extract(GTE) in dairy cows during the transition period can attenuate proinflammatory conditions and prevent endoplasmic reticulum(ER) stress in the liver of these cows. Thirty Holstein cows with an average parity of 3.06(± 1.31, SD) were divided into a control group and a group that received a daily amount of 10 g of GTE from d 7 before the calving day and a daily amount of 20 g of GTE from the day of calving until d 7 of ***: Cows supplemented with GTE did not show differences in energy intake or milk yield in weeks 2–7 of lactation. However, these cows had a lower milk fat concentration and a lower energy corrected milk yield than the control cows and showed a trend of improved energy balance. The relative m RNA concentrations of proinflammatory genes, genes involved in the acute phase reaction and antioxidant genes in the liver in weeks 1, 4 and 7 of lactation were not different between the two groups of cows. The concentrations of α-tocopherol and the Trolox equivalent antioxidant capacity in plasma were not different between the two groups. However, the group supplemented with GTE showed significant reductions of some genes of the unfolded protein response(UPR) in week 1 and a trend of lower liver triacylglycerol(TAG) concentrations in the liver compared to the control ***: This study shows that supplementation of GTE in dairy cows lowers the fat concentration in the milk but overall has no effect on the expression of inflammatory genes and the antioxidative status in dairy cows during early lactation. The finding of reduced m RNA levels of genes involved in the UPR at week 1, however, supports other results showing that supplementation of polyphenols could prevent the development of ER stress in the liver of cows during early lactation. Long-term effects of environmentally relevant concentrations of silver nanoparticles on microbial biomass, enzyme activity,and functional genes involved in the nitrogen cycle of loamy soil

The increasing production and use of engineered silver nanoparticles（AgNP） in industry and private households are leading to increased concentrations of AgNP in the environment. An ecological risk assessment of AgNP is needed, but it requires understanding the long term effects of environmentally relevant concentrations of AgNP on the soil microbiome. Hence, the aim of this study was to reveal the long-term effects of AgNP on soil microorganisms. The study was conducted as a laboratory incubation experiment over a period of one year using a loamy soil and AgNP concentrations ranging from 0.01 to 1 mg AgNP/kg soil. The short term effects of AgNP were, in general, ***, after one year of exposure to 0.01 mg AgNP/kg, there were significant negative effects on soil microbial biomass（quantified by extractable DNA; p = 0.000） and bacterial ammonia oxidizers（quantified by amo A gene copy numbers; p = 0.009）. Furthermore, the tested AgNP concentrations significantly decreased the soil microbial biomass, the leucine aminopeptidase activity（quantified by substrate turnover; p = 0.014）, and the abundance of nitrogen fixing microorganisms（quantified by nif H gene copy numbers; p = 0.001）. The results of the positive control with Ag NO3 revealed predominantly stronger effects due to Ag＋ion release. Thus, the increasing toxicity of AgNP during the test period may reflect the long-term release of Ag^＋ions. Nevertheless, even very low concentrations of AgNP caused disadvantages for the microbial soil community, especially for nitrogen cycling, and our results confirmed the risks of releasing AgNP into the environment.

The risk of thromboembolism is increased in inflammatory bowel disease and its symptoms may be overlooked. Furthermore, its treatment can be complex and is not without complications. We describe a case of an adolescent boy who developed a cerebral sinus venous thrombosis during a relapse of his ulcerative colitis and who, while on treatment with heparin, developed heparin-induced thrombocytopenia (HIT). The treatment was then switched to fondaparinux, a synthetic and selective inhibitor of activated factor .

Since the mid-nineteenth century., when the first of the Nordic Geological Surveys were established, they have generated a substantial amount of information on the Earth＇s crust, its natural resources, its processes, and on the geological history of Nordic areas. The collective mission of the geological surveys is to carry out ＂Geology for Society＂, by doing research and providing services, and by making geological information and data easily accessible to all the varied end users in industry, government agencies, government institutes, public administrations, technical offices, academia and research institutes, as well as for private individuals. The present paper gives a brief overview of a few, selected, research areas and projects currently undertaken by the Nordic geological surveys. These serve as practical examples of how the Nordic geological surveys address important societal problems and challenges that require geological input for their solution.

Recent trends in liver transplantation for alcoholic liver disease in the United States

AIm To examine temporal changes in the indications for liver transplantation(LT) and characteristics of patients transplanted for alcoholic liver disease(ALD).mETHODS We performed a retrospective cohort analysis of trends in the indication for LT using the United Network for Organ Sharing(UNOS) database between 2002 and 2015. Patients were grouped by etiology of the liver disease and characteristics were compared using χ~2 and t-tests. Time series analysis was used identifying any year with a significant change in the number of transplants per year for ALD, and before and after eras were modeled using a general linear model. Subgroup analysis of recipients with ALD was performed by age group, gender, UNOS region and etiology(alcoholic cirrhosis, alcoholic hepatitis and hepatitis C-alcoholic cirrhosis dual listing).RESULTS Of 74216 liver transplant recipients, ALD(n = 9400, 12.7%) was the third leading indication for transplant after hepatitis C and hepatocellular carcinoma. Transplants for ALD, increased from 12.8%(553) in 2002 to 16.5%(1020) in 2015. Time series analysis indicated a significant increase in the number of transplants per year for ALD in 2013(P = 0.03). There were a stable number of transplants per year between 2002 and 2012(linear coefficient 3, 95%CI:-4.6, 11.2) an increase of 177 per year between 2013 and 2015(95%CI: 119, 234). This increase was significant for all age groups except those 71-83 years old, was observed for both genders, and was incompletely explained by a decrease in transplants for hepatitis C and ALD dual listing. All UNOS regions except region 9 saw an increase in the mean number of transplants per year when comparing eras, and this increase was significant in regions 2, 3, 4, 5, 6, 8, 10 and *** There has been a dramatic increase in the number of transplants for ALD starting in 2013.

Characterizing the patient experience during neoadjuvant therapy for pancreatic ductal adenocarcinoma:A qualitative study
