Delayed Pseudoaneurysms of Vertebral Artery Post Penetrating Trauma: A Case Report

Objective: Vascular injuries usually present immediately after the penetration, but the delayed onset of vascular symptoms caused by vessel dissection or aneurysm after a traumatic event is extremely rare. Vertebral artery injury is a low-frequency but high-mortality injury. We aim to report evidence of delayed onset of vascular symptoms following penetrating trauma in the neck. Materials and Methods: A case report. Results: A 19-year-old boy was referred to our hospital and complained of a mass in the right part of his neck (right mandibular angle). He gave a history of penetrating trauma to his neck 2 months ago. The mass was expanding during these 2 months and doesn’t respond to antibiotic therapy. In the examination, 3 × 3 cm, firm, nonmobile, and without tenderness and pain mass was palpated in the second level of his neck. Doctors ordered a Doppler sonography in the hospital where a yin-yang pattern was reported. A 36 × 43 × 40 mm heterogeneous, solid, and hypodense area close to C1-C2-C3 with vascular flow was discovered in the right submandibular area after computed tomography (CT)-angiography. The patient was referred to an interventional neurologist for angiography and due to the lack of flow at the distal of the V3 segment, he decided to sacrifice this artery by two coils. Conclusion: Penetrating neck injuries are usually asymptomatic, but these injuries are often accompanied by hemorrhage, neurological symptoms, dysphagia, odynophagia, and windpipe. Penetrating lesions of the vertebral artery are rare and very difficult to diagnose. Also, these lesions are challenging for surgeons due to complex anatomy and difficult surgical exposure. So, endovascular treatment was used to treat the patient.

Spontaneous Uterine Artery Pseudoaneurysm Rupture Following Cesarean Section: A Case Report

A pseudoaneurysm, also known as false aneurysm, is a collection of blood in perivascular soft tissue. Iatrogenic cause such as arterial catheterization and abdominopelvic surgery is one of the most common etiologies of pseudoaneurysm formation. Pseudoaneurysm in pelvic vessels is a complication of pelvic surgery or vessel catheterization. Cesarean section is the most common reported cause. In this article we introduced a 25 years old woman presented with abdominal pain and vaginal bleeding 1 week after Cesarean section. Imaging modalities confirmed the diagnosis of left uterine artery pseudoanurysm. Patient was admitted to treat by endovascular intervention. Several hours after admission blood pressure decreased and abdominal pain became more sever, ultra sound study showed evidence of free fluid in abdominopelvic cavity suggestive of pseudoaneurysm rupture. Emergence laparotomy surgery and hysterectomy were done due to active bleeding secondary to pseudoaneurysm rupture. Patient was discharged from hospital 5 days after surgery without any new complication.

Renal Doppler sonography has proved efficacious in detection of several important underlying causes of secondary hypertension. Its application in diagnosis of congenital arterial abnormalities has also recently received attention. Herein, we presented a patient who was diagnosed with coarctation of the aorta (COA) using renal Doppler sonography during work-up for secondary hypertension.

Intravesical Migration of Intrauterine Device and Calculi Formation 12 Years Post Missing: A Case Report

Intraunterine device (IUD) is a safe, effective and feasible method used for reversible contraception worldwide. Transvesical migration of an Intrauterine device (IUD) is a rare complication. We describe a case in whom initially a plain abdominopelvic radiography demonstrated calcified string of an IUD in a 42-year-old woman with recurrent lower urinary symptoms (LUTS) and urinary tract infection since 2 years earlier. The IUD had been inserted 12 years earlier and 3 years after placement of the IUD, the patient experienced an uneventful pregnancy and a successful delivery. Sonographic images and later on the cystoscopic procedure confirmed the diagnosis of transvesical migration of the IUD. The IUD was removed using cystoscopic procedure, leaving no complication.

Testicular Lipomatosis without Evidence of Cowden’s Disease

Background: Testicular lipomatosis is a very rare and benign disorder of the testicles. It usually presents as multiple bilateral ill-defined hyper-echoic intra-testicular lesions of different sizes but generally with maximum 4 mm. Testicular lipomatosis is usually reported in association with Cowden’s syndrome. Aim: We aimed to show that there are cases of testicular lipomatosis occurring in the absence of Cowden’s syndrome. Case Presentation: We present a 28-year-old man with testicular pain, who was finally diagnosed as having isolated testicular lipomatosis without other clinical and biochemical abnormalities using magnetic resonance imaging (MRI) technique. Conclusion: We showed that the testicular lipomatosis may occur and be detected without any evidence of Cowden’s disease.

Huge Atypical Appendicitis in a 14-Year-Old Male: A Case Report

Normal appendix is 4 to 5 mm in diameter and approximately 8 cm in length;although it may be up to 30 cm long. Appendicitis is the inflammation of the appendix and is the most common cause of acute abdominal pain. Right lower quadrant pain, nausea, vomiting and fever are common signs of appendicitis but 20% to 33% of patients with suspected appendicitis present with atypical features. Case presentation: we presented a 14-year-old boy with abdominal pain predominantly in hypogastric area and LLQ (Left Lower Quadrant) that after ultrasound and CT study a large appendix 22 mm in diameter and 17 cm in length was detected. Conclusion: appendix position and size is very variable and appendicitis could be presented by different clinical features. In this case, the patient was presented with LLQ pain rather than RLQ (Right Lower Quadrant) pain due to its large size and elongation to the left side.

An Extremely Rare Case Report of Foramen Magnum Dermoid Cyst Presenting as an Acute Progressive Neurologic Deficit

Intracranial dermoid cysts are nonneoplastic lesions that present a rare clinical entity. They account for 0.04% to 0.6% of all intracranial tumors. Such cystic lesions usually contain different ectodermal derivatives including hairs, sebaceous glands, apocrine gland and teeth. Dermoid cysts of foramen magnum occur very rarely, with few cases reported in the literature. Computed tomography (CT) and magnetic resonance imaging (MRI) are two main diagnostic tools. Here in this paper, we described a case of intracranial (foramen magnum) dermoid cyst presented with neurologic deficits that completely removed surgically.

Hydatid Cyst of Para-Spinal Muscles as a Rare Cause of Back Pain in a Woman: A Case Report and Review of Literature

Introduction: Hydatid cyst formation is rare in organs such as muscle, bone and spine. Para-spinal involvement is an uncommon finding with a prevalence of less than 0.5% in the literatures. Presentation of Case: In the present paper, we reported a case of primary hydatid cyst in lumbar para-spinal area. The patient presented with complaint of chronic back pain and swelling in right thoracolumbar area. A suspicious mass was revealed in lumbar spine x-ray presenting as multi-loculated cystic mass on further evaluation by magnetic resonance imaging (MRI). The patient underwent surgery and the cyst was excised completely. Histopathological evaluations confirmed the hydatid disease. Discussion: Hydatid disease is a zoonotic infectious disease caused by Echinococcus granulosus. The parasitic cysts can form in any part of the host body. The liver and lungs are the most commonly involved organs. The occurrence of these lesions in certain areas such as para-spinal muscles is rare. The cystic mass is detected by radiologic modalities and is confirmed by pathologic examination. Surgery is the mainstay of treatment. Conclusion: Hydatid disease should be on differential diagnosis list facing a cystic lesion in any part of the body especially in the endemic areas. Moreover, radiologic and serologic assessments are important in confirming the diagnosis.

Right Inguinal Varicose Vein in Connection with Femoral Vein Following Intravenous Drug Abuse: A Rare Radiologic Finding

Varicose veins are enlarged, protuberant superficial veins that are palpable beneath the skin. The causes of such a venous pathology may be primary, secondary, or congenital. The major agents leading to the development of varicose veins include: Hereditary, prolonged standing, Increasing age, Heavy lifting, Prior superficial or deep vein clots, Female gender and Multiple pregnancies. In this manuscript, we report a case of inguinal varicose vein in connection with femoral vein, resulted from direct intravenous injection of drug. The diagnosis was made based on Doppler sonography.

A Rare Presentation of Popliteal Vein Aneurysm: Case Report

Popliteal venous aneurysms (PVAs) are very rare. More often it is an incidental finding found on duplex ultrasonography. Venous thrombosis and pulmonary emboli is the primary presentation in most cases of PVAs, hence it is a clinically significant abnormality and a potentially morbid and fatal condition. Early surgical repair should be performed immediately after the diagnosis is made because of the risk of pulmonary emboli. We report a case of symptomatic PVA presenting primarily with local symptoms and chronic pain in popliteal region.
