Planar Scintigraphic Images Denoising

Scintigraphic images are generally affected by a Poisson type random noise which diminishes qualitatively and quantitatively the images. Restoration techniques aim to “find” an object from one (or several) degraded observation(s). The objective of the restoration is then to produce an image closer to the physical reality. So that the restoration is successful, it is very useful to know the nature of degradation. In this work, we present a planar scintigraphic acquisition chain modeling. This model takes into account the Poisson noise and its stationarity aspect. Then, we present a comparative study of the multi-resolution methods used to reduce the noise in scintigraphic images. Scintigraphy is a tool for exploring functionally several pathologies: the ventricular ejection fraction, the renal clearance and the thyroid activity. Given the fact that scintigraphic images are strongly affected by noise, the objective in this work is to enhance scintigraphic images for a reliable diagnosis and better orientation and understanding of the pathological phenomenon. This paper focuses on two main parts: the first deals with the degradation of model while the second takes into consideration the comparison of the multi-resolution methods for assessing the quality of scintigraphic images to reduce noise using wavelet, contourlet, curvelet, ridgelet and bandelet transformations.

Hemothorax Revealing Aortic Aneurism and Aortic Dissection

Hemothorax caused by rupture of aortic aneurysm or aortic dissection is an uncommon manifestation and carries a high mortality rate. We report two cases of 75- and 80-year-old men, who were referred to our department for exploration of a left pleural opacity. Thoracentesis produced a hemorrhagic fluid. The computed tomography scan revealed an aortic dissection type B of Stanford in the first case and an aneurysm of the descending thoracic aorta in the second patient. Emergency operation was performed in the first case but the patient died of multiple organ failure 48 hours after surgery. Aortic dissection or aneurysm can result in a hemothorax especially in patient with favourable conditions such as elderly patient and systemic hypertension. Diagnosis relies on computed tomography (CT) scan and echocardiography.

A seropositive nodular rheumatoid polyarthritis without arthritis, does it exist?

Introduction: The rheumatoid polyarthritis is the most frequent chronic polyarthritis. It affects essentially woman between 40 and 60 years old. Rheumatic subcutaneous nodules and tenosynovitis are usually associated with seropositive symptomatic rheumatoid polyarthritis. However, it is rare that they constitute the essential clinical expression of the disease. Case report: A 60-year-old woman was hospitalized for tumefaction of the dorsal face of the right hand evolved two months before. The clinical examination was found isolated subcutaneous nodules. The anatomopathological exam of one nodule ended in its rheumatoid origin and the MRI of a second one confirmed a tenosynovitis. Anti-CCP antibodies were positive. The patient has never suffered from arthralgias. The evolution was favourable under corticosteroid therapy, methotrexate and colchicine. Discussion: Rheumatic subcutaneous nodules and tenosynovitis are usually associated with seropositive symptomatic rheumatoid polyarthritis. It is rare that they constitute the essential clinical expression of the disease. Conclusion: The extra-articular appearances of the rheumatoid polyarthritis must not be underestimated especially since they can constitute—even rarely—the essential clinical expression of the disease.

Metastatic Colorectal Cancer: Evolution of the Response Rate and Survival with Therapeutic Progress

Introduction: The therapeutic strategy for metastatic colorectal cancer (mCRC) has evolved greatly in the last two decades. This study aimed to compare patients with mCRC treated at the University Hospital of Sousse (Tunisia) through three periods, in terms of treatment strategy;objective response rate (ORR) to chemotherapy;median overall survival (OS) and prognostic factors conditioning OS. Methods: This was a retrospective study covering the period from January 1994 to December 2011. It involved 401 patients with mCRC divided into three groups: group 1 (G1) including patients treated in the period 1994-1998, group 2 (G2) including patients treated in the period 1999-2005 and group 3 (G3) including patients treated in the period 2006-2011. Results: The ORR after 3 cycles of first line chemotherapy was 33%, 50% and 57.8% for G1, G2 and G3 respectively. The median OS was 13.8 months for G1, 19 months for G2 and 23 months for G3. The prognostic factors associated with a better OS for G3 were absence of initial tumor complication, normal initial level of carcinoembryonic antigen, normal liver function tests, surgery of the primary tumor, surgery of liver metastases and the ORR after three cycles of first line chemotherapy. Conclusion: The development of the therapeutic strategy, whatever of medical or surgical nature, has led to improved response rates and survival of patients treated for mCRC.

Evaluation of Targeted Therapy for Locally Advanced or Metastatic Renal Cell Carcinoma in Tunisia

Introduction: Renal cell carcinoma (RCC) is known to be chemo resistant but with the introduction of targeted therapies;there has been a “revolution” in its treatment strategies. The only targeted therapy available in Tunisia for the treatment of metastatic and/or locally advanced RCC is sunitinib. Objective of the Study: To evaluate therapeutic results and tolerance of sunitinib in metastatic and/or locally advanced RCC. Subjects and Methods: This was a retrospective study covering a period of six years (from January 2008 to January 2014) conducted in 5 medical oncology departments in Tunisia. The population of the study consisted of 29 patients treated with sunitinib for metastatic and/or locally advanced RCC. Results: The mean age of patients was 51 years. Three patients had tumor recurrence and 26 patients had a metastatic RCC. The prognosis was good for 5 patients, intermediate for 19 patients and poor for 5 patients. The median duration of treatment was 5 months. Because of side effects, treatment was discontinued in 12.5% of cases and the dose was reduced in 10.3% of cases. Side effects consisted of asthenia (95.8%), stomatitis (70.8%), anemia (50%), hand-foot syndrome (55.8%) in addition to nausea and vomiting (54.2%). Objective response was observed in 37.5% of patients after 3 months of treatment and in 50% after 6 months. The median progression-free survival was 14 months (95% CI, 7.9 to 20.6). The median overall survival was 22 months (95% CI, 15.6 to 28.7). Conclusion: The prognosis of RCC in Tunisian patients has clearly improved with the introduction of sunitinib, but other therapies with a proven efficacy as a first and second line therapy should be considered.

Introduction: The association of the professional exposure to the decline of the male reproductive function had been the subject of several studies but without arriving to one envisagement undeniable of its existence. The ICSI, technique of reference in severe male infertilities, is visavis unexplained failures whose environmental and professional exposure could be about its cause partly. In this work, we tried to highlight an association between certain factors of professional exposure and the deterioration of the spermatic parameters, then to evaluate the effect of such factors on the result of the ICSI. Materials and Methods: It is about a study of the exposed type—no exposed, realized through collaboration between the Unit of Medicine of the Reproduction and the Service of Medicine of Work of the CHU Farhat Hached of Sousse. This study was based on a questionnaire of which part of the items was collected medical files of the patients raising the general characteristics, the profile of exposure as well as the spermatic parameters and the biological assessment of ICSI. Results: Significant associations between the deterioration of sperm and the professional risk factors such as the exposure to the formaldehydes (p = 0.01) and heat (p = 0.001) were noted. The exposure to formaldehydes was associated at the higher risk of oli gozoospermy, of asthenozoospermy with a significant reduction in mobility (p = 0.05), whereas the talks with cement had a risk higher of teratozoospermy. In addition an exposure to heat was associated at the high risk of oligoteratozoospermy. The total rate of pregnancies after ICSI among 220 patients of our series was of 14.47%. The ICSI balance showed a reduction in total fertility rate (76.6% exposed vs 75.3% no exposed). Conclusion: In spite of the limited number of the patients, this study had shown a correlation between certain professional exposures of origin and the deterioration of sperm. Study of the Crosslinking of PVA with Glyoxal in LbL Nanocomposites

Crosslinking is a common practice to improve the barrier properties of polymers. In this study, Montmorillonite (MMT) was used with Polyvinyl alcohol (PVA) to deposit nanocomposite coatings which were crosslinked with glyoxal (Gly) by Layer by Layer (LbL) on a PET substrate. Two crosslinking conditions were studied, under mild condition and with an acidic environment. Mild condition was useful to identify the reversibility steps and the optimum crosslinking times while the acidic environment was essential to investigate the crosslinking mechanism, by determining the permeability for different crosslinking times. PVA and PVA-MMT coatings showed a strong correlation between the permeability coefficients for different crosslinking times and the FTIR results.

Localized bronchial Amyloidosis is an uncommon disease of unknown origin. Clinical signs are not specific. Tracheobronchial symptoms are the most frequent. We report a case of a 31 year-old man complaining of asthma-like dyspnea. Bronchoscopy was performed because of ineffectiveness of antiasthmatic treatment, showed a submucosal infiltration with stenosis of both right and left upper bronchi and a complete stenosis of intermediate troncus. Multiple biopsies were performed and concluded to Amyloidosis of AL type. Oral corticosteroids were indicated with clinical improvement.

Exohedral van der Waals （vdW） hybrids comprising pristine carbon nanotubes （CNTs） and C60 fullerenes are expected to enable the engineering of carbonaceous materials with tunable physical and chemical properties due to the unperturbed sp2 hybridization of the comprising allotropes. In spite of theoretical predictions, experimental realization of exohedral non-covalent hybridization of unmodified C60 fullerenes and CNTs was not reported before, probably due to the relatively strong inter-fullerene and inter-tube interactions that drive the system toward phase separation and the formation of fullerene crystals and CNTs bundles. Here we describe a two-step process for preparation of C60-CNTs hybrids. The first step is based on solution-assembly of random, 3D-networks of individual, pristine CNTs followed by fast solvent-quenching. In the second step sublimated fullerenes are deposited onto the pre-assembled networks where structural defects serve as nucleation points for fullerite nanocrystals. Annealing at low temperature （80 ℃） results in re-organization of the C6o nanocrystals into nanometric layers, forming a CNT-C6o core-shell like cylindrical hybrids. Raman characterization and the observed high mobility of the fullerenes at the CNT surface support the assumption that the exohedral hybrids are of vdW type. The approach presented here offers a pathway for preparation of exohedral hybrids with controlled nano- morphology： CNTs-networks decorated by nanometric C60 crystals and CNTs-3D-networks coated by a shell of non-covalently bonded mono- or multi-layers of C60 molecules that may serve to shape the nanomorphology of the active layer in polymer-fullerene bulk-heterojunction solar cells.
