Early testing of insulin resistance: a tale of two lipid ratios in a group of 5th graders screened by the Coronary Artery Risk Detection in Appalachian Communities Project (CARDIAC Project)

Background In West Virginia (WV),47% of fifth-grade children are either overweight or *** is no clear consensus regarding the definition of insulin resistance in children,and directly measuring insulin on the population level is *** proposed measures examined further in this study include triglyceride (TRIG)/high-density lipoprotein cholesterol (HDL-C) ratio and TRIG/low-density lipoprotein (LDL-C) *** purpose of this study is to examine the relationship between TRIG/HDL-C ratio,TRIG/LDL-C ratio and insulin resistance in fifth-graders with acanthosis nigricans (AN).Methods Between 2007 and 2016,52,545 fifth-grade students in WV were assessed for *** glucose and insulin levels were collected only for a sub-group of students who were AN-positive and was used to determine insulin resistance using the Homeostatic Model for Insulin Resistance (HOMA-IR) *** analysis included t tests and logistic regression with receiver operating characteristic *** Of the students assessed for AN,4.5% (n =2360) tested *** prevalence of insulin resistance was 79% (n =814) among 1030 with AN and complete ***/HDL-C ratio and TRIG/LDL-C ratio were significantly associated with insulin resistance (TRIG/HDL-C:Est.=0.36,P < 0.0001,AUC =0.68;TRIG/LDL-C:Est.=0.87,P < 0.0001,AUC =0.69).Multivariate analysis showed that increased body mass index (Est.=0.05,P < 0.0001),gender (Est.=0.49,p < 0.0001) and TRIG/HDL-C ratio (Est.=0.21,P < 0.0001) were significantly associated with insulin *** TRIG/HDL-C is a better surrogate marker of insulin resistance in AN-positive children compared to TRIG/ LDL-C ratio;so,on a population-level,cholesterol rather than insulin may be obtained for preliminary testing of early insulin resistance in children.

研究了金属间化合物厚度对蜂窝状棒材热性能和力学性能的影响。在200℃下采用重复静液挤压制备Al/Cu蜂窝状棒材。在制备过程中,Al/Cu界面生成1μm厚的金属间化合物层。经420℃退火0.5-2 h后续热处理后,可获得不同厚度的金属间化合物层,最厚可达10.1μm。经鉴定,金属间化合物层为Al2Cu（θ）、AlCu（η2）和Al4Cu9（γ1）相。随着金属间化合物层厚度的增加,纵向和截面方向上的热导率分别减小11.9%（从（268±4.8）到（236±4.4）W/（m·K））和10.4%（从（210±3.2）到（188±2.8）W/（m·K））,Al/Cu蜂窝状棒材的抗拉强度和伸长率分别是（103±8.4）MPa和（73±6.2）%。当金属间化合物层厚度达到7.7μm时,抗拉强度增加至（131±6.5）MPa,随着金属间化合物层厚度增加至10.1μm时,抗拉强度降低至（124±3.9）MPa。随着金属间化合物层厚度增加,蜂窝状棒材的伸长率急剧下降至（29±2.5）%。

Effect of heat treatment on LPSO morphology and mechanical properties of Mg-Zn-Y-Gd alloys

The mechanical properties and microstructure were investigated under different Zn content and heat treatment conditions in a Mg-Zn-YGd cast alloy.A part of the long period stacking order(LPSO)phases transformed to W-M^ZnaRE?phases with an increase in Zn content from 0.9 at.%to 1.8 at.%,and the ultimate tensile strength(UTS)increased from 229 MPa to 248 *** solution treatment at 480°C,the content of the LPSO phase and strength sharply decreased in the Mg-1.8Zn-0.8Y-0.8Gd alloy,whereas this change was not significantly observed in the Mg-0.9Zn-O.8Y-O.8Gd *** solution treatment,the elongation significantly improved and the UTS sharply decreased in both *** lamellar and filminess LPSO phases were observed with aging treatment at 200℃.Moreover,the strengthening efficiency of lamellar and filminess LPSO phases was lower than that of the block LPSO ***,the UTS of the T6 state was lower than that of the as-cast alloy.

SENSITIVIty STUDY OF THE EFFECTS OF WAVE-INDUCED VERTICAL MIXING ON VERTICAL EXCHANGE PROCESSES

The hydrodynamic model COHERENS-SED, developed by the present authors through introducing wave-enhanced bottom stress, wave dependent surface drag coefficient, wave-induced surface mixing, SWAN to COHERENS, is modified to account for wave-induced vertical mixing. The COHERENS-SED model can also be used for one-dimensional, two-dimensional, three-dimensional current and salinity calculations. One-dimensional model and three-dimensional model are used to study the effects of the wave-induced vertical mixing. The horizontal current velocity profiles obtained by the model are in good agreement with the analytical velocity profiles under the same input conditions. Numerical results show that higher wave height would generally generate larger vertical eddy viscosity and lower horizontal velocity. The results for fresh water in Yellow River Delta show that the wave-induced vertical mixing increases the momentum of fresh water transferring ability downwards to seabed and salt water＇s mixing with upper fresh water. Fresh water flume length is compressed considerably.

Liver function alterations after laparoscopy-assisted gastrectomy for gastric cancer and its clinical significance

AIM: To evaluate the factors associated with liver function alterations after laparoscopy-assisted gastrectomy (LAG) for gastric cancer. METHODS: We collected the data of gastrectomy patients with gastric cancer and divided them into 2 groups: open gastrectomy (OG) and LAG. We also collected the data of patients with colon cancer to evaluate the effect of liver manipulations during surgery on liver function alterations. Serum aspartate aminotransferase (AST), alanine aminotransferase (ALT), total bilirubin, and alkaline phosphatase were measured on the preoperative day and postoperative day 1 (POD1), POD3, POD5, and POD7. RESULTS: No changes in liver function were observed after the operation in patients with colon cancer (n = 121). However, in gastric cancer patients (n = 215), AST and ALT levels increased until POD5 compared to those in colon cancer patients and these findings were observed both in the LAG and OG without a significant difference except at POD1. The mean hepatic enzyme levels at POD1 in the LAG group were significantly higher than those in the OG group (P = 0.047 for AST and P = 0.039 for ALT). The factors associated with elevated ALT on POD1 in patients with gastric cancer were body mass index (P type of operation (OG vs LAG, P = 0.094). CONCLUSION: We conclude that the liver function alteration after LAG may have been caused by direct liver manipulation or aberrant hepatic artery ligation rather than the CO2 pneumoperitoneum.

Selecting and applying quantification models for ecosystem services to forest ecosystems in South Korea

There is growing interest in using ecosystem services to aid development of management strategies that target sustainability and enhance ecosystem support to humans. Challenges remain in the search for methods and indicators that can quantify ecosystem services using metrics that are meaningful in light of their high priorities. We developed a framework to link ecosystems to human wellbeing based on a stepwise approach. We evaluated prospective models in terms of their capacity to quantify national ecosystem services of forests. The most applicable models were subsequently used to quantify ecosystem services. The Korea Forest Research Institute model sat- isfied all criteria in its first practical use. A total of 12 key ecosystem services were identified. For our case study, we quantified four ecosystem functions, viz. water storage capacity in forest soil for water storage service, reduced suspended sediment for water purification service, reduced soil erosion for landslide prevention service, and reduced sediment yield for sediment regulation service. Water storage capacity in forest soil was estimated at 2142 t/ha, and reduced suspended sediment was estimated at 608 kg/ ha. Reduced soil erosion was estimated at 77 m^3/ha, and reduced sediment yield was estimated at 285 m^3/ha. These results were similar to those reported by previous studies. Mapped results revealed hotspots of ecosystem services around protected areas that were particularly rich in bio- diversity. In addition, the proposed framework illustrated that quantification of ecosystem services could be sup- ported by the spatial flow of ecosystem services. However, our approach did not address challenges faced when quantifying connections between ecosystem indicators and actual benefits of services described.

Inadvertent placement of a urinary catheter into the ureter: A report of 3 cases and review of the literature

We describe three cases of inadvertent placement of the urinary catheter into the *** 85-year-old gentleman on long-term indwelling catheter(IDC)for neurogenic bladder presented with fever and right flank *** of abdomen and pelvis demonstrated the tip of the IDC to be located within the right vesicoureteric junction with acute right hydronephrosis and acute pyelonephritis.A 74-year-old woman,on long-term IDC for neurogenic bladder was found to have hydronephrosis on ultrasound ***-enhanced CT intravenous pyelography done subsequently showed the IDC was in the right distal ureter.A 47-year-old lady,on IDC for urinary retention and voiding dysfunction likely secondary to schizophrenia and anti-psychotic medications,presented with raised creatinine.A non-enhanced CT of her abdomen and pelvis was done and showed that the tip of the urethral IDC was located up to the left vesicoureteric *** all patients,the hydronephrosis resolved after changing the catheter and they were well on *** also review the literature to identify the incidence,outcomes and possible risk *** our knowledge,only 20 cases have been reported thus far in the English *** serious complications can occur,the incidence is very *** risk factor that has been identified is long-term catheterization in patients with neurogenic *** do not recommend routine imaging after catheterization in this group of ***,we should still be mindful of the possibility of this occurrence and evaluate and treat as necessary when clinical suspicion arises.

We have been experimentally demonstrated an active control technique of dynamic gain-clamped spectra for theerbium-doped fiber amplifiers (EDFA's) by a backward-injected Fabry-Perot (F-P) laser into this EDFA. In addition, employing a short length erbium-doped fiber (EDF) that not cause any gain saturation for preamplification in front of this amplifier module, it can reduce the noise figure degradation and simultaneously achieve gain variation from 11.1 dB to 0.5 dB for 10 dB input power level change.

对机械合金化制备的玻璃工业中用作结构零件的Pt-5%ZrO2(体积分数)的进行表征。通过锆丝电爆炸法制备氧化锆(ZrO2)纳米颗粒(粒径<100 nm),并在室温下与铂粉(粒径<44?m)球磨2~72 h。普通球磨至48 h时,铂粒径遵循经典的递减趋势,至72 h时观测到颗粒团聚现象。晶粒尺寸演变规律与颗粒尺寸规律相似,48 h后减至50 nm左右。但是,铂晶粒的均方根应变规律相反,一直在增长,直至48 h时得到最大值,然后出现弛豫。粉末球磨48 h后,采用放电等离子体烧结制成块体。根据测量得到烧结体的质量损失表明:合金虽然相对密度较低,但有较好的热稳定性。
