Many patients prioritize the ability to return to sports following shoulder replacement surgeries, including total shoulder arthroplasty(TSA), reverse total shoulder arthroplasty(RTSA), and hemiarthroplasty(HA). While activity levels after hip and knee replacements have been well-established in the literature, studies on this topic in the field of shoulder arthroplasty are relatively limited. A review of the literature regarding athletic activity after shoulder arthroplasty was performed using the PubMed database. All studies relevant to shoulder arthroplasty and return to sport were included. The majority of patients returned to their prior level of activity within six months following TSA, RTSA, and shoulder ***, low demand activities are permitted by most surgeons postoperatively and generally have higher return rates than contact sports or high-demand activities. In some series, patients reported an improvement in their ability to participate in sports following the arthroplasty procedure. The rates of return to sports following TSA(75%-100%) are slightly higher than those reported for HA(67%-76%) and RTSA(75%-85%). Patients undergoing TSA, RTSA, and shoulder HA should be counseled that there is a high probability that they will be able to return to their preoperative activity level within six months postoperatively. TSA has been associated with higher rates of return to sports than RTSA and HA,although this may reflect differences in patient population or surgical indication.

Protein secondary structure prediction and high-throughput drug screen data mining are two important applications in bioinformatics. The data is represented in sparse feature spaces and can be unrepresentative of future data. There is certainly some noise in the data and there may be significant noise. Supervised learners in this context will display their inherent bias toward certain solutions, generally solutions that fit the training set well. In this paper, we first describe an ensemble approach using subsampling that scales well with dataset size. A sufficient number of ensemble members using subsamples of the data can yield a more accurate classifier than a single classifier using the entire dataset. Experiments on several datasets demonstrate the effectiveness of the approach. We report results from the KDD Cup 2001 drug discovery dataset in which our approach yields a higher weighted accuracy than the winning entry. We then ex-tend our ensemble approach to create an over-generalized classifier for prediction by reducing the individual subsample size. The ensemble strategy using small subsamples has the effect of averaging over a wider range of hypotheses. We show that both protein secondary structure prediction and drug discovery prediction can be improved by the use of over-generalization, specifically through the use of ensembles of small subsamples.

Conversion of inert N2 molecules into NH3 via electrochemical methods is an environmentally friendly alternative to replace the traditional Haber-Bosch ***,the development of highly efficient catalyst is still ***,we report a density functional theory(DFT)based high-throughput screening to investigate the potential of 23 atomic transition metals(Sc,Ti,v,Cr,Mn,Fe,Co,Ni,Cu,Zn,Y,Zr,Nb,Mo,Tc,Ru,Rh,Pd,Ag,Cd,W,Pt and Au)supported on phosphorene monolayer as electrocatalyst for nitrogen reduction reaction(NRR).Our theoretical results demonstrate that v single atom anchoring on phosphorene monolayer exhibits good thermal stability,selectivity and excellent catalytic activity with a low overpotential of 0.18 ***,rational design principles and electronic descriptor between the intrinsic electronic properties and activation barrier have been *** work offers a new promising noble metal-free catalyst for NRR and reveals profound insights into the activity origin to guide further design.

Attention is given to the role of Ca^2＋ at the interface between the cell wall and the cytoplast, especially as seen in pollen tubes. While the cytoplasm directs the synthesis and deposition of the wall, it is less well appreciated that the wall exerts considerable self control and influences activities of the cytoplasm. Ca^2＋ participates as a crucial factor in this two way communication. In the cytoplasm, a [Ca^2＋] above 0.1 μM, regulates myriad processes, including secretion of cell wall components. In the cell wall Ca^2＋ , at 10μM to 10 mM, binds negative charges on pectins and imparts structural rigidity to the wall. The plasma membrane occupies a pivotal position between these two compartments, where selective channels regulate influx of Ca^2＋ , and specific carriers pump the ion back into the wall. In addition we draw attention to different factors, which either respond to the wall or are present in the wall, and usually generate elevated[Ca^2＋ ] in the cytoplasm. These factors include：（i） stretch activated channels; （ii） calmodulin; （iii） annexins; （iv） wall associated kinases; （v） oligogalacturonides; and （vi） extracellular adenosine 5＇-triphosphate. Together they provide evidence for a rich and multifaceted system of communication between the cytoplast and cell wall, with Ca^2＋ as a carrier of information.

vascularization on newly implanted metal orthopedic implants has remained a challenge in the field of tissue engineering. To address this challenge, in this research,an interconnected foam structure of Ti-6 Al-4 v was microfabricated by electron beam melting(EBM) technique. The foam in question has a density of 1.77 g cm-3 with 60% porosity and a tensile strength of 18 GPa. An extracellular matrix based hydrogel was added as an aqueous matrix to the *** mimetic stress has been closely related to many wound healing biomedical applications as it increases survival and proliferation molecular signals. To that end, increased expression of hypoxia-inducible factor-1α(Hif-1α) and vascular endothelial growth factor(vEGF) in the aqueous hydrogel matrix was achieved by the addition of a hypoxia mimetic deferoxamine mesylate(DFM). In this study, the formation of an endothelial network was achieved in a hydrogel matrix in the presence of the before mentioned 3 D printed metal foam. Cellular viability, fluorescent microscopy and scanning electron microscopy imaging analysis demonstrate that pre-osteoblasts undergo proliferation and also attach efficiently to the foam when exposed to DFM. Human umbilical vascular endothelial cells(HUvECs) were grown in an extracellular matrix-like 3 D hydrogel and a hypoxia-like stress was achieved. This research demonstrates that pre-osteoblast cells undergo cell differentiation and increase the production of hydroxyapatite on exposure to the hypoxia mimetic molecule. This proposed approach encompasses an ideal prototype for a completely living implanted structure for future orthopedic implants.

AIM: To compare the trauma experience gained by a trainee at a United Kingdom major trauma centre and a secondary level hospital in South ***: A profile of inpatient trauma cases during a five-week period in Addenbrooke's Hospital, Cambridge and Somerset Hospital, Cape Town was created. This was achieved by recording various parameters for each patient admitted including age, gender, injury, mechanism of injury and postal/area code. This, together with details of the departments themselves, allows a comparison of the amount and variety of orthopaedic trauma cases experienced by an individual trainee in each setting. RESULTS: The trauma profiles differed significantly. Patients in Cape Town were younger and more likely to be male. In the young, injury in Cape Town was more likely to occur due to assault or being struck by a vehicle, whilst patients in Cambridge were more likely to be injured whilst in a vehicle or in high energy falls. In older patients, trauma at both centres was almost exclusively due to mechanical falls. In a given age group, injuries at the two centres were similar, however the majority of patients admitted to Addenbrooke's were elderly, resulting in less variation in the overall injury ***: The trauma profile of a major trauma centre in the United Kingdom is less varied than that of a South African secondary centre, with significantly fewer cases per surgeon. This suggests a more variedtraining experience in the developing world with a greater caseload.

The interplay between inflammation and cancer progression is a growing area of research. A combination of clinical, epidemiological, and basic science investigations indicate that there is a relationship between inflammatory changes in the pancreas and neoplastic progression. Diets high in ω-6 polyunsaturated fatty acids provide increased substrate for arachidonic acid metabolism by cyclooxygenase-2 (COX-2) and 5-lipoxygenase (5-LOX) to form eicosanoids. These eicosanoids directly contribute to pancreatic cancer cell proliferation. Both COX-2 and 5-LOX are upregulated in multiple cancer types, including pancreatic cancer. In vitro studies using pancreatic cancer cell lines have demonstrated upregulation of COX-2 and 5-LOX at both the mRNA and protein levels. When COX-2 and 5-LOX are blocked via a variety of mechanisms, cancer cell proliferation is abrogated both in vitro and in vivo. The mechanism of COX-2 has been shown to include effects on apoptosis as well as angiogenesis. 5-LOX has been implicated in apoptosis. The use of COX-2 and 5-LOX inhibitors in clinical studies in patients with pancreatic cancer has been limited. Patient enrollment has been restricted to those with advanced disease which makes evaluation of these drugs as chemopreventive agents difficult. COX-2 and 5-LOX expression have been shown to be present during the early neoplastic changes of pancreatic cancer, well before progression to invasive disease. This indicates that the ideal role for these interventions is early in the disease process as preventive agents, perhaps in patients with chronic pancreatitis or hereditary pancreatitis.

AIM： To conduct a retrospective study in 400 chronic hepatitis B patients in order to identify hepatitis B viral factors associated with complications of liver disease or development of hepatocellular carcinoma. METHODS： The mean follow-up time was 83.6 ± 39.6 mo. Alpha-fetoprotein test and abdominal ultrasound were used for cancer surveillance. Hepatitis B basal core promoter mutants, precore mutants, genotypes, hepatitis B viral DNA （HBv DNA） level and hepatitis B e antigen （HBeAg） were measured. Univariate analysis and logistic regression were used to assess odds ratios for viral factors related to liver deaths and hepatocellular carcinoma development. RESULTS： During follow-up, 38 patients had liver deaths not related to hepatocellular carcinoma. On multivariate analysis, older age [odds ratio： 95.74 （12.13-891.31）, P 〈 0.0001], male sex [odds ratio： 7.61 （2.20-47.95）; P = 0.006], and higher Iogzo HBv DNA [odds ratio： 4.69 （1.16-20.43）; P 〈 0.0001] were independently predictive for these liver related deaths. Also, 31 patients developed hepatocellular carcinoma. Multivariate analysis showed that older age [odds ratio： 26.51 （2.36-381.47）; P = 0.007], presence of precore mutants [odds ratio： 4.23 （1.53-19.58）, P = 0.02] and presence of basal core promoter mutants [odds ratio： 2.93 （1.24-7.57）; P = 0.02] were independent predictors for progression to hepatocellular carcinoma. CONCLUSION： Our results show that high levels of baseline serum HBv DNA are associated with non- hepatocellular carcinoma-related deaths of liver failure, while genetic mutations in the basal core promoter and precore regions are predictive for development of hepatocellular carcinoma.
