Objective： The complexity, heterogeneity and capacity of malignant neoplastic cells and tumors for rapid change and evolution suggest that living-cell-based biological-systems approaches to cancer treatment are merited. Testing this hypothesis, the tumor marker, metabolic activity, and overall survival（Os） responses, to the use of one such system, implantable macrobeads [RENCA macrobeads（RMBs）], in phase I and IIa clinical trials in advanced,treatment-resistant metastatic colorectal cancer（m CRC） are described ***： Forty-eight m CRC patients（30 females; 18 males）, who had failed all available, approved treatments,underwent RMB implantation（8 RMB/kg body weight） up to 4 times in phase I and phase IIa open-label ***, labs [tumor and inflammation markers, lactate dehydrogenase（LDH）] and positron emission tomography-computed tomography（PET-CT） imaging to measure number/volume and metabolic activity of the tumors were performed pre-and 3-month-post-implantation to evaluate safety and initial efficacy（as defined by biological responses）. PET-CT maximum standard uptake value（sUVmax）（baseline and d 90; sUVmax ≥2.5）, LDH,and carcinoembryonic antigen（CEA） and/or cancer antigen 19-9（CA 19-9） response（baseline, d 30 and/or d 60）were assessed and compared to ***： Responses after implantation were characterized by an at least 20% decrease in CEA and/or CA 19-9 in75% of patients. Fluorodeoxyglucose（FDG）-positive lesions（phase I, 39; 2 a, 82） were detected in 37/48 evaluable patients, with 35% stable volume and stable or decreased sUV（10） plus four with necrosis; 10, increased tumor volume, sUV. LDH levels remained stable and low in Responders（R）（d 0–60, 290.4–333.9）, but increased steadily in Non-responders（NR）（d 0–60, 382.8–1,278.5）（d 60, P=0.050）. Responders to RMBs, indicated by the changes in the above markers, correlated with Os（R mean Os=10.76 months; NR mean Os=4.9 months; P=0.0006）.Conclusions： The correlations of the tumor marker, tumor volume

Dual Instinct and structural Theories: DsM-5-TR

The genetic based Dual Instinct Theory is determined by evolutionary history. Aggression is required for survival and sex for propagation of species. Aggressive and sexual drive derivatives with their corresponding defence mechanisms are combined with biologically based stages of infant/child development and the functional entity of structural Theory. This model of human nature is the applied to the diagnostic categories of DsM-5-TR. Objectives: To follow the innate Dual Instinct Theory from life to death and through Artificial Intelligence;connect it with biological stages of development of the structural Theory and illustrate its manifestations in DsM-5-TR classifications. Method: Review of selected published literature. Applying the principle of focus and cognition of informed clinical observation of innate drive derivatives in conjunction with The structural Theory. Both theories are functional entities with no structures involved. sigmund Freud’s biologically based stages of development;oral, anal, genital pubic, adult, and geriatric along with a variety of unconscious, automatic, and persistent defense mechanisms are selectively folded into diagnosis listed in the manual.

Faced with autism, motherhood and parenthood are turned upside down in many ways. Coping with stress and mental health problems, continuing to assume a rewarding parental role and finding suitable care are the trials and tribulations that mark out the journey of African parents. Faith and belief have been described as providing meaning and coping mechanisms in the frightening ordeal of disability. An encounter with a young girl and her parents provided an opportunity to analyse the mother’s experience and the impact of beliefs and discourses on her commitment to care. Based on this clinical story, we discuss the place of other-actors (parents and carers) and the Other-God in relation to the psychopathological dynamics of the mother.

spatial Heterogeneity Modeling Using Machine Learning Based on a Hybrid of Random Forest and Convolutional Neural Network (CNN)

spatial heterogeneity refers to the variation or differences in characteristics or features across different locations or areas in space. spatial data refers to information that explicitly or indirectly belongs to a particular geographic region or location, also known as geo-spatial data or geographic information. Focusing on spatial heterogeneity, we present a hybrid machine learning model combining two competitive algorithms: the Random Forest Regressor and CNN. The model is fine-tuned using cross validation for hyper-parameter adjustment and performance evaluation, ensuring robustness and generalization. Our approach integrates Global Moran’s I for examining global autocorrelation, and local Moran’s I for assessing local spatial autocorrelation in the residuals. To validate our approach, we implemented the hybrid model on a real-world dataset and compared its performance with that of the traditional machine learning models. Results indicate superior performance with an R-squared of 0.90, outperforming RF 0.84 and CNN 0.74. This study contributed to a detailed understanding of spatial variations in data considering the geographical information (Longitude & Latitude) present in the dataset. Our results, also assessed using the Root Mean squared Error (RMsE), indicated that the hybrid yielded lower errors, showing a deviation of 53.65% from the RF model and 63.24% from the CNN model. Additionally, the global Moran’s I index was observed to be 0.10. This study underscores that the hybrid was able to predict correctly the house prices both in clusters and in dispersed areas.

This study aims to introduce the protocol for ultrasonic backscatter measurements of musculoskeletal properties based on a novel ultrasonic backscatter bone diagnostic(UBBD)***-energy X-ray absorptiometry(DXA)can be adopted to measure bone mineral density(BMD)in the hip,spine,legs and the whole *** muscle and fat mass in the legs and the whole body can be also calculated by DXA body composition *** on the proposed protocol for backscatter measurements by UBBD,ultrasonic backscatter signals can be measured in vivo,deriving three backscatter parameters[apparent integral backscatter(AIB),backscatter signal peak amplitude(BsPA)and the corresponding arrival time(BsPT)].AIB may provide important diagnostic information about bone *** and BsPT may be important indicators of muscle and fat *** standardized backscatter measurement protocol of the UBBD instrument may have the potential to evaluate musculoskeletal characteristics,providing help for promoting the application of the backscatter technique in the clinical diagnosis of musculoskeletal disorders(MsDs),such as osteoporosis and muscular atrophy.

Effective Utilization of Government-Provided CTI by small Businesses within the Defense Industrial Base

There are two broad objectives of the research reported in this paper. First, we assess whether government-provided cyber threat intelligence (CTI) is helpful in preventing, or responding to, cyber-attacks among small businesses within the U.s. Defense Industrial Base (DIB). second, we identify ways of improving the effectiveness of government-provided CTI to small businesses within the DIB. Based on a questionnaire-based survey, our findings suggest that government-provided CTI helps businesses within the DIB in preventing, or responding to, cyber-attacks providing a firm is familiar with the CTI. Unfortunately, a large percentage of small firms are not familiar with the government-provided CTI feeds and consequently are not utilizing the CTI. This latter situation is largely due to financial constraints confronting small businesses that prevent firms from having the wherewithal necessary to effectively utilize the government-provided CTI. However, we found a significant positive association between a firm’s familiarity with the government-provided CTI and whether a firm is being periodically reviewed by the Defense Counterintelligence and security Agency (DCsA) or is compliant with the Cybersecurity Maturity Model Certification (CMMC) program. The findings from our study also show that the participating firms believe that external cyber threats are more likely to be the cause of a future cybersecurity breach than internal cybersecurity threats. Finally, our study found that the portion of the IT budget that small businesses within the DIB spend on cybersecurity-related activities is dependent on the perception that a firm would be the target of an external cyber-attack.

Collecting umbilical cord stem cells is widely practiced due to its numerous benefits. Over the past decade, umbilical cord stem cells (UCsCs) have shown effectiveness in treating various conditions, such as bone pathologies, neuropsychiatry disorders, hereditary diseases, and metabolic disorders. However, factors like immunization affect the quantity and quality of cord harvesting. studies suggest that antibodies from the mother pass through the umbilical cord to protect the infant against infections. Cleaning the umbilical cord before stem cell extraction is crucial to maintain sterility and cell integrity. Vaccinating a female donor, including for COVID-19, typically does not directly affect the stem cells. Although vaccines aim to trigger an immunological response, they generally do not affect the donor’s stem cells.

Cloud-based robotics systems leverage a wide range of Information Technologies(IT)to offer tangible benefits like cost reduction,powerful computational capabilities,data offloading,***,the centralized nature of cloud computing is not well-suited for a multitude of Operational Technologies(OT)nowadays used in robotics systems that require strict real-time guarantees and *** computing and fog computing are complementary approaches that aim at mitigating some of these challenges by providing computing capabilities closer to the *** goal of this work is hence threefold:i)to analyze the current edge computing and fog computing landscape in the context of robotics systems,ii)to experimentally evaluate an end-to-end robotics system based on solutions proposed in the literature,and iii)to experimentally identify current benefits and open challenges of edge computing and fog *** show that,in the case of an exemplary delivery application comprising two mobile robots,the robot coordination and range can be improved by consuming real-time radio information available at the ***,our evaluation highlights that the existing software,wireless and virtualization technologies still require substantial evolution to fully support edge-based robotics systems.

A novel Epstein-Barr virus subtype associated with nasopharyngeal carcinoma found in south China

Nasopharyngeal carcinoma(NPC)is a unique malignancy closely associated with Epstein-Barr virus(EBV)infection and with a restricted geographical distribution[1].According to a recent World Health Organization(WHO)report,there were around 129,000 new NPC cases reported worldwide in 2018 with greater than 70%of cases in south China and southeast Asia[2].In Hong Kong and Guangdong in south China,NPC incidence is as high as 12.8-25.0/100,000 per year[1,2].

Epstein-Barr virus at 50——future perspectives

The special November and December issues of the Chinese Journal of Cancer celebrate the 50th anniversary of the discovery of Epstein-Barr virus(EBV) with a series of reviews covering the association of the virus with various cancers, with special emphasis on the role of EBV in the pathogenesis of nasopharyngeal cancer(NPC). The restricted geographic prevalence of NPC along with the tumor's consistent association with EBV infection has fascinated scientists and clinicians ever since it was first suggested in 1966. As in all cancers, NPC development reflects the complex interplay between host genes and environmental factors, but the essential role of EBV infection provides important insight into the etiology of this tumor. Indeed, it is this understanding that is now translating into exciting diagnostic and therapeutic opportunities.
