What is the prognosis of emphysematous pyelonephritis associated with extended-spectrum beta-lactamases producing microorganisms?

Objective:To describe the microbiological characteristics in emphysematous pyelonephritis(EPN),demonstrate the frequency of extended-spectrum beta-lactamase(ESBL)microorganisms,and determine if these microorganisms are associated with the prognosis of patients with EPN.Methods:We conducted a retrospective study in patients with a diagnosis of EPN in a tertiary care hospital of the northeast region of Mexico during the period from January 2011 to January 2016.Clinical variables were analyzed to determine association with the presence of ESBL-producing microorganisms.Statistical significance was set with pge of 55(interquartile range:45-65)years.Conservative management was indicated in 38.1%;42.9%were treated with ureteral stent;12.7%with open or percutaneous drainage;15.8%with early nephrectomy;and 9.5%with delayed nephrectomy.Reported mortality was 13(20.6%)cases;23(36.5%)cases required admission to the intensive care unit.The most frequent microorganism isolated was Escherichia coli(n=34,53.9%).ESBL microorganisms were found in 31.7%of the population.No significant association of ESBL was found with admission to the intensive care unit,or with increased mortality.Conclusions:To our knowledge,this is the first study that evaluates ESBL microorganisms as a prognostic factor in EPN.Risk factors associated with a poor prognosis in patients with EPN have been described.The microbiological factors,specifically ESBL-producing bacteria,do not seem to influence in the prognosis of these patients.

Dear Editor,Persistent hematuria could trigger urinary retention,leading the patient to the emergency department for abdominal pain,decreased urinary output,anemia,and azotemia[1,2].Conventional bladder clot evacuation requires a three-way catheter with continuous irrigation and aspiration with a syringe or Ellik evacuator[3].Sometimes a large and well-organized hematoma requires an open cystostomy[1,4].We want to introduce a combined method using hydrogen peroxide as well as suction technique with an endotracheal catheter to evacuate intravesical clots.

Impact of tacrolimus intra-patient variability in adverse outcomes after organ transplantation

Tacrolimus(Tac)is currently the most common calcineurin-inhibitor employed in solid organ transplantation.High intra-patient variability(IPV)of Tac(Tac IPV)has been associated with an increased risk of immune-mediated rejection and poor outcomes after kidney transplantation.Few data are available concerning the impact of high Tac IPV in non-kidney transplants.However,even in kidney transplantation,there is still a controversy whether high Tac IPV is indeed detrimental in respect to graft and/or patient survival.This may be due to different methods employed to evaluate IPV and distinct time frames adopted to assess graft and patient survival in those reports published up to now in the literature.Little is also known about the influence of high Tac IPV in the development of other untoward adverse events,update of the current knowledge regarding the impact of Tac IPV in different outcomes following kidney,liver,heart,lung,and pancreas tran-splantation to better evaluate its use in clinical practice.

An Earthquake Catalogue for El salvador and Neighboring Central American Countries （1528-2009） and Its Implication in the Seismic Hazard Assessment

Merging all available databases, a comprehensive and updated earthquake catalogue for El salvador and surrounding areas has been compiled, containing a total of 2,584 events for the period 1528-2009, covering the geographic window delimited by the coordinates 11.0°-16.5° N and 85.5°-92.0° W, focal depths of 0.0 to 304 km, and the moment magnitudes in the interval 5.0 ≤ MW ≤ 8.1. Events in the catalogue are distributed into six seismogenic sources taking into consideration the tectonic regime affecting El salvador, the interplay and complexities between shallow crustal, intraplate and interface subduction seismicity has been thoroughly investigated, primarily with the aim of developing detail criteria to delimit the seismogenic sources in order to perform a consistent seismic hazard assessment. A uniform hazard spectrum for San salvador and seismic hazard maps and their uncertainty have been calculated for the horizontal component of ground motion for rock site conditions using zone and zone free methods. The references cited in the article constitute a comprehensive list of sources of information on the tectonics and seismicity of El salvador and neighboring Central American countries.
