Endoscopic treatment of biliopancreatic pathology in patients with Whipple's pancreaticoduodenectomy surgical variants: Lessons learned from single-balloon enteroscopy-assisted ERCP

Background: Endoscopic treatment of biliopancreatic pathology is challenging due to surgically altered anatomy after Whipple's pancreaticoduodenectomy. This study aimed to evaluate the feasibility and safety of single-balloon enteroscopy-assisted endoscopic retrograde cholangiopancreatography(SBE-ERCP) to treat biliopancreatic pathology in patients with Whipple's pancreaticoduodenectomy surgical variants. Methods: We retrospectively analyzed 106 SBE-ERCP procedures in 46 patients with Whipple's variants. Technical and clinical success rates and adverse events were evaluated. Results: Biliary SBE-ERCP was performed in 34 patients and pancreatic SBE-ERCP in 17, including 5 with both indications. From a total of 106 SBE-ERCP procedures, 76 were biliary indication with technical success rate of 68/76(90%) procedures and clinical success rate of 30/34(88%) patients. Mild adverse event rate was 8/76(11%), without serious adverse events. From a total of 106 SBE-ERCP procedures, 30 were pancreatic indication with technical success rate of 24/30(80%) procedures( P = 0.194 vs. biliary SBEERCP) and clinical success rate of 11/17(65%) patients( P = 0.016 vs. biliary SBE-ERCP). Mild adverse event rate was 6/30(20%)( P = 0.194 vs. biliary SBE-ERCP), without serious adverse events. After SBE-ERCP failure, endoscopic ultrasound-guided drainage, percutaneous drainage and redo surgery were alternative therapeutic options. Conclusions: Biliopancreatic pathology after Whipple's pancreaticoduodenectomy variants can be treated using SBE-ERCP without serious adverse events. Technical and clinical success rates are high for biliary indications, whereas clinical success rate of pancreatic indications is significantly lower. SBE-ERCP can be considered as first-line treatment option in this patient group with surgically altered anatomy.

globally,2023 was the warmest observed year on record since at least 1850 and,according to proxy evidence,possibly of the past 100000 *** in recent years,the record warmth has again been accompanied with yet more extreme weather and climate events throughout the ***,we provide an overview of those of 2023,with details and key background causes to help build upon our understanding of the roles of internal climate variability and anthropogenic climate *** also highlight emerging features associated with some of these extreme *** extremes are occurring earlier in the year,and increasingly simultaneously in differing parts of the world(e.g.,the concurrent hot extremes in the Northern Hemisphere in July 2023).Intense cyclones are exacerbating precipitation extremes(e.g.,the North China flooding in July and the Libya flooding in September).Droughts in some regions(e.g.,California and the Horn of Africa)have transitioned into flood *** extremes also show increasing interactions with ecosystems via wildfires(e.g.,those in Hawaii in August and in Canada from spring to autumn 2023)and sandstorms(e.g.,those in Mongolia in April 2023).Finally,we also consider the challenges to research that these emerging characteristics present for the strategy and practice of adaptation.

Realizing the full potential of ultrahigh-intensity lasers for particle and radiation generation will require multi-beam arrangements due to technology ***,we investigate how to optimize their coupling with solid ***,we show that overlapping two intense lasers in a mirror-like configuration onto a solid with a large preplasma can greatly improve the generation of hot electrons at the target front and ion acceleration at the target *** underlying mechanisms are analyzed through multidimensional particle-in-cell simulations,revealing that the self-induced magnetic fields driven by the two laser beams at the target front are susceptible to reconnection,which is one possible mechanism to boost electron *** addition,the resistive magnetic field generated during the transport of the hot electrons in the target bulk tends to improve their *** simulations also indicate that such effects can be further enhanced by overlapping more than two laser beams.

Distribution and Contamination of Arsenic in Fish, gastropods and Bivalves in the Aby and Tendo Lagoons in East of Ivory Coast

Lagoons are ecosystems for biodiversity and the livelihoods of coastal communities. The main objective of the study was to analyze the variability of arsenic concentrations in gastropods and bivalves in the Aby and Tendo lagoons, taking into account spatial, seasonal and hydrological variations. The study was carried out in four stages spread over two successive hydroclimatic cycles, including two seasons during the rainy season and two more during the dry season. The samples were taken in two areas of the Aby and Tendo lagoons. Arsenic levels were measured by ICP-MS. The results showed that mean arsenic concentrations in the muscles of organisms in Aby Lagoon ranged from 0.01 to 1.26 μg As/g, with a mean and median of 0.17 and 0.06 μg As/g, respectively. Fish had the highest levels of arsenic, followed by crustaceans, while molluscs and plants had lower and comparable concentrations of As. Arsenic concentrations in tilapia and jawbones varied significantly between sites and seasons, with higher concentrations at Tendo and during the rainy season. Arsenic concentrations in gastropods and bivalves were significantly higher than those of other species, with averages of 0.74 and 1.03 mg As/kg, respectively.

gomphrena serrata L. (Amaranthaceae) is a medicinal plant widely used traditionally in rural areas to treat many diseases, including gastrointestinal helminthiasis of small ruminants. This work aims to estimate the plant extracts phytochemical potential and study the activities of the aqueous decoction (gS_AD), aqueous macerate (gS_AM), and hydroalcoholic macerate (gS_HEM) of gomphrena serrata extracts on two developmental stages (eggs and adult worms) of Haemonchus contortus. Phytochemical screening was carried out. The acute toxicity test was performed according to the Organisation for Economic Co-operation and Development (OECD) test guideline 423. The Haemonchus contortus eggs hatching inhibition test was performed left to incubate for 48 hours in the presence of the different plant extracts at increasing concentrations of 0.125;0.25;0.5;0.625;1;2;3 mg/mL. For the motility inhibition test on adult worms, the worms were contacted with the extracts at concentrations of 0.75;1;1.25;1.5;2;3;4 mg/mL and left to incubate for 24 hours. Albendazole and levamisole were used as standard. Phytochemical screening revealed the presence of tannins, saponins, reducing compounds, coumarins and derivatives, anthocyanins, steroids, triterpenes, and flavonoids. An acute toxicity study showed that the oral LD50 value of g. serrata was estimated to be 5000 mg/kg. All extracts showed an effect on both stages of parasite development. The hatching inhibition rate was 100% from 3 mg/mL for all three extracts. The rate of inhibition of motility was 100% from 4 mg/mL for each extract. HEM is more efficient on eggs and Haemonchus contortus adult worms than other extracts with IC50 of 0.457 mg/mL and LC50 of 1.329 mg/mL, respectively. These results suggest that gomphrena serrata, a plant used by farmers, has anthelmintic properties.

The Minamata Convention in the aim to protect human life and the environment, seeks to reduce mercury (Hg) by monitoring it concentrations in the environment. Artisanal and Small-scale gold Mining (ASgM) has been identified as the most important anthropogenic source of human exposure to Hg. In this context, the main goal of this study was to assess the level of mercury in hair of two (2) populations living along two lagoons respectively Aby and Tendo, in Ivory Coast. To reach this goal, hair samples of 138 residents were collected and analyzed by using Cold Vapor Atomic Fluorescence Spectrophotometry (CV-AFS) technique for mercury concentration. Results showed that for the entire population the Hg mean was 2.34 μg/g. Also, they were ranged between 0.15 to 8.53 μg/g and presented substantial differences amongst the villages. In Aby village, we observed the highest Hg concentrations (Mean = 2.62 μg/g). Our findings showed that almost the entire sample group (82%) exceed the USEPA recommended limit, furthermore 56% of them exceed the normal level of WHO and 2% of the respondent has the unhealthy levels of mercury (≥6 μg Hg/g) of hair by WHO standards. gender differences in hair mercury varies greatly among reports. Lower levels in women’s hair compared to men were reported in the both village. Considering age, the lowest concentrations were observed with children. However, when we take in account the age groups, data suggested that the most exposed sub-population of [18-29] years old is from Aby village in opposite at Frambo village, the same case those who were ≥ 40 years old. It’s convenient to note that, the proportion of Mercury levels would not to be neglected among studied population especially with the resident from Aby village. So, some measures need to be taken at the political level to control mercury contamination.

Purpose: Diet and eating habits are major risk factors for the health and the development of disease, such as, for example, metabolic disorder leading to cardiovascular pathology and cancer, decreased immunity exposing to infections. This study of the physico-chemical and nutritional properties of a soumara-based food broth was carried out with the aim of promoting the consumption of organic broth made from nere seeds (soumara). That is to alleviate certain metabolic diseases, which is a matter of food safety, and also to limit the risk for the health about the consumption of some cooking stocks on the market. Methods: Several natural ingredients such as nere seeds (soumara), ginger, black pepper, parsley and garlic were used to create a nere-based stock. All these ingredients were freeze-dried and the powder obtained was used to make the broth, regarding their physical and chemical properties. Results: The broth had a good protein content of 17.41 ± 0.367 g/100g, a lipid content of 16.80 ± 0.08 g/100g and a fiber content of 8.66 ± 0.04 g/100g. In terms of nutritional values, the broth showed good levels of calcium 184.21 ± 0.09 mg/100g, potassium 50.04 ± 1.45 mg/100g and iron and zinc. In terms of antioxidant activity, the broth also showed good antioxidant activity. Conclusion: Regarding the properties of our food broth, whose composition is based on natural ingredients, could be recommended for consumption and, its properties, could play an important role in preventing and combating certain metabolic diseases.
