A Retrospective Renal Study from a Lupus Vasculitis Clinic

Aim: In July 2009, a combined Renal Rheumatology Lupus Vasculitis (RRLV) clinic, the first of its kind for adult patients in Australia, was started at Royal Brisbane & Women’s Hospital. This is an audit of progression of renal disease to assess if patients attending this clinic had comparable results to published studies of similar cohorts with lupus Nephritis (LN) and vasculitis. Methods: We conducted a retrospective audit of all the patients who attended this clinic from July 2009 to October 2013. There were 33 patients followed up in the vasculitis group and 36 in the LN group. Patients with other connective tissue disorders were excluded from the analysis as the numbers were insignificant. Results: The mean estimated glomerular filtration rate of vasculitis and LN patients improved from 32.06 to 45.82 ml/min/1.73m2 and 62.42 to 65.53 ml/min/1.73m2 respectively. The mean urine protein/creatinine ratio of vasculitis and LN patients improved from 420 to 85 and 406 to 70 respectively. No patients died in either group. One vasculitis and two LN patients required maintenance dialysis. Three LN patients underwent renal transplantation. Conclusion: The results show excellent patient and renal survival and support the concept of a combined renal rheumatology clinic in managing renal disease from systemic connective tissue disorders.

AfterWards:A Narrative Medicine Program at Johns Hopkins Medicine and in China

Narrative medicine is a multidisciplinary field of inquiry and practice based on the premise that medical care takes place in the context of *** on narrative medicine training suggests that it conveys benefits such as improved communication skills and personal and professional growth to physicians,medical students,and other health care *** medicine can promote empathy and trust between patients and physicians and foster *** 2014,the author and a colleague started an ongoing inter-disciplinary narrative medicine program in the Children’s Center of the Johns Hopkins Hospital called *** program,which meets monthly,is open to all on a volunteer *** literature,art,and writing,AfterWards nurtures empathy,encourages reflective practice,and builds community among a diverse group of health care *** a series of lectures and workshops at Johns Hopkins Medicine,Peking Union Medical College,and Fudan Hospital in Shanghai,the author has introduced AfterWards to Chinese medical educators and *** with *** Hanson,she created an AfterWards Facilitator’s Guide for the use of Chinese practitioners.A recent white Paper on Chinese health care indicates that an infusion of humanities-based education,of which narrative medicine forms a part,can help rebuild patient-physician ***,there has been an increase in interest in narrative medicine in the United States and ***,more research is needed to demonstrate the impact of programs like *** to the implementation of narrative medicine programs remain,most significantly in terms of expertise,resources,and time.

Improvement of whole wheat dough and bread properties by emulsifiers

Whole wheat bread is widely available worldwide,but it is always associated with less desirable dough processibility,small loaf volume,firm and gritty texture,and other distinctive attributes compared to white *** are commonly used to improve dough handling and baking quality during bread *** present study,five emulsifiers(diacetyl tartaric acid esters of mono-and diglycerides(DATEM),polysorbate 80,sodium stearoyl lactylate(SSL),soy lecithin,and sucrose esters)were added during dough preparation of the whole wheat flour at 0.2%,0.5%,and 1.0%(flour weight basis).Dough rheological behavior and bread quality attributes,such as specific loaf volume and hardness,were *** results showed that DATEM,sucrose esters,and SSL increased the resistance to extension of the dough,whereas soy lecithin and polysorbate 80 increased the *** lecithin and polysorbate 80 were the only emulsifiers that significantly increased loaf volume compared to the *** higher levels(1.0%)of sucrose esters,polysorbate 80,and SSL increased the formation of amylose-lipid complex and mitigate the crumb staling during *** results suggested that the emulsifiers could be applied to contribute to optimum functional quality of whole wheat bread.

Differences in incidence,staging,and survival of urologic cancers in patients under 65 living in the US-Mexico border region

Objectives:To describe and compare the incidence,stage at diagnosis,and survival for genitourinary cancers in the border regions and in Hispanic-Americans.Materials and methods:A population-based search was performed using the Surveillance,Epidemiology,and End Results Program 18 database and the Texas Cancer Registry from 2000 to *** regression models were performed with adjusted for age,gender,race,cancer type,cancer stage,insurance status,and cause of death were used to compare cancer-specific survival.Results:A total of 63,236 kidney and renal pelvis,38,398 bladder,1 70,640 prostate,24,313 testicular cancer cases were ***-specific survivalwas found to beimproved in Hispanic-Americans in kidney and renal pelvis(hazard ratio[HR],0.903,95％con-fidence interval[CI],0.856-0.952,p=0.0001),and bladder cancers(HR,0.81 7,95％CI,0.743-0.898,p＜0.001),despite a more ad-vanced stage at diagnosis in Hispanics with bladder cancer(p＜0.0074).Testicular cancer has a survival disadvantage for individuals living in the border region(HR,1.31 5,95％CI,1.124-1.539,p=0.0006).Conclusions:Disparities exist between Hispanic-Americans and Non-Hispanic white and also between individuals living in the border counties when compared to other *** is most significant in individuals with testicular cancer residing in the border region who demonstrate worse overall survival.

Maternal one-carbon metabolism plays an important role in early life *** is a well-established connection between the fetal environment and the health status of the *** r,there is a knowledge gap on how maternal nutrition impacts stro ke outcomes in *** aim of our study was to investigate the role of maternal dietary deficiencies in folic acid or choline on stroke outcomes in 3-month-old *** female mice were fed a folic acid-deficient diet,choline-deficient diet,or control diet 4 weeks before *** we re continued on diets during pregnancy and la *** and female offspring were weaned onto a control diet and at 2 months of age were subjected to ischemic stroke within the sensorimotor cortex via photothrombotic *** maintained on either a folic acid-deficient diet or choline-deficient diet had reduced levels of S-adenosylm ethionine in the liver and S-adenosylhomocysteine in the *** ischemic stro ke,motor function was impaired in 3-month-old offspring from mothers receiving either a folic acid-deficient diet or choline-deficient diet compared to the animals receiving a control *** brain tissue,there was no difference in ischemic damage *** protein levels were assessed in ischemic brain tissue,there were lower levels of active caspase-3 and hypoxia-inducible factor 1α in males compared to females and betaine levels were reduced in offspring from the mothers receiving a choline-deficient *** results demonstrate that a deficient maternal diet at critical time points in neurodevelopment results in worse stro ke *** study emphasizes the importance of maternal diet and the impact it can have on offspring health.

Given the likelihood of regional extirpation of several once-common bat species in eastern North America from white-nose syndrome,it is critical that the impacts of forest management activities,such as prescribed fire,are known in order to minimize potentially additive negative effects on bat *** wildfires may offer a suitable surrogate to assess long-term burn impacts on bats for planning,implementing and assessing burn *** examine the effects of historic fire on bats,we sampled bat activities at 24 transect locations in burned and unburned forest stands in the central Appalachian Mountains of Shenandoah National Park(SNP),Virginia,*** was limited evidence of positive fire effects over time on hoary bats(Lasiurus cinereus Beauvois)and big brown bats(Eptesicus fuscus Beauvois)***,there were few or mostly equivocal relationships of bat occupancy relative to burn conditions or time since fire in SNP across species using a false-positive occupancy *** results suggest that fire does not strongly affect bat site occupancy short-or long-term in the central Appalachians.

Evaluation of Guardrail Posts Installed in Asphalt Mow Strips by Static Finite Element Simulation

Asphalt mow strips are typically used as vegetation barriers around guardrail posts in the design of roadside safety structures. Asphalt mow strips have historically been modeled as a rigid layer in simulations;this assumption results in significant ground level restraint on the guardrail post. However, experiments have shown that asphalt rupture should be considered in the analysis of the response of guardrail posts embedded in mow strips. The present study investigates the effect of asphalt material properties and mow strip geometry on guardrail post performance using finite element simulations. Numerical simulations are performed and correlated with results from static experiments and material testing. The test simulations and experimental results are used to evaluate the response of guardrail posts with various mow strip designs to predict the level of restraint from the asphalt layer. The model is then used to investigate the effects of asphalt material properties and mow strip geometry on the overall performance of the system. The results demonstrate that including asphalt rupture in numerical simulations is essential in accurately predicting the behavior of guardrail posts installed in asphalt mow strips. In addition, mow strip geometry along with asphalt material properties significantly affect the guardrail post response.

80%以上的发展中国家和经济转轨国家正在进行着某种形式的分权试验。以11个国家（拥有全球森林总量60%以上）为例,简要描述了实施联邦制的主要林业国家当前的森林行政管理类型以及施政形式,认为分权后强化了跨部门联系的重要性,许多政府机构都介入了有关森林资源的各项决策;有效的森林治理取决于各种管理机构的能力;私营部门和民间组织在成功治理各种森林资源中也发挥着重要作用。从11国经验中分析了各种潜在优势、潜在危险,指出分权管理减少了中央政府的官僚和决策障碍,但会使全国性的政策协调、实施和监督变得困难。
