Studies have shown that phosphorus (P) recovered from wastewater as the mineral struvite [MgnH4PO4·6(H2O)] may be a viable alternative fertilizer-P source. This study aimed to compare the effectiveness of electrochemically precipitated struvite (ECST), reclaimed from synthetic wastewater, to other commercial fertilizer-P sources in cultivated soils from Arkansas [AR;silt loam (SiL) and loam (L)], Missouri (MO;SiL), and nebraska [nE;SiL and sandy loam (SL)]. A plant-less, moist-soil incubation experiment, including ECST, chemically precipitated struvite (CPST), monoammonium phosphate (MAP), triple superphosphate (TSP), and an unamended control (UC), was conducted to quantify soil pH, nitrate (nO3-n), ammonium (nH4-n), and Mehlich-3 (M3)-P, -Ca, -Mg, and -Fe concentrations at 0.5, 1, 2, 4, and 6 months. All measured soil properties differed (P ·kg-1 for AR-L-TSP after 1 month and nE-SiL-MAP after 6 months, respectively. Soil M3-P ranged from -29.6 mg·kg-1 in the AR-L-UC after 1 month to 429 mg·kg-1 AR-SiL-TSP after 0.5 months. Results showed that, over time, ECST had comparable pH and soil nO3-n, nH4-n, and M3-P, -Ca, -Mg, and -Fe behavior compared to CPST, MAP, and TSP across various soil textures.

The authors have undertaken the determination of pH values for one buffer solution of TES without naCl and nine buffer solutions with naCl yielding an ionic strength I = ***-1. similar to that of blood. These buffer solutions have been evaluated in the temperature range of 5℃ to 55℃ using an extended version of the Debye-Hückel equation. The pH values are reported using 1) the Debye-Hückel extension of the Bates-Guggenheim convention in the tempera- ture range 5℃ to 55℃ and 2) with and without liquid junction correction at 25℃ and 37℃. These TES buffer solutions are recommended as secondary standard references for pH measurements in the range of pH 7.2 to 7.5 for physiological application with an ionic strength of I = 0.16 ***-1.

Multidisciplinary Breast Conference Improves Patient Management and Treatment

Introduction: Multidisciplinary breast conference (BC) that discusses different clinicopathological aspects of disease creates a sound, complete and unique treatment plan for each patient. The aim of this study was to investigate the role of weekly breast tumor boards in patient management and treatment modifications. Methods: From June 2010 to June 2011, 242 patients (Stage 0 to Stage 4 breast cancer) were prospectively presented before a multidisciplinary team. Presentation included clinical data, imaging and pathological review followed by prospective treatment plan formulated by the physician group. If postoperative presentation, final histopathology of the tumor and adjuvant treatment relevant to each patient was discussed. Breast conference was performed via teleconference, including video projection of pathology slides and images. Feedback evaluations were completed and returned to the Tumor Registry for consistent weekly improvement of BC. Results: 102 patients (42%) had modified management after being presented at BC, ?38% (n = 39) surgical, 33% (n = 34) medical (chemotherapy/endocrine therapy), 17% (n = 17) radiation, 6% (n = 6) both medical and radiation treatment and 6% (n = 6) imaging changes. Majority of changes were made to patients with Stage IA (27%) followed by Stage 0 (21%) breast cancer. Conclusion: Breast conference presents a significant impact on patient management and serves as a model for best practice in cancer care. Consultative discussion of each case permits every team member to be acutely aware of the patient’s condition, contribute to treatment plan and permit a harmonious and cooperative team approach while assuring best treatment for the patient.

On exploratory class missions to other planets astronauts will be exposed to types and doses of radiation that are not experienced in low earth orbit. While it is likely that the crew will consist of both male and female astronauts, there has been little research on the effects of exposure to space radiation on central nervous system function and cognitive performance in female subjects. Because estrogen can function as a neuroprotectant, the present experiments were designed to evaluate whether or not the presence or absence of estrogen at the time of irradiation would affect the susceptibility to the neurocognitive effects of exposure to 56Fe particles in female rats. Capsules containing 17β-estradiol or vehicle were implanted in ovariectomized rats three days prior to exposure 56Fe particles (50 - 200 cGy, 1000 MeV/n). Cognitive performance was evaluated using novel object recognition memory to measure learning and memory and operant responding on an ascending fixed-ratio schedule to measure changes in motivation and in the responsiveness to environmental contingencies. The results indicated the estrogen does not function as a neuroprotectant to minimize the cognitive effects of exposure to 56Fe particles. However, the presence/absence of estrogen at the time of irradiation could modulate the responsiveness of the subject to the disruptive effects of exposure to HZE particles on the performance of specific cognitive tasks.

Barriers and facilitators to an outreach rehabilitation program delivered in nursing homes after hip fracture surgical repair

Objective: To identify and understand facilitators and barriers to implementing an Outreach rehabilitation program designed to improve post-operative recovery following hip fracture in long-term care residents. Residents of nursing home facilities are at considerable risk of hip fracture and minimal recovery following a hip fracture. Methods: Data were gathered over June-August, 2012 through semi-structured interviews or focus groups. Fifteen persons (n = 15) who were members of the Outreach rehabilitation team (n = 8) or relevant nursing home staff (n = 7) were interviewed. Data analysis was guided by principles of grounded theory method. Findings: Three major themes that contributed to or hindered the Outreach rehabilitation program emerged, namely, 1) the division, the separate operation and delivery of rehabilitation services;2) building bridges, or negotiating ways to communicate and work together, and 3) strength in the structure, the acceptance of the program and the perceived benefits of the program. One main challenge to program implementation con- cerned coordinating additional rehabilitation with the rehabilitation provided within the nursing homes. Facility staff was largely unaware of the program and were unprepared to work with Outreach team members. As the program progressed, the facility staff and Outreach team were able to collaborate to overcome resident health issues impeding recovery such as cognitive impairment, language barriers and post-surgical pain control needs. Facilitators included the consistency of Outreach team members and accessible facility staff, which contributed to effective communication and trust between the Outreach team and facility staff. Facilitators also included support for the program by the Outreach team and facility staff, as well as the potential benefits of improved mobility and functional status among some program recipients. n-acetylbenzoquinoneimine (nABQI) is a toxic metabolite of the common analgesic acetaminophen (APAP). nABQI is an electrophilic intermediate formed via the oxidation of APAP within the cytochrome P450 system. Within the normally recommended low-dose use of APAP, nABQI is a minor metabolite which is either quickly reduced back to APAP or conjugated to Glutathione (GSH) producing an innocuous by-product. However, with overdose or prolonged high-dose usage of acetaminophen, GSH levels can become depleted and the bioactive nABQI is thought to form adducts with proteins and oxidize protein sulfhydryls producing intra- and intermolecular disulfide bridges in proteins. In this work we investigated the effect of nABQI on purified kidney na,K-ATPase to see if the clinical renal insufficiencies seen in APAP overdose may be linked to inhibition of the na,K-ATPase. Our work has shown that nABQI does indeed inhibit the na,K-ATPase in a dose dependent (IC50 = 19.8 ± 2.9 μM) and irreversible manner. Interestingly, brief storage of nABQI at -20°C eliminates the irreversible effects of the compound, and leads to a product that remains a potent reversible inhibitor of the na,K-ATPase (IC50 = 58.7 ± 19.5 μM). Further, the reversible inhibition produced by stored nABQI competes with para-nitrop.
