Factors associated with cervical cancer screening in a safety net population

AIM To identify factors associated with Papanicolaou-smear(Pap-smear) cervical cancer screening rates in a safety net *** From January 2012 to May 2013, the use of Pap-smear was determined for all patients seen at the breast clinic in a safety net hospital. Health literacy assessment was performed using the validated Newest Vital Sign. The records of patients were reviewed to determine if they had undergone Pap-smears for cervical cancer screening. Sociodemographic information was collected included age, education, monthly income, race/ethnicity, employment, insurance status, and primary care provider of the patient. Logistic regression analysis was then performed to determine factors associated with utilization of Papsmears. Crude and adjusted odds ratios derived from multivariate logistic regression models were calculated as well as the associated 95% CIs and *** Overall, 39% had Pap-smears in the prior 15 mo, 1377 consecutive women were seen during the study period and their records were reviewed. Significantly more patients with adequate health literacy underwent Papsmears as compared to those with limited health literacy(59% vs 34%, P e analysis, patients with adequate health literacy, younger patients, and those with later age of first live birth were more likely to undergo Pap-smears. Patients whose primary care providers were gynecologists were also significantly more likely to have Pap-smears compared to other specialties(P ents younger than 21 years or older than 65 years underwent screening less frequently(11% and 11%, respectively) than those 21-64 years(41%, P e, ethnicity, language, and insurance status were not associated with Pap-smear screening *** Patient health literacy and primary care physician were associated with Pap-smear utilization. Development of interventions to target low health literacy populations could improve cervical cancer screening.

Pelleting Diets Impairs TRAMP Prostate Carcinogenesis

Diets rich in soy products may reduce the risk of prostate cancer (PCa). Daidzein, the major isoflavone present in soy germ, can be metabolized by the gut microbiota into equol. The effects of daidzein and equol on PCa have not been well studied. The objective of this study was to investigate the effect of feeding 2% soy germ, 92 ppm daidzein, or 88 ppm equol diets on the progression of PCa in the transgenic adenocarcinoma of the mouse prostate (TRAMP) model. 3-week old male C57BL/6 X FVB TRAMP mice were weaned from our breeding colony and immediately acclimated to an AIN-93G control diet for one week. At 4 weeks of age, mice (n = 30 per diet group) were randomized to one of four pelleted study diets until 18 weeks of age. Unexpectedly, we did not detect any statistical differences in cancer incidence between diets. We suggest that these results are due to the physical attributes of the pelleted diets in the current study. Mice fed pelleted diets had reduced food intake and significantly decreased body weights (p ed to previous studies. A reduction in food intake is known to reduce cancer incidence in a number of cancer models and is likely to have contributed to the decrease in expected cancer incidence in the current study. In conclusion, we suggest that the hardness of the diets pellets could result in a decreased cancer incidence in TRAMP mice.

Role of microbial dysbiosis in the pathogenesis of esophageal mucosal disease:A paradigm shift from acid to bacteria?

Genomic sequencing,bioinformatics,and initial speciation(e.g.,relative abundance)of the commensal microbiome have revolutionized the way we think about the“human”body in health and *** interactions between the gut bacteria and the immune system of the host play a key role in the pathogenesis of gastrointestinal diseases,including those impacting the *** relatively stable,there are a number of factors that may disrupt the delicate balance between the luminal esophageal microbiome(eM)and the *** changes are thought to be a product of age,diet,antibiotic and other medication use,oral hygiene,smoking,and/or expression of antibiotic products(bacteriocins)by other *** effects may lead to persistent dysbiosis which in turn increases the risk of local inflammation,systemic inflammation,and ultimately disease *** has suggested that the etiology of gastroesophageal reflux disease-related esophagitis includes a cytokine-mediated inflammatory component and is,therefore,not merely the result of esophageal mucosal exposure to corrosives(i.e.,acid).emerging evidence also suggests that the eM plays a major role in the pathogenesis of disease by inciting an immunogenic response which ultimately propagates the inflammatory ***,we discuss the potential role for manipulating the eM as a therapeutic option for treating the root cause of various esophageal disease rather than just providing symptomatic relief(i.e.,acid suppression).

For the last 40 years,the study of cooperative breeding(CB)in birds has proceeded primarily in the context of discovering the ecological,geographical,and behavioral drivers of *** advent of molecular tools in the early 1990s assisted in clarifying the relatedness of helpers to those helped,in some cases,confirming predictions of kin selection *** for genome-wide analysis of sequence variation,gene expression,and epigenetics promise to add new dimensions to our understanding of avian CB,primarily in the area of molecular and developme.tal correlates of delayed breeding and dispersal,as well as the ontogeny of achieving pare.tal status in ***,we outline key ways in which modern-omics approaches,in particular genome sequencing,transcriptomics,and epigenetic profiling such as AtaC-seq,can be used to add a new level of ana-lysis of avian *** on recent and ongoing studies of avian social behavior and sociogenom-ics,we review how high-throughput sequencing of a focal species or clade can provide a robust foundation for downstream,context-dependent destructive and non-destructive sampling of spe-cific tissues or physiological states in the field for analysis of gene expression and epigenetics.-Omics approaches have the potential to inform not only studies of the diversification of CB over evolutionary time,but real-time analyses of behavioral interactions in the field or *** of birds represents a new branch in the network of methods used to study CB,and can help clarify ways in which the different levels of analysis of CB ultimately interact in novel and unexpected ways.

In animal societies, behavioral idiosyncrasies of the individuals often guide which tasks theyshould perform. Such personality-specific task participation can increase individual task efficiency,thereby improving group performance. While several recent studies have documented group-levelbenefits of within-group behavioral （i.e., personality） diversity, how these benefits are realized atthe individual level is unclear. Here we probe the individual-level benefits of personality-driven taskparticipation in the social spider Stegodyphus dumicola. In S. dumicola, the presence of at leastone highly bold individual catalyzes foraging behavior in shy colony members, and all group con-stituents heavily compete for prey. We assessed boldness by examining how quickly spidersresumed normal movement after a simulated predator attack. We test here whether （1） participantsin collective foraging gain more mass from prey items and （2） whether bold individuals are less re-sistant to starvation than shy spiders, which would motivate the bold individuals to forage ***, we assembled colonies of shy spiders with and without a bold individual, added one preyitem, and then tracked the mass gain of each individual spider after this single feeding event. Wefound that spiders that participated in prey capture （whether bold or shy） gained more mass thannonparticipators, and colonies containing a single bold spider gained more total mass than purelyshy colonies. We also found that bold spiders participated in more collective foraging events andwere more susceptible to starvation than shy spiders, suggesting that the aggressive foraging ofbold individuals may represent a strategy to offset starvation risk. These findings add to the bodyof evidence that animal personality can shape social organization, individual performance, andgroup success.

Allograft loss from acute Page kidney secondary to trauma after kidney transplantation

We report a rare case of allograft loss from acute Page kidney secondary to trauma that occurred 12 years after kidney transplantation. A 67-year-old Caucasian male with a past surgical history of kidney transplant presented to the emergency department at a local hospital with left lower abdominal tenderness. He recalled that his cat, which weighs 15 lbs, jumped on his abdomen 7 d prior. On physical examination, a small tender mass was noticed at the incisional site of the kidney transplant. He was producing a normal amount of urine without hematuria. His serum creatinine level was slightly elevated from his baseline. Computer tomography revealed a large subscapular hematoma around the transplant kidney. The patient was observed to have renal trauma grade Ⅱ at the hospital over a period of three days, and he was finally transferred to a transplant center after his urine output significantly decreased. Doppler ultrasound demonstrated an extensive peri-allograft hypoechoic area and abnormal waveforms with absent arterial diastolic flow and a patent renal vein. Despite surgical decompression, the allograft failed to respond appropriately due to the delay in surgical intervention. This is the third reported case of allograft loss from acute Page kidney following kidney transplantation. This case reinforces that kidney care differs if the kidney is solitary or a transplant. early recognition and aggressive treatments are mandatory, especially in a case with Doppler signs that are suggestive of compression.
