Nutritional and health benefits of semi-elemental diets: A comprehensive summary of the literature

AIM:To critically review and summarize the literature on nutritional and health outcomes of semi-elemental formulations on various nutritionally vulnerable patient populations who are unable to achieve adequate nutrition from standard oral ***:We conducted a comprehensive literature search of Pubmed and embase *** manually screened articles that examined nutritional and health outcomes(e.g.,growth,disease activity,gastrointestinal impairment,mortality,and economic impact)among various patient groups receiving semi-elemental *** review focused on full-text articles of randomized controlled clinical trials and other intervention studies,but pertinent abstracts and case studies were also *** pertaining primarily to tolerance,digestion,and absorption were summarized for each patient population in this systematic ***:Results pertaining primarily to tolerance,digestion,and absorption were summarized for each patient *** efficacy of semi-elemental whey hydrolyzed protein(WHP)diet have been reported in various nutritionally high risk patient populations including-Crohn’s disease,short bowel syndrome,acute and chronic pancreatitis,cerebral palsy,cystic fibrosis,cerebrovascular accidents,human immunodeficiency virus,critically ill,and ***,the evidence from the medical literature indicates that feeding with a semi-elemental diet performs as well or better than parenteral or amino acid based diets in terms of toler-ance,digestion,and nutrient assimilation measures across various disease ***:Based on this comprehensive review of the literature,patient populations who have difficulty digesting or absorbing standard diets may be able to achieve improved health and nutritional outcomes through the use of semi-elemental WHP diets.

Rationale Haematoma growth is common early after intracerebral haemorrhage(ICH),and is a key determinant of *** acid,a widely available antifibrinolytic agent with an excellent safety profile,may reduce haematoma *** and design Stopping intracerebral haemorrhage with tranexamic acid for hyperacute onset presentation including mobile stroke units(STOP-MSU)is a phase Ⅱ double-blind,randomised,placebo-controlled,multicentre,international investigator-led clinical trial,conducted within the estimand statistical *** In patients with spontaneous ICH,treatment with tranexamic acid within 2 hours of onset will reduce haematoma expansion compared with *** size estimates A sample size of 180 patients(90 in each arm)would be required to detect an absolute difference in the primary outcome of 20%(placebo 39%vs treatment 19%)under a two-tailed significance level of *** adaptive sample size re-estimation based on the outcomes of 144 patients will allow a possible increase to a prespecified maximum of 326 *** Participants will receive 1 g intravenous tranexamic acid over 10 min,followed by 1 g intravenous tranexamic acid over 8 hours;or matching *** efficacy measure The primary efficacy measure is the proportion of patients with haematoma growth by 24±6 hours,defined as either≥33%relative increase or≥6 mL absolute increase in haematoma volume between baseline and follow-up CT *** We describe the rationale and protocol of STOP-MSU,a phase Ⅱ trial of tranexamic acid in patients with ICH within 2 hours from onset,based in participating mobile stroke units and emergency departments.

AIM:To study the epidemiology of gastric malignancies in Jordan as a model for Middle east countries where such data is scarce. MeTHODS:Pertinent epidemiological and clinicopathological data for 201 patients with gastric malignancy in north of Jordan between 1991 and 2001 were analyzed. ReSULTS:Male:female ratio was 1.8:*** mean age was 61.2 years,and 8.5% of the patients were younger than 40 years of *** overall age-adjusted incidence was 5.82/100 000 population/*** age specific incidence for males raised from 1.48 in those aged 30-39 years to 72.4 in those aged 70-79 ***, gastric lymphomas,malignant stromal tumors,and carcinoids were found in 87.5%,8%,2.5%,and 2% *** was an average of 10.1-month delay between the initial symptoms and the *** 82 patients underwent“curative”*** adenocarcinoma groups,lauren intestinal type was the commonest(72.2%)and the distal third was the most common localization(48.9%).The mean follow up for patients with gastric adenocarcinoma was 25.1 mo(range 1-132mo).The 5-year survival rates for stages Ⅰ(n=15), Ⅱ(n=41),Ⅲ(n=59),and Ⅳ(n=53) were 67.3%,41.3%, 5.7%,and 0% respectively(P=0.0001).The overall 5 year survival was 21.1%. CONCLUSION:Despite low inddence,some epidemiological features of gastric cancer in Jordan mimic those of high- risk *** are detected and treated after a relatively long *** justification in favor of a possible gastric cancer screening effort in Jordan is supported by our study;rather,the need of an earlier diagnosis and subsequent better care.

背景:在家庭中进行父母干预是改善儿童膳食妁一种有效方法。目的:本综述旨在评估以父母为目标的家庭干预增加儿童水果和蔬菜(FV)摄入量的有效性。数据来源:本研究搜索了5个电子数据库:MeDLINe,embase,PubMed,CINAHL和PsycINFO。研究选择:选择2000至2016年间以英文发表的在2岁～12岁儿童中进行的随机或非随机试验。数据提取:对18篇文献进行综述,对12个随机试验进行分析。以研究结果和干预类型为核心进行分析,产生3个独立的meta分析结果。结果:营养教育干预对增加儿童水果摄入量具有正性但较小的效果(Hedges’g=0.112;P=0.028)。味觉暴露干预对增加儿童蔬菜摄入量具有中等正性效果(Hedges’g=0.438;P<0.001)。每天或每周进行干预与每月干预相比,更多得报告出阳性结果。结论:未来的干预应该采取有规律的味觉暴露来最大限度地增加儿童蔬菜的摄入量。这一点尤为重要,因为与水果相比,达到国家蔬菜推荐摄入量的儿童更少。

AIM: To evaluate the role of CDX2 homeobox protein as a predictor for cancer progression and prognosis as well as its correlation with MUC2 expression. CDX2 represents a transcription factor for various intestinal genes (including MUC2) and thus an important regulator of intestinal differentiation, which could previously be identified in gastric carcinomas and intestinal metaplasia. MeTHODS: Formalin-fixed and paraffin-embedded tissues from 190 gastric carcinoma patients were stained with monodonal antibodies recognizing CDX2 and MUC2, respectively. Immunoreactivity was evaluated semiquantitatively and statistical analyses including x2 tests, uni- and multi-variate survival analyses were performed. ReSULTS: CDX2 was mostly expressed in a nuclear or supranuclear pattern,whereas MUC2 showed an almost exclusive supranuclear *** antigens were present in >80% of areas exhibiting intestinal metaplasia. An immunoreactivity in >5% of the tumor area was observed in 57% (CDX2) or in 21% (MUC2) of the *** presence of both molecules did not correlate with WHO, lauren and Goseki classification (with the exception of a significantly stronger MUC2 expression in mucinous tumors). CDX2 correlated with a lower pT and pN stage in the subgroups of intestinal and stage I cancers and was associated with MUC2 positivity.A prognostic impact of CDX2 or MUC2 was not observed. CONCLUSION: CDX2 and MUC2 play an important role in the differentiation of normal, inflamed, and neoplastic gastric tissues. According to our results, loss of CDX2 may represent a marker of tumor progression in early gastric cancer and carcinomas with an intestinal phenotype.

目的研究Drosha蛋白在胃腺癌发展进程中的意义及与患者预后的相关性,探究Drosha对胃癌细胞SGC-7901侵袭能力的影响。方法免疫组织化学染色及Image-Pro Plus软件检测组织芯片889例胃腺癌组织样本中Drosha的表达量,统计分析Drosha与各临床参数及309例胃腺癌患者预后的关系;小干扰RNA(siRNA)干扰人胃腺癌细胞株SGC-7901中Drosha的表达,TranswellTM侵袭实验检测细胞侵袭能力。结果 Drosha在高分化胃腺癌组织中高表达(吸光度值中位值为0.4195),在中分化胃腺癌中表达降低、在低分化腺癌中低表达;Drosha的表达与肿瘤大小、lauren分型、肿瘤浸润深度、淋巴结转移、肿瘤病理分级及病理分期相关;生存分析显示Drosha高表达的患者拥有更高的生存率;敲低SGC-7901细胞的Drosha水平后,细胞侵袭能力显著增强。结论 Drosha蛋白水平随胃腺癌分化程度降低而逐渐降低,敲低Drosha水平可促进胃癌细胞的侵袭。
