Malate dehydrogenase （MDH） is an enzyme widely distributed among living organisms and is a key protein in the central oxidative pathway. It catalyzes the interconversion between malate and oxaloacetate using NAD＋ or NADP＊ as a cofactor. Surprisingly, this enzyme has been extensively studied in eukaryotes but there are few reports about this enzyme in prokaryotes. It is necessary to review the relevant information to gain a better understanding of the function of this enzyme. Our review of the data generated from studies in bacteria shows much diversity in their molecular properties, including weight, oligomeric states, cofactor and substrate binding affinities, as well as differences in the direction of the enzymatic reaction. furthermore, due to the importance of its function, the transcription and activity of this enzyme are rigorously regulated. Crystal structures of MDH from different bacterial sources led to the identification of the regions involved in substrate and cofactor binding and the residues important for the dimer-dimer interface. This structural information allows one to make direct modifications to improve the enzyme catalysis by increasing its activity, cofactor binding capacity, substrate specificity, and thermostability. A comparative analysis of the phylogenetic reconstruction of MDH reveals interesting facts about its evolutionary history, dividing this superfamily of proteins into two principle clades and establishing relationships between MDHs from different cellular compartments from archaea, bacteria, and eukaryotes.

Objective: To identify the presenting features of a laryngeal cleft in children. To compare rigid and flexible endoscopic methods available for identifying laryngeal cleft and determine their utility. Methods: The charts of all patients diagnosed with laryngeal cleft in a tertiary care institution between 2009 and 2010 were evaluated retrospectively for age, gender, comorbidity, presenting features, and results of bedside swallow evaluation. findings on flexible and direct laryngoscopy, both performed under general anesthesia, were compared. Results: Eleven patients had a diagnosis of laryngeal cleft, confirmed by direct laryngoscopy. Nine of eleven had signs of aspiration on modified barium swallow study (MBSS). Of the eight subjects who underwent flexible laryngoscopy by a pulmonologist, a deep interarytenoid groove was only reported in four cases. In all eleven cases, the arytenoids could be physically separated during direct laryngoscopy, allowing for definitive diagnosis of the cleft and identification of its type and severity. Conclusion: Video swallow studies and flexible laryngoscopy may raise suspicion or even diagnose a laryngeal cleft, however, a laryngeal cleft must be confirmed by direct laryngscopy in which the interarytenoid space is palpated. furthermore, a patient in whom symptoms persist but no laryngeal cleft is identified on flexible examination should have a direct laryngoscopy to rule out a cleft.

Background:Recent investigations highlighted that Hermetia illucens(BSf)larva meal can be a valuable alternative protein source for aquafeed *** this study,in substitution of fishmeal,we used increasing inclusion levels of a partially defatted BSf larva meal in low fishmeal-based diets for rainbow trout(Oncorhynchus mykiss Walbaum)and we evaluated the related implications in terms of growth performance,physical characteristics,proximate and fatty acid(fA)compositions of the fillets,gut and liver histology,and diet *** a 131-day trial,576 fish(100.1±9.29 g)were randomly allotted to 24 tanks(24 fish/tank,4 replicates/treatment).Six experimental diets were produced to have partial replacement of fishmeal(0,10%,20%,30%,40%and 50%)by increasing levels of BSf meal(0%[BSf0],3%[BSf3],6%[BSf6],9%[BSf9],12%[BSf12]and 15%[BSf15]on as fed basis,respectively).Results:No differences were observed among the treatments for all the considered growth performance *** viscero and hepato-somatic indexes showed significant differences among the treatments,with the highest values observed in the BSf15 *** differences were recorded in terms of fillet’s physical characteristics,dry matter(DM),crude protein(CP)and ether extract(EE)*** saturated and monounsaturated fA increased,while polyunsaturated fA(particularly n-3 fA)decreased while increasing the HI meal inclusion in the *** of liver and gut was not affected,whereas,in posterior gut,villi were higher in BSf6 and BSf9 compared to BSf3 *** apparent digestibility of DM,CP,EE and gross energy did not vary among the ***:These results suggest that a partially defatted BSf meal could be included up to 15%in low fishmeal-based diets for rainbow trout with no adverse effects on growth performance,fillet’s physical characteristics,gut and liver health,and diet *** the contrary,the fillet fA composition worsened while increasing the level of BSf

A parametric study was performed to design a device capable of treating small targeted regions within the prostate using high intensity focused ultrasound, while sparing the surrounding organs and minimizing the number of elements. The optimal focal length (L), operating frequency (f), element size (a) and central opening radius for lodging an imaging probe (r) of a device that would safely treat tissue within the prostate were obtained. Images from the Visible Human Project were used to determine simulated organ sizes and treatment locations. Elliptical tumors were placed throughout the simulated prostate and their lateral and axial limits were selected as test locations. Using graphics processors, the acoustic field and Bio-Heat Transfer Equation were solved to calculate the heating produced during a simulated treatment. L, f, a and r were varied from 45 to 75 mm, 2.25 to 3 MHz, 1.5 to 8 times the wavelength and 9 to 12.5 mm, respectively. The resulting optimal device was a 761-element concentric-ring transducer with L = 68 mm, f = 2.75 MHz, a = 2.05λ and r = 9 mm. Simulated thermal lesions showed that it was possible to treat target tumors consistent with reported locations and sizes for prostate cancer.

AIM: To investigate the safety/efficacy of Boceprevirbased triple therapy in hepatitis C virus(HCV)-G1 menopausal women who were historic relapsers, partial-responders and null-responders. METHODS: In this single-assignment, unblinded study, we treated fifty-six menopausal women with HCV-G1, 46% f3-f4, and previous PEG-α/RBV failure(7% null, 41% non-responder, and 52% relapser) with 4 wk lead-in with PEG-IfNα2b/RBV followed by PEGIfNα2b/RBV+Boceprevir for 32 wk, with an additional 12 wk of PEG-IfN-α-2b/RBV if patients were HCV-RNApositive by week 8. In previous null-responders, 44 wk of triple therapy was used. The primary objective of retreatment was to verify whether a sustained virological response(SVR)(HCV RNA undetectable at 24 wk of follow-up) rate of at least 20% could be obtained. The secondary objective was the evaluation of the percent of patients with negative HCV RNA at week 4(RVR), 8(RVR BOC), 12(EVR), or at the end-of-treatment(ETR) that reached SVR. To assess the relationship between SVR and clinical and biochemical parameters, multiple logistic regression analysis was ***: After lead-in, only two patients had RVR; HCV-RNA was unchanged in all but 62% who had ≤1 log10 decrease. After Boceprevir, HCV RNA became undetectable at week 8 in 32/56(57.1%) and at week 12 in 41/56(73.2%). Of these, 53.8% and 52.0%, respectively, achieved SVR. Overall, SVR was obtained in 25/56(44.6%). SVR was achieved in 55% previous relapsers vs. 41% non-responders(P = 0.250), in 44% f0-f2 vs 54% f3-f4(P = 0.488), and in 11/19(57.9%) of patients with cirrhosis. At univariate analysis for baseline predictors of SVR, only previous response to antiviral therapy(OR = 2.662, 95%CI: 0.957-6.881, P = 0.043), was related with SVR. AIM To evaluate viral hepatitis knowledge among individuals from different resource areas and health conditions to identify possible *** A cross-sectional, descriptive study was carried out among 447 individuals from five distinct populations in Brazil: Southeast Viral Hepatitis Ambulatory(n = 100), South(n = 89) and Northeast(n = 114) Health Center, Southeast(n = 77) and Northeast(n = 67) low resource areas. All individuals answered a questionnaire assessing sociodemographic characteristics and viral hepatitis awareness. The perception was scored based on the average number of correct answers of all participants and categorized as "low"(0-28 correct answers) or "desirable"(29-46 correct answers). Associations between sociodemographic characteristics and perception were also *** A low level of knowledge was observed in individuals from Northeast Health Center, Northeast and Southeast low resource areas while desirable knowledge was observed in individuals from Viral Hepatitis Ambulatory and South Health Center. According to sociodemographic characteristics, desirable scores were more common among those with secondary education(47.1%), those who declared themselves as white(46.3%), and those who lived in houses with three individuals(25.5%). Multivariate analysis showed an association between viral hepatitis perception and type of population. CONCLUSION The results demonstrated high level of knowledge among study participants from health clinics from the Southeast region of Brazil and the importance of education programs in increasing the level of knowledge in low resource areas.
