Large mucinous biliary cystadenoma with "ovarian-like"stroma:A case report

Hepatobiliary cystadenomas are rare tumors that are difficult to diagnose preoperatively. They can reach large sizes that make them real intraoperative ＂surprises＂. A 63-year-old woman, presented with a symptomatic huge abdominal cystic mass, underwent complete resection of the mass with safety margins. Histopathological report revealed mucinous hepatic cystadenoma with ＂ovarian- like＂ stroma and areas of sclerohyalinization. The differential diagnosis of the large cystic tumors which occupy the right hemiabdomen must include the biliary cystadenoma; the complete resection of the tumor with safety margins avoids local recurrence, and therefore represents the optimal therapy because of the malignant potential of the disease. The postoperative follow-up includes abdominal ultrasound or CT scan and CA 19-9 measurement.

Damage of the Co-Cr-Mo Femoral Head of a Total Hip Prosthesis and Its Influence on the Wear Mechanism

Co-Cr alloy is one of the most used alloys for artificial hip joints and offers a good combination of mechanical properties, corrosion resistance and biocompatibility. To increase its resistance to wear are commonly used hard coating thin layers of Co-Cr-Mo. Analyzing a femoral head of a Co-Cr modular total hip prosthesis coated Co-Cr-Mo, we found peeling coating, abrasion of the substrate and hardening of abrasion traces against subsurface initial hardness. This paper presents an analysis by the Student test of the microhardness peeling a portion of a scratch from a femoral head explant covered with Co-Cr-Mo, and that demonstrate the transformation of wear mechanism from three bodies abrasion in two bodies abrasion.

Preliminary Study on the Seizure Trend of a MOM-THP with Self-directed Balls

This work continues the approach of one of our topics relating to a MOM-THP （metal on metal-total hip prostheses） with self-directed movement balls. Experiments revealed a certain seizure in some strain conditions. Laboratory trials for balls/plane Hertzian contacts have been restarted in order to determine seizure behaviour depending on the roughness of the flat area. The trials have been carried out in BSF （body simulated fluid） lubrication conditions, much closer to the real operating conditions up against the initial tests with distilled water. Seizure burdens to different loadings and contact surfaces roughness influence over the seizure burden have been determined. Even though the minimum value of the wear must be the same with the minimum value of the surfaces roughness, given the experimental conditions, it came out from the trials results on wear that the lowest level of wear is acquired at a certain value of roughness, not at the lowest level of roughness.

Hepatocellular carcinoma in the setting of interferon-free treatment for chronic HCV hepatitis - experience of a single center

Aim: This study aims to analyze the particularities of hepatitis C induced hepatocellular carcinoma (HCC), developed during or after treatment with direct-acting antivirals. Methods: We conducted an observational prospective study on 278 patients, who underwent treatment for hepatitis C related liver cirrhosis and respectively for F3 chronic hepatitis C. Liver status was assessed using biological parameters and imagistic evaluation (ultrasonography, computed tomography scan, magnetic resonance imaging). Results: The follow-up time was 14 months. Before therapy, 69.3% of the cirrhotic patients and 26.7% of those with F3 degree of liver fibrosis had high levels of alpha-fetoprotein, with no imagistic evidence of HCC. During treatment, HCC was confirmed in 5 patients, 2 of them presenting portal vein thrombosis (PVT). Antiviral therapy was not interrupted. Two patients developed HCC at the end of treatment, while 4 of them were diagnosed with HCC after three months of ending the treatment. Excepting the ones with PVT, all patients underwent trans-arterial chemoembolization. Conclusion: All patients acquired sustained virological response. The screening for HCC should not be stopped after achievement of sustained virological response. Patients who develop HCC after antiviral treatment often need to be evaluated by magnetic resonance imaging in order to detect the extensionof the disease.
