Objective To assess the prognostic impact of a routine invasive strategy according to the frailty burden in patients with non-ST-segment elevation myocardial infarction(NSTEMI)from the MOSCA-FRAIL clinical *** The MOSCA-FRAIL trial randomized 167 frail patients,defined by a Clinical Frailty Scale(CFS)≥4,with NSTEMI to an invasive or conservative *** primary endpoint was the number of days alive and out of hospital(DAOH)one year after *** this subanalysis,we compared the impact of an invasive strategy on the outcomes between vulnerable(CFS=4,n=43)and frail(CFS>4,n=124)*** Compared to vulnerable patients,frail patients presented lower values of DAOH(289.8 vs.320.6,p=0.146),more read-missions(1.03 vs.0.58,p=0.046)and higher number of days spent at the hospital during the first year(10.8 vs.3.8,p=0.014).The cau-ses of readmission were mostly non-cardiac(56%).Among vulnerable patients,DAOH were similar regardless of strategy(invasive ***:325.7 vs.314.7,p=0.684).Among frailest patients,the invasive group tended to have less DAOH(267.7 vs.311.1,p=0.117).Indeed,patients with CFS>4,invasively managed lived 29 days less than their conservative *** contrast,the-re were no differences in the subgroup with CFS=*** Adult patients with frailty and NSTEMI showed different prognosis according to the degree of frailty.A routine in-vasive strategy does not improve outcomes and might be harmful to the frailest patients.

Current approaches for simulating the energy performance of buildings on a large scale are limited by numerous assumptions and simplifications,which can lead to inaccurate *** new tools and procedures are emerging to improve accuracy,there remains a need for more user-friendly *** study proposes a new tool based on online maps to create the geometry of districts in a simple *** tool also enables an automatic evaluation of all buildings through dynamic hourly simulations,using a building simulation software and allowing to consider different weather *** illustrate the procedure,a district at risk of energy poverty in Seville(Spain)is modeled,where hourly temperature data for a whole year are available to demonstrate the need for building *** tool is used to evaluate the energy demands of the district under several retrofitting alternatives,and free-floating simulations are also performed to evaluate the improvement of thermal comfort without air-conditioning *** aim is not to discuss the actual values for this particular case,but rather to identify the correct direction for large-scale studies,so as to make them more easily ***,it may be concluded that the results provided by comprehensive tools,such as the one proposed in this study,enable easy yet accurate evaluations of buildings on a large scale with significant time savings,as well as the identification of locations where retrofitting interventions would have the greatest impact.

Effects of organic chromium supplementation to finishing lambs diet on growth performance, carcass characteristics and meat quality

The objective of this study was to evaluate supplemental organic chromium（Cr） to finishing lambs on growth performance, carcass characteristics, and meat quality. Eighteen Suffolk lambs（age（4.5±0.2） mon;（25.8±3.6） kg body weight（BW）） were randomly assigned to three levels of supplemental organic Cr（0.0, 0.2 and 0.4 mg kg^–1 dry matter（DM）） in a complete random design. Growth performance was evaluated for 70 d, and then lambs were slaughtered to study carcass characteristics and chemical composition of meat. Orthogonal contrasts were performed（contrast one-average level 0.2 ppm Cr vs. average level 0.4 ppm Cr; contrast two-level 0 vs. average levels（0.2＋0.4） ppm Cr）. Orthogonal polynomials were used to estimate the linear and quadratic effects of Cr concentrations. Growth and carcass performance were not affected by supplemental organic Cr. Muscle conformation and leg perimeter linearly increased（p〈0.05） as organic Cr level increased in the diet. Kidney fat decreased linearly（p〈0.05） as supplemental Cr increased. In Longissimus dorsi（LD）, the ash content decreased linearly, and shear force（kg cm^–2） increased（p〈0.05） as organic Cr level increased in the diet. It is concluded that organic Cr did not affect growth performance, but it improved positively the muscle conformation, reduced kidney fat, whereas in LD there was an increment in shear force in finishing carcass lambs.

BACKGROUND Life-long removal of gluten from the diet is currently the only way to manage celiac disease(CeD). Until now, no objective test has proven useful to objectively detect ingested gluten in clinical practice. Recently, tests that determine consumption of gluten by assessing excretion of gluten immunogenic peptides(GIp) in stool and urine have been developed. Their utility, in comparison with conventional dietary and analytical follow-up strategies, has not been fully *** To assess the performance of enzyme-linked immunosorbent assay(ELISA) and point-of-care tests(poCTs) for GIp excretion in CeD patients on gluten-free diet(GFD).METHODS We conducted an observational, prospective, cross-sectional study in patients following a GFD for at least two years. Using the Gastrointestinal Symptom Rating Scale questionnaire, patients were classified at enrollment as asymptomatic or symptomatic. Gluten consumption was assessed twice by 3-d dietary recall and GIp excretion(by ELISA in stool and poCTs(commercial kits for stool and urine) in two consecutive samples. These samples and dietary reports were obtained 10 day apart one from the other. patients were encouraged to follow their usual GFD during the study *** Forty-four patients were enrolled, of which 19(43.2%) were symptomatic despite being on a GFD. Overall, 83 sets of stool and/or urine samples were *** out of 44 patients(25.0%) had at least one positive GIp test. The occurrence of at least one positive test was 32% in asymptomatic patients compared with 15.8% in symptomatic patients. GIp was concordant with dietary reports in 65.9% of cases(Cohen′s kappa: 0.317). poCT detected dietary indiscretions. Organic matter in the environment is involved in many biogeochemical processes,including the mobilization of geogenic trace elements,such as arsenic,into *** this paper we present the use of fluorescence spectroscopy to characterize the dissolved organic matter (dom) pool in heavily arsenicaffected groundwaters in Kandal province,*** fluorescence dom (fdom) characteristics between contrasting field areas of differing dominant lithologies were compared and linked to other hydrogeochemical parameters,including arsenic and dissolved methane as well as selected sedimentary ***-corrected fluorescence indices were used to characterize depth profiles and compare field *** fdom was generally dominated by terrestrial humic and fulvic-like components,with relatively small contributions from microbially-derived,tryptophan-like *** fdom from sand-dominated sequences typically contained lower tryptophan-like,lower fulvic-like and lower humic-like components,was less bioavailable,and had higher humification index than clay-dominated *** concentrations were strongly correlated with fdom bioavailability as well as with tryptophan-like components,suggesting that groundwater methane in these arsenic-prone aquifers is likely of biogenic origin.A comparison of fdom tracers with sedimentary OM tracers is consistent with the hypothesis that external,surface-derived contributions to the aqueous dom pool are an important control on groundwater hydrogeochemistry.

溶解的天然有机质(dom)会影响水环境中砷(As)的存在形式。在环境相关的砷/溶解有机质(DOC)比率和p H条件下,使用平衡透析法研究了As(Ⅲ)和As(Ⅴ)络合到两种商业腐植酸的条件分布系数(dom)。在所有p H值范围内,As(Ⅴ)比As(Ⅲ)能更好地络合到腐植酸上,p H值为7左右时腐植酸的络合能力最强,因为在低p H值时,H+会与As竞争络合位点;而在高p H值时,OH-会与腐植酸竞争As。对于这两种氧化态,dom的值会随着As/DOC比率减少而增加。对As(Ⅲ)和As(Ⅴ)来说,dom值可以和As/DOC比值拟合为一个函数。在相同的条件下,陆生的腐植酸与As的络合能力比水生的腐植酸高1.5~3倍。在As/DOC比值较低时,Al3+也可以与As竞争络合位点。在正常环境条件下,约10%的As(Ⅴ)可以与天然有机质络合,而只有在As/DOC比值较低时,才会有>10%的As(Ⅲ)被络合到腐植酸上。在将As络合到自然有机物上时,应考虑到自然环境。
