Hepatitis B virus(HBV)infection is a serious global public health *** infection may be transmitted through sexual intercourse,parenteral contact or from an infected mother to the baby at birth and,if contracted early in life,may lead to chronic liver disease,including cirrhosis and hepatocellular *** the basis of the HBV carrier rate,the world can be divided in 3 regions of high,medium and low *** major concern is about high endemicity countries,where the most common route of infection remains vertical transmission from mother to *** of all pregnant women and passive immunization with human hepatitis B immunoglobulin are not affordable for many developing *** infection rate can be reduced by modifying behavior,improving individual education,testing all blood donations,assuring asepsis in clinical practice and screening all pregnant ***,availability of a safe and efficacious vaccine and adoption of appropriate immunization strategies are the most effective means to prevent HBV infection and its *** unsolved problem for poorest countries,where the number of people currently infected is high,is the cost of the vaccine.A future challenge is to overcome the social and economic hurdles of maintaining and improving a prevention policy worldwide to reduce the global burden of the disease.

A totally mini-invasive approach for colorectal laparoscopic surgery

AIM:To study the short-term outcome of patients treated with laparoscopic right colectomy and how intracorporeal anastomosis has improved the ***:We retrospectively examined all patients affected by colorectal cancer who underwent a laparoscopic right colectomy between January 2006 and December 2010 in our *** evaluation criteria were:diagnosis of colorectal carcinoma at presurgical biopsy,elective surgery,and the same *** excluded:emergency surgery,conversions from laparotomic colectomy,and other *** endpoints we examined were:surgical time,number of lymph nodes removed,length of stay(removal of nasogastric tube,bowel movements,gas evacuation,solid and liquid feeding,hospitalization),and major ***-two patients were divided into two groups:intracorporeal anastomosis(39 patients)and extracorporeal anastomosis(33 patients).RESULTS:Significant differences were observed between intracorporeal vs extracorporeal anastomosis,respectively,for surgical times(186.8 min vs 184.1 min,P < 0.001),time to resumption of gas evacuation(3 d vs 3.5 d,P < 0.001),days until resumption of bowel movements(3.8 d vs 4.9 d,P < 0.001),days until resumption of liquid diet(3.5 d vs 4.5 d,P < 0.001),days until resuming a solid diet(4.6 d vs 5.7 d,P < 0.001),and total hospitalization duration(7.4 d vs 8.5 d,P < 0.001).In the intracorporeal group,on average,19 positive lymph nodes were removed;in the extracorporeal group,on average,14 were removed P < 0.001).Thus,intracorporeal anastomosis for right laparoscopic colectomy improved patient outcome by providing faster recovery of nutrition,faster recovery of intestinal function,and shorter hospitalization than extracorporeal ***:Short-term outcomes favor intracorporeal anastomosis,confirming that a less traumatic surgical approach improves patient outcome.

A high-resolution integrated study has been performed in a super-expanded marine record(sedimentation rate spanning from 11 cm/100 years to 20 cm/100 years)from the continental shelf area of the southeastern Tyrrhenian *** foraminiferal distribution illustrates 6 major environmental changes during the past 2000 years:(i)the Roman Period-Dark Age transition(from herbivorous-opportunistic to carnivorous species);(ii)the Dark Age-MCA transition(from carnivorous to herbivorous-opportunistic species);(iii)the Medieval Classic Anomaly-Little Ice Age transition(a further and definitive change from carnivorous to herbivorous-opportunistic species);(iv)the period during the Maunder event between approximately 1720 AD and 1740 AD(turnover from the carnivorous planktonic foraminifer Globigerinodes ruber to the herbivorous-opportunistic planktonic foraminifer Turborotalita quinqueloba);(v)the Industrial Period(dominance of herbivorous-opportunistic planktonic foraminifera);and(vi)the Modern Warm Period at approximately 1940 AD(the last turnover in favor of herbivorous-opportunistic planktonic foraminifers,associated with an increase in benthic foraminifera).Our studies lead us to link this latter feature to an anthropogenic impact associated with the damming of Sele River(Salerno Gulf)at 1934 AD,which induced a change in the sediment input with a strong decrease in coarse-grained fraction and a probable alteration in nutrient ***δ^(18)O_(***) record of the past 2000 years shows the alternation of warm/wet and cold/dry events related to the Roman Period,the Dark Age,the Medieval Classic Anomaly,the Little Ice Age,the Industrial Period and the Modern Warm *** 5 evidentδ^(18)O_(***) oscillations(between approximately 1325 AD and 1940 AD)coincide with the 5 minima in the solar activity record(Wolf,Spörer,Maunder,Dalton and Damon events).

Right ventricular septal pacing: Safety and efficacy in a long term follow up

AIM: To evaluate the safety and efficacy of the permanent high interventricular septal pacing in a long term follow up, as alternative to right ventricular apical pacing. METHODS: We retrospectively evaluated:(1) 244 patients(74 ± 8 years; 169 men, 75 women) implanted with a single(132 pts) or dual chamber(112 pts) pacemaker(PM) with ventricular screw-in lead placed at the right ventricular high septal parahisian site(SEPTAL pacing);(2) 22 patients with permanent pacemaker and low percentage of pacing( 80%) right ventricular apical pacing(RVA). All patients had a narrow spontaneous QRS(101 ± 14 ms). We evaluated New York Heart Association(NYHA) class, quality of life(Qo L), 6 min walking test(6MWT) and left ventricular function(end-diastolic volume, LV-EDV; end-systolic volume, LVESV; ejection fraction, LV-EF) with 2D-echocardiography. RESULTS: Pacing parameters were stable duringfollow up(21 mo/patient). In SEPTAL pacing group we observed an improvement in NYHA class, Qo L score and 6MWT. While LV-EDV didn't significantly increase(104 ± 40 m L vs 100 ± 37 m L; P = 0.35), LV-ESV slightly increased(55 ± 31 m L vs 49 ± 27 m L; P = 0.05) and LV-EF slightly decreased(49% ± 11% vs 53% ± 11%; P = 0.001) but never falling < 45%. In the RVA pacing control group we observed a worsening of NYHA class and an important reduction of LV-EF(from 56% ± 6% to 43% ± 9%, P < 0.0001).CONCLUSION: Right ventricular permanent high septal pacing is safe and effective in a long term follow up evaluation; it could be a good alternative to the conventional RVA pacing in order to avoid its deleterious effects.
