Challenges for Nurses in Providing Patient-Centered Care in Rural Primary Health Care Clinics in Nigeria

Globally, patient-centered care has become the focus of the healthcare system. It is imperative to note that during a global pandemic crisis, patient-centered care principles seek to empower partnering approaches in Primary Health Care (PHC), and have recently gained prominence in nursing practice and applied nursing research. However, nurses are faced with challenges in achieving the desired results in the PHC system. Thus, the study aimed to explore the factors that influence PCC utilization in the PHC facilities in Nigeria. A qualitative exploratory-descriptive design was used for this study. Data collection was conducted with the nurses from PHC facilities through individual interviews. Data saturation was achieved with 35 participants from 30 PHC facilities in Osun State, Nigeria, using a purposive sampling technique. All interviews were audio-recorded, later transcribed verbatim, and analyzed using the thematic analysis approach. NVivo 12 software was used for data management. The results from this study were categorized into two factors: Organizational factors and individual factors. Six themes that emerged from the study include inadequate management support, insufficient opportunities for further training, work overload and time constraints, health personnel readiness to use PCC, dearth enthusiasm for change, and poor nurses’ Accountability. The study shows that nurses encountered diverse challenges in providing patient-centered care. To improve the quality of healthcare delivery in the PHC facilities, there is a need to reinforce adequate management support, education, continued training, and the internal motivation of nurses to achieve transformative health outcomes in the community.

New Polymer Syntheses Part 60: A Facile Synthetic Route to Polyamides Based on Thieno[2,3-b]thiophene and Their Corrosion Inhibition Behavior

Polyamides containing thieno[2,3-b]thiophene moiety were prepared via a simple polycondensation reaction of the diaminothieno[2,3-b]thiophene monomer la with different kinds of diacid chlorides （including oxalyl, adipoyl, sebacoyl, isophthaloyl, terephthaloyl, 4,4＇-azodibenzoyl, 3,3＇-azodibenzoyl, p-phenylene diacryloyl） in the presence of LiC1 and NMP as a solvent through lowtemperature solution polycondensation. The chemical structures of model compound and synthesized polyamides were confirmed by FTIR, nuclear magnetic resonance spectroscopy （including 1 H-NMR and 13 C-NMR） and elemental analysis. In addition, the thermal stability, crystallinity structure and surface morphology of synthesized polyamides were characterized via thermogravametric analysis （TGA）, wide-angle X-ray diffraction analysis （WAXD） and scanning electron microscopy （SEM）. Also, the corrosion inhibition behavior of selected examples ofpolyamides was investigated; the inhibitive effect of the investigated polymers for carbon steel in 1.0 mol＇L-1 HCI was studied using potentiodynamic polarization （PDP） and electrochemical impedance spectroscopy （EIS） methods. PDP results displayed that the polyamides containing thieno[2,3-b]thiophene moiety can be as mixed-type inhibitors. The inhibition efficiency （P, %） was found to be in the range from 67.13% to 96.01%. There is an increase in P by the synthesized polymers in comparison to the starting monomer. The adsorption of these polymers was found to obey Langmuir adsorption isotherm.

Elm bark beetle Scolytus kashmirensis is a key pest of elm nurseries in the social forestry ecosystem of Kashmir. The biological and morphological characters of this species were determined both in field and in the laboratory. The larvae remained inactive throughout the winter which resumed their activity from the third week of March. Pupation started from the first week of April and the first swarming adults appeared from the last week of April. After emergence, the adults fly to suitable trees and undergo maturation feeding for 8.4 (±0.68 SD) days and females laid 46 (±9.45 SD) eggs on an average. The eggs hatched after an incubation period of 11 - 13 days. The five larval instars were distinguished from each other on the basis of the width of head capsule measurements. The larvae completed their development in 40 - 46 days. The pupal stage lasted for 10 - 20 days and finally adults emerge that lived for 45 - 60 days. The single longitudinal maternal gallery measured 42.67 (±11.07 SD) mm (on average) long and the larval gallery was 43 (±10.96 SD) mm (on average) in length. The seasonal distribution of various life stages and the number of generations were also recorded.

New Matrix Tablet from Okra Gum: Effects of Method of Preparation and Gum Concentration on Tablet Properties

The objective of this investigation is to study the effect of methods of preparation and concentration of gum on the compressional and mechanical properties of Okra gum matrix. The compressional behavior of Okra gum matrices prepared by direct compression and wet granulations is analyzed using density measurements, Heckel and Kawakita analysis while the mechanical properties of the formulations were assessed using crushing strength (CS) and friability (FR) as well as CSFR ratio. Formulations prepared by direct compression had lower Pk values than those prepared by wet granulation while there was no significant difference between Py values of formulations prepared by direct compression and wet granulations. Therefore, formulations prepared by direct compression underwent plastic deformation more easily and rapidly than those prepared by wet granulation. The results show that DB values increased with decrease in concentration of the gum and granules undergo higher degree of fragmentation than powders. Formulations containing 90% w/w Okra gum exhibited the highest amount of total plastic deformation and gave the best packing. Tablets prepared by direct compression showed lower bond strength and higher friability values than those prepared by wet granulations. The crushing strength generally decreases with a decrease in the concentration of the gum while there was an inverse relationship between friability and gum concentration. CSFR decreases with a decrease in gum concentration and tablets prepared by wet granulations showed significantly higher values of CSFR

Background: Self-medication is the inappropriate use of drugs to treat self-diagnosed disorders or symptoms. Even though Self-medication has benefits as it enhances better use of clinical skills and increases access to medication;it is also associated with risks such as adverse drug reactions and antibiotics resistance. Objectives: To determine prevalence of self-medication practices among dental out-patients, triggering factors that influence self-medication practice and common source of drugs. Materials and methods: This descriptive cross-sectional study was conducted at the Clinic of the Department of Preventive Dentistry, Lagos University Teaching Hospital over a period of 6 months using self-administered questionnaires adapted from previous studies. Data obtained was analysed and expressed as frequencies and percentages. Test of associations was done using chi-square (p Results: Age of respondents ranged from 16 to 78 years (32.24 ± 13.47). M:F was 1:1. Prevalence of self-medication was 45.5%;toothache was the most prevalent reason for self-medication (71%). The frequently used drugs for self-medication were analgesics (65%) while the most common source for the drugs was the pharmacy (60%). Educational status (X2 = 12.85, P 2 = 6.42, P 2 = 10.38, P Conclusion: This study revealed high prevalence of self-medication with toothache as the predominant trigger and pharmacy, the most common source.

The manufacturing company under consideration recorded the high accident rates for last few years. These accidents cause the organization the heavy man-day loss, the production loss and heavy costs of insurance. The objective of health and safety department at the manufacturing company was to set and improve accidents prevention system. The paper presents how does the six-sigma technique will help to evaluate the safety and environmental hazards in performance of organizations? It is observed that the study helped the management to measure, analyze and improve overall safety plan to protect the life and health of the employees.

Nitrate is one of the problematic and wide spread contaminations. Indiscriminate disposition of human and animal wastes in Nigeria especially in urban region, and lack of proper environmental sanitation geared the need for the monitoring of groundwater contamination. This research work monitored the concentration of nitrate and nitrate-nitrogen in portable water from wells in Odo Ado, Ado-Ekiti, Ekiti State, and examined the relationship between this concentration and the well depth as well as its nature （ringing）. The results showed that out of 20 water wells sampled, 50% of samples contain high level of nitrate with the highest having concentration of 140 mg/L and least 49 mg/L while the remaining has acceptable concentration ranging between 3.5 mg/L and 35 mg/L with most ringed wells. The mean concentration of nitrates is 48.06 mg/L and the nitrate-nitrogen is 10.85 mg/L. The statistical correlation between the concentrations and depth showed that there is a significant difference between their means at 95% confidence using T-test. Out of ringed wells, five have high concentration and eight have low concentrations while two of the wells （not ringed） fall below the acceptable limit （EPA） which indicates that there are other factors such as closeness to the point source and soil texture which were not considered in this work.

Zinc is common metal used for steel protection from *** addition of further element,such as Ni,can modify the corrosion rate and maintain sacrificial *** anodic dissolution behavior of Zn,Ni and Zn-Ni alloys with different Ni contents（from 0.5% to 10%,mass fraction） in 3.5% Na Cl solution was investigated using potentiodynamic,potentiostatic and galvanostatic *** composition and microstructure of the corrosion layer on Zn,Ni and Zn-Ni alloys were characterized by energy-dispersive X-ray spectroscopy analysis（EDX） and scanning electron microscopy（SEM）.The galvanostatic curves show that the anodic behavior of all investigated electrodes exhibits active/passive transition and the tendency of the alloys to passivity decreases with the increase in Ni content,except for 99.5Zn-0.5Ni *** the potentiodynamic curves exhibit active passive transition only for pure *** analysis reveals the presence of oxides,chlorides and metal hydroxide chloride in corrosion products,and very small cracks are observed for 90Zn-10 Ni alloy compared with that of Zn.
