Effectiveness of Treatment of Chronic Viral Hepatitis C by Direct-Acting Antivirals in Togo

Background: Viral hepatitis C is the second leading cause of hepatocellular carcinoma after hepatitis B in Africa and Togo in particular. The advent of direct acting antivirals has revolutionized the care and prognosis of patients infected with hepatitis C virus (HCV). Objective: To evaluate the sustained virological response (SVR) 12 weeks after oral treatment without interferon in HCV infected patients with genotypes 1 and 2. Patients and Method: Descriptive and analytical study based on the retrospective collection of data in the hepatogastroenterology unit of the University Hospital Campus of Lome (Togo) from July 11, 2016 to April 22, 2018. All patients who had a chronic viral hepatitis C with viral replication, naive, regardless of the genotype, regardless of the degree of liver fibrosis, and who had completed their treatment with direct-acting antivirals were included. Results: We recruited 84 patients, 60 of whom were infected with HCV genotype 2 (71.43%) and 24 with HCV genotype 1 (28.57%). There were 58 men and 26 women (sex ratio: 0.45). In HCV genotype 1 patients, the median age was 54.29 years and Sofosbuvir/Ledipasvir was the most used combination (62.50%). In HCV genotype 2 patients, the median age was 54.5 years and Sofosbuvir associated with Ribavirin was the most used treatment (81.66%). The virological response at the end of treatment was 100% (genotype 1) and 93.30% (genotype 2). The SVR 12 was 100% (genotype 1) and 91.70% (genotype 2). Five patients were in treatment failure (genotype 2). Conclusion: Direct-acting antivirals were effective in our patients. The rate of sustained virological response was above 90%.

Background: Anemia is multifactorial and very frequently observed in the evolution of cirrhosis. Only biological investigations can clarify its mechanisms. Objective: To determine the frequency of anemia in cirrhosis patients and to identify the different types of anemia encountered. Patients and Methods: Descriptive and analytical study based on the retrospective collection of data was carried out over 12 months in the hepato-gastroenterology unit of the University Hospital Campus of Lome (Togo). This study included hospitalized cirrhotic patients with a complete medical file including a blood count and presenting anemia. Results: During the study period, we collected 253 cases of cirrhosis, of which 153 patients had anemia (60.5%);there was a male predominance of 73.2%. The mean age was 51 ± 13 years. The B viral origin of cirrhosis was the most common (60.1%). Oedemato-ascitic decompensation (82.4%) and hepatocellular carcinoma (34%) were the main complications

Objective: To find a more accessible and less restrictive mean such as serum-ascites albumin gradient (SAAG) to predict esophageal varices (EV) in cirrhotics with ascit. Patients and methods: Descriptive and analytical studies based on the retrospective collection of data on 125 patients’ records in the hepatogastroenterology unit of the University Hospital Campus of Lome (Togo) from January 1, 2008 to March 31, 2018 were included. Cirrhotic patients aged 15 years and older had performed cytochemical analysis of ascites fluid and upper gastrointestinal fibroscopy and had a protidogram. Statistical analysis was done by R Studio Software 3.4.2. Results: The mean age was 48.70 years;there was a male predominance (70.40%) with a sex ratio of 2.38. Protein levels in ascitic fluid was Conclusion: Our study noted that at an upper threshold of 1.10 g/dl, the SAAG significantly predicted the presence of EV.

Aims: To determine the prevalence and risk factors of heartburn in Abidjan, a black African city. Patients and Methods: cross-sectional study from June 15 to September 30, 2003. One thousand nine hundred forty (1940) persons from five health zones of the city Abidjan were included after their informed consent. Socio demographic and clinical parameters were collected using a questionnaire in an interview format. Heartburn was defined as a discomfort or burning sensation extending from the sternal manubrium to the base of the neck. Factors related to the complaint were asked such as predisposing factors, habits (tobacco, alcohol and coffee intake) and body mass index. Stepwise multiple logistic regression analyses were used to examine associations between these factors and heartburn. Results: Among 1940 respondents (mean age 28 ± 9 years;sex ratio (M:F) 0.86), heartburn occurred in 433 persons (once a week in 9.2% of case (178 persons)). Five factors were statistically associated with heartburn: male sex (p = 0.025, OR = 0.555 [CI95% 0.331 - 0.930]), heartburn in a family member (p = 0.010, OR = 1.765 [95%CI 1.143 - 2.725]), constipation (p = 0.011, OR = 2.182 [95%CI 11,953,983]), right lateral decubitus (p = 0.001, OR = 6.247 [95%CI 2.079 - 18.775]) and after a meal (p = 0.000, OR = 2.643 [95%CI 1.594 4.383]). Conclusion: Heartburn is common in this black African population. Male sex appears to be less associated. Constipation, right lateral decubitus and after a meal are trigger factors for heartburn. Heartburn in a family member is a risk factor.

The capability of error detection of patient-specific QA tools plays an important role in verifying MLC motion accuracy. The goal of this study was to investigate the capability in error detection of portal dosimetry, MapCHECK2 and MatriXX QA tools in IMRT plans. The 9 fields IMRT for 4 head and neck plans and 7 fields IMRT for 4 prostate plans were selected for the error detection of QA devices. The measurements were undertaken for the original plan and the modified plans, where the known errors were introduced for increasing and decreasing of prescribed dose (±2%, ±4% and ±6%) and position shifted in X-axis and Y-axis (±1, ±2, ±3 and ±5 mm). After measurement, the results were compared between calculated and measured values using gamma analysis at 3%/3 mm criteria. The average gamma pass for no errors introduced in head and neck plans was 96.9%, 98.6%, and 98.8%, while prostate plans presented 99.4%, 99.0%, and 99.7%, for portal dosimetry, MapCHECK2 and MatriXX system, respectively. In head and neck plan, the shifted error detections were 1 mm for portal dosimetry, 2 mm for MapCHECK2, and 3 mm for MatriXX system. In prostate plan, the shifted error detections were 2 mm for portal dosimetry, 3 mm for MapCHECK2, and 5 mm for MatriXX system. For the dose error detection, the portal dosimetry system could detect at 2% dose deviation in head and neck and 4% in prostate plans, while other two devices could detect at 4% dose deviation in both head and neck and prostate plans. Portal dosimetry shows slightly more capability to detect the error compared with MapCHECK2 and MatriXX system, especially in the complicated plan. It may be due to higher resolution of the detector;however, all three-detector types can detect various errors and can be used for patient-specific IMRT QA.

Using an experimental setup, the series configurations (SC) and the parallel configurations (PC) of the PV cell connection are studied to compare their performance under the condition of partial shading s. The performance of the configurations is evaluated by comparing the open-circuit voltage, the short-circuit current, the maximum power point (MPP), the voltage and current corresponding to MPP, and the Fill Factor (FF). The variations of the series resistance and the shunt resistance of a PV module under different irradiance levels are also determined by considering the effect of thermal voltage. Finally, a comparison between the performance losses in the different configurations is presented. The results of this study show that the parallel configuration has the best performance under the conditions of partial shade in the context of this work.

The development of alternatives energies illustrates the common interest of all countries in reducing greenhouse gas emissions and combating climate change. Thermochemical treatment of municipal solid waste, agricultural and forestry wastes is a major challenge for this XXIst century to replace petroleum fuels. Thermogravimetric analysis (TGA) makes it possible to elucidate the thermal behavior of Casamance (Senegal) biomass residues, mass losses and decomposition rate, under inert (N2) atmosphere and oxidizing (O2) atmosphere. Carbonization and briquetting techniques of these various residues encountered in this part of Senegal country, by densification in order to produce fuel briquettes (call biochars) will be used to improve stoves for cooking. Samples used in this study are peanuts shells (PNS), cashew nut shells (CNS), palm nut shells (PLS) and millet stems (MS). Elemental and approximate analyses make it possible to determine the CHNSO* composition, volatile matter, fixed carbon and ashes content of the samples used. Higher heating values (HHV) of the former residues are ranging from 28.60 MJ·kg-1, 26.51 MJ·kg-1, 29.69 MJ·kg-1 and 24.93 MJ·kg-1 respectively. The chars are obtained by slow pyrolysis with a heating rate of 5°C·min-1 from ambient temperature up to 800°C under inert atmosphere. The morphology of the samples is different for the four biomasses studied, from biomass in the form of wood fibers to a more compact biomass. The parietal composition of different samples presented was determined by Van Soest method using neutral detergents (NDS), acid detergent (ADS) and sulfuric acid (H2SO4, 72%);to solubilize successively extractable, hemicellulose and cellulose respectively;lignin was obtained by balance (Table 2). The samples show a high level of cellulose, this pseudo-component is very rich in carbon directly linked to the calorific value, whose values vary from 32.35%;24.20%;34.94% and 39.67% for PNS, PLS,

The African rice midge, Orseolia oryzivora H. & G., is an important insect pest in Burkina Faso. Our study was conducted from 2017 to 2019 in three irrigated rice schemes including Banzon, Karfiguéla and Vallée du Kou in Western Burkina Faso. The objective of this study was to investigate the ecology of the insect pest. In each rice scheme, four plots of 500 m2 each, spread 50 m apart were randomly selected in farmers’ fields and used for the study. In each plot, a light trap was implemented along one of the diagonals of the plot. Each light trap consisted of a yellow plastic round bowl containing soapy water almost at half the bowl and a torch. The bowl had a capacity of 4.5 l, a diameter of 28 cm and a height of 10.5 cm. The torch had 3 batteries of 1.5 V each, 30 cm long and 1 cm diameter. The torches were lit every night at 6 pm and off at 6 am. Four hundred tillers were collected per month from the cultivated rice and from each of the alternative hosts of the rice midge in order to evaluate the monthly gall count, pre-imaginal populations and parasitism associated with O. oryzivora and its cousin, O. bonzii that is hosted by Paspalum scrobiculatum. The number of O. oryzivora’s adults caught in the light traps was higher during the wet season than the dry season. The level of larval parasitism ranged from 6.16% (Banzon) to 14.29% (Vallée du Kou). The highest level of pupal parasitism (13.73%) was recorded in Vallée du Kou. The damage of O. oryzivora was observed on the wild rice species, O. longistaminata in all three sites. The highest levels of parasitism associated with O. bonzii were recorded in Karfiguéla (20.08% and 25.21% for larval parasitism and pupal parasitism, respectively). These findings are useful in the development of a sustainable control strategy of O. oryzivora.
