Otolaryngology residency program factors associated with female resident representation

Objective:Female representation in the field of otolaryngology is lacking.Residency is the first point at which medical school graduates specialize in a chosen field and thus represents an opportunity to recruit and train more female otolaryngologists.This study sought to identify program factors associated with greater female representation among resident physicians.Methods:Departmental websites of all 124 otolaryngology residency programs in the United States and Puerto Rico were examined for a list of residents.For programs with a resident roster available,the genders of residents,faculty,program directors,and chairpersons were recorded.Location and city population for each program was also recorded,as was female resident representation.Programs were compared using Pearson Chi‐squared univariate tests.Results:1,632 residents and 2,605 faculty were included in the analysis of 109 programs.The median female resident representation was 40%.Programs with larger faculty sizes,more female faculty,and urban location were associated with an above‐median female resident representation.Programs with a larger residency cohort approached significance regarding above‐median female resident representation.Higher female faculty representation,program director gender,chairperson gender,and US region were not associated with variation in female resident representation.Conclusions:Greater female otolaryngology residency representation was associated with programs having an urban location and greater numbers of female and total faculty.It was also likely that a larger resident cohort size may affect female resident representation.The proportions of female faculty,program director,and chairperson gender,as well as the US region,were not associated with variation in female resident gender representation.

We report the confirmation of a sub-Saturn-size exoplanet,TOI-1194 b,with a mass of about 0.456+0.055-0.051M_(j),and a very low mass companion star with a mass of about 96.5±1.5 Mj,TOI-1251 *** candidates provided by the Transiting Exoplanet Survey Satellite(TESS)are suitable for further follow-up observations by ground-based telescopes with small and medium apertures.The analysis is performed based on data from several telescopes worldwide,including telescopes in the Sino-German multiband photometric campaign,which aimed at confirming TESS Objects of Interest(TOIs)using ground-based small-aperture and medium-aperture telescopes,especially for long-period targets.TOI-1194 b is confirmed based on the consistent periodic transit depths from the multiband photometric data.We measure an orbital period of 2.310644±0.000001 days,the radius is 0.767+0.045-0.041Rj and the amplitude of the RV curve is 69.4_(-7.3)^(+7.9)m s^(-1).TOI-1251 B is confirmed based on the multiband photometric and high-resolution spectroscopic data,whose orbital period is 5.963054+0.000002-0.000001days,radius is 0.947+0.035-0.033 R_(j) and amplitude of the RV curve is 9849_(-40)^(+42)ms^(-1).

Imaged guided surgery during arteriovenous malformation of gastrointestinal stromal tumor using hyperspectral and indocyanine green visualization techniques:A case report

BACKGROUND This case report demonstrates the simultaneous development of a gastrointestinal stromal tumour(GIST)with arteriovenous malformations(AVMs)within the jejunal mesentery.A 74-year-old male presented to the department of surgery at our institution with a one-month history of abdominal pain.Contrast-enhanced computed tomography revealed an AVM.During exploratory laparotomy,hyperspectral imaging(HSI)and indocyanine green(ICG)fluorescence were used to evaluate the extent of the tumour and determine the resection margins.Intraoperative imaging confirmed AVM,while histopathological evaluation showed an epithelioid,partially spindle cell GIST.CASE SUMMARY This is the first case reporting the use of HSI and ICG to image GIST intermingled with an AVM.The resection margins were planned using intraoperative analysis of additional optical data.Image-guided surgery enhances the clinician’s knowledge of tissue composition and facilitates tissue differentiation.CONCLUSION Since image-guided surgery is safe,this procedure should increase in popularity among the next generation of surgeons as it is associated with better postoperative outcomes.

Cost-effectiveness of patient specific vs conventional instrumentation for total knee arthroplasty:A systematic review and meta-analysis

BACKGROUND Over the past years,patient specific instrumentation(PSI)for total knee arthroplasty(TKA)has been implemented and routinely used.No clear answer has been given on its associated cost and cost-effectiveness when compared to conventional instrumentation(CI)for TKA.AIM To compare the cost and cost-effectiveness of PSI TKA compared to CI TKA.METHODS A literature search was performed in healthcare,economical healthcare,and medical databases(MEDLINE,EMBASE,CINAHL,Web of Science,Cochrane Library,EconLit).It was conducted in April 2021 and again in january 2022.Relevant literature included randomised controlled trials,retrospective studies,prospective studies,observational studies,and case control studies.All studies were assessed on methodological quality.Relevant outcomes included incremental cost-effectiveness ratio,quality-adjusted life years,total costs,imaging costs,production costs,sterilization associated costs,surgery duration costs and readmission rate costs.All eligible studies were assessed for risk of bias.Meta-analysis was performed for outcomes with sufficient data.RESULTS Thirty-two studies were included into the systematic review.Two were included in the metaanalysis.3994 PSI TKAs and 13267 CI TKAs were included in the sample size.The methodological quality of the included studies,based on Consensus on Health Economic Criteria-scores and risk of bias,ranged from average to good.PSI TKA costs less than CI TKA when considering mean operating room time and its associated costs and tray sterilization per patient case.PSI TKA costs more compared to CI TKA when considering imaging and production costs.Considering total costs per patient case,PSI TKA is more expensive in comparison to CI TKA.Meta-analysis comparing total costs for PSI TKA,and CI TKA showed a significant higher cost for PSI TKA.CONCLUSION Cost for PSI and CI TKA can differ when considering distinct aspects of their implementation.Total costs per patient case are increased for PSI TKA when compared to CI TKA.

The idea of using ultrashort X-ray pulses to obtain images of single proteins frozen in time has fascinated and inspired many.It was one of the arguments for building X-ray free-electron lasers.According to theory,the extremely intense pulses provide sufficient signal to dispense with using crystals as an amplifier,and the ultrashort pulse duration permits capturing the diffraction data before the sample inevitably explodes.This was first demonstrated on biological samples a decade ago on the giant mimivirus.Since then,a large collaboration has been pushing the limit of the smallest sample that can be imaged.The ability to capture snapshots on the timescale of atomic vibrations,while keeping the sample at room temperature,may allow probing the entire conformational phase space of macromolecules.Here we show the first observation of an X-ray diffraction pattern from a single protein,that of Escherichia coli GroEL which at 14 nm in diameter is the smallest biological sample ever imaged by X-rays,and demonstrate that the concept of diffraction before destruction extends to single proteins.From the pattern,it is possible to determine the approximate orientation of the protein.Our experiment demonstrates the feasibility of ultrafast imaging of single proteins,opening the way to single-molecule time-resolved studies on the femtosecond timescale.

High-resolution peat records for Holocene monsoon history in the eastern Tibetan Plateau

The variations of summer and winter monsoons during the Holocene in the eastern Ti-betan Plateau are shown to follow two basic models based on the reliable dating and high-resolution monsoon proxies determinations, one being a synchronous model in that both summer and winter monsoons are strengthening or decreasing, and the other to form a complementary pattern. These two different patterns evenly interact with each other on different time scales and together compose a complicated monsoon climatic model in this region. The climatic condition integrated by winter and summer monsoons is synchronous to the global pattern, which also shows the instability of the Holocene climate on centennial-millennial timescale. The abrupt monsoon event in about 6.2 ka cal. BP is much more severe than that in ca. 8.0 ka cal. BP, which indicates the regional character of the Asian monsoon and that the Asian monsoon climate is indeed a window on the global climate system.

Chronic fatigue is associated with increased disease-related worries and concerns in inflammatory bowel disease

AIM:To investigate the impact of chronic fatigue on disease-related worries in inflammatory bowel disease (IBD) and the potential multicolinearity between subjective questionnaires.METHODS:Patients in remission or with mild-tomoderate disease activity completed the fatigue questionnaire (FQ),the rating form of IBD patient concerns (RFIPC),the Short-Form 36 (SF-36),and IBD questionnaire (N-IBDQ).In addition,clinical and epidemiological data were obtained.RESULTS:In total,140 patients were included;of which 92 were diagnosed with ulcerative colitis and 48 with Crohn's disease.The mean age of patients with chronic fatigue was 44.2 years (SD=15.8) and for non-fatigued patients was 44.7 years (SD=16.0).Chronic fatigued patients had clinically significantly increased levels of disease-related worries,as measured by Cohen's d effect size.Worries about having an ostomy bag,loss of bowel control,and energy levels were most prominent in both chronic fatigued and non-chronic fatigued IBD patients.Variance inflation factor (VIF) and tolerance indicated that there were no problematic multicolinearity among the FQ,RFIPC,SF-36 and N-IBDQ responses (VIF 2).CONCLUSION:Chronic fatigue is associated with increased levels of disease-related worries and concerns in IBD.Increased levels of worries were also associated with impaired health-related quality of life.

Highly efficient passive Tesla valves for microfluidic applications

A multistage optimization method is developed yielding Tesla valves that are efficient even at low flow rates,characteristic,e.g.,for almost all microfluidic systems,where passive valves have intrinsic advantages over active ones.We report on optimized structures that show a diodicity of up to 1.8 already at flow rates of 20μl s^(−1) corresponding to a Reynolds number of 36.Centerpiece of the design is a topological optimization based on the finite element method.It is set-up to yield easy-to-fabricate valve structures with a small footprint that can be directly used in microfluidic systems.Our numerical two-dimensional optimization takes into account the finite height of the channel approximately by means of a so-called shallow-channel approximation.Based on the threedimensionally extruded optimized designs,various test structures were fabricated using standard,widely available microsystem manufacturing techniques.The manufacturing process is described in detail since it can be used for the production of similar cost-effective microfluidic systems.For the experimentally fabricated chips,the efficiency of the different valve designs,i.e.,the diodicity defined as the ratio of the measured pressure drops in backward and forward flow directions,respectively,is measured and compared to theoretical predictions obtained from full 3D calculations of the Tesla valves.Good agreement is found.In addition to the direct measurement of the diodicities,the flow profiles in the fabricated test structures are determined using a two-dimensional microscopic particle image velocimetry(μPIV)method.Again,a reasonable good agreement of the measured flow profiles with simulated predictions is observed.

Solid-State NMR Spectroscopic Study of Chromophore-Protein Interactions in the Pr Ground State of Plant Phytochrome A

Despite extensive study, the molecular structure of the chromophore-binding pocket of phytochrome A （phyA）, the principal photoreceptor controlling photomorphogenesis in plants, has not yet been successfully resolved. Here, we report a series of two-dimensional （2-D） magic-angle spinning solid-state NMR experiments on the recombi- nant N-terminal, 65-kDa PAS-GAF-PHY light-sensing module of phytochrome A3 from oat （Avena sativa）, assembled with uniformly 13C- and lSN-labeled phycocyanobilin （u-[13C,15N]-PCB-As.phyA3）. The Pr state of this protein was studied regarding the electronic structure of the chromophore and its interactions with the proximal amino acids. Using 2-D 13C-13C and 1HjSN experiments, a complete set of 13C and 15N assignments for the chromophore were obtained. Also, a large number of 1H-13C distance restraints between the chromophore and its binding pocket were revealed by inter- facial heteronuclear correlation spectroscopy. 13C doublings of the chromophore A-ring region and the C-ring carbox- ylate moiety, together with the observation of two Pr isoforms, Pr-I and Pr-II, demonstrate the local mobility of the chromophore and the plasticity of its protein environment. It appears that the interactions and dynamics in the binding pocket of phyA in the Pr state are remarkably similar to those of cyanobacterial phytochrome （Cphl）. The N-terminus of the region modeled （residues 56-66 of phyA） is highly mobile. Differences in the regulatory processes involved in plant and Cphl phytochromes are discussed.
