Myocardial perfusion imaging in patients with a recent, normal exercise test

AIM: To investigate the added value of myocardial perfusion scintigraphy imaging (MPI) in consecutive patients with suspected coronary artery disease (CAD) and a recent, normal exercise electrocardiography (ECG). METHODS: This study was a retrospective analysis of consecutive patients referred for MPI during a 2-year period from 2006-2007 at one clinic. All eligible patients were suspected of suffering from CAD, and had performed a satisfactory bicycle exercise test (i.e. , peak heart rate > 85% of the expected, age-predicted maximum) within 6 mo of referral, their exercise ECG was had no signs of ischemia, there was no exercise-limiting angina, and no cardiac events occurred between the exercise test and referral. The patients subsequently underwent a standard 2-d, stress-rest exercise MPI. Ischemia was defined based on visual scoring supported by quantitative segmental analysis (i.e. , sum of stress score > 3). The results of cardiac catheterizationwere analyzed, and clinical follow up was performed by review of electronic medical files. RESULTS: A total of 56 patients fulfilled the eligibility criteria. Most patients had a low or intermediate ATPⅢ pretest risk of CAD (6 patients had a high pre-test risk). The referral exercise test showed a mean Duke score of 5 (range: 2 to 11), which translated to a low postexercise risk in 66% and intermediate risk in 34%. A total of seven patients were reported with ischemia by MPI. Three of these patients had high ATPⅢ pre-test risk scores. Six of these seven patients underwent cardiac catheterization, which showed significant stenosis in one patient with a high pre-test risk of CAD, and indeterminate lesions in three patients (two of whom had high pre-test risk scores). With MPI as a gate keeper for catheterization, no significant, epicardial stenosis was observed in any of the 50 patients (0%, 95% confidence interval 0.0 to 7.1) with low to intermediate pre-test risk of CAD and a negative exercise test. Otolaryngology residency program factors associated with female resident representation

Objective:Female representation in the field of otolaryngology is *** is the first point at which medical school graduates specialize in a chosen field and thus represents an opportunity to recruit and train more female *** study sought to identify program factors associated with greater female representation among resident ***:Departmental websites of all 124 otolaryngology residency programs in the United States and Puerto Rico were examined for a list of *** programs with a resident roster available,the genders of residents,faculty,program directors,and chairpersons were *** and city population for each program was also recorded,as was female resident *** were compared using Pearson Chi‐squared univariate ***:1,632 residents and 2,605 faculty were included in the analysis of 109 *** median female resident representation was 40%.Programs with larger faculty sizes,more female faculty,and urban location were associated with an above‐median female resident *** with a larger residency cohort approached significance regarding above‐median female resident *** female faculty representation,program director gender,chairperson gender,and US region were not associated with variation in female resident ***:Greater female otolaryngology residency representation was associated with programs having an urban location and greater numbers of female and total *** was also likely that a larger resident cohort size may affect female resident *** proportions of female faculty,program director,and chairperson gender,as well as the US region,were not associated with variation in female resident gender representation.

High-resolution peat records for Holocene monsoon history in the eastern Tibetan Plateau

The variations of summer and winter monsoons during the Holocene in theeastern Tibetan Plateau are shown to follow two basic models based on the reliable dating andhigh-resolution monsoon proxies determinations, one being a synchronous model in that both summerand winter monsoons are strengthening or decreasing, and the other to form a complementary *** two different patterns evenly interact with each other on different time scales and togethercompose a complicated monsoon climatic model in this region. The climatic condition integrated bywinter and summer monsoons is synchronous to the global pattern, which also shows the instability ofthe Holocene climate on centennial-millennial timescale. The abrupt monsoon event in about 6.2 kacat. BP is much more severe than that in ca. 8.0 ka cal. BP, which indicates the regional characterof the Asian monsoon and that the Asian monsoon climate is indeed a window on the global climatesystem.

The Role of Attitude Strength in Predicting Organ Donation Behaviour by Implicit and Explicit Attitude Measures

Explicit attitude measures seem to be rather poor predictors of organ donation behaviour. This study examined whether implicit attitude measures acted as better predictors. Special attention was paid to the moderating role played by attitude strength in predicting deliberate donation behaviour. In this study (N = 78), explicit and implicit measures of attitudes towards organ donation, as well as behavioural commitment, were assessed. Implicit associations were measured by an Implicit Association Test;five constructs assessed self-reported—understood as reflected—attitude strength. The explicit attitude measure appeared to be the best single predictor of whether the participant ended up taking an organ donor card. More importantly, test results demonstrated the moderating role of attitude strength. In the case of low attitude strength, the likelihood of taking a donor card increased with an increasing positive implicit association. In contrast, increasing strength was associated with a weaker link between card taking and the implicit attitude measure. The results are discussed in light of the power of implicit associations to predict more deliberate behaviours.

Liquid water structure from X-ray absorption and emission, NMR shielding and X-ray diffraction

Here we investigate to what extent X-ray absorption（XAS） and emission（XES） spectroscopy, the oxygen-oxygen radial distribution function and σ（1H） and σ(17O) NMR shielding can be represented by a common set of model structures of liquid water. This is done by using a Monte Carlo-based fitting technique which fits the spectra based on a library of ～1400 precomputed spectra and assigns weights to contributions from different model structures. These are then used to reweight the contributions from the structures in the library to reveal classes of structures that are over-or under-represented in the library. The goal is to include different experimental data sets which are sensitive to different aspects of liquid water structure and thus narrow down which types of structures must exist in the real liquid.

Targeted proteomics for biomarker discovery and validation of hepatocellular carcinoma in hepatitis C infected patients

Hepatocellular carcinoma(HCC)-related mortality is high because early detection modalities are hampered by inaccuracy,expense and inherent procedural *** there is an urgent need for minimally invasive,highly specific and sensitive biomarkers that enable earlydisease detection when therapeutic intervention remains practical. Successful therapeutic intervention is predicated on the ability to detect the cancer early. Similar unmet medical needs abound in most fields of medicine and require novel methodological approaches. Proteomic profiling of body fluids presents a sensitive diagnostic tool for early cancer detection. Here we describe such a strategy of comparative proteomics to identify potential serum-based biomarkers to distinguish high-risk chronic hepatitis C virus infected patients from HCC patients. In order to compensate for the extraordinary dynamic range in serum proteins,enrichment methods that compress the dynamic range without surrendering proteome complexity can help minimize the problems associated with many depletion methods. The enriched serum can be resolved using 2D-difference in-gel electrophoresis and the spots showing statistically significant changes selected for identification by liquid chromatographytandem mass spectrometry. Subsequent quantitative verification and validation of these candidate biomarkers represent an obligatory and rate-limiting process that is greatly enabled by selected reaction monitoring(SRM). SRM is a tandem mass spectrometry method suitable for identification and quantitation of target peptides within complex mixtures independent on peptidespecific antibodies. Ultimately,multiplexed SRM and dynamic multiple reaction monitoring can be utilized for the simultaneous analysis of a biomarker panel derived from support vector machine learning approaches,which allows monitoring a specific disease state such as early HCC. CFD-simulation of a circulating fluidized bed riser

In the current work, a model of the fluid mechanics in the riser of a circulating fluidized bed （CFB） has been implemented using computational fluid dynamics （CFD）. The model developed shall be used in future as the basis of 3D-reactor model for the simulation of large scale CFB combustors. The two-fluid model （TFM） approach is used to represent the fluid mechanics involved in the flow. The computational implementation is accomplished by the commercial software FLUENT. Different closure formulations are tested on a simplified geometry. Two different turbulence formulations, namely the swirl modified RNG k-e model and the Realizable k-e model, are tested in combination with two different approaches to solid phase turbulence, namely the dispersion and per phase approach. One focus of the current work is put on the study of different drag correlations. Besides the drag correlations by Syamlal et al. [Syamlal, M., Rogers, W., ＆ O＇Brien, T. j. （1993）. MFIX documentation theory guide. Technical Report DOE/METC-9411004, U.S. Department of Energy （DOE）. Morgantown Energy Technology Center： Morgantown, WV] and Gidaspow [Gidaspow, D. （1994）. Multiphaseflow andfluidization. New York： Academic Press] the EMMS model has been used to determine the momentum exchange between the two phases. Clinical and dietary predictors of common carotid artery intima media thickness in a population with type 1 and type 2 diabetes: A cross-sectional study

AIM To determine the clinical and dietary predictors of common carotid artery intima media thickness(CCA IMT) in a cohort of subjects with type 1 and type 2 diabetes. METHODS Participants with type 1(n = 23) and type 2 diabetes(n = 127) had mean and mean maximum CCA IMT measured using B mode ultrasound. Dietary intake was measured using a food frequency questionnaire. Clinical and dietary predictors of mean and mean maximum CCA IMT were determined using linear regression analysis adjusted for potential confounders. RESULTS The main predictors of mean and mean maximum CCA IMT were age and weight. After multivariate adjustment there were no dietary predictors of CCA IMT. However,in subjects that were not prescribed a lipid lowering medication alcohol consumption was positively associated with CCA IMT after multivariate adjustment. No difference existed in CCA IMT between subjects with type 1 or type 2 diabetes once age was adjusted for. CONCLUSION CCA IMT was predominantly predicted by age and weight in these subjects with diabetes. The finding that CCA IMT was not different between people with type 1 and type 2 diabetes warrants further investigation in a larger cohort.
