Ductility demands on buckling-restrained braced frames under earthquake loading

Accurate estimates of ductility demands on buckling-restrained braced frames(BRBFs)are crucial to performance-based design of BRBFs.An analytical study on the seismic behavior of BRBFs has been conducted at the ATLSS Center,Lehigh University to prepare for an upcoming experimental program.The analysis program DRAiN-2DX was used to model a one-bay,four-story prototype BRBF including material and geometric nonlinearities.The buckling- restrained brace(BRB)model incorporates both isotropic and kinematic hardening.Nonlinear static pushover and time- history analyses were performed on the prototype BRBF.Performance objectives for the BRBs were defined and used to evaluate the time-history analysis results.Particular emphasis was placed on global ductility demands and ductility demands on the BRBs.These demands were compared with anticipated ductility capacities.The analysis results,along with results from similar previous studies,are used to evaluate the BRBF design provisions that have been recommended for codification in the United States.The results show that BRB maximum ductility demands can be as high as 20 to 25.These demands significantly exceed those anticipated by the BRBF recommended provisions.Results from the static pushover and time- history analyses are used to demonstrate why the ductility demands exceed those anticipated by the recommended provisions. The BRB qualification testing protocol contained in the BRBF recommended provisions is shown to be inadequate because it requires only a maximum ductility demand of at most 7.5.Modifications to the testing protocol are recommended.

A Systematic Approach to Pulmonary Valve Replacement in the Current Era

Background:Pulmonary valve replacement(PVR)can be accomplished via surgical,transcatheter,or hybrid approaches.There are inherent advantages to transcatheter PVR and hybrid PVR without cardiopulmonary bypass.We review the methods and results of a standardized institutional approach to PVR.Methods:Retrospective review of all PVR cases between February 2017 and February 2020.Hybrid PVR entailed off-pump RVOT plication with percutaneous transcatheter PVR.Results:Primary transcatheter PVR was attempted in 37,hybrid PVR was performed in 11,and on-pump surgical PVR was performed in 9.Median age at PVR was 27 years(6–65).Primary transcatheter PVR was successful in 35/37(2 converted to surgical).Standard surgical PVR was utilized for positive coronary compression testing(n=4),stent/valve system migration(n=2),or patient preference(n=3).in the hybrid group mean RVOT diameter was 34 mm(32–38).Median length of stay was 1 day for transcatheter PVR,5 for surgical,and 3 for hybrid(p=0.02).Median follow-up was 1.5 years.Re-interventions were one balloon valve dilation in a transcatheter PVR,and one valve dilation with subsequent transcatheter valve-in-valve PVR in the surgical cohort.One hybrid patient expired 11 months post procedure.Conclusions:A systematic approach to PVR utilizing all approaches in pre-defined order of preference leads to consistent outcomes in a wide variety of anatomic configurations.Transcatheter PVR may be accomplished in the majority of patients.When necessary,hybrid off-pump RVOT plication with transcatheter PVR avoids the need for cardiopulmonary bypass.

Acute Syphilitic Hepatitis in the Early Secondary Phase: A Case Report and Literature Review

Background: The incidence of syphilis is clearly increasing. it is a source of visceral damage, particularly in the secondary phase. Acute syphilitic hepatitis is a rare clinical entity classically described but under-diagnosed because of its non-specific presentation. Case Presentation: We report the case of a 25-year-old French woman, without comorbidity, sexually active, having unprotected sex with only one partner. She was admitted to our unit for jaundice and a disturbed liver function test preceded 3 weeks earlier by a rash. The patient did not consume alcohol and did not report the introduction of a new medication. A skin rash compatible with syphilitic roseola and generalized adenopathy were found. The liver workup showed alanine aminotransferase (ALT) at 1984 U/L, aspartate aminotransferase (AST) at 1377 U/L, total bilirubin at 221 μmol/L, alkaline phosphatase activity (APL) at 419 U/L, gamma glutamyl transferase activity (GGT) at 229 U/L and a prothrombin concentration at 73%. The search for the most common etiologies of acute hepatitis was negative. Syphilitic serology was positive with a Treponema Pallidum Hemagglutinations Assay (TPHA) titration of 5120 iU and Veneral Disease Research Laboratory (VDRL) of 64 iU. Abdominal ultrasound revealed only homogeneous hepatomegaly and splenomegaly with no focal lesions. Other sexually transmitted infections (STi) were negative and her partner had positive syphilis serology. After a single dose of Benzathine benzylpenicillin, the clinical signs regressed after one week and the hepatic balance tended to normalize 20 days later. Conclusion: This highlights the need for a high index of suspicion for syphilitic hepatitis in sexually active patients presenting with acute hepatitis associated with a cutaneous rash.

Effect of Pregnancy on TMJ Nociception in Rats

Objective:Does TMJ nociception,as measured by a behavioral assay,differ during pregnancy,potentially due to the effect of sex steroids?Methods:Thirty-two Sprague Dawley rats were randomly allocated to either mid-(E11)or late-(E17)pregnancy groups.The animals within each group were further allocated to a saline or CFA treatment.At E11 or E17,the animals were injected bilaterally into TMJ with either saline(15μL)or 1μg/μL CFA(15μL).Nociception was measured with a feeding assay(pellet recording)and analyzed by comparing intra-meal rates using a distance-based permutation method.Results:CFA injection resulted in the animals eating longer in both the mid-pregnant and late-pregnant groups.Plasma estradiol was higher in the late-pregnant group versus the mid-pregnant group.importantly,the CFA injected late-pregnant group ate for a shorter amount of time(i.e.,shorter meal rate)than the CFA injected rats at mid-pregnancy.Conclusion:The reduced nociceptive response(shorter meal rate)in the CFA injected late-pregnant group may be due to greater estradiol versus the CFA injected mid-pregnancy rats.Thus,one potential reason why women report fewer TMJ symptoms during pregnancy is the higher level of circulating estradiol.

Odontogenic Herpes Zoster infection: A Case Report and Review

Herpes zoster is a commonly encountered entity of which all clinicians should be aware. The diagnosis is most often considered and made based on a characteristic vesicular rash that presents in a unilateral dermatomal distribution and is usually accompanied by a painful neuritis. Not uncommonly, the pain presents a few days before the rash becomes evident and may even occur without the rash appearing. When this happens patients may be subject to further diagnostic testing seeking alternative diagnoses, as herpes zoster is known to mimic a variety of other non-cutaneous organ system entities. Although the thoracic and lumbar dermatomes are the affected most frequently, in approximately one-fifth of cases the cranial nerves are involved either singularly or in combination. Trigeminal nerve zoster is of particular concern as it poses a risk of developing into zoster ophthalmicus with subsequent keratitis and uveitis resulting in permanent vision impairment. involvement of the second (maxillary) and third (mandibular) branches of this 5th cranial nerve are less common and may present with signs and symptoms of a primary dental process. The infrequency and unfamiliarity of herpes zoster odontogenic manifestations can lead to unnecessary investigation and treatment. Apropos such a case we review the epidemiology, pathophysiology, signs, and symptoms of odontogenic herpes zoster.

Experimental and Modeling Study of Forest Fire Effect on Soil Thermal Conductivity

An understanding of soil thermal conductivity after a wildfire or controlled burn is important to land management and post-fire recovery efforts. Although soil thermal conductivity has been well studied for non-fire heated soils, comprehensive data that evaluate the long-term effect of extreme heating from a fire on the soil thermal conductivity are limited. The purpose of this study was to evaluate the long-term impact of fire on the effective thermal conductivity of soils by directly comparing fire-heated and no-fire control soils through a series of laboratory studies. The thermal conductivity was measured for ten soil samples from two sites within the Manitou Experimental Forest, Colorado, USA, for a range of water contents from saturation to the residual degree of saturation. The thermal conductivity measured was compared with independent estimates made using three empirical models from literature, including the Campbell et al.(1994), Cté and Konrad(2005), and Massman et al.(2008) models. Results demonstrate that for the test soils studied, the thermal conductivity of the fire-heated soils was slightly lower than that of the control soils for all observed water contents.Modeling results show that the Campbell et al.(1994) model gave the best agreement over the full range of water contents when proper fitting parameters were employed. Further studies are needed to evaluate the significance of including the influence of fire burn on the thermal properties of soils in modeling studies.
