Impact of Association between Functional Training and Respiratory Muscle Training in Elderly: A Randomized Controlled Trial

The elderly population in Brazil has been growing exponentially according to data presented by the IBGE. It is estimated that by 2025 the population will have reached 32 million, placing the country sixth in the world in the population of elderly people. To assess the results of respiratory training associated with functional training on respiratory muscle strength, lung function, and functionality of the elderly. The sample was composed of 40 elderlies, divided into four groups, the experimental group with functional training, one experimental group with functional training associated with respiratory training, one experimental group with respiratory training, and the fourth group, the control group. After the initial and final evaluations, the variables measured were: The Breathing Test, Sit to Stand Test, the SF-36 Quality of Life Questionnaire, and Spirometry. Twenty appointments were conducted twice a week. There were significant differences for variables maximum inspiratory and expiratory pressure;more accentuated in the associated group when compared to the respiratory group;forced vital capacity and forced expiratory volume in the first second/forced vital capacity were meaningful in the associated group;peak expiratory flow means values increased after the intervention;in the six-minute walk test, we noticed a trend to increase values after the intervention. Functional training as well as associated respiratory muscle training provide the most beneficial changes compared only to functional and respiratory training. Therefore, this may be an effective clinical training method for patients’ elderly.

Missed Pancreaticobiliary Malignancy: The Flaw of the Expedited Cholecystectomy

Background: Early cholecystectomy has been recommended for patients with acute cholecystitis and gallstone pancreatitis. However, patients with pancreaticobiliary malignancy may present acutely with similar symptoms. We hypothesize that the diagnoses of these malignancies may potentially be delayed as an unintended consequence of expedited cholecystectomies. This study reviews a cohort of patients who underwent pancreaticoduodenectomy (PD) to identify those who underwent a separate cholecystectomy before their PD. Methods: We retrospectively reviewed 162 PDs performed between 2012 and 2022. Data collected included: demographics, disease etiology and the presence of cholelithiasis. We identified patients who had a previous cholecystectomy and the time elapsed before PD as well as procedures done during the interval. We reported detailed case summaries on those patients who had a cholecystectomy within 1 year of PD. Results: In the entire cohort, mean age was 65 years, 54% were males, and 83% had a malignant reason for PD. Thirty-one patients had cholelithiasis with 23 (14%) patients having had previous cholecystectomy. Six patients had cholecystectomy within 1 year of PD. They had the following malignancies: ampullary—3, pancreas—1, cholangiocarcinoma—1 and neuroendocrine—1. Four of these patients had expedited cholecystectomy on their index hospital admission and were later found to have a periampullary malignancy with further work up. Conclusions: Pancreaticobiliary malignancies can be difficult to diagnose, and surgeons should not overlook these potential diagnoses when considering expedited cholecystectomy. Future studies in large cohorts are needed to identify high risk candidates who should undergo more detailed testing to exclude malignancy before proceeding with cholecystectomy.

Feasibility of performing the 3-minute step test with remote supervision in children and adolescents with cystic fibrosis: A comparative study

Importance:The 3-min step test is a simple option to monitor submaximal exercise capacity,although its use via remote video monitoring has not been investigated in children with cystic fibrosis(CF).Objective:This study aimed to assess the feasibility and reproducibility of performing the 3-min step test with remote ***:A cross-sectional study including CF patients(6-18 years)from two CF services were ***,anthropometric,clinical,and lung function data were collected and two 3-min step tests were performed:(i)in-person supervision,and(ii)remotely supervised by video *** and after the tests,heart rate(HR),oxygen saturation(SpO2),and the Borg score for dyspnea and lower limb fatigue were ***:Twenty-three patients(10.7±3.7 years)with a mean FEV1 of 89.5%±23.2%were *** were no significant differences between tests,with mean differences(95%confidence intervals)in final HR of-3.3(-8.9,2.4),change in HR of-1.9(-6.1,2.1),final SpO2 of 0.3(-0.4,1.0),and final dyspnea of 0.1(-0.8,0.9).The intraclass correlation coefficient was 0.852(final HR),0.762(final SpO2),and 0.775(final lower limb fatigue).Significant and moderate correlations were found between tests for final HR(r=0.75),change in HR(r=0.61),and final SpO2(r=0.61).The Bland-Altman analysis showed a mean difference in final SpO2 between tests of 0.3%(limit of agreement-3.0%,3.5%).Interpretation:Physiological responses between tests were similar,indicating it was feasible to perform the 3-min step test with remote supervision in CF children.

Metabolic syndrome and the urinary microbiome of patients undergoing percutaneous nephrolithotomy

Objective:To identify possible stone-promoting microbes,we compared the profiles of microbes grown from stones of patients with and without metabolic syndrome(MetS).The association between MetS and urinary stone disease is well established,but the exact pathophysiologic relationship remains *** evidence suggests urinary tract dysbiosis may lead to increased nephrolithiasis ***:At the time of percutaneous nephrolithotomy,bladder urine and stone fragments were collected from patients with and without *** sample types were subjected to expanded quantitative urine culture(EQUC)and 16 S ribosomal RNA gene ***:Fifty-seven patients included 12 controls(21.1%)and 45 MetS patients(78.9%).Both cohorts were similar with respect to demographics and non-MetS *** controls had uric acid stone *** EQUC,bacteria were detected more frequently in MetS stones(42.2%)compared to controls(8.3%)(p=0.041).Bacteria also were more abundant in stones of MetS patients compared to *** validate our EQUC results,we performed 16 S ribosomal RNA gene *** 12/16(75.0%)sequence-positive stones,EQUC reliably isolated at least one species of the sequenced *** were detected in both“infectious”and“non-infectious”stone ***:Bacteria are more common and more abundant in MetS stones than control *** findings support a role for bacteria in urinary stone disease for patients with MetS regardless of stone composition.

The authors report a case of deficient sensory neuropathy secondary to vitamin B12 deficiency, diagnosed in the neurology department of the Sino-Central African Friendship University Hospital in Bangui. The diagnosis was made possible by electroneuromyography which showed subclinical neurological damage associated with hematological damage (anemia). Through this observation, we recall the diagnostic criteria of the disease in a context of difficult medical practice. .

目的:采用静息态功能磁共振成像(rs-f MRI)技术探究杨氏絮刺火罐治疗湿疹瘙痒的临床疗效和中枢机制。方法:50例湿疹瘙痒患者纳入观察组,50名健康受试者纳入对照组。对照组不行任何干预;观察组于大椎及双侧曲池、血海、三阴交行杨氏絮刺火罐治疗,每周1次,共治疗6周。比较观察组治疗前后12项瘙痒严重程度评估量表(12-PSS)、湿疹面积及严重程度(EASI)、皮肤病生活质量指数(DLQI)、匹兹堡睡眠质量指数(PSQI)和焦虑自评量表(SAS)评分。观察组治疗前后及对照组入组时均接受rs-fMRI扫描并进行局部一致性(ReHo)分析,将差异脑区ReHo值与上述量表评分进行相关性分析。结果:与治疗前比较,观察组治疗后12-PSS、EASI、DLQI、PSQI、SAS评分均降低(P<0.01,P<0.05)。与对照组比较,观察组治疗前右侧尾状核、右侧颞中回、右侧眶部额上回、右侧丘脑、左侧角回ReHo值增高(P<0.001);与治疗前比较,观察组治疗后上述脑区ReHo值下降(P<0.001)。与对照组比较,观察组治疗前左侧颞中回、左侧顶上小叶、左侧补充运动区ReHo值下降(P<0.001);与治疗前比较,观察组治疗后上述脑区ReHo值增高(P<0.001)。治疗前,观察组左侧补充运动区ReHo值与12-PSS评分呈正相关(r=0.432,P=0.004);右侧眶部额上回ReHo值与PSQI评分呈负相关(r=-0.318, P=0.04)。治疗后,观察组左侧顶上小叶ReHo值与12-PSS评分呈正相关(r=0.384,P=0.012)。结论:湿疹瘙痒患者多个参与刺激反应、情绪调节、行为控制、注意力等功能的脑区存在活动异常。杨氏絮刺火罐能有效改善湿疹患者瘙痒症状和皮肤损害,其作用机制可能和逆转瘙痒所导致的脑区功能活动异常有关。

Diversity among Isolates of the Tangerine Pathotype of Alternaria alternata

The tangerine pathotype of Alternaria alternata is the aetiological agent of Alternaria brown spot on tangerines. In the state of Paraíba, Brazil, its occurrence on “Dancy” tangerine trees is associated with genetic aspects as well as the influence of environmental conditions on reproduction and dissemination within and between populations. The aim of the present study was to evaluate the diversity of isolates of this pathogen using morphophysiological and molecular markers. For the analysis of mycelial growth and sporulation, 30 isolates from different locations were examined at 24-hour intervals until the seventh day, when the spores were quantified. The 30 isolates were characterised based on molecular markers (ISSR) and genetic similarity (Jaccard index). A factor arrangement was used: 30 isolates, four media (ODA, PDA, LEA and V8), three light regimes (continuous dark, alternating light and continuous light) and three temperatures (15°C, 25°C and 35°C), with 12 repetitions. Groups 1, 2 and 3 presented low genetic variability. Group 4 showed high genetic variability of the isolates obtained from the Massaranduba (state of Paraíba-Brazil) producing region and higher mycelial growth and sporulation of A. alternata. The continuous light regime and the temperature 25°C in PDA and V8 media were the ideal conditions for the mycelial growth and sporulation, respectively, of the isolates of A. alternata.
