Auriculotherapy in primary health care: A large-scale educational experience in Brazil

Auriculotherapy consists of physical stimuli applied to the outer ear and is commonly associated with traditional Chinese medicine (TCM). The authors present and discuss the development of a course that offers a semi-on-site auriculotherapy course for Brazilian primary health care (PHC) professionals. The course was funded by the Brazilian Ministry of Health and developed at the Federal University of Santa Catarina in 2015 by a team of experts in auriculotherapy. It consisted of 75 h of distance learning (five sequential modules) and 5 h of on-site learning. The modules included the following items:(1) introduction to integrative practices;(2) ear reflexology;(3) introduction to TCM;(4) biomedical view of auriculotherapy;and (5) auriculotherapy in PHC. The teaching material in eluded a workbook for each module, 14 video lectures and an interactive ear(on line resource) to study location and application to the main auricular points. The on-site lectures follow a structured script of ear palpation techniques, auricular seed insertion practice and clinical case discussions, under the supervision of trained instructors. The course was offered in 2016 and 2017 and on-site lectures took place in 25 cities, covering all Brazilian regions, in coordination with municipal or state boards of health. A total of 4273 health professionals concluded the training and their evaluation of the course was highly positive. The Brazilian experience of large-scale training shows the potential to disseminate auriculotherapy in the context of PHC, given that its practice is fast, easy to learn, safe, effective for different health problems and well accepted by the patients.

Purpose: The involvement of the ocular anterior segment by SARS-CoV-2 has been the subject of many studies, however, the repercussions on the posterior segment, particularly on the different layers of the retina and optic nerve, are still little known. The purpose of this study was to evaluate the impact of severe COVID-19 on the retinal ganglion cell layer (RGCL) thickness. Methods: This observational, prospective and analytical study was performed in the Ophthalmology Department of the FACISA University Center, Campina Grande. Three groups were included: group I (control), 29 healthy individuals who had not severe COVID-19;group II (infirmary), 24 individuals who had COVID-19 and were hospitalized in the infirmary;and group III, 25 individuals who had severe COVID-19 and required Intense Care Unit (ICU). All individuals had ophthalmologic examination and assessment of RGCL thickness using Optical Coherence Tomography (OCT). Statistical tests required p ≤ 0.05 to reject the null hypothesis. Results: The mean of RGCL thickness was significantly reduced in individuals from GIII (77.9 ± 8.9 µm), as compared with GII (83.9 ± 10.9 µm) and GI (82.8 ± 6.5 µm) (p = 0.0027). The mean measurements from the retinal neve fiber layer (RNFL) of the optic nerve head were similar. However, when evaluated sectoral, the mean of RNFL at the temporal sector of the optic disc was significantly lower in group GIII (p Conclusion: The RGCL thickness from patients with severe COVID-19 was significantly reduced. This finding supports that the SARS-CoV-2 has systemic action and affinity for nerve cells, including those from the retina and are related to the severity of the infection.

Effect of silymarin on biochemical indicators in patients with liver disease: systematic review with meta-analysis

To evaluate the effect of silymarin on the serum levels of alanine aminotransferase (ALT), aspartate aminotransferase (AST) and gamma glutamyl transpeptidase (γGT) in patients with liver diseases. METHODSA systematic review with meta-analysis of ramdomized and controlled clinical trials was performed, evaluating the effects of sylimarin in patients with hepatic diseases, published by January 31, 2016. Clinical trials were sought on the basis of The Cochrane Central Register of Controlled Trials in the Cochrane Library, PubMed/Medline, Scopus, Web of Science, Lilacs and Clinical Trials. The trials with adult and elderly patients of both sexes, with Liver Diseases who took oral silymarin supplementation, as extract or isolated, as well as Silymarin combined with other nutrients, were included. The trials should provide information about the intervention, such as dosages and detailing of the product used, besides the mean and standard deviation of serum levels of ALT, AST and γGT of the baseline and at the end of the intervention. RESULTSAn amount of 10904 publications were identified. From those, only 17 were included in the systematic review and 6 in the meta-analysis, according to the used selection criteria. In this meta-analysis, the results indicated a reduction of 0.26 IU/mL (95%CI: -0.46-0.07, P = 0.007) at the level of ALT and 0.53 IU/mL (95%CI: -0.74-0.32, P = 0.000) at the serum levels of AST after using the silymarin, both, statistically significant, but with no clinical relevance. There was no significant change in the γGT levels. Subgroup analyzes were also performed for the biochemical markers in relation to the type of intervention, whether silymarin isolated or associated with other nutrients and the time of intervention (whether ≥ 6 mo or < 6 mo). In order to assess the prevalence of physical activity (PA) and its relationship with lipid profile among adolescents, a cross-sectional study with a quantitative approach was conducted with 102 adolescents aged 15 to 19 years from public schools of Campina Grande, Paraiba, Brazil. The following variables were analyzed: age, gender, ethnicity, maternal education, socioeconomic status, nutritional status, the practice of PA and lipid profile. The variables were obtained through validated form, anthropometry (weight and height) and blood collection by contract laboratory. Double entry and SPSS (Statistical Package for Social Sciences) 22.0 were performed. First, a descriptive analysis of all variables was conducted, as well as measures of central tendency and dispersion for continuous variables. Accordingly, distribution was tested for normality using the Kolmogorov-Smirnov test. According to results, correlation was evaluated by Pearson/Spearman tests. Significance level of 5% was adopted for all analyses. The study was approved by the Ethics Approach Committee (EAC) of the State University of Paraíba. The most prevalent sex in the sample was female and regarding maternal education, 35.4% had complete high school. Regarding serum lipoproteins, 16% showed elevated total cholesterol and HDL (High Density Lipoprotein) 66% below the desired value. With regard to physical activity, 60.8% had insufficient PA levels. The sample showed changes in lipid profile and insufficiently active life style, besides having overpowered the eutrophic nutritional status. No statistically significant correlations between variables of the lipid profile and the time of PA were observed. These findings highlight the importance of physical exercise for maintaining health and suggest studies with larger populations in order to check the scientific literature regarding this topic.

Introduction: The more intense and early development of obesity, the greater the risk of persistence and severity of comorbidities, such as cardiovascular disease. There is evidence that high serum concentrations of lipoprotein (a) [Lp (a)] and C-reactive protein (CRP) are associated with increased risk of cardiovascular diseases. Objectives: To verify the change in the levels of Lp (a) and ultrasensitive CRP and its relationship with the nutritional status of adolescents. Methods: Cross-sectional study conducted with overweight children and adolescents between August 2012 and July 2013 attended at the Center for Childhood Obesity. The measurement of inflammatory markers was performed in the Clinical Laboratory of the State University of Paraiba. Comparison of sociodemographic variables by sex was tested by chi-square;the association of the risk markers according to age was evaluated by Student’s t test, and the body mass index by analysis of Pearson correlation. The normality distribution was tested by the Kolmogorov-Smirnov test. Confidence interval of 95% was adopted in all analyses. The study was approved by the Ethics Research Committee of UEPB (CAAE 0256.***.133.000-11). Results: Of the 133 children and adolescents evaluated, 60.9% were female and 72.2% were adolescents. Body Mass Index, lipo (a) and u-CRP showed statistically significant association with age (p < 0.01). There was a positive ascending correlation (r = 0.273, p < 0.01) of u-CRP with BMI, which was not verified for lipoprotein (a). Conclusion: As a cardiovascular risk marker already established in literature, the association of u-CRP with the nutritional status of adolescents, proportionally, shows the need for losing weight in this population, especially at early age. A deeper and long-term investigation should be carried out for more effective and consistent contribution to public health.

Differential Pulse Voltammetric Studies on Lamivudine:An Antiretroviral Drug

Lamivudine (also known as 3TC) is a dideoxynucleoside analogue, which undergoes intracellular phosphorylation in the putative active metabolite, lamivudine triphosphate. Lamivudine triphosphate prevents HIV replication by competitively inhibiting viral reverse transcriptase. Lamivudine has been extensively used in the treatment of HIV patients owing to its antiretroviral activity. For the determination of lamivudine in pharmaceuticals, an analytical methodology using voltammetry was developed. Lamivudine was reduced at a hanging mercury drop electrode (HMDE) at –1.16 V vs Ag/AgCl at pH 2.0. The influence of electroanalytical parameters such as scan rate (20 mV.s–1), amplitude (50 mV), nature of the support electrolyte (Clark-Lubs), and pH (2.0) on the voltammetric signal was optimized. Under these optimized conditions, the method had been validated using pharmaceutical formulations. The lamivudine peak current varied linearly with its concentration from 1.15 to 10.40 mg.L–1, detection and determination limits of 0.46 and 1.0 mg.L–1, respectively, and recovery of 95.15% with a relative standard deviation of 1.10%.

Contraception Knowledge, Attitudes and Practices of Adolescents at Risk of Pregnancy in Northeastern Brazil

Background: Contraceptive methods (CM) are important resources in the prevention of adolescent pregnancy. This study aimed to investigate adolescent knowledge, attitudes and practices related to contraception. Study Design: A cross-sectional and exploratory study was conducted with 7th to 9th grade adolescents in northeastern Brazil. Their knowledge, attitudes, and contraceptive method use were analysed by gender, age and sexual initiation groups. Results: A sexual debut had been experienced by 21% (n = 120) of all respondents (n = 570). The majority of the respondents who had experienced a sexual debut were male (73.3%, n = 88), their mean age was 15 years (SD = 1.46), and 49.3% (n = 59) never used any CM. The participants displayed low knowledge and unfavourable and ambivalent attitudes concerning contraception. Conclusion: Adolescents initiate sexual intercourse with knowledge and attitudes that limit the use of CM, which makes them more at risk of pregnancy. Further research that will be representative of the specific population in need of interventions is necessary.

Brackish Shrimp Farming in Northeastern Brazil: The Environmental and Socio-Economic Impacts and Sustainability

Despite the economic importance of farmed shrimp, a number of technical, environmental, economic and social problems have been widely reported in the international literature. This paper focuses on the environmental and socio-economic impacts of semi-intensive and intensive shrimp farming in the coastal region of Northeastern Brazil and the identification of options for sustainable production. In this Region, the total area dedicated to shrimp farming is approximately 18,500 ha, of which 5750 ha are located in Ceará State. The estuary of Jaguaribe river has the largest number of shrimp farms in the state of Ceará. Currently, the industry has 64 participating farms with a total area dedicated to shrimp farming of 2411.3 ha. In 2011, the total production was 13,110 tons of shrimp with an average yield of 6330 kg ***-1 in a pond area of 2071.2 ha. This industry employs 2350 people that represent 23.2% of jobs generated in the two municipalities where the Jaguaribe river estuary is inserted. Compared with other countries, Brazil has reduced its exports due to high cost inputs for shrimp farming. However, the Brazilian shrimp industry has benefited from high domestic prices, despite the decreases in international price of shrimp. In 2011, the prices for size category ranged between U$ 4.67 - 6.04 for 80/100 (count of head-on shrimp), U$ 4.95 - 6.60 for 70/80 and U$ 5.
