Multidisciplinary comprehensive treatment of massive hepatocellular carcinoma with hemorrhage:A case report and review of literature

BACKGROUND Hepatocellular carcinoma(HCC),a major contributor to cancer-related deaths,is particularly prevalent in Asia,largely due to hepatitis B virus infection.Its prognosis is generally poor.This case report contributes to the medical literature by detailing a unique approach in treating a large HCC through multidisciplinary collaboration,particularly in patients with massive HCC complicated by ruptured bleeding,a scenario not extensively documented previously.CASE SUMMARY The patient presented with large HCC complicated by intratumoral bleeding.Treatment involved a multidisciplinary approach,providing individualized care.The strategy included drug-eluting bead transarterial chemoembolization,sorafenib-targeted therapy,laparoscopic partial hepatectomy,and standardized sintilimab monoclonal antibody therapy.Six months after treatment,the patient achieved complete radiological remission,with significant symptom relief.Imaging studies showed no lesions or recurrence,and clinical assessments confirmed complete remission.This report is notable as possibly the first docu-mented case of successfully treating such complex HCC conditions through integrated multidisciplinary efforts,offering new insights and a reference for future similar cases.CONCLUSION This study demonstrated effective multidisciplinary treatment for massive HCC with intratumoral bleeding,providing insights for future similar cases.

Histological Features of Uterine Myometrial Dysfunction:Possible Involvement of Localized Inflammation

Objective The latest perspective suggests that elevated levels of inflammation and cytokines are implicated in atonic postpartum hemorrhage.Lipopolysaccharide(LPS)has been widely used to induce inflammation in animal models.Therefore,this study aimed to induce uterine inflammation using LPS to investigate whether local inflammation triggers dysfunction and atrophy in the myometrium,as well as the potential underlying molecular mechanisms involved.Methods In vivo,an animal model was established by intraperitoneal injection of 300μg/kg LPS in rats on gestational day 21.Hematoxylin-eosin(H&E)staining and Masson staining were employed to determine morphological changes in the rat uterine smooth muscle.Enzyme-linked immunosorbent assay(ELISA)was used to detect inflammatory cytokines.Immunohistochemistry,tissue fluorescence,and Western blotting were conducted to assess the expression levels of the uterine contraction-related proteins Toll-like receptor 4(TLR4)and the nuclear factor kappa-B(NF-κB)signaling pathway.In vitro,human uterine smooth muscle cells(HUtSMCs)were exposed to 2μg/mL LPS to further elucidate the involvement of the TLR4/NF-κB signaling pathway in LPS-mediated inflammation.Results In this study,LPS induced uterine myometrial dysfunction in rats,leading to a disorganized arrangement,a significant increase in collagen fiber deposition,and widespread infiltration of inflammatory cells.In both in vivo animal models and in vitro HUtSMCs,LPS elevated IL-6,IL-1β,and TNF-αlevels while concurrently suppressing the expression of connexin 43(Cx43)and oxytocin receptor(OXTR).Mechanistically,the LPS-treated group exhibited TLR4 activation,and the phosphorylation levels of p65 and IκBαwere notably increased.Conclusion LPS triggered the TLR4/NF-κB signaling pathway,inducing an inflammatory response in the myometrium and leading to uterine myometrial dysfunction and uterine atony.

Plasmonic sensing technology has attracted considerable attention for high sensitivity due to the ability to effectively localize and manipulate light.In this study,we demonstrate a refractive index(RI)sensing scheme based on open-loop twisted meta-molecule arrays using the versatile nano-kirigami principle.RI sensing has the features of a small footprint,flexible control,and simple preparation.By engineering the morphology of meta-molecules or the RI of the ambient medium,the chiral surface lattice resonances can be significantly enhanced,and the wavelength,intensity,and sign of circular dichroism(CD)can be flexibly tailored.Utilizing the relation between the wavelength of the CD peak and the RI of the superstrate,the RI sensor achieves a sensitivity of 1133 nm/RIU.Additionally,we analyze these chiroptical responses by performing electromagnetic multipolar decomposition and electric field distributions.Our study may serve as an ideal platform for applications of RI measurement and provide new insights into the manipulation of chiral light–matter interactions.

Globally,2023 was the warmest observed year on record since at least 1850 and,according to proxy evidence,possibly of the past 100000 years.As in recent years,the record warmth has again been accompanied with yet more extreme weather and climate events throughout the world.Here,we provide an overview of those of 2023,with details and key background causes to help build upon our understanding of the roles of internal climate variability and anthropogenic climate change.We also highlight emerging features associated with some of these extreme events.Hot extremes are occurring earlier in the year,and increasingly simultaneously in differing parts of the world(e.g.,the concurrent hot extremes in the Northern Hemisphere in July 2023).Intense cyclones are exacerbating precipitation extremes(e.g.,the North China flooding in July and the Libya flooding in September).Droughts in some regions(e.g.,California and the Horn of Africa)have transitioned into flood conditions.Climate extremes also show increasing interactions with ecosystems via wildfires(e.g.,those in Hawaii in August and in Canada from spring to autumn 2023)and sandstorms(e.g.,those in Mongolia in April 2023).Finally,we also consider the challenges to research that these emerging characteristics present for the strategy and practice of adaptation.

Repair of Second-Generation Recycled Fine Aggregate of Waste Concrete from Freeze-Thaw Environment by Carbonation Treatment

The reuse of waste recycled concrete from harsh environments has become a research hotspot in the field of construction.This study investigated the repair effect of carbonation treatment on second-generation recycled fine aggregate(SRFA)obtained from recycled fine aggregate concrete(RFAC)subjected to freeze-thaw(FT)cycles.Before and after carbonation,the properties of SRFA were evaluated.Carbonated second-generation recycled fine aggregate(CSRFA)at five substitution rates(0%,25%,50%,75%,100%)to replace SRFA was used to prepare carbonated second-generation recycled fine aggregate concrete(CSRFAC).The water absorption,porosity and mechanical properties of CSRFAC were tested,and its frost-resisting durability was evaluated.The results showed after carbonation treatment,the physical properties of SRFA was improved and met the requirements of II aggregate.The micro-hardness of the interfacial transition zone and attached mortar in CSRFA was 50.5%and 31.2%higher than that in SRFA,respectively.With the increase of CSRFA replacement rate,the water absorption and porosity of CSRFAC gradually decreased,and the mechanical properties and frost resistance of CSRFAC were gradually improved.Carbonation treatment effectively repairs the damage of SRFA caused by FT cycles and improves its application potential.

水环境中过量的氮、磷和高锰酸盐会对流域造成严重污染,准确预测这三类指标的含量对流域污染治理具有重要意义。然而,现有的模型预测精度低,输入因子的选择缺乏数理依据。基于此,以邕江为研究区域,提出一种RF-BiLSTM的混合网络模型。该模型具有利用RF算法提取水质指标最优特征和利用BiLSTM模型提取输入数据的时间特征的优势,采用先降维后预测的方式对TN、TP和COD Mn进行预测,并将深度学习中的CNN、LSTM、BiLSTM和RF-LSTM作为基准模型与本研究所提模型作对比研究。研究结果表明,本研究模型预测TN、TP和COD Mn的平均绝对百分比误差(MAPE)分别达到了4.330%、6.781%和7.384%,均低于其他基准模型,预测结果具有较高的准确性和实用性,可为水环境的污染治理提供有效的技术支持。

非相似冗余架构被广泛使用到信息系统中,提高系统的安全性和可靠性。非相似冗余架构中的执行体之间存在差异,当系统正常工作时,执行体表现一致,但在面对恶意攻击行为时,执行体会表现出不一致。架构通过比较执行体的表现监控系统、感知威胁,从而提升系统安全可靠性。执行体的同步监控,是所有非相似冗余架构都需要解决的难题。目前没有针对同步技术比较系统性的描述和总结。该文首先对执行体同步问题进行了抽象建模,然后提出基于同步点的同步技术分类方法,并分别对每种技术的基本方式、流行度、优缺点进行了总结。该文还提出了影响同步效果的3个重要指标:同步点、误报率和性能,同时给出了同步技术的数学模型,可用于同步技术的设计评估。最后,结合网络弹性工程领域和软件定义晶上系统领域的发展,指出了同步技术未来的发展潜力和可能的发展方向。

Effect of an Ecological Executive Skill Training Program for School-aged Children with Attention Deficit Hyperactivity Disorder： A Randomized Controlled Clinical Trial

Background： As medication does not normalize outcomes of children with attention deficit hyperactivity disorder （ADHD）, especially in real-life functioning, nonpharmacological methods are important to target this field. This randomized controlled clinical trial was designed to evaluate the effects of a comprehensive executive skill training program for school-aged children with ADHD in a relatively large sample. Methods： The children （aged 6-12 years） with ADHD were randomized to the intervention or waitlist groups. A healthy control group was composed of gender- and age-matched healthy children. The intervention group received a 12-session training program lbr multiple executive skills. Executive function （EF）, ADHD symptoms, and social functioning in the intervention and waitlist groups were evaluated at baseline and the end of the final training session. The healthy controls （HCs） were only assessed once at baseline. Repeated measures analyses of variance were used to compare EF, ADHD symptoms, and social function between intervention and waitlist groups. Results： Thirty-eight children with ADH D in intervention group, 30 in waitlist group, and 23 healthy children in healthy control group were included in final analysis. At posttreatment, intervention group showed significantly lower Behavior Rating Inventory of Executive Function （BRIEF） total score （135.89 ±16.80 vs. 146.09 ± 23.92, P = 0.04） and monitoring score （18.05 ± 2.67 vs. 19.77 ± 3.10, P = 0.02）, ADHD-IV overall score （41.11 ± 7.48 vs. 47.20 ± 8.47, P 〈 0.01 ）, hyperactivity-impulsivity （HI） subscale score （ 18.92 ± 5.09 vs. 21.93 ± 4.93, P = 0.02）, and inattentive subscale score （22.18 ±3.56 vs. 25.27± 5.06, P 〈 0.01 ）, compared with the waitlist group. Repeated measures analyses of variance revealed significant interactions between time and group on the BRIEF inhibition subscale （F = 5.06, P 0.03）, working memory （F- 4.48, P = 0.04）, A DH D-IV overall score （F = 21.72, P 〈 0.01）, HI subscale score （F = 19.08, P 〈 0.01）, and inattentive subscale sc

Respiratory disease caused by coronavirus infection remains a global health crisis.Although several SARS-CoV-2-specific vaccines and direct-acting antivirals are available,their effcacy on emerging coronaviruses in the future,including SARS-CoV-2 variants,might be compromised.Host-targeting antivirals provide preventive and therapeutic strategies to overcome resistance and manage future outbreak of emerging coronaviruses.Cathepsin L(CTSL)and calpain-1(CAPN1)are host cysteine proteases which play crucial roles in coronaviral entrance into cells and infection-related immune response.Here,two peptidomimetic a-ketoamide compounds,14a and 14b,were identified as potent dual target inhibitors against CTSL and CAPN1.The X-ray crystal structures of human CTSL and CAPN1 in complex with 14a and 14b revealed the covalent binding of a-ketoamide groups of 14a and 14b to C25 of CTSL and C115 of CAPN1.Both showed potent and broad-spectrum anticoronaviral activities in vitro,and it is worth noting that they exhibited low nanomolar potency against SARS-CoV-2 and its variants of concern(VOCs)with ECso values ranging from 0.80 to 161.7 nM in various cells.Preliminary mechanistic exploration indicated that they exhibited anticoronaviral activity through blocking viral entrance.Moreover,14a and 14b exhibited good oral pharmacokinetic properties in mice,rats and dogs,and favorable safety in mice.In addition,both 14a and 14b treatments demonstrated potent antiviral potency against SARS-CoV-2 XBB 1.16 variant infection in a K18-hACE2 transgenic mouse model.And 14b also showed effective antiviral activity against HCoV-OC43 infection in a mouse model with a final survival rate of 60%.Further evaluation showed that 14a and 14b exhibited excellent antiinflammatory effects in Raw 264.7 mouse macrophages and in mice with acute pneumonia.Taken together,these results suggested that 14a and 14b are promising drug candidates,providing novel insight into developing pan-coronavirus inhibitors with antiviral and anti-inflammatory properties.
