Epidemiological, Clinical, Therapeutic, and Evolutionary Aspects of Heart Failure in the Medical Department at Gao Regional Hospital

Introduction: Heart failure is a major public health pandemic, particularly in Africa, where its prevalence continues to increase. In northern Mali, few data exist, hence the interest of this study with the objective of studying the epidemiological, clinical and therapeutic and evolutionary aspects of Heart failure at the regional hospital of Gao. Patients and Methods: This was a cross-sectional, descriptive study that took place from July 2022 to June 2023 in the medical department at Gao Hospital. Results: The hospital prevalence of heart failure was 44.1%. The mean age was 47.30 ± 20 years (range: 16-88). Hypertension was the most common with 46.1%, followed by a sedentary lifestyle, and diabetes with 18.2% and 8.3% respectively;NYHA stage III-IV dyspnea was found in 83.9%. Reduced EF heart failure was present in 110 patients (76.9%), seventeen cases with moderately reduced EF (11.9%) and sixteen patients had preserved EF (11.2%). Global heart failure was the dominant (91.6%). The main etiologies of heart failure were dominated by hypertensive heart disease in 46 patients (32.2%), followed by postpartum cardiomyopathy with 43 cases (30.1%), primary dilated cardiomyopathy in 18 patients (12.6%), ischemic heart disease in 16 patients with 11.2%. Seven cases of valvular heart disease, or 4.9%. The evolution was favorable under treatment in 104 patients or 72.7%. In-hospital mortality was 14.7%. Conclusion: Heart failure is a common condition in sub-Saharan Africa, particularly in our country.

Background: Noncommunicable diseases are the leading cause of death in the world and low and middle-income countries suffer from preventable premature death. The aim of this study was to assess the risk factors for non- communicable disease (NCDs) in general and particular cardiovascular diseases (CVDs) among the outpatients of our department of medicine. Methods: We performed a cross-sectional study from April to December 2017 by the consecutive enrollment of outpatients who attended in our department of medicine of Hôpital Sominé DOLO de Mopti, Mali. Clinical and laboratory data were measured for cardiovascular risk assessment. Framingham Risk Score (FRS) and Systemic Coronary Risk Estimation (SCORES) were computed by using Framingham and SCORE equations. Metabolic syndrome was defined using the harmonized criteria from the International Diabetes Federation (IDF) and the American Heart Association/National Heart, Lung, and Blood Institute (AHA/NHLBI). Data were captured in excel and analyzed with R version 4.0.3. The statistical significance was set at p = 0.05. Results: A total of 292 patients were enrolled in this study. The prevalence of traditional cardiovascular risk factors was 36.64%, 21.57%, 14.04%, and 13.01% for high blood pressure, hyperglycemia, smoking, and alcohol consumption, respectively. The metabolic syndrome accounted for 23.63%. The mean body mass index was 26.10 ± 7 kg/m2. The overall 10-year risk for cardiovascular events or death was 26.3% and 8.6% according to the FRS and SCORE equation, respectively. Gas Chromatographic Analysis of the Methanogenic Potential of Lignocellulosic Biomass Consisting of Banana Residues in Tambacounda, Senegal

The residual biomass composed of pseudo trunks and banana leaves is very important and poorly valued. There is very little quantified data on the deposits of residual biomass from banana plantations in Senegal and in particular in the Tambacounda region. In this work, we seek to evaluate the methanogenic potential and to valorize this biomass in biogas and biofertilizer. The laboratory experiment lasted approximately 35 days. During this time, the methanogenic microorganisms degrade the organic residue provided, which results in the production of biogas. At the end of the reactions, the rate of biogas production drops, indicating the end of the biodegradation of organic matter. Biogas production is measured over time and the composition of the biogas produced is analyzed by gas chromatography (GC) or by an infrared analyzer. The methane potential of each sample is determined from the cumulative quantity of methane produced in each flask representing a digestion system. The measurement can be expressed in m3 of CH4 per tonne of dry matter or per tonne of raw material. The first challenge of this study therefore lies in the acquisition of reliable and usable data to quantify the methanizable biomass. This study will allow us not only to evaluate the quantities of pseudo trunks and banana leaves available in a plot after harvest but also to test the biogas and methane production potential (BMP test) of this substrate and therefore determine the expected biogas production of this biomass.

We consider the problem of viscosity solution of integro-partial differential equation( IPDE in short) with one obstacle via the solution of reflected backward stochastic dif ferential equations(RBSDE in short) with jumps. We show the existence and uniqueness of a continuous viscosity solution of equation with non local terms, if the generator is not monotonous and Levy's measure is infinite.

A method to determine relevant geomechanical parameters for evaluating the hydraulic erodibility of rock

Among the methods used for evaluating the potential hydraulic erodibility of rock,the most common methods are those based on the correlation between the force of flowing water and the capacity of a rock to resist erosion,such as Annandale’s and Pells’*** capacity of a rock to resist erosion is evaluated based on erodibility indices that are determined from specific geomechanical parameters of a rock *** indices include unconfined compressive strength(UCS)of rock,rock block size,joint shear strength,a block’s shape and orientation relative to the direction of flow,joint openings,and the nature of the surface to be potentially ***,it is difficult to determine the relevant geomechanical parameters for evaluating the hydraulic erodibility of *** assessment of eroded unlined spillways of dams has shown that the capacity of a rock to resist erosion is not accurately *** more than 100 case studies,we develop a method to determine the relevant geomechanical parameters for evaluating the hydraulic erodibility of rock in unlined *** UCS of rock is found not to be a relevant parameter for evaluating the hydraulic erodibility of *** the other hand,we find that the use of three-dimensional(3D)block volume measurements,instead of the block size factor used in Annandale’s method,improves the rock block size ***,the parameter representing the effect of a rock block’s shape and orientation relative to the direction of flow,as considered in Pells’method,is more accurate than the parameter adopted by Annandale’s method.

Open Fractures of Limbs by the Bite of Domestic Donkeys: An Unusual Aetiology

Introduction: The aim of this study is to describe the epidemiological, lesional, therapeutic and evolutionary profile of open fractures of limbs by bites of domestic donkeys. Patients and Methods: This is a prospective descriptive study over 28 months made in the Orthopedics-Traumatology department of the Tambacounda Regional Hospital in Senegal, concerning patients bitten by donkeys, resulting in an open fracture of the thoracic and pelvic limbs. Results: Twelve male patients were included. The average age was 10.50 years ± 2.60. The most common circumstance was the bite when two donkeys were separated. The thoracic limbs were the most affected by the occurrence of the arm. Cauchoix-Duparc type II and AO A2 and B2 were predominant. The essential trimming/osteosynthesis was carried out within 24 hours of the bite. Serovaccination was systematic, but on the other hand, a single dose of anti-rabies serum was administered. Pinning was the most commonly used bone synthesis. Complications were mainly infectious, followed by a case of radial nerve palsy and non-union on pins. Conclusion: The typical profile of an open donkey bite fracture is a male child/adolescent who is bitten during the separation of two fighting donkeys. The lesions are most often found in the thoracic limbs with a predominance of Cauchoix-Duparc type II and type A2 and B2 fractures of the AO, whose treatment consisted of trimming and pinning. The infectious complications of the soft tissues marked the evolution.

Determining relative block structure rating for rock erodibility evaluation in the case of non-orthogonal joint sets

The most commonly used method for assessing the hydraulic erodibility of rock is Annandale's *** method is based on a correlation between the erosive force of flowing water and the capacity of rock resistance. This capacity is evaluated using Kirsten's index, which was initially developed to evaluate the excavatability of earth materials. For rocky material, this index is determined according to certain geomechanical factors related to intact rock and rock mass, such as compressive strength of intact rock, rock block size, discontinuity shear strength and relative block structure. To quantify the relative block structure, Kirsten(1982) developed a mathematical expression that accounts for the shape and orientation of the blocks relative to the direction of flow. Kirsten's initial concept for assessing the relative block structure considers that the geological formation is mainly fractured by two joint sets forming an orthogonally fractured system. An adjusted concept is proposed to determine the relative block structure when the fractured system is non-orthogonal where the angle between the planes of the two joint sets is greater or less than 90°. An analysis of the proposed relative block structure rating shows that considering a non-orthogonally fractured system has a significant effect on Kirsten's index and, as a consequence, on the assessment of the hydraulic erodibility of rock.

Giant Cell Tumor of the Proximal Phalanx of the Little Finger about One Case in Our Milieu

Giant cell tumor is a benign primary bone tumor. The phalangeal localization is rare. It is distinguished from other locations by the local aggressiveness of the tumor and a high rate of recurrence. We report one case of a 25-year-old right-handed nurse with no reported pathological history who complained about an ulcero-budding mass of the right little finger evolving one year ago. She would have initially consulted a bonesetter who would have taken care of her by using decoctions. The hand X-ray showed an osteolytic tumor of the first phalanx of the right little finger with a “honeycomb” appearance invading the soft tissues. We carried out the amputation of the radius. The postoperative course was simple with healing of the surgical wound and disappearance of the axillary adenopathy. The surgical specimen after anatomopathology exam concluded to be a grade 2 giant cell tumor of Senerkin.

The general objective of our study was to describe the clinical and epidemiological aspects of anxiety disorders. This was a retrospective descriptive cross-sectional study, lasting 5 years, from January 01, 2019 to December 31, 2023, on 225 patients seen in consultation for anxiety disorders in the department. The prevalence of anxiety disorders was 2.5% in consultation. The 20 - 29 age group was the most represented at 33.3%. Females were slightly represented at 55.1%, with a sex ratio of 1.2. The secondary level was the most affected, accounting for 36.4% of cases. Insomnia was the most frequent reason for consultation, accounting for 71.1% of cases. A larger general population study would be necessary to better assess the extent of anxiety disorders in Guinea.

Design Study of a Cable-based Gait Training Machine

This paper deals with a design approach of a gait training machine based on a quantitative gait analysis. The proposed training machine is composed of a body weight support device and a cable-driven parallel robot. This paper is focused on the cable-driven robot, which controls the pose of the lower limb through an orthosis placed on the patient＇s leg. The cable robot reproduces a normal gait movement through the motion of the orthosis. A motion capture system is used to perform the quantitative analysis of a normal gait, which will be used as an input to the inverse dynamic model of the cable robot. By means of an optimization algorithm, the optimal design parameters, which minimize the tensions in the cables, are determined. Two constraints are considered, i.e., a non-negative tension in the cables at all times, and a free cable/end-effector collision. Once the optimal solution is computed, a power analysis is carried out in order to size the robot actuators. The proposed approach can be easily extended for the design study of a similar type of cable robots.
