Life style modification improves insulin resistance and liver histology in patients with non-alcoholic fatty liver disease

AIm: To study the effect of regular aerobic exercise on insulin resistance, serum aminotransferase and liver histology in nonalcoholic fatty liver disease (NAFLD) patients. mETHODS: Sixty (mean age 40.0 ± 8.5 years, 75% male) NAFLD patients were included in the study. After baseline anthropometric measurement i.e., body mass index (BmI), waist circumference (WC); all patients were advised regular aerobic exercise for 30 min/d, for at least 5 d/wk and trained to achieve around 70% of his maximal heart rate. In addition, moderately energy restricted diet was advised to patients with high BmI (> 25 kg/m 2 ). monthly follow up was done by measuring BmI, WC, aspartate aminotransferase, and alanine aminotransferase (ALT). Insulin resistance was calculated using homeostasis model assessment (HOmA) of insulin resistance (HOmA-IR) model, at baseline and after 6mo. Insulin resistance was arbitrarily considered altered when it was ≥ 2. Liver biopsy was done in a section of patients at baseline and after 6 mo. RESULTS: Seventy percent (42/60) patients were overweight or obese; 95% (57/60) had central obesity (WC > 90 cm in men, > 80 cm in women). In the 45 exercise compliant patients insulin resistance decreased from 6.4 ± 6.1 to 1.3 ± 1.0, BmI from 26.7 ± 3.3 kg/m 2 to 25.0 ± 3.3 kg/m 2 , WC from 95.7 ± 8.9 cm to 90.8 ± 7.3 cm and ALT from 84.8 ± 43.5 U/L to 41.3 ± 18.2 U/L (P mpliant patient's insulin resistance, BmI, WC and ALT did not show significant change at 6 mo follow up. Six of 8 patients in compliant group on repeat liver biopsy showed significant change in steatosis and necroinflammation. Nonalcoholic steatohepatitis scores improved form 5.3 ± 1.5 to 3.35 ± 1.5. The decline in insulin resistance correlated with decline in ALT (P = 0.01, r s = 0.90) and liver histology (P = 0.03, r s = 0.73). CONCLUSION: Life style modification improves insulin resistance resulting in improvement in ALT and liver histology in NAFLD patients.

Cholera is re-emerging as an infectious disease with an increase in its overall incidence in Africa since 2008. To analyze the epidemiological aspects of the cholera outbreak in endemic areas in Benin. The study is a retrospective investigation on 2560 data of cholera patients received in health facilities from 2012 to 2016 in Benin. Besides, a prospective study was conducted. This prospective study includes 513 person (health professionals, community workers and people exposed to cholera) of five different areas of the country. It was conducted from April 10 to may 10, 2017. A survey was conducted over 513 in order to collect data on cholera symptoms, hygiene guidelines suggested by the respondents, excreta disposal, drinking water, household waste management and direct observation. Software R 3.4.0 was used for simple correspondence factor analysis (CFA). From 2012 to 2016, 16.48% of samples were taken and 48.34% were positive for Vibrio cholera O1, El Tor biotype, Ogawa serotypes. The Littoral area is leading in the number of cases (33.83%) followed by Atlantic (23.75%), Ouémé (16.79%) and Colline (8.91%). The survey revealed that the exposed populations are unaware of the symptoms of cholera and do not perceive the quality of drinking water as a strong guidance in the prevention of cholera. nevertheless, the majority of respondents (86%) are aware for the best times to wash their hands and know that they need to go to hospital in case of symptoms. Our study highlights the persistence of vulnerability factors to cholera in the targeted population, despite a good knowledge of prevention rules among staff and the population.

Background: Otitis media is one of the most common childhood infections, the leading cause of doctor’s visit by children in ENT clinic. The acute form if not recognized early is commonly characterized by suppuration from the middle ear following perforation of the tympanic membrane otherwise referred to as acute suppurative otitis media (ASOm). If not adequately treated, it progresses to chronic suppurative otitis media with attendant challenges in management. Ceasation of ear discharge is an indication of resolution of disease. The objective of this study is to appraise the value of local ear suction toileting and dressing in the ceasation of ear discharge in acute suppurative otitis media. materials and method: Consecutive new cases of acute suppurative otitis media seen at the Ear, Nose and Throat clinic of a tertiary health centre in north western Nigeria were assessed for efficacy of aural toileting and dressing with antibioctic impregnated guaze in the treatment of acute suppurative otitis media and compared with patients with similar disease without aural toileting and dressing or dressing with normal saline soaked guaze in addition to systemic antibiotic and antihistamine medications. Ear toileting was done by suctioning the discharge with suction machine, and cleaning the external auditory canal with hydrogen peroxide and methylated spirit. They were then dressed with gauze impregnated with antibiotic ear drops daily until there was no more ear discharge. The period of ceasation of ear discharge in each group was assessed. Results: There were 39 patients with acute suppurative otitis media (ASOm). Eighteen were females (46.2%) while 21 were males (53.8%), giving m:F ratio 1:2.1. Their ages ranged between 1 month and 45 years. About 36 (92.3%) were children and out of this, 28 (77.8%) were less than 5 years old. The average period of ceasation of discharge was 24 - 48 hours with those undergoing ear toileting and dressing but varied between 7 and 10 days wit

Differential cross sections for the elastic scattering of halo nucleus 6He on proton target were measured at 82.3 meV/u. The experimental results axe well reproduced by optical model calculations using global potential KD02 with a reduction of the depth of real volume part by a factor of 0.7. A systematic analysis shows that this behavior might be related to the weakly bound property of unstable nuclei.

The application of biostimulants in agriculture represents an environmentally friendly alternative while increasing agricultural production. The aims of the study were to develop solid biostimulants based on five rhizobacteria native to Benin’s soils and to evaluate their efficacy on the growth and biomass yield of maize under greenhouse conditions on ferrallitic and ferruginous soils. Clay and peat were used as a conservation binder for the preparation of the biostimulants. These binders were used alone or combined in the different formulations with maize flour and sucrose. 10 g of biostimulants were applied at sowing in pots containing five kilograms of sterilised soil. The experimental design was a completely randomised block of 24 treatments with three replicates. The results obtained showed significant improvements (m>Pm> m diameter (32.7%), leaf area (66.10%), above-ground biomass (97.12%) and below-ground biomass (53.98%) on ferrallitic soil with the application of the clay + m>Pseudomonas putidam> biostimulant compared to the control. On the other hand, the use of the peat biostimulant + m>Pseudomonas syringaem> was more beneficial for plant growth on ferruginous soil. The height, stem diameter, leaf area, above-ground biomass and below-ground biomass of the plants under the influence of this biostimulant were improved by 83.06%, 44.57%, 102.94%, 86.84% and 42.68%, respectively, compared to the control. Therefore, these results confirm that Rhizobacteria express their potential through biostimulants formulated on maize. The formulated biostimulants can later be used by producers to improve crop productivity for sustainable agriculture.

The lifetime of the first excited 2+ state in 18C was measured using an upgraded recoil shadow method to determine the electric quadrupole transition. The measured mean lifetime is 18.9±0.9(stat)±4.4(syst)ps,which corresponds to a B(E2; 2+1→0+gs) value of (4.3± 0.2±1.0) e2fm4,or about 1.5 Weisskopf units. The mean lifetime of the first 2+ state in 16C was remeasured to be about 18 ps,about four times shorter than the value reported previously. This discrepancy was explained by incorporating the γ-ray angular distribution measured in this work into the previous measurement. The observed transition strengths in 16,18C are hindered compared to the empirical transition strengths,indicating that the anomalous hindrance observed in 16C persists in 18C.

market garden products can carry several types of microorganisms, and their consumption is the source of many cases of food poisoning. This work aimed to improve food safety in Benin. In characterizing strains of K. pneumoniae and fluorescent Pseudomonas spp. at the biochemical and molecular level, the target was to identify contaminated watering water and garden products sold during Cotonou in both the dry and rainy seasons. A total of 164 samples of market garden products and 22 samples of watering water were investigated. The results showed that 5.91% of market garden products and watering water were contaminated by K. pneumoniae and 20.43% by fluorescent Pseudomonas spp. During the dry season, cabbage was most contaminated by fluorescent Pseudomonas spp. (50%). Pool water was more contaminated with K. pneumoniae (17%). All isolated strains were resistant to both amoxicillin and penicillin. All strains of K. pneumoniae and fluorescent Pseudomonas spp. were not resistant to imipenem, and 22% of them produced penicillinase. Among the 49 strains producing penicillinase isolated, 64.29% and 21.43% carried blaTEm and blaSHV respectively while 14.28% carried blaCTX-m genes. In light of the previously-developed results and considering the importance of horticultural products in Beninese food habits, we must improve national awareness of the risk for foodborne illness.
