Dynamic earth pressure induced by machine foundations on a neighboring retaining wall is analyzed with emphasis on factors which control the intensity and location of the design forces. The meshless local Petrov-Galerkin (MLPG) method is used to analyze the problem for a variety of retaining wall and machine foundation geometries. The soil medium is assumed to be homogeneous and visco-elastic. The machine foundation is idealized as a harmonic sinusoidal dynamic force often encountered in practice. A number of analyses have been made to reveal the effect of the loading frequency, the location and size of the foundation and the soil shear wave velocity on the distribution and magnitude of the dynamic earth pressure. Results indicate that there is a critical frequency and a critical location for which the passive pressure takes the maxima in the entire duration of the dynamic load.

Computational fluid dynamics(CFD)provides a powerful tool for investigating complicated fluid *** paper aims to study the applicability of CFD in the preliminary design of linear and nonlinear fluid viscous *** fluid viscous dampers were designed based on CFD *** first device was a linear viscous damper with straight *** second was a nonlinear viscous damper containing a one-way pressure-responsive valve inside its *** dampers were detailed based on CFD simulations,and their internal fluid flows were ***-scale specimens of both dampers were manufactured and tested under dynamic *** to the tests results,both dampers demonstrate stable cyclic behaviors,and as expected,the nonlinear damper generally tends to dissipate more energy compared to its linear *** compatibility was achieved between the experimentally measured damper force-velocity curves and those estimated from CFD *** a thermography camera,a rise in temperature of the dampers was measured during the *** was found that output force of the manufactured devices was virtually independent of temperature even during long duration ***,temperature dependence can be ignored in CFD models,because a reliable temperature compensator mechanism was used(or intended to be used)by the damper manufacturer.

Full thickness gastrointestinal defects such as perforations, leaks, and fistulae are a relatively common result of many of the endoscopic and surgical procedures performed in modern health care. As the number of these procedures increases, so too will the number of resultant defects. Historically, these were all treated by open surgical means with the associated morbidity and mortality. With the recent advent of advanced endoscopic techniques, these defects can be treated definitively while avoiding an open surgical procedure. Here we explore the various techniques and tools that are currently available for the treatment of gastrointestinal defects including through the scope clips, endoscopic suturing devices, over the scope clips, sealants, endoluminal stents, endoscopic suction devices, and fistula plugs. As fistulae represent the most recalcitrant of defects, we focus this editorial on a multimodal approach of treatment. This includes optimization of nutrition, treatment of infection, ablation of tracts, removal of foreign bodies, and treatment of distal obstructions. We believe that by addressing all of these factors at the time of attempted closure, the patient is optimized and has the best chance at longterm closure. However, even with all of these factors addressed, failure does occur and in those cases, endoscopic therapies may still play a role in that they allow the patient to avoid a definitive surgical therapy for a time while nutrition is optimized, and infections are addressed.

Endoscopic submucosal dissection(EsD) is the most advanced and representative technique in the field of therapeutic endoscopy and has been used for the treatment of gastrointestinal neoplasms,including early gastric *** major difference and advantage of EsD compared to existing endoscopic resection techniques,such as endoscopic mucosal resection(EMR) and polypectomy,are the width and depth of the *** developed cutting devices,distal attachable endoscopic accessories,and an advanced electrosurgical unit have helped to overcome the limitations of therapeutic endoscopy in terms of lesion size,location,presence of fibrotic scarring,and accompanying *** a result,the indications for EsD have been expanded from the classical indication for EMR and polypectomy,and there is now support for a further expansion of EsD *** present,the most critical factor to consider in the decision of whether to perform EsD is the probability of unexpected lymph node *** guidelines for EsD are continually being updated and *** this review,we discuss the strengths and weaknesses of the expanded guidelines,based on evidence found in the literature.

A Rare Case of Anterior sacral Meningocele (AsM) in a Young Patient

We report a 24-year-old male who presented with abdominal distension, constipation and left sided groin pain. CT and MRI of the abdomen/spine were performed which showed a large anterior sacral meningocele occupying most of the pelvic and abdominal cavity and displacement of their respective contents. Initially a posterior approach with lumbosacral laminectomy (L5 - s4) was performed. The ostium of the meningocele was identified with several nerve roots identified passing through, adjacent and into the defect. As nerve roots traversed the ostium, watertight closure was not feasible without sacrificing nerve roots. subsequent MRI demonstrated recurrence of approximately 60% of the anterior sacral meningocele. We therefore opted to approach the AsM anteriorly via an anterior approach with the help of colorectal surgical colleague. The AsM was completely embedded within the sigmoid and upper to mid rectal mesentry, with its own vascular supply to the thick walled capsule. This case highlights the need for a combined approach due to the incorporation of the pseudomeningocele into the omentum with the development of its own blood supply.

Background:New-onset diabetes after transplantation(NODAT)is a serious complication following liver transplantation(LT).The present study aimed to investigate the incidence of and risk factors for NODAT using the Korean Organ Transplantation Registry(KOTRY)***:Patients with history of pediatric transplantation(age≤18 years),re-transplantation,multi-organ transplantation,or pre-existing diabetes mellitus were excluded.A total of 1,919 non-diabetic adult patients who underwent a primary LT between May 2014 and December 2017 were *** factors were identified using Cox regression ***:NODAT occurred in 19.7%(n=377)of adult liver transplant *** analysis showed steroid use,increased age,and high body mass index(BMI)in recipients,and implantation of a left-side liver graft was closely associated with NODAT in adult *** living donor liver transplant(LDLT)patients(n=1,473),open donor hepatectomy in the living donors,steroid use,small for size liver graft(graft to recipient weight ratio≤0.8),increased age,and high BMI in the recipient were predictive factors for *** use of antimetabolite and basiliximab induction reduced the incidence of NODAT in adult LT and in adult ***:Basiliximab induction,early steroid withdrawal,and antimetabolite therapy may prevent NODAT after adult *** BMI or advanced age in liver recipients,open donor hepatectomy in living donors,and small size liver graft can predict the occurrence of NODAT after adult LT or LDLT.
