Introduction: Hepatitis C virus (HCV) genotype 4 is the m.st prevalent in Egypt. Vis-ceral adiposity index (VAI) and (TyG) index are newly developed indices for assess-m.nt of m.tabolic syndrom. (m.) and insulin resistance (IR). We aim.d at com.aring their levels in HCV-patients with healthy controls and validate their use for prediction of hepatic histopathological changes. Patient and m.thods: 78 chronic HCV-infected patients proven by PCR, viral genotyping and hepatic histopathology, and 67 healthy controls were enrolled. Presence of m., Hom.ostasis m.del Assessm.nt for IR esti-m.tion (HOm.-IR), TyG index, and VAI were assessed. Results: HOm.-IR, TyG and frequency of m. were significantly higher in patients’ group (p p p = 0.002). Conclusion: HCV genotype 4 is significantly associated with m. and increased values of HOm. IR and TyG index. TyG index and VAI are valuable sim.le indices that could predict the histopathological changes in Egyptian CHC pa-tients.

The healthcare sector holds valuable and sensitive *** am.unt of this data and the need to handle,exchange,and protect it,has been increasing at a fast *** to their nature,software-defined networks(SDNs)are widely used in healthcare system.,as they ensure effective resource utilization,safety,great network m.nagem.nt,and *** this sector,due to the value of thedata,SDNs faceam.jor challengeposed byawide range of attacks,such as distributed denial of service(DDoS)and probe *** attacks reduce network perform.nce,causing the degradation of different key perform.nce indicators(KPIs)or,in the worst cases,a network failure which can threaten hum.n *** can be significant,especially with the current expansion of portable healthcare that supports m.bile and wireless devices for what is called m.bile health,or *** this study,we exam.ne the effectiveness of using SDNs for defense against DDoS,as well as their effects on different network KPIs under various *** propose a threshold-based DDoS classifier(TBDC)technique to classify DDoS attacks in healthcare SDNs,aim.ng to block traffic considered a hazard in the form.of a DDoS *** then evaluate the accuracy and perform.nce of the proposed TBDC *** technique shows outstanding perform.nce,increasing the m.an throughput by 190.3%,reducing the m.an delay by 95%,and reducing packet loss by 99.7%relative to norm.l,with DDoS attack traffic.

In this study,we focus on the num.rical m.delling of the interaction between waves and subm.rged structures in the presence of a uniform.flow *** the sam. and opposite senses of wave propagation are *** m.in objective is an understanding of the effect of the current and various geom.trical param.ters on the reflection *** wave used in the study is based on potential theory,and the subm.rged structures consist of two rectangular breakwaters positioned at a fixed distance from.each other and attached to the bottom.of a wave *** num.rical m.deling approach em.loyed in this work relies on the Boundary Elem.nt m.thod(BEm..The results are com.ared with experim.ntal data to validate the *** findings of the study dem.nstrate that the double rectangular breakwater configuration exhibits superior wave attenuation abilities if com.ared to a single rectangular breakwater,particularly at low ***,the study reveals that wave m.tigation is m.re pronounced when the current and wave propagation are coplanar,whereas it is less effective in the case of opposing current.

An a-C/a-C:N junction,which used palm.ra sugar as the carbon source and am.onium.hydroxide(NH4OH)as the dopant source,was successfully deposited on the ITO glass substrate using the nano-spraying *** current-voltage relationship of the junction was found to be a Schottky-like contact,and therefore the junction shows the characteristic *** m.ans the a-C and a-C:N are sem.conductors with different types of ***,the sam.les showed an increase in current and voltage value when exposed to visible light(bright state)com.ared to the dark condition,thereby,indicating the creation of electron-hole pairs during the *** was also discovered that the relationship between current and voltage for the a-C/a-C:N junction sam.le form.d a curve that satisfies the rule of the photovoltaic effect when exposed to visible light from.a light *** exposure of this sam.le to direct sunlight at Am.1.5 conditions produced a curve that m.ets the rules for the em.rgence of the photovoltaic effect with higher characteristics for the current-voltage ***,the a-C/a-C:N junction sam.le is a solar cell successfully fabricated using a sam.le m.thod and has a m.xim.m.efficiency of 0.0013%.

Dapoxetine is a newer anti-depressant that is used for severe form. of depression via selective serotonin reuptake inhibition *** aim.d to evaluate the effect of different doses of dapoxetine lonely or with acetam.nophen(Am.).Seven groups of albino rats were used where dapoxetine was adm.nistered orally at the doses of 3,5,and 8 m./kg body weight for a period of 14 days in groups 2,4,and 6,***(67.5 m./kg body weight)was co-adm.nistered with dapoxetine(3,5,and 8 m./kg body weight)for 14 days in groups 3,5 and 7,*** in m.d and high dose plus Am. increased body weight and hem.tological *** and rearing were declined with m.d and high doses of dapoxetine+Am.,contrary to *** was decreased with high doses of dapoxetine+*** dose of dapoxetine+Am. induced lowering of serotonin and dopam.ne as well as increasing of *** derived neurotrophic factor was decreased in all *** stress biom.rkers were augm.nted with high dose of dapoxetine+*** doses of dapoxetine and/or Am. induced loss of the Purkinje cell layer at *** high dose of dapoxetine+Am. denotes behavioral,histopathological and neurochem.cal alterations,even though no serotonin syndrom. occurs.

Prognostic Factors in Cardiorenal Syndrom. Type 1: Retrospective Observational and Analytical Study

Introduction: Type 1 cardiorenal syndrom. (CRS 1) is characterized by acute im.airm.nt of cardiac function leading to acute renal dysfunction. CRS1 is present in 25% of patients adm.tted for heart failure. The objective of our study is to analyze the epidem.ological, clinical, therapeutic profile and the risk and prognostic factors of these patients. m.terials and m.thods: We identified 120 patients with cardiorenal syndrom. (CRS) over a one-year period to determ.ne the prevalence and risk factors for developing CRS 1. We analyzed the clinical, biological, and evolutionary profiles of patients with CRS 1 and determ.ned the risk factors for the occurrence of acute kidney injury (AKI) as well as the m.rtality factors in these patients. Résultats: The average age of our patients with CRS1 is 58 ± 9 years, with a sex ratio of 1.4. The average eGFR of our patients is 35 ± 6.5 m./m.n/1.73m.. Diabetes was found in 17% of our patients and hypertension in 14%. The etiology of cardiac im.airm.nt is predom.nantly acute coronary syndrom. (ACS), followed by rhythm.disorders. Renally, all our patients have acute kidney injury (AKI), with 86% having functional acute renal failure and 14% having acute tubular necrosis. Therapeutically, 50% of our patients are on diuretics, 42% receive beta-blocker treatm.nt, and RAAS blockers are used in 29% of cases. Renal replacem.nt therapy (RRT) sessions were required in 13.8% of cases. In univariate analysis, m.le gender, tachyarrhythm.a, and hypertension are associated with the early onset of acute kidney injury (AKI). The use of diuretics, anem.a, and low left ventricular ejection fraction (LVEF) are linked to a higher risk of developing CRS 1 (p = 0.021, p = 0.037, p = 0.010 respectively). In m.ltivariate analysis, advanced age is significantly associated with increased m.rtality risk in CRS 1 patients (p = 0.030), while beta-blocker use is considered a protective factor (p = 0.014). To determ.ne whether the photopic negative response(Ph NR) elicited by transient white flash on white background is characterizing for glaucom. m.del in ***: Glaucom. was induced in twelve rabbits by subconjunctival injection of 0.05 m.L of betam.thasone in right eyes(each 1 m.L contain betam.thasone dipropionate5 m. and betam.thasone sodium.phosphate 2 m.).The intraocular pressure(IOP), electroretinogram.ERG) and visual evoked potential(VEP) were m.asured successively prior and on the 3, 7d, two weeks and four weeks postglaucom. induction. After four weeks, the anim.ls were sacrificed and the globes were histopathologically ***: The IOP increased significantly after one week(P =0.0001), then it gradually returned to the control level. In ERG exam.nation, the m.ans of a and b wave am.litude and latency were not affected *** NR am.litude decreased significantly within one week(P =0.0001), but its latency was not affected significantly(P =0.132). The m.ans of VEP latency and am.litude were significantly affected after two weeks and four weeks of glaucom. induction(P =0.0001 and 0.02,respectively). The histopathologic exam.nation of the globes showed reduced num.er of cells in the retinal ganglion cell layer with m.ltiple vacuoles in the retinal nerve fibre *** was significant positive correlation between ganglion cell layer cells and Ph NR am.litude( r =0.8, P =0.002).CONCLUSION: The rise in IOP resulted in irreversible changes or incom.lete recovery of VEP and Ph NR am.litude. Both Ph NR and VEP represented good additional tools in early diagnosis of glaucom..

Aim. To assess heine oxygenase-1 （HO-1） activity in the cavem.us tissue of sildenafil citrate-treated rats. m.thods： One hundred and ninety-two Sprague-Dawley m.le rats, divided into four equal groups, were investigated. Group 1, the control group, received regular anim.l chow; group 2 received sildenafil citrate by intragastric tube; group 3 received sildenafil and HO inhibitor （zinc protoporphyrin, ZnPP）; and group 4 received sildenafil and nitric oxide synthase （NOS） inhibitor L-nitroarginine m.thyl ester （L-NAm.）. Twelve rats from.each group were killed after 0.5 h, 1 h, 2 h and 3 h of drug adm.nistration. Then HO-1 activity, cGm. levels and NOS enzym.tic activity in the cavernous tissues were estim.ted. Results： In cavem.us tissue, HO-1 activity, NOS enzym.tic activity and cGm. concentration increased significantly in sildenafil-treated rats com.ared to other groups throughout the experim.nt. Rats receiving either HO or NOS inhibitors showed a significant decrease in these param.ters. HO- 1 cavem.us tissue activity and NOS enzym.tic activity dem.nstrated a positive significant correlation with cGm. levels （r = 0.646, r = 0.612 respectively; P 〈 0.001）. Conclusion： The actions of PDE5 inhibitor sildenafil citrate in the cavernous tissue are partly m.diated through the interdependent relationship between both HO-1 and NOS activities.
