Modified preoperative score to predict disease-free survival for hepatocellular carcinoma patients with surgical resections

BACKGROUND No prognostic models specific to hepatocellular carcinoma patients receiving surgical resection have been considered strong and convincing enough for survival prediction thus far,and there are no models including only preoperative predictors.We derived a nomogram to predict disease-free survival in a previous study.AIM To simplify our score and compare research outcomes among other scoring systems.METHODS We retrospectively reviewed data from 1106 patients with hepatocellular carcinoma who underwent liver resection at the Linkou Chang Gung Memorial Hospital between April 2003 and December 2012.Multivariate analyses were conducted to identify the significant survival predictors.Homogeneity,Harrell’s C-index,and Akaike information criterion were compared between our score,AJCC 8^(th)edition,Tokyo score,and Taipei Integrated Scoring System(TTV-CTPAFP model).RESULTS Among the 1106 patients,731(66.1%)had tumor recurrence at a median followup of 83.9 mo.Five risk factors were identified:platelet count,albumin level,indocyanine green retention rate,multiplicity,and radiologic total tumor volume.Patients were divided into three risk groups,and the 5-year survival rates were 61.7%,39%,and 25.7%,respectively.The C-index was 0.617,which was higher than the Tokyo score(0.613)and the Taipei Integrated Scoring System(0.562)and equal to the value of the AJCC 8th edition(0.617).CONCLUSION The modified score provides an easier method to predict survival.Appropriate treatment can be planned preoperatively by dividing patients into risk groups.

Neural plasticity secondary to carpal tunnel syndrome: a pseudo-continuous arterial spin labeling study

Conventional neuroimaging techniques cannot truly reflect the change of regional cerebral blood flow in patients with carpal tunnel syndrome.Pseudo-continuous arterial spinning labeling(pCASL)as an efficient non-invasive neuroimaging technique can be applied to directly quantify the neuronal activities of individual brain regions that show the persistent symptoms owing to its better spatial resolution and increased signal-to-noise ratio.Therefore,this prospective observational study was conducted in 27 eligible female carpal tunnel syndrome,aged 57.7±6.51 years.Psychometric tests,nerve conduction studies and pCASL neuroimaging assessment were performed.The results showed that the relevant activated brain regions in the cortical,subcrotical,and cerebral regions were correlated with numbness,pain,functionality,median nerve status and motor amplitude of median nerve(K=21–2849,r=–0.77–0.76,P<0.05).There was a tendency of pain processing which shifted from the nociceptive circuitry to the emotional and cognitive one during the process of chronic pain caused by carpal tunnel syndrome.It suggests the necessity of addressing the ignored cognitive or emotional state when managing patients with carpal tunnel syndrome.Approval for this study was obtained from the Institutional Review Board of The University of Hong Kong/Hospital Authority Hong Kong West,China(HKU/HA HKW IRB,approval No.UW17-129)on April 11,2017.This study was registered in Clinical Trial Registry of The University of Hong Kong,China(registration number:HKUCTR-2220)on April 24,2017.

Pancreatic lymphangioma:two case reports from an institutional experience with a rare entity and review of literature

Pancreatic lymphangiomas are very rare benign cystic lesions that pose a diagnostic dilemma due to their resemblance to other non-neoplastic and neoplastic pancreatic cystic lesions.As such,pancreatic lymphangiomas are frequently diagnosed only after histological examination of the excised lesion.We present 2 cases of pancreatic lymphangioma,determine its prevalence at our institution and perform a detailed review of published literature since 2010.Case 1 is a 36-year-old male and case 2 is a 35-year-old female,both of which presented with abdominal pain.These were the only cases of pancreatic lymphangiomas reported at our institution since 2010.We reviewed 69 cases of pancreatic lymphangiomas from 52 publications.It affects predominantly females with a median age of 43 and such patients typically present with abdominal pain(58.8%)or are asymptomatic(27.9%).The median size is 8.6 cm,the most common location is the head of pancreas and the most common imaging finding is that of a multilocular cyst.Majority of patients underwent surgical resection(69.6%).Endoscopic ultrasound-guided fine-needle aspiration features of pancreatic lymphangiomas include chylous cyst fluid,elevated fluid triglyceride levels(15/16 cases)and numerous lymphocytes on cytology.The majority of patients with elevated fluid triglyceride levels(13/15 cases)were managed conservatively.Pancreatic lymphangiomas are rare pancreatic cystic lesions that may be diagnosed preoperatively using a multidisciplinary and multimodal approach involving clinical,radiological,biochemical and cytological features,allowing greater confidence in the selection of patients who can be managed conservatively.

A ruptured recurrent small bowel gastrointestinal stromal tumour causing hemoperitoneum

Hemoperitoneum is a rare and potentially life-threatening complication of GIST. We reported a 54- year-old man who developed disseminated intra-abdominal recurrence from a previously resected gastrointestinal stromal tumour （GIST） of the small bowel, and the patient presented with hemoperitoneum. Emergent debulking surgery was performed. A high dose imatinib was prescribed. Despite the presence of residual disease, the patient was well clinically 8 months after the operation. Even though, there is no evidence to support the routine use of debulking surgery in the management of GIST. In our patient, disease progression after second line targeted therapy and the absence of alternative treatment options for spontaneous rupture and hemoperitoneum prompted us to treat the patient aggressively. Resection of the ruptured GIST was carried out for control of bleeding and to prevent recurrent bleeding in this patient with good surgical risks. During the treatment decision-making, the patient＇s general condition, the risk of surgery and the extent of dissemination were taken into consideration. In this patient who presented with spontaneous rupture of a small intestinal GIST, the novel use of targeted therapy and aggressive surgical treatment produced reasonably good survival outcome.

Dear Editor,Plant tissue culture involves callus formation and de novo shoot regeneration.First,explants from differentiated tissues are used to generate a pluripotent cell mass,called callus,on auxin-rich callus-inducing medium(CIM),followed by shoot regeneration on cytokinin-rich shoot-inducing medium(SiM).Callus results from division of pericycle-like cells(Atta et al.,2009;Sugimoto et al.,2010);its cellular identity resembles that of lateral root primordia(Atta et al.,2009;Sugimoto et al.,2010).Callus acquires cellular pluripotency by forming root stem cell niches on CIM(Sugimoto et al.,2010)。

Exploratory use of ultrasound to determine whether demyelination following carpal tunnel syndrome co-exists with axonal degeneration

Carpal tunnel syndrome （CTS） accompanied by secondary axonal degeneration cannot be clearly dis- criminated using the current cross-validated ultrasound severity classification system. This study aimed at exploring cut-off values of ultrasound parameters, including wrist cross-sectional area （W-CSA）, wrist perimeter （W-P）, ratio of cross-sectional area （R-CSA） and perimeter （R-P）, changes of CSA and P from wrist to one third distal forearm （△CSA＆AP） for differentiation. Seventy-three patients （13 male and 60 female） were assigned into group A （demyelination only, n = 40） and group B （demyelination with secondary axonal degeneration, n = 33） based on the outcomes of nerve conduction studies （NCS）. Receiver Operative Characteristics （ROC） curves were plotted to obtain sensitivity, specificity, and accuracy of cut- off values for all the ultrasound parameters. The overall identified cut-off values （W-CSA 12.0 mm2, W-P 16.27 mm, R-CSA 1.85, R-P 1.48, △CSA 6.98 mm2, △P 5.77 mm） had good sensitivity （77.1-88.6%）, fair specificity （40-62.2%） and fair-to-good accuracy （0.676-0.758）. There were also significant differences in demographics （age and severity gradation, P 〈 0.001）, NCS findings （wrist motor latency and conduction velocity, P 〈 0.0001; wrist motor amplitude, P 〈 0.05; distal sensory latency, P 〈 0.05; sensory amplitude, P 〈 0.001） and ultrasound measurements （W-CSA, W-P, R-CSA, R-P, △CSA＆△P, P 〈 0.05） between groups. These findings suggest that ultrasound can be potentially used to differentiate demyelinating CTS with sec- ondary axonal degeneration and provide better treatment guidance.

Recently,there has been an upsurge of activity in image-based non-photorealistic rendering(NPR),and in particular portrait image stylisation,due to the advent of neural style transfer(NST).However,the state of performance evaluation in this field is poor,especially compared to the norms in the computer vision and machine learning communities.Unfortunately,the task of evaluating image stylisation is thus far not well defined,since it involves subjective,perceptual,and aesthetic aspects.To make progress towards a solution,this paper proposes a new structured,threelevel,benchmark dataset for the evaluation of stylised portrait images.Rigorous criteria were used for its construction,and its consistency was validated by user studies.Moreover,a new methodology has been developed for evaluating portrait stylisation algorithms,which makes use of the different benchmark levels as well as annotations provided by user studies regarding the characteristics of the faces.We perform evaluation for a wide variety of image stylisation methods(both portrait-specific and general purpose,and also both traditional NPR approaches and NST)using the new benchmark dataset.

Fractal Analysis of Rainfall-Induced Landslide and Debris Flow Spread Distribution in the Chenyulan Creek Basin, Taiwan

The Chenyulan Stream in Central Taiwan follows the Chenyulan fault line which is a major boundary fault in Taiwan. In recent years, many destructive landslides have occurred in the Chenyulan Creek Basin after heavy rainfall accompanied by several strong typhoons. Three examples of landslide distributions in the Chenyulan Creek Basin, before and after 1996 and after 2004 are analyzed. The box dimension and two-point correlation dimension are employed to describe the landslide area size distribution and distance distribution between every two landslides, respectively. It is found that the number of landslides increased in this period. However, the average landslide area decreased. The correlation dimension gradually increased from 1.15 to 1.32 during this period（before and after 1996 and after 2004）. This implies that the landslide distribution in the Chenyulan Creek Basin has become diffuse and extensive. The box dimension value shows the degree of the landslide density occupied in a space. The box dimension also increased from 0.3 to 0.69 during this period. The increasing box dimension means that the landslide presented in this creek basin has gradually increased. This indicates that the slopes of this creek basin have become more unstable and susceptible.

Are herbal medicinal products less teratogenic than Western pharmaceutical products?

AIM: To determine the use and teratogenicity of herbal medicinal products (HMP). METHODS: A retrospective study was conducted in a University hospital to compare the difference in the pattern of use and fetal outcomes between pregnant women who took HMP and Western pharmaceutical products (WPP). RESULTS: From 1995 Jan to 2001 Dec, 61 and 372 women took HMP and WPP one month before or during their current pregnancies respectively. There was an increase in the prevalence of pregnant women who took HMP from 0 % in 1995 to 0.8 % in 2001. Among HMP users, 51.6 %, 82.8 % and 58.6 % of them had low monthly family income (<15 000), low education level (secondary education or below) and were unemployed respectively. In comparison to WPP, pregnant women used smaller number of HMP (1.4 vs 3.0, P<0.01) at a later gestation (4.8 weeks vs 3.1 weeks, P <0.01) and within a shorter duration (11.1 d vs 47.9 d, P<0.01). An Investigation of Effect of Stand-Off Distance on the Material Removal Characteristics and Surface Generation in Fluid Jet Polishing

Fluid jet polishing(FJP)is a versatile polishing process that has many advantages compared to other polishing processes.Stand-off distance(SOD)is one of the key parameters in flu id jet polishi ng.However,relatively little research work has been carried out to investigate its effect of SOD on material removal characteristics and surface generation in FJP.In this paper,a systematic investigation of the effect of SOD on the tool influence function and surface topography in FJP was conducted.Experiments were designed for FJP two kinds of materials corresponding to ductile and brittle materials.They are nickel copper(NiCu)alloy and BK7 optical glass,respectively.In this study,the SOD was varied from 2 to 35 mm.Analysis and discussions were made on its effect on the shape of TIF,material removal rate,and surface topography.It is interesting to note that the TIF shape becomes a Gaussian-like shape with large SOD both on NiCu and BK7,which provides a novel way to optimize the TIF in FJP.The variation of the material removal rate and surface roughness versus SOD on NiCu and BK7 were also determined from the experimental results.Moreover,the surface topography of NiCu and BK7 were characterized from the results measured from the white light interferometer and scan electron microscope.The outcome of the study provides a better understanding of the material removal characteristics and surface generation mechanism in FJP.
