The activating effect of IFN-y on monocytes/macrophages is regulated by the LIF-trophoblast-lL-lO axis via Statl inhibition and Stat3 activation

Interferon gamma （IFN-7） and leukemia inhibitory factor （LIF） are key gestational factors that may differentially affect leukocyte function during gestation. Because IFN-,/induces a pro-inflammatory phenotype in macrophages and because trophoblast cells are principal targets of LIF in the placenta, we investigated whether and how soluble factors from trophoblast cells regulate the effects of IFN-7 on macrophage activation. IFN-7 reduces macrophage motility, but enhances Statl activation, pro-inflammatory gene expression and cytotoxic functions. Soluble factors from villous cytotrophoblasts （vCT＋LIF cells） and BeWo cells （BW/ST＋LIF cells） that were differentiated in the presence of LIF inhibit macrophage Statl activation but inversely sustain Stat3 activation in response to IFN-7. vCT＋LIF cells produce soluble factors that induce Stat3 activation; this effect is partially abrogated in the presence of neutralizing anti-interleukin 10 （IL-IO） antibodies. Moreover, soluble factors from BW/ST＋LIF cells reduce cell proliferation but enhance the migratory responses of monocytes. In addition, these factors reverse the inhibitory effect of IFN-y on monocyte/macrophage motility. BW/ST＋LIF cells also generate IFN-y-activated macrophages with enhanced IL-IO expression, but reduced tumor-necrosis factor alpha （TNF-cd, CD14 and CD40 expression as well as impaired cytotoxic function. Additional assays performed in the presence of neutralizing anti-lL-lO antibodies and exogenous IL-IO demonstrate that reduced macrophage cytotoxicity and proliferation, but increased cell motility result from the ability of trophoblast IL-IO to sustain Star3 activation and suppress IFN-y-induced Star1 activation. These in vitro studies are the first to describe the regulatory role of the LIF-trophoblast-lL- 10 axis in the process of macrophage activation in response to pro-inflammatory cytokines.

Return on Investment after Implementation of a Centralized Automated Storage System in a Hospital Pharmacy

An ADS （automated dispensing system） was implemented in our hospital pharmacy in 2008 to optimize and secure the medication process. The main objective of this study was to compare the projected and the real ROI （return on investment）, after seven years of use. ROI was calculated annually （from 2008 to 2015）, by deducting the cost of investment （ADS buying and implementation, maintenance, repairs and ADS upgrade） from the cost saving （drugs stock reduction and decrease of pharmacy staff dedicated to global dispensing）. In 2015, total costs saving （＋$1,141,987） were divided between decreasing drug stock value and reduction of pharmacy staff. Total costs of investment （-$978,656） were acquisition, maintenance, repairs and an unplanned upgrade costs. Finally, the real ROI was ＋$163,331 after seven years of use. In 2008, projected ROI was ＋$410,786. The real payback period has been increased by approximately two years （six years of use, instead of four years as expected）. Despite their cost, ADSs are a worthwhile investment, leading to a ROI within a few years. These economic considerations should be put into perspective with optimization of drugs stock management, greater efficiency of the global dispensing process, securitization of medication process and redeployment of pharmacy staff.

Nanostructured hybrid materials for bone-tooth unit regeneration

As a part of regenerative medicine, biomaterials are largely used in this field of nanotechnology and tissue engineering research. We have recently developed a new scaffold using electrospun nanofibers of Poly (ε-caprolactone), PCL which is able to mimic the collagen extracellular matrix of cells. The aim of this study was to engineer a biological and implantable structure leading the regeneration of the tooth-bone unit. For this aim, we have cultured mouse osteoblasts embedded in a collagen gel on the nanofibrous membrane and coupled this structure with an embryonic dental germ before implantation. To follow bone and tooth regeneration, we have performed RT-PCR, histology and immunofluorescence analysis. We showed here that this leaving implantable structure represents an accurate strategy for bone-tooth unit regeneration. We report here the first demonstration of bone-tooth unit regeneration by using a strategy based on a synthetic nanostructured membrane. This electrospun membrane is manufactured by using an FDA approved polymer, PCL and functionalized with osteoblasts before incorporation of the tooth germs at ED14 (the first lower molars) to generate bone-tooth unit in vivo after implantation in mice. Our technology represents an excellent platform on which other sophisticated products could be based.

Prebiotic-Like Effects of SweetPearl®Maltitol through Changes in Caecal and Fecal Parameters

Prebiotic-like effects of maltitol were investigated supplementing two groups of rats with either 5% maltodextrin (control group) or 5% maltitol (maltitol group). A third group was supplemented with 5% maltitol at first and then with 5% maltodextrin (maltitol/maltodextrin group). Faecal parameters were monitored throughout the experiment and caecal parameters at the end. The weights of caecal content and caecal wall were significantly higher in the maltitol group than in the control group, but not in the maltitol/maltodextrin group. Propionic acid concentration was significantly higher in the maltitol group compared to both control and maltitol/maltodextrin group. Faecal parameters were also influenced by the dietary supplementation with maltitol: the amount of dry matter in feces decreased and alpha-glucosidase activity increased. These effects lasted 28 days in the maltitol only group, whereas they stopped some days after the switch to maltodextrin in the maltitol/maltodextrin group. Maltitol could induce prebiotic-like effects.

Predictive Factors for Persistent Pain and Poor Recovery of Health Status 1 Year after Whiplash Injury (Quebec Grade 1 and 2): Results from the ESPARR Cohort

Background: A large number of studies have addressed whiplash injury, and many meta-analyses have sought to highlight chronicity factors;the implicated processes, however, remain a matter of debate. The present study used data from the ESPARR cohort (an on-going prospective study of a representative cohort of road accident victims in the Rh?ne administrative département of France). The objectives were to describe the consequences of whiplash injury and to determine prognostic factors for poor recovery and persistent pain at 1 year post-accident. Methods: The cohort included 255 “pure” whiplash victims, 173 of whom responded to the 1-year follow-up questionnaire. Correlations between explanatory variables and health and pain status were explored by modified Poisson regression to provide adjusted relative risk (RR) values. Results: Half of the victims had not fully recovered health status by 1 year. The main factor associated with non-recovery was pain (RR = 1.3;1.0-1.7). A birth in the family preceding the accident emerged as another factor (RR=1.5;1.2-1.9). Victims responsible for their accident were twice as likely to report being free of pain as those not responsible (RR = 0.5;0.3-0.8). No correlation emerged with accident-related characteristics or PTSD. Conclusions: The present results extend our understand- ing of whiplash injury. Residual pain is the fundamental factor causing whiplash victims to feel that they have not recovered good health. Our findings suggest this may be bound up with physical factors (gender susceptibility);external factors such as having to carry weights (such as a baby) and with perceiving oneself as a victim are not incompatible with this hypothesis.

CT Profile of Hydrocephalus in Children at the Charles de Gaulle Pediatric Teaching Hospital in Ouagadougou

Aim of the Study: The aim is to analyse the epidemiological, clinical and CT aspects of hydrocephalus in children. Method: This was a cross-sectional descriptive study with retrospective collection from 1 June 2021 to 31 December 2022 within the radiology department of Charles De Gaulle Pediatric Teaching Hospital in Ouagadougou, Burkina Faso. The study included the records of patients with hydrocephalus who underwent cerebral computed tomography within this radiology department. Results: Hydrocephalus is due to a disorder of the hydrodynamics of the cerebrospinal fluid causing an increase in the volume allocated to this fluid in the brain and being accompanied by an increase in the pressure of this fluid. Its diagnosis is established by Doppler ultrasound or MRI prenatally, transfontanellar ultrasound in newborns and young children whose fontanel is permeable and by CT or MRI in older children, who can also determine the etiology. MRI remains an imaging technique that is less available and less accessible than CT in developing countries like ours. In Burkina Faso, MRI is only available in two private health facilities. The objective of this study was to describe the epidemiological, clinical and computed tomography aspects of hydrocephalus in children at the Charles de Gaulle pediatric teaching hospital in order to contribute to the diagnostic assessment and better management of this pathology. Over 19 months, 105 cases of hydrocephalus were recorded out of a total population of 115 children, or an average of 6 cases per month. The mean age was 17.52 months, with extremes of 03 days and 13 years, and the sex ratio was 1.38. Macrocrania, convulsions and psychomotor retardation were the main clinical signs, with rates of 49%, 34% and 30% respectively. On cerebral CT scan, hydrocephalus was triventricular (41.90%), tetraventricular (40%), and predominantly non-communicating (60%), with a mean Evans score of 0.53.
